It is new day. It is wednesday, april 8th, 6 00 in new york. This morning, we try to see where the coronavirus hits next in the United States. As new york city grapples with its highest numbers of deaths yet, there are some signs that the worst projections of the death toll might be avoided. On tuesday, United States had its deadliest day so far. Coronavirus has now killed nearly 13,000 americans. But the cdc director believes that thanks to the stayathome orders and social distancing, we may, may see fewer fatalities than the Trump Administration projected. But still, this morning, many governors and mayors around the country are warning that they are still dangerously close to running out of medical supplies for whatever comes next. New statistics show the coronavirus is disproportionately killing black americans. Dr. Sanjay gupta investigates why and well bring you his findings in a moment. Also, developing overnight, the lockdown in wuhan, china, has been lifted. 11 Million People in the city where this pandemic started are now free to leave their homes for the first time in ten weeks. Well show you what that looks like and if were seeing a glimpse into our future, john. This morning, President Trump who on february 24th stayed the stock market looked good to him and on february 26th said the number of cases in the u. S. Was a couple of days away from being zero, hes threatening to withhold funds from the World Health Organization for its response. Also this norng, President Trump, who himself voted by mail in florida last month is dismissing mailin voting as corrupt. It comes after thousands of wisconsin voters stood in long lines on tuesday with little or no protective gear. State republicans cnn released new polling about what the public thinks about the pandemic and the federal governments response. Well bring you those numbers shortly. We wanted to get to with athena jones with the latest on the battle against coronavirus. The highest number of deaths yet in new york, athena. But continuing hope that it might be leveling off. Reporter good morning, john. Thats exactly right. New york and new jersey now account for more than half of all coronavirusrelated deaths in the u. S. Andrew cuomo said the curve could be flattening with the number of people being hospitalized and the number of people neilding breathing tubes dropping. Experts say the gains are dependent on people continuing to practice social distancing. Heres more of what the governor had to say. Right now, were projecting that we are reaching plateau in the total number of hospitalizations and you can see the growth and you see its starting to flatten. Again, this is a projection. It still depends on what we do. And what we do will affect those numbers. This is not an act of god that were looking at. Its an act of what society actually does. So the governor is saying it is up to us to make sure that these gains continue to go in the right direction. The governor state that the rate of increase for cases in new york appears to be flattening. Meanwhile, the epicenters of the outbreak, here in new york and in new jersey and in connecticut, i should mention, all saw their highest single day death toll. But on the good news front, the cdc director says the stunning model suggesting as many as 240,000 people could die of the coronavirus were based on only about 50 of americans following these social distancing guidelines. It shows you that this can work. New york is averaging about 200 deaths a day at home. That is roughly ten times the normal average. Not all of these are tested for coronavirus. But that is one issue theyre facing here. More than 200 members of the National Guard are now assigned to new york citys office of the chief medical examiner to help with the dignified recovery of these bodies. This Mission Began last week and the teams are working 24 7. Theyre also joined by 52 members of the army. These are mortuary, military specialists trained to process bodies. Theyre also helping the medical examiner. Really big operation going on here. John, alisyn . Athena, thank you very much for that reporting. Join us now, doctors from two of the hardest hit states. We have dr. Rob gore. Hes an emergency physician at suny, down state Medical Center in Brooklyn New York and dr. Say gar, the chief of Infectious Disease department at holy name Medical Center in nuchblgt great to have you both. Dr. Gore, i want to start with you. Youve been so helpful for us over these past weeks. That piece that our reporter, Miguel Marquez did in your hospital, i cant stop thinking about it. When he was there, there kept being the announcements overhead of code 99, just one after another. When you hear that things in new york may be, may be leveling off or plateauing, are you seeing that yet in your hospital . One can hope that the numbers continue to drop. Our Emergency Departments are still filled with new patients every shifty come in. Were still putting patients on ventilators. Were still seeing people die. Like the other day, there may be three or four code 99s within the span