Danger. John, there is also development on hydroxychloroquine. Thats the drug that President Trump and many fox hosts have touted repeatedly. Theres a new study that shows that those who took it had higher death rates than those who did not. Now the National Institutes of health is recommending against using this drug in combination with the socalled zpak that the president has also been pushing. Meanwhile, the Senate Passing a 480 billion relief package for small businesses. It also provides money for hospitals and for more testing. The house is expected to vote on this measure tomorrow. Lets start with the significant change to the timeline of the Virus Outbreak here in the u. S. Cnns dan simon is live in San Francisco with the details. What have we learned, dan . Hi, alisyn. Experts have been speculating that some time that the virus has been in the country earlier than anybody ever knew. Now this adds to the growing body of evidence. What Santa Clara County is saying, keep in mind this is the heart of silicon valley. That somebody who died in their home on february 6th had the coronavirus. Keep in mind, the first reported deaths in the country actually came on the february 29th in the seattle area. So were talking weeks earlier. Santa clara also saying that two other deaths are linked to the virus, one on february 17th and one on march 9th. Not only does it mean that the virus had been here longer than we knew. But it represents many more infections according to Santa Clara County officials. Now at the time to get tested for the coronavirus, you had to you need to have traveled over to asia or to china. Known travel history. You had to have had certain symptoms. None of these people had history of travel. Were talking about community spread. Santa clara collected tissue samples from deceased individuals, sent them to the cdc lab and got the results back yesterday. John, back to you. Dan simon in california for us. This is really interesting. Join us now is chief medical correspondent, dr. Sanjay gupta and well speak to dr. Hotez in a minute at Baylor College of medicine in houston. Sanjay, this news that the first known death three weeks earlier than previously thought from community spread, it is believed, which means i presume, that this virus was here maybe midjanuary. What are the implications of this. I think this is pretty stunning, john. Not surprising at the same time. Not surprising and stunning at the same time. I think for some time, we thought the first patient diagnosed in the United States, did we happen to catch the very first patient . We were looking for specific people who traveled back from china at that point or had known exposure to coronavirus. The idea that this was a virus, that it was already spreading in the community, i think, was suspected for a while. We just didnt know for sure. So if you put up that timeline again and say february 6th and february 17th, i believe, when these patients now when they went back and looked, they were suspicious. Maybe they were thought to have had the flu before. Many other patients out there as well. Because if these are patients who died, how many how much of this coronavirus is spreading in the community and when. Probably much more and a lot earlier. Right in the middle of those two dates, between february 6th and february 17th, february 12th you remember is when the cdc also disclosed that the testing that they had was not working. The testing was flawed. So this is a combination of things. Inadequate testing, not testing the right people and now as a result, recognizing that this is probably spreading much earlier in the community. Im talking maybe early, mid january, probably at least between the time someone was exposed and the time that someone died from this. Also, just one other thing ill add to this. January 31st is when that travel ban from china was implemented. You know, obviously i think it made sense to a lot of people at the time. We start layering in this new information, its going to become clearer, i think, that the virus was already here by that point. Theres a lot of interesting things. This was california, not Washington State. We had previously thought that this was not the Washington State nursing home. That we had heard so much about. These are people that have not traveled, not off of a cruise ship. Not the previous assumptions that we thought about how it got here and how it was spreading. Heres what the chief medical officer said, sanjay, alluding to what youre talking about in terms of the cdc. She says each one of those deaths is probably the tip of an iceberg of unknown size. It feels quite significant. We had to ask the cdc every single time, does this person meet the case definition. May we send a sample. We had this uncomfortable feeling we were hearing about a lot of patients who felt they were cases but we couldnt test. So how does that inform where we are now . You know, i think that the notion that this has been around for a lot longer than we realize, you know, within the United States certainly maybe other places around the world, its interesting information to have. But it speaks to how we need to move forward. This is sort of this the rearview mirror. This is probably spreading. If we look at other patients who were thought to have died of flu or flulike symptoms, you may find more patients had coronavirus. I think, at the time we werent doing enough testing. At the time we were talking almost exclusively to the federal government about the diamond princess cruise ship around that time. It highlights, again, that the testing was the key. Because some of this was missed. If it had been caught at that point, maybe we would have had a better chance at containing this. Maybe not. I think a lot of this is stuff that we start to look at in the rearview mirror. But hopefully Going Forward. Just to be clear, the february 6th date the first fatality before that ship left port. Dr. Hotez, if we can, i want your quick take on this before we move on to next the next wave of the