Theyre warning the country to brace for disaster on the scale of september 11th or pearl harbor. Its clear that the saddest, hardest days are still ahead. Dr. Anthony fauci is warning that the outbreak is not yet contained and caution that is as many as half the people infected with coronavirus may be asymptomatic. Deaths in the country are about to surpass 10,000. The u. S. Is losing more than 1,000 people a day. Dr. Deborah birx of the Coronavirus Task force is asking people to avoid the pharmacy or the Grocery Store this week. New york is actually reporting its first drop in deaths. Governor cuomo warns its too early to tell if that will stick. Well take you inside an emergency room to witness the fight firsthand in a moment. John, President Trump continues to push for the use of the antimalaria drug hydroxychloroquine despite Scientific Evidence of the effectiveness of the drug. Well speak to the family of the youngest person to die from the virus in the state of massachusetts. His family says he was given the antimalaria treatment but his parents claim it did not help. Moon while, british Prime Minister Boris Johnson remains hospitalized for coronavirus. Cautionary measures because of the persistent symptoms that hes had for nearly two weeks. Lets begin with our coverage at the Javits Center where coronavirus patients are expected to start arriving today. Whats the situation . Reporter good morning, alisyn, were outside the Javits Center. It will open its doors to covid19 patients. This is as we begin with the federal task force saying its going to be a tough week in the fight against the coronavirus. Experts claim a difficult week ahead for the nation. This is going to be the hardest and the saddest week of most americans lives quite frankly. This will be our pearl harbor moment and 9 11 moment. Its not localized. It will be all over the country. As the numbers of cases are expected to peak in new york in the coming days. If you look at the projections of the curves or the kinetics of the curves, well continue to see an escalation. This is a moment not to go to the Grocery Store or the pharmacy but doing everything to keep your family and friends safe. More than 1100 americans were reported dead. Were struggling to get it under control. Thats the issue thats at hand right now. In new york, hospitals are bracing for the possibility of hitting the apex of covid19 cases this week. Mayor bill de blasio warns the city only has enough ventilators and medical supplies for the next few days. This as the state saw a decrease in deaths over the weekend. The number of deaths over the past few days has been dropping for the first time. What is the significance of that . Its too early to tell. In some states, like illinois and michigan, the rates for ventilators and other medical supplies have governors growing more frustrated at the federal governments lack of an early response. If they had started in february building ventilators, getting ready for this pandemic, we wouldnt have this happening. And fewer people would die. President trump expressing hope. Were starting to see light at the end of the tunnel. Before doubling down at the benefits of treating patients with an antimalaria drug. Even though theres no scientific data. Why are you promoting this drug. Im not. You talked about it may work and it may not work. But if it doesnt work, nothing lost by doing it, nothing. Because we know longterm what i want, i want to save lives. I dont want to to be in a lab as people are dying all over the globe. Later in the briefing, cnns Jeremy Diamond who you saw speaking there, trying to ask fauci if he agreed. But the ept refused to allow him to answer. Fauci made news when he said as many as half of the people infected with coronavirus may not show symptoms. Thats a much larger estimate than the cdc director gave. Fauci said that other experts differ with his opinion. Alisyn . Yes, that was a big headline. Athena. Thank you very much for all of that. Joining us is cnn medical analyst, professor of medicine and infectious diseases. And the chief executive at Hackensack Meridian Health in new jersey. Doctors, thank you to both of you from being here chld dr. Gounder, the Surgeon General is trying to tell americans to brace for themselves for what is about to happen. He likened it to two huge traumas in our national memory. Thats pearl harbor and 9 11. But you think that those are not the best analogies. Why . Well, pearl harbor and 9 11 were on the american psyche in a way that very few things have in national history. That said, some 2400 people died at pearl harbor, 3,000 at 9 11. Were already close to 10,000 deaths and counting from the coronavirus in the United States. Its on a completely different scale, unprecedented scale. Not to mention that with pearl harbor and 9 11, youre talking about attacks by other people, a Foreign Government and were somewhat less predictable. Weve been warning about a pandemic of this scale for decade, quite literally. This is not something that could not have been anticipated. We could have taken the time to prepare for it. Japanese Aircraft Carrier on november 7th instead of december 7th so that you could prepare for that month. Dr. Moraga, i want to ask you about the peak. The idea we may see the peak in new york city sometime in the next seven to ten days. Is that your predicted timeline, where you are in new jersey, and what does that mean exactly for your hospital . So, just looking at specific data from our own health system, starting the week of the 22nd of march to the 29th of march, we went from 600 case toss 1400 cases. Last week we went to 2100 cases. We havent necessarily seen a change in our trends. New york is certainly experiencing starting to see some change it looks like a little bit. Were anticipating the models, it could be