of 30 to 40 minutes. And so i dont work on the floors, but when i contact the internal medicine docs upstairs or talk to the icu docs, the floors are still saturated with patients and still overwhelmed. Doctor, when you look at this and i know people are at home and they want to know what does this mean for me . What does it mean if the number of deaths, while still high and tremendously upsetting, is leveling off . What does it mean if the number of hospitalizations is leveling off . What does it mean for me at home as we try to get through this pandemic . Sure. I echo the sentiments of my e. R. Colleague. I work on the floors and the e. R. We see a high acute and oftentimes requiring advance support, including intubation. I think whats important for the general public to understand is that these models are based on assumptions. Assumptions include social distancing. The early information is such that it shows that the social distancing, the mitigation is indeed working. Those at home should feel empowered that what theyre doing is just as important as what were doing on the front lines. Dr. Gore, President Trump is getting a different message somehow or different information. Last night he said something to the effect of everyone is doing great. Lets listen to this moment. I think were reaching a level where it will start coming down, sloping down. The good thing is that the number of beds needed, i think we were right about that. I was right. My group was right. Theyre not needing nearly as many bids as they thought or ventilators as they thought. In fact, i saw on your show and a couple of other people just reported back to me that everyone is in great shape from the standpoint of ventilators. That is not reflected in what were hearing from governors. In massachusetts, in terms of ppe, theyre in a dangerous place, kentucky, and oregon say the same thing, kansas says the same thing in terms of the masks, the gowns, everything youre grappling with. What are you seeing in terms of supplies and the need . Any time right now we still have a lot of patients were taking care of. Right now were seeing a plat u plateau. At least theyre saying theres a plateau. If those numbers decide remember, the incubation period is about in any disaster situation, were mindful of the supplies were using any time were working in our Emergency Department. Just in medicine in general, were cautious of what were using, how much were spending. Are we going to have enough medication and enough protective gear. Because if we dont and we get hit hard, were not prepared for t but im still very cautious. When the president says oh, everybody is great. Any time you have somebody who loses their life, then people are not doing great. Any time you have someone dying because of a disease thats spread via communication in close proximity, were not doing okay. If we let our guard down at least once, the number of new cases [ inaudible ] the crucial staff that are required to care for patients in the Emergency Departments, on the floors, the death toll will continue to increase. Dr. Saggar, one of the other interesting things when youre in a Public Health phenomenon alike we are now, hospital admissions, maybe theyre going down because the standards are getting higher, right . Another thing interesting and upsetting perhaps is that in new york city at least, the number of people dying at home has spiked. Its 20 times what it normally is. We dont know for sure if all these people are diving from covid19 or dying from covid19 or coronavirus. But theres a suspicion they are. What do you see when you look at the dying at home numbers . Thats a great point. As this pandemic continues, were learning more and more about the virus every day. Were learning that it doesnt just affect the respiratory system. It can affect the cardiac system. There may be people having heart attacks, blood clots, having strokes at home and dying that may not be readily evidenced from the virus which in fact it very well may be. As we go along, we unfortunately, find a lot of that, but not necessarily directly but indirectly. Either indirectly because patients werent coming in routinely. Theyre scared to come to the e. R. And dying of heart attacks and strokes. Or the effect of virus, as were learning, the virus can have a lot of systemic effects. It may be causing strokes and blood clots, causing people to die at home. Dr. Gore, on that note, we spoke to a young woman. I believe her husband was 31 years old. They were were hearing this from more people. I cant remember if they were turned away from the hospital. But i think anyway, he died at home is my point. When people are showing up, were hearing this story over and over. Unless theyre in acute distress, the hospitals cant take them and theyre told to go home and some of them are dying at home with their loved ones. What are your thoughts, dr. Gore . Some people are dying at home with their loved ones. There are a couple of things. One, if you have an opportunity to spend the end of this loved ones life here with them, you and the rest of