coronavirus pandemic. First on this, how likely is it that there were more deaths from coronavirus in february than we realized now given this information . So everything sanjay said as it always is, is spot on. Hes absolutely right. This really does highlight the dangers of silent Community Transmission. Heres why. Mark lip ships and others looking at the covid19 in china. A very interesting analysis to show that the number of patients ir going to have in your intensive care unit in your community is pretty much related to how long silent transmission continues before you intervene. For instance, whether 2,000 icu patients is because Community Transmission went on for six weeks most of it silently before an intervention with social distancing, as opposed to another chinese city where you intervened for a week. For a city like new york which has been devastated, it means we had silent Community Transmission going on. The number of cases doesnt rise in a straight linear fashion. Theres a lag and then an exponential rise that catches you off guard. This is going to be a very important Lesson Learned when we look at relaxing social distancing as we move into the summer month. Sanjay, i want to ask you about what dr. Redfield of the cdc, the director of the cdc said that has gotten a lot of attention. Which is that the next round of coronavirus, which he expects in the late fall and winter, could be even more difficult than the one we just went through. Now, sanjay, how is that possible . When we all know so much more about how to protect ourselves now than months ago. Doctors know more about how to i mean, we dont have a treatment but they do know more about the virus and about some things that are, i guess, ameliorating some symptoms for patients. How can it possibly be worse next time around . Well, you know, i think you raise an important point, alisyn. First of all, even though we learn more, we have to act on that knowledge as well. I mean, i happen to be living in a state right now where theyre starting to think about relaxing some of these guidelines despite all the Public Health evidence suggesting that we shouldnt. What really, i think, is going to be striking, people will get exposed as a result of relaxing these guidelines. We wont know it for a couple, three weeks later. There will be a couple, three weeks where people will say, were doing fine. We relaxed the guidelines, nothing happened. Everything is fine. And then a few weeks later, all of a sudden people will become sick. Hopefully not that many. But people will need to be hospitalized. As a result, other places around the country may say everything is fine in georgia, well start relaxing guidelines as well. Even though we have the knowledge, we have to act on it. Theres another issue, alisyn. This outbreak in the United States, as were seeing the timeline is changing but it seemed to happen as flu season was starting to wane off. This time around in the fall you might get both at the same time, the flu and coronavirus. Flu is very significant. Causes lots of people to be hospitalized and all that uses the same resources. So you compound these two things and thats part of what i think dr. Redfield was saying. Hes said this a few times in the past as well. This isnt the first time he brought this up. If youre relaxing things. You compound this with flu. The good news in all of this, to keep in mind, physical distancing has worked. Its hard. Its tough. It requires a lot of sacrifice from people. It has worked. We got to make sure we can continue to do it. The concept of a second wave is not new. In some ways its more than 100 years old. All we have to do is look at the 1918 flu. Heres a graph of that right now. There are three waves there. It was the second wave in the fall that was, by far the most deadly. Dr. Hotez, ive been reading what youve been saying about what dr. Redfield of the cdc said in an interview with the washington post. Your reaction is something along the lines of duh. I dont know about that. But we have had for a week or so the coming out of harvard that showing the predictive will be seeing seasonal waves in the winter, around january, february, 2021, 2022. This could be a regular occurrence until ultimately we have several vaccines out there where we can vaccinate the population. But you know, i think another point to reinforce what sanjay said again, dr. Redfield talked about the second wave in january or february. I agree thats a likelihood. But i would just revise that to say that wont be the second wave. That might be the third wave. The second wave could easily be this summer and fall. As were relaxing social distancing prematurely in several states. Some of the models coming out of the metrics in seattle show that for many states, including states like texas, we really need to maintain social distancing until june 1, or the middle of june until we prevent new cases. Most states are not willing to do that. I understand the economic urgencies. The problem is this. We dont have the Health System in place in order to manage opening up social distancing prematurely. By that i mean in the workplace. We dont the Contact Tracing or surveillance in most cities. Im starting to talk to City Health Directors and its pretty scary. Theyre not getting any guidance at the federal level from the krd in some cases. Theyre not getting guidance from the states. So theyre pretty much on their own. They dont have the bandwidth to know how to manage this in their cities. I understand the urgency of opening things up sooner than those projected dates from the models, but if youre going to do that, at least have a system ready to go in order to manage those detection of new cases. Once again, we all know that the reason why we have this tragedy in new york is we werent able to monitor that first wave of infections. Sanjay, quickly, new developments on that drug that weve heard so much about from President Trump as well as fox hosts. High clocks chloroqui hydroxychloroquine. Theres a new study out of the