anywhere between the next week or so into the middle of may. So dr. Gounder, if 9 11 is not the right analogy and pearl harbor isnt the right analogy. I think you make a compel reason theyre not. What should americans be thinking as we start this week . Well, i think americans need to be thinking that this needs to be taken very seriously. Yes, while we in new york city may be approaching a peak, this is the first city to truly be affected on this scale. Were going to see the dominos falling. City after city being affected. Unless the cities take measures before this truly hits. I think a lot of local leaders are waiting for it to hit before they take measures. The message here is that is too late. Dr. Varga, i was struck by deborah birx saying dont go to the pharmacy or Grocery Store this week or next week. Weve been in the social distancing measures, but theres always the exception. If you need to buy food or get drugs, go ahead and do so. Shes saying if you can avoid it, dont do that now. I would agree. While this may not be a pearl harbor or a 9 11 moment, still a chance for peeople to be heroes. You can do that by staying home and washing your hands. You can be a hero by checking in on someone who is alone and not have someone to support them. This is going to be a critical next couple of weeks to bend the curve. Dr. Gounder, as you know, theres a debate. I guess i think in the White House Task force about the use and efficacy of the hydroxychloroqui hydroxychloroquine. Dr. Fauci had been pumping the brakes and reminding everybody that it is not approved for use with coronavirus. The president is very bullish about it. He, i think, wants it to work for coronavirus and i think that anecdotally there are scattered cases that sound promising. Where does that leave americans who are listening to this and these briefings. Should they rush out and try to get some . Alisyn, wanting something to work does not make it so. I think we all want to see something that will work for this. But hydroxychloroquine is a medication that has very serious side effects including cardiac arrhythmias, which means an abnormal heart rhythm which can be fatal. The people at risk for the most severe cases of covid19 are precisely the most at risk for the cardiac arrhythmias. In other words, people with underlying heart and lung conditions. So thats very concerning that this is being promoted as something people should be going out and rushing to get or asking their doctors to prescribe them. Frankly, only a doctor should be making recommendations about what medications are appropriate for the treatment of this. No politics, i dont care what your strife is, should be prescribing medications to the american people. Its not a disagreement about wanting it to work. Everybody wants it to work. If its hydroxychloroquine, we can get behind that. The disagreement is how much to claim it does work before the appropriate scientific testing has been completed. Dr. Varga, i am curious, its out there. People are in some cases taking it with differing degrees of claimed success on this. I know well talk to people on this show who have used it and havent seen success. Dr. Varga, what are you seeing . Its definitely too early to tell whether theres any utility to it at all as celine was saying. That said, i would say that probably three quarters of the patients that we have, particularly in the icus in our system, we have over 500 people in icus. Almost 500 on ventilators. The majority of those folks are getting some sort recipe with hydroxychloroquine. You said that they have included you have given it to them . Its too early to tell whether its having any efficacy as all. Just saying a large percentage of our folks are being treated with hydroxychloroquine. Well, we will wait and see. Obvious obviously, if could be fast tracked, everybody would be grateful if it works. Dr. Varga and dr. Gounder, thank you both very much. As much as we heard from doctors and nurses on the frontlines, it has been rare to get a look inside these hospitals. Cnn was given exclusive access inside the emergency room at a new york city hospital. It is one of three in the state now treating coronavirus patients. And only coronavirus patients. Manuel marquez joins us with the exclusive. You saw this firsthand. Reporter look, this was at suny, theyre receiving some of the sickest patients. A quarter of the people admitted to the hospital, they end up dying. Theyve invented two e. R. S. They want the world to see what this crisis actually looks like. The frontline in the fight against coronavirus. The brooklyn emergency room of suny down state Health Sciences university. Patient after patient struggling to breathe. This morning has been brutal. Today is pretty intense. Weve had a bunch of people die in a very short period of time. Were prepared for it, but when it happens so many times in one shift, its pretty hard to tolerate. As we arrive in the e. R. , the latest victim of coronavirus at suny is being wrapped up in the emergency room bay where doctors try to save her. We visit suny down state for about three hours midday friday. In the short time we were there in the emergency room alone, six patients coded, in other words, they suffered heart or respiratory failure. Four of them died. A devastating part of just one day. This is what we train to do. Its what we signed up for. Just not in this volume. You know, you may have a code maybe on a bad shift, you may have two codes where you carry that emotion and you wonder if you did everything that you could. I think its emotionally hard to prepare for this level of sickness and suffering and morbidity and mortality in such a short period of time. I dont think any of us are wellprepared for it. Have you ever seen this e. R. Jammed like this . Not quite like these days, no. Youre not at max