the family in a more supportive setting, youd rather do that. If they come to the Emergency Department and they do die here, were not letting families in because were concerned that we may increase the risk of those family members being infected by other patients in the Emergency Department. So the question is, do we have the person come into the e. R. And die in a hospital with nobody that they know or do we have them die at home surrounding their loved ones . This is something i recommended yesterday with a close family friend whose mother is pretty sick. If we keep her in the hospital, even though she wasnt in super distress, we keep her there, shell wind up getting an infection and dying nat hospital. If we can keep them around their loved ones, im sure thats something people like and prefer. Thats a very great point snien. Thank you both very much. New this morning, disturbing new statistics show the coronavirus is disproportionately killing black americans. John and alisyn, as we get closer to the apex of this coronavirus curve, the peak of the curve as everyone is talking about, another risk factor has ee mernld. A significant one. That is race. Just how big a risk factor is it . Turns out pretty big. Why is that . I i decided to find out myself. In the beginning, covid19 was far away and it didnt have a name. It was coming over from shores from people who traveled. Once it got a foothold in the United States, its true self was revealed. Once people got infected, because of their diabetes and Heart Disease and the like, they would get it more severely and be at higher risk of dying. Dr. Kamara jones is i was struck, dr. Jones, were both doctors and as i was hearing the story, it sounded just all of a tragically familiar. Is this a familiar story . What we see is familiar because the condition of our lives hasnt changed. Whats happening is that we are carrying the burden of these limited opportunities in our body, the diabetes, the Heart Disease, the hypertension, even in terms of immune compromised and all of that. Already, were hearing from some officials that blacks have been disproportionately hit. In michigan, where you grew up, 14 of the population is black, though they make up 41 of coronavirus deaths. In illinois, 15 identify as African American, but they make up 42 of deaths. Louisianas population is 32 black, which accounts for about 70 of coronavirus deaths. But heres the problem according to dr. Jones. As inadequate as testing had been for the country, the problem is even worse for African Americans. Our whole National Testing strategy has started out as a clinical strategy, not a Public Health strategy. That, of course, has disadvantaged the whole nation in terms of knowing who has contracted. Im told also that it has been harder for many people of color to get the test just because of where testing stations have been located. While blacks are less likely to be tested, less likely to be treated, they are more likely to be on the front lines. Essential workers. Upon whom we all depend. Were out here as Public Workers doing our jobs trying to make an honest living to take care of our families. Detroit bus driver, Jason Hargrove posted this online on march 21st. It is not as easy for them to shelter in place. And its not as easy because they might have front facing jobs like Home Health Aides or bus drivers or working at amazon and the like. For you to get on the bus and staying on the bus, and cough several times without covering up your mouth and you know that were in the middle of a pandemic, that lets me know that some folks dont care. Four days later, hargrove, who is 50 years old, became ill. He died last wednesday. Its still unclear how he got covid19. John and alisyn, i think theres a couple of important things that are sort of more immediate. First of all, we need to collect the data. Data is actually hard to come by. First of all, theres been inadequate testing as everybody knows. Some of the results, the data was not broken down by demographics. Something we should continue to insist on. Also, there may be a tragic sort of story of the rationing of care because theres not enough of some of these resources weve been talking about. I think its really important to be sure that the structural inequalities dont manifest, arent affected by that rationing of care as well. John, alisyn . All right. Thanks to sanjay gupta for that. Dr. Rob gore is back with us. Dr. Gore, your hospital serves a largely Minority Community and as upsetting as these statistics are, i have a feeling that theyre not one bit surprising to you. No. These statistics arent surprising at all. Ive always worked in black neighborhoods. The one im in right now happens to be where i grew up which is predominantly African American and afro caribbean. Our death rates are higher in central brooklyn. Our rates of homicide are higher. Hypertension and diabetesrelated deaths are higher. Rates of spread of hiv infections are higher. Hospitalizations due to asthma. If you can think about potential about healthrelated problems, our o numbers are a lot