veterans hospital. They studied 368 patients. They did not find that those who took the drug needed less ventilation. It was the same amount as those who did not take the drug. Those who took the drug had higher death rates. That seems significant. And when President Trump says what do you have to lose . I think now doctors have an answer to that. Yeah. I think this is part of another evidence that these are still small studies, alisyn. Thats the problem. Whether the studies are good, showing benefit or not good, theyre still small studies. We wouldnt be talking about this on cnn were it not for the situation that were in right now. I do want to point out, its not a randomized study. Thats significant. That means that the people werent randomly assigned to different treatment groups. If doctors had im not suggesting they do. But if people have inherent bias going into the study, that would influence the outcomes. They think this doesnt work, were giving it to people who are sicker, conscious bias. The point is, you have to random eyes. You need larger trials. Were going to get that data. Youre right, so far when you look at whats come out of brazil and sweden and france and now the v. A. Study out of the United States, it certainly hasnt shown benefit. In some cases its shown harm. We need the larger studies where we dont know its blinded. Who got what, we dont know. Now we look at the results and it shows what it shows. I want a treatment. You want a treatment. All the other statistics could be beneficially affected by that. But we dont have that yet. We still need more data in either direction, good or bad. The lesson clearly seems to be just wait for the science in both cases. Yeah. Dr. Sanjay gupta, dr. Now were seeing a lot of improvement as far as people testing positive but we still have a lot of work to do to slow the spread of the virus. I think one of the things were pushing for in a big way here is more testing. Then the tracing as well, the Contact Tracing. Thats really whats going to ultimately, whats going to make us better here in the city of boston, the commonwealth of massachusetts. Interesting that you say you havent seen the improvement that dr. Birx said she saw last night. I also have heard you say, you cant even think about the process of reopening right now. What do you need to happen for that to even begin . I think its too early. The information, i mean, one thing were lacking is information. I think what the doctor might be referring to is hospital availability, which is consistent and when you talk about hospitals in boston is consistent. We did build out a hospital at the Boston Convention center bcec we call it. We have about 175 people in there. We have two sides, one is a homeless and one is a hospital side. Were putting people in there and keeping availability in our hospitals open. But really, at the end of the day, it comes down to testing and data to see how we kien of get back to society here. Theres a lot of focus on it. Its a conversation were having internally as well. Right now were still working on it. The important thing to do is stop the spread of the virus by social, physical distancing. All of the things you hear about every day on the show that dr. Gupta talks about all day. How much help are you getting from the federal government when it comes to testing, which you clearly believe is a major need this morning. When you think about 1 of the last number i heard was 1 of the American Public has been tested. That clearly is not enough to be able to get a good gauge where we have to go as a country. When you break that down to the city of boston, clearly the Testing Capacity needs to be expanded. [ inaudible ] we have ten Health Centers and five hospitals that have the capacity to do the testing. Schools closed for the rest of the year in the commonwealth. That announcement from governor baker yesterday. I heard from members of my family who will not be in school for the rest of the year in massachusetts. Whats your reaction . How tough will that be in boston . When you think about it, when kids go back to school in september, they will have been out of school for six months. Thats just something thats never seen here in the United States. Thats going to be challenging. Our superintendent is working on a plan on how we take the next two months, 2 1 2 month and make sure our kids have a focus on Education Learning through electronics. Then as we get back to work in september, america has a lot of work to do, particularly those we have a lot of work to do to get i dont know we fully understand what this is going to do to those kids and how they will adjust. Its a missing month, period, its a gap for a whole generation of americans that we have to address as soon as the fall. Mayor, boston not even thinking about as you said reopening yet. Youre not there yet. There are other states that are. Georgia, South Carolina. As you look at those decisions being made, what do you think of that . I was watching cnn last night. It could be pretty scary. Im watching a lot of the pushback. My colleague pushing back on some of the decisions being made, opening beaches, opening different movie theaters, things like that. That can be a very dangerous situation in those cities and towns and states, particularly if they dont have the testing, which we dont. If they dont have the tracing, which we dont. If they dont have all of that. All of the process set up and what could happen is well see another surge in cases. People are already talking about a second surge here in the fall, in the winter. I cant imagine having to go through what were going through again. We need to prepare for that. When we open up society and its going to be a different normal, we when reopen society, we have to think about what does a second surge look like and how do we keep our economy moving during that time. The way they do this is by using cell phones to figure out if people are actually complying with the guidelines. Joining us now is lauren myers, a professor at the university