higher. People seem surprised that theyre higher in chicago, the state of louisiana with marginalized communities. What the pandemic is showing us what weve already known. Our Health System is fractured and marginalized communities dont have the resources required to keep them alive and thriving. The social determinants that have always impacted health. Education, economic development, the more we start to see these economic divides, the more we see patients in our communities not receiving quality education, were going to still see the numbers continue to skyrocket as it relates to the pandemic and every other healthrelated outcome that can potentially happen in the future. Dr. Gore, that theory about rationing of care, the reason i ask because on new day weve interviewed several patients or family members of patients who are black and they have told the story of having a horrible time getting tested. I know getting tested is challenging for many, many people across the board. But there was something about their stories, about getting the runaround. Of having to go from this place to this place and being denied that testing, that we cant tell if thats racially biased. Are you seeing anything like that . Well, in our Emergency Departments, the only testing that were doing are people who we are considering to admit to the hospital. Were not testing everybody who comes in with a cold or runny nose or a little bit of a cough. It doesnt make sense because if you test lets say you test negative, maybe about 20 of those tests may be false negative and you may have the disease process itself. If you still come in contact with other people, that still increases the risk of you being infected, even if you tested negative to begin w but we dont have the numbers and the resources to adequately test every single patient that comes into our hospital for every complaint. But were trying to save those tests only for those people who are highest at risk. If somebody does have symptoms, well tell them go home, you probably do have coronavirus or covid19. These are the supportive measures that we wanted to make sure you take at home. By keeping you in the hospital, doing testing, it just exposes you to other people who might be sick. If you didnt have it before, you will contract it being in our spaces. Dr. Gore, it also is the fact that African Americans are subjected to a kind of double jeopardy when it comes to coronavirus, which is that they have because of structural issues largely, many of the Underlying Health conditions that make covid19 even more dangerous. Whether its diabetes or high Blood Pressure or higher rates of obesity, things like that. A lot of that can be explained through structural or socioeconomic reasons. But it does put you at a higher risk. Well, you cut off. I didnt hear the last bit of question. Im sorry. I was talking about the double jeopardy. Dealing with the Underlying Health conditions that happen at higher rates in the African American community. That is definitely the case. The viral infection, your immune system has to kick in and help fight off the infection. If you have all sorts of other stressors, whether societal and environmental stressors, theyre preexisting Health Conditions like diabetes and longstanding hypertension which can lead to kidney failure. If youre suffering from chronic obstructive pulmonary disease, chronic bronchitis, on steroids or on any immunosuppressant, those are additional factors which inhibit your ability to come back from this disease. Its not surprising the deaths are much higher. A lot of issues havent been if we want to change the scope and flatten the curve, there are a couple things that need to happen. We need to mitigate the transmission of the disease process. Number two, if were looking at longterm goals, you have to fixed entire Health System, fix the economic system. If you dont have the resources that it takes for your basic survival, how can you expect to take on and be able to combat covid19. Dr. Rob gore, 2018 cnn hero and 2020 hero forever. He was criticized after firing the commander of an Aircraft Carrier who warned about a coronavirus outbreak on his ship and then he called that commander stupid. Cnns ryan brown is live with more at the pentagon. Its been a whirlwind few days for the navy. As you mentioned, after the captain sent that warning, the navy decided to fire him. The acting director decided to fire him for too widely distributing that warning saying he created a firestorm and shown poor judgment. After that, captain got a rousing farewell from the sailors on his ship. The secretary decided to fly out to guam and address the same sailors, slamming the captain calling him at that naive or stupid for intentionally leaking that warning to the press. Later, he was ordered by the secretary of the defense to issue an apology. But after mounting criticism from defense officials and from members of congress, started to call for his resignation, he stepped down. Resigning, were told, over this issue. He did not address his comments to that crew in his Resignation Letter but did address them in a memo to the navy saying in his memo, i lost Situational Awareness and decided to speak with them as if i was their commander or shipmate rather than their secretary. They deserve better. The crew deserved more empathy and a lot less lecturing. I lost sight of that at the time. Now the pentagon has appointed the number two civilian in the army to step in as acting Navy Secretary in the interim. Good morning, john. You just noted there is a majority of the American Public who thinks the federal government is doing a poor job. Look at this by partisan breakdown. We see partisanship playing into the assessment. Among republicans, 80 say the federal government is doing a good job. Only 17 poor job. Look among independents and democrats. Majority believe that the federal government is doing a poor job in responding to the outbreak. The other thing that i think is interesting and important to look at is just the concern the American People have about the current state of affairs. Take a look at the notion of the pessimism that is out there. 80 of respondent in this poll say the worst is yet to come. 80 . 17 say the worst is behind us. How about that ppe weve been talking about, the personal protective equipment gear for the front line responders. 69 of americans say the federal government should be doing more. Only 28 says the federal government is doing enough when it comes to getting that ppe stuff out and dealing with the shortage. Then finally, look at this, though. The partisanship is not as stark here on the issue of ppe. Its across the board. 87 of democrats, 72 of independents and 42 of republicans think that the federal government needs to be doing more to address this shortage. Of course, would you be comfortable returning to your regular routine after april 30th if indeed the soicial distancin guidelines lifted. 60 of americans say no, they would not be comfortable at april 30th if the guidelines are lifted returning to their routine. Thats an important number. That could have a big Economic Impact when the public is ready medically to go back to normal. It will be interesting to see if theyre ready. What are people saying about the president S Performance . Yeah. Thises critical. As you know. This president will be entirely judged on the response here, the economic fallout. So take a look at him fighting the outbreak. Only 43 say hes doing everything he can. A majority 55 say President Trump could be doing more. How about his overall just handling of the outbreak, his approval on that. Take a look here. Hes at 45 approval right now overall on handling the outbreak. Thats a slight bit up, john, from where he was in march when we asked that. So too is his disapproval a slight bit up. Hes at 52 . A slim majority now disapprove of the way President Trump is handling the coronavirus outbreak. I mentioned partisan drives a lot. When you enter President Trump into the equation, you see that play out. Look at the partisanship here in terms of his handling of the coronavirus outbreak. 88 of republicans approve how hes doing. 45 of the democrats. Only 45 of independents, sorry, only 14 of democrats approve of his handling of the outbreak. His overall job approval in this poll, john, 44 . It doesnt budge. The program is more than ten times what the Small Business administration guaranteed one year ago. Government officials had seven days to turn legislation. The bill is passed. Into an economysaving program. The whole point of this program is to keep 50 of america back at work. So that Small Businesses can reopen quickly. One that will provide loans of up to 10 million to businesses with under 500 employees. But more than that, those loans would be forgiven, essentially turned into grants if that money was used to pay salaries. Payroll expenses or business costs like mortgages and rent. The exceedingly tight timeline left many of the details too open ended for some lenders to even get online in its initial days. Multiple Bank Executives told cnn the Largest Community trade group firing off a saturday night letter noting, quote, they remain frustrated with a lack of clear instruction on how to complete loans through the sba. When guidance was provided, it came hours before the program was set to launch. The largest banks initially put limits on which customers they would serve, infuriating some Business Owners. While others repeatedly ran into notices like this. Websites going down or under maintenance due to the sheer volume of applications, all as lenders face the governments own technological issues as the system for processing loans was down for hours at a time, several sources told cnn. The rocky start something trump himself tried to look past. Its only been going for a couple of days. Its really been performing well. Couple of little glitches, minor glitches that have already been taken care of. Yet, there has been some progress in the days since the initial launch. As of today, sba has processed over 70 billion in guaranteed loans. Each day, Administration Officials, lenders and Small