of texas at austin. She leads a team of researchers working on this other model. Its great to have you. We hear so much about the university of washington model that the white house has used, the ihme. And your model now at the university of texas is getting a lot of attention. First, explain how are we doing with social tis tansing based on distancing based on cell phone use . I think what our forecasts show is that were doing a very good job. The reason that the projections for covid19 deaths are not continuing to skyrocket and do seem to be subsiding is because of the excessive social distancing. People have actually stopped going to commonplaces and are spending more time at home. I know it is awkward for you as an academic and a researcher to criticize your colleagues at the university of washington. I know you have a lot of respect for them and the model that they do. Clearly, you thought that there was a different way to do things because you all are putting out a different model. What were they not getting exactly right or what did you want to build on . Yeah. I think its more what did we want to build on. So the original model makes projections for what is going to happen in the United States and states around the country based on data from china and from europe. The assumption is what happened in those countries after social distancing measures were enacted will be the same here. Our model looks exclusively at whats happening in the United States under foot. Were using data from cell phones and cell phones of tens of millions of people that tell us on a daily basis how much time are people spending at home. How often are they in Grocery Stores or pharmacies or other places where they could have contact that lead to disease transmission. That is what is fueling our forecast. The daytoday changes and the extent to which were actually coming in contact with each other or effectively social distancing. Can you quantify that for us . How much time are people spending at home . Well, the data suggests that people really have dramatically reduced over the last two weeks the extent to which theyre going to points of interest and spending a lot more time at home. That is the secret to success. That is why we have seen a trailing off of mortality in this country. That is why its going to be important Going Forward that we continue to take measures to prevent transmission by just keeping out of contact with each other or reducing the likelihood of transmission when we have to come in contact with each other. What does your model suggest about whats about to happen in South Carolina and georgia, places that are lifting some of the social distancing guidelines or i guess i should say reopening some of their key businesses this week . So i need to make a distinction. Our model and the these models are based on the first wave of transmission. Our model can project what will happen in the next three weeks with respect to covid19 deaths. It assumes that we are undertaking social distancing and its going to continue to carry us in a downward slope. We use other models to project what is going to happen if people start to come in contact with each other. What the models say is that as people start to come in contact with each other again, there are going to be more opportunities for transmission. Since not a lot of people have been infected yet, most most of our population is susceptible. Unless we continue to take measures, we expect to see cases and hospitalizations and mortality start to climb again in many of our cities. Does your model suggest that the u. S. At this moment has passed the peak in terms of deaths . Yes. Our model says its pretty likely that the u. S. Has passed its peak and many of the states have also passed their peak. That is under the current social distancing measures. We can only one of the things our model and our website does is try to communicate not only what we can predict but what we cant predict. One of the things we cant predict is how behavior is going to change in the next week or two or month or two. To the extent that changes, the forecasts may look very different. Since you only look at three weeks out, do you know what the death toll, do you have a prediction for what the death toll will be three weeks from now . In the United States as a whole . Yeah. Id have to look it up. If you go to our website i think what youre saying, the latest number is 53,000. 53,000 were at 45 right now. That mean by may 5th, 53,000, which is just another bitter pill to have to swallow. Absolutely. Knowing that there will be that many thousands more. Absolutely. I think, you know, its a horribly bitter pill. I also could be so much worse if we werent doing the social distancing that were doing today. Really fair. Ill never see him here again. Im sorry. Nursing Homes Across America are taking the brunt of the outbreak. This is just a small sample of states that publicly report coronavirus cases at Nursing Homes. More than 10,000 in new jersey to 1700 in california. Now a federal agency that oversees Nursing Homes says they must report their coronavirus deaths and infections directly to the centers for Disease Control and prevention. Whats causing the problem inside of Nursing Homes when this comes to covid19 . Its really two things, sara. Understaffing and Infection Control and the two things go hand in hand. In new jersey at andover subacute and rehabilitation center, 17 bodies were discovered in their morgue. In richmond, virginia, 45 residents dead from covid19 at this facility. In massachusetts, holy oak soldiers home, coronavirus killed 47 veterans, including this womans husband of 38 years. She said she believes the administrator there may not have initially taken the virus seriously enough. The commander in chief was saying it was nothing. You know. The very first major outbreak in all of america happened at a nursing home outside of seattle. 129 people linked to the facility infected with covid19. 