Businesses say the process is starting to smooth out. Lawmakers already considering adding another 250 billion to the program under scoring both the urgency and volume the crisis for Small Businesses. Also, their faith that the problems will be worked out. Even the participants say some of the current optimism. It has become so popular, its really a tribute to government, really wellrun government. Is congratulations on the success. Isnt merited just yet. Phil, stick with us if you can. We have many questions that have been raised. Also joining us is julia chatterley. Julia, youve been telling us about the glitches, if thats the word for it, for days now. Are they getting closer . What does this mean for people desperate for money . The desperation continues, alisyn. If we came back in two, three weeks and had this conversation, it would be a completely different story. This whole program is reeling. Its overwhelmed. What i hear from some of the largest lenders, theyve approved hundreds of thousands of loans to the tunes of billions of dollars. Theyre ready to hand the cash out, but they need to get approval from the fda. The Small Business administration and this has been the choke point. It was never set up to deal with this avalanche of requests that weve seen. Thats the first problem. Then to go back to the Community Banks, theyre critical. They give money to some of the smallest business this is the country. Over a third of those Community Banks still dont have access to the Small Business administration to even ask for these loans. So it is imperative that we raise the size of this, one, because obviously theres so much demand and businesses are desperate. But two, because a lot of the flow and requests that weve had, people are afraid of missing out. They fear that theyre not going to be able to get their handson any money. If they believe it comes eventually, they may make different decisions today and that may save jobs, alisyn. Thats ultimately what were trying to do here, save jobs. Some of the Small Businesses dont have two to three weeks. Some have ten days or less of cash reserves on hand right now. Thats something i know theyre trying to deal with in congress. Phil, its interesting to hear members say, 250 more billion dollars into this program, which they all support. This may happen before any other larger stimulus. It seems to be a priority right now. Why . Yeah. Theres no question it is. A couple pieces of it. One, its bipartisan. This program itself for the entire 2. 2 trillion package, was the most bipartisan aspect of this. Both republicans and democrats agreeing its needed, agreeing on how its structured and what the goals were for the program. Julia hits on a key point, too. The way the program is structured, its first come first serve. Theres a lot of concern when you talk to members hearing from their states or districts that those people that maybe have really good tax lawyers, good access to be able to find work an arounds to specific banks are getting in front of perhaps some of the smaller Business Owners that havent taken the loans out before or dont have the best connections or using the best counsel as they move through the process. They want to ensure that those individuals, those Small Businesses still have opportunities to take advantage of this program. I will say, its completely wild that with the snap of a finger both chamber of commerce are going to approve 250 billion additional dollars . But i also think that under scores what theyre hearing back home. These Small Businesses, as julia points out, are falling offer a cliff and they need that help and lawmakers are trying to give it to them. Youre so right, phil. So its been 36 hours now, just over that hes been in the intensive care unit. Were being told that he is still stable, which is good news in the sense that his condition is not deteriorating at all. Its also concerning news in that his condition is not yet improving. We know that hes conscious and reportedly comfortable. Hes still receiving oxygen treatment but hes breathing on his own. According to the british cabinet, he is still the one making some of the most important decisions. So, of course, we know he has deputized some of the daytoday affairs to the foreign secretary, dominick robb. He has no doubt that Boris Johnson will pull through all of this because he is a fighter. But it is fair to say, skron, th john, that the British Public is concerned, theyre anxious. They want him they desperately want to see boris get well. Weve seen trucks driving around the city with the big sort of signs saying get well soon, boris. People are going to be clanning, as well for the clapping as well for the Prime Minister later today. Theres a sense of the public rallying behind Boris Johnson. Underlying that, i would say theres a sense of growing anxiety about who is really in charge. Officials here saying there are some signs potentially of a plateau in the number of hospital admissions, but this is still a critical situation and everybody would like to see the countrys leader back in the job.