35 people died. All of a sudden there were so many patients, everybody needed medications and treatments. Their facility was fined in part for not doing proper Infection Control. Watchdog groups say that is one of the most common citations for many facilities, like care centers Life Care Centers disputes the finding. What went wrong . An unprecedented viral outbreak which we didnt know enough about entered our country. Because we have a vulnerable population, it injured our population. Thats what went wrong. He stands by the frontline workers saying they were the first in america to heroically battle a new and invisible enemy. Representatives of many Nursing Homes warn the governments failure to provide enough testing and the scarcity of personal protection can be a lethal bin nation. Can they selfdistance from the patients . Dont they have to clean them and lift them and help them rehabilitate. Thats exactly right, sara. Many of these patients have to be fed. Nurses are touching and handling patients all the time. They cant avoid it. They have literally no way of protecting themselves. This morning, several states start to reopen some businesses, there are growing concerns over the health and safety of the people going back to work. Are employers ready to have them back without widespread testing. Joining me is seth harris, former acting labor secretary under president obama. Always great to have you with us. You say there has been too much focus on the when to reopen, and not on the how. Explain. Well, we hear the president of the United States talking about a rush to open and, you know, supporting protesters who want everything in our economy open right now. But there are very complicated details of how individual employers open up their work places. Were not getting a lot of guidance on that from the white house or the former labor department. This is a difficult task. Theyre subject it a legal obligation to make their workplace as safe and healthy as possible. It is different in every workplace. They need a lot of help and i dont think theyre getting it. Georgia is way out in front on this with Governor Brian Kemp allowing nail salons, barbershops, tattoo parlors, massage parlors to reopen friday. They have some safety guidelines that they have put out. Let me put those up on the screen. Well, i think Governor Kemp is listening reading Donald Trumps tweets rather than reading the plan that came out of the white house. The white house plan said that in order to get to phase one opening, you have to see 14 days of decline in the number of cases in your state. Thats not whats happening in georgia right now. They also talk about maintaining social distancing during that time and yet Governor Kemp is opening up massage parlers and nail salons, im not sure how you sustain social distancing in those contexts. So i dont think that what Governor Kemp is doing and some of the other governors opening up parts of their economy is doing is safe. Employers are not obligated to open. They should only reopen their workplaces if they know that they have everything they need to keep the place safe. Do they have the kind of social distancing that you were talking about, are they able to keep sick and infected workers out of the workplace, are they able to create barriers where there is where you cant have social distancing, do workers have the personal protective equipment they need . Employers need to make that decision for themselves, they shouldnt necessarily follow the lead of politicians. You said something really interesting, employers are not legally required to reopen. It begs the question, what do employees, what are the requirements of employees . And the president was asked a version of this, the issue of liability yesterday during the briefing by kaitlan collins. Listen to this. These companies that open and they have employees come back to work and they get sick, will these companies be liable . Give you a legal answer to that when we look it up, but we tried to take liability away from the companies. We just dont want that. We want the companies to open and to open strong. But ill get you a legal opinion on that. Let me ask your opinion, mr. Secretary, what are the rights of someone who works at a company that reopens . If you dont want to go back to work . Right, well, employees are going to be in a difficult position. Because your Unemployment Insurance is no longer available to you if your employer calls you back to work. But workers do have the right to work in a safe and healthy workplace under the federal Occupational Safety and health act in some states under state Occupational Safety and health act, and for those workers who are not employees, who are contractors, they have a right to sue in tort, a negligence claim against their employer. The answer to kaitlans question is, yes, employers are potentially subject to liability. And the federal government has not waved liability. So employers should not rely on that fudge that the president gave at the podium. There is potential legal liability. Now, it might be hard to make out a case, but i think most employers in america try to do what is right under the law, and that means keeping workers safe. How do you mack a cake a cas though, if the president of the United States is suggesting that states should reopen and the governor of the state suggesting it is okay for companies to reopen, how do you make that case in court . The argument has to be that there are certain guidelines from the cdc, from the Occupational Safety and Health Administration and from the white house itself that the employer is not following. If youre Standing Shoulder to shoulder with your coworkers and your coworkers are sick and they are coughing and youre subjected to that, then there is potentially a case that there is a violation of the Occupational Safety and health act and workers have the right to complain to federal osha or state osha and it is illegal for their employer to retaliate against them. Workers should use that right to make sure that they are being kept safe by their employers.