yet. But youre pushing it . Yes. Were pushing it. At times the emergency room here, there is no room to move. But we have a system where we decompress them out here in the hallways, the other rooms is the fast track. Not many people have been bringing in their children. So taken over part of pediatrics. Theres clinics back here we can turn into beds and they wait there once theyre stabilized from here and then go upstairs. The corridors in the e. R. Here lined with those suffering from coronavirus. Patients unresponsive struggling to breathe. It is the hard reality of this virus. For some patients it attacks the lungs, depriving them of oxygen, slowly suffocating them. With covid, the pneumonia is not one lung but in both lungs leaving the patient with no good lungs. Its also widespread with a massive inflammatory reaction thats damaging the lungs. Keeping the most critically ill patients suffering from coronavirus breathing is as simple as it gets in medicine. As coronavirus resists treatments. Confounds doctors and kills patients. Here in terms of the airway, we have to manage their airway once they become so altered from the lack of oxygen that theyre unable to keep it open themselves. What is the mortality rate . Once they go on a vent, what happens to mortality . It increases exponentially. More patients dying, in other words. They do. The numbers are not exactly the same from country to country. There are various factors for that. We all agree it skyrockets. Showing some fluid in the lungs. Dr. Lorenzo palladino has done groundbreaking work in putting more than one person on a single ventilator. Its research he hopes never has to be used here. What keeps us up at night is what motivates us to do the research here at suny down state. Its one alternative to give to the patients. Not just the ventilators, but for cpap and isolation and high flow. We try all sorts of ma jufrs to keep them breathing and suffocating from having a cardiac arrest. Its not just in the emergency room where patients struggle to breathe and code. While interviewing doctors in other parts of the hospital, nearly constant overhead announcements that another patient has coded. Those announcements for patients already admitted, not those in the e. R. Can i stop you for a second . This is the fifth or sixth code 99. Code 99 is typically a rare event. Were having, i would say, ten code 99s every 12 hours at least. Weve been here for about 30, 40 minutes and thats the fifth or sixth one. A lot of that, what that represents is calling for a team to put an individual, a patient on a breathing machine. What is most jarring about seeing the inside of an e. R. In hospitals making the transition to being one of three in new york state that will only treat patients suffering from coronavirus, outside it is quiet and feels like an early spring day. Its slowmoving and its damn boring for a lot of people. But this is a disaster. This is definitely a disaster. Its difficult for people from the general public who dont work in the public. When you drive down the street, youre driving down new york avenue and nostrand avenue, theyre busy thoroughfares, its almost crickets. Its an intensity you only see in disaster zone that have been televised around the world. Reporter heres another disturbing data point about what suny down state is expecting in the days ahead. The regular morgue is overwhelmed. They have two semi tractortrailer trucks. Theyre talking about adding shelves to that. Right now all of the bodies are just on the base of the truck. Theyre also talking about shutting down streets around the hospital and bringing in three more trailers. John . Miguel, i have to say what a remarkable look inside that hospital. Thank you first of all for showing us whats going on there and risking your own wellbeing and that of your crew. I saw you dressed up in that protective gear. What were you wearing . We had the proper gear unlike brookdale earlier in the week. We had the tyvek suit, the jumpsuit. Andy buck went into suny down state. We had proper gear. N95 masks, the goggles. But pro tech, the gloves, the whole bit. They have that now at suny down state. They have it for today. Theyre not sure theyll have it next week. The code 99s you kept on hearing, its a constant death or near death . Whats disturbing, we heard it for areas outside of the e. R. Inside the e. R. Its always an emergent situation. They dont call those codes out in the e. R. They had six people code in the e. R. , four died there. All the codes were happening elsewhere in hospital. Its terrifying. Cnn took you inside a brooklyn hospital, one of three in the state dedicated to dealing with coronavirus. Joining me now is one of the Emergency Physicians at suny down state Medical Center featured on the cnn special, inside the e. R. Dr. Rob gore. Hes also the founder of the kings against Violence Initiative and was honored as a cnn hero in 2018. Dr. Gore, we really appreciate the work youre doing and thank you for being with us. Tell us how youre holding up. Thanks for having me this morning. Im okay right now. I got a little bit of sleep this past night. But its tough. Every day you come in, you wonder is it going to be the same situation. I know yesterday or the other day when the team from cnn was in the Emergency Department, you kept hearing code 99, code 99, code 99. That same was present last night on my shift. Maybe three or four of those within the span of 30 minutes. But it is scary knowing whats going on upstairs and around the Emergency Department and wondering if its going to happen to you. Code 99, just to our viewers who werent watching means that a patient likely needs to be intubated immediately and it is at extreme, extreme risk. I understand, dr. Gore, youre staying at an airbnb to keep your family safe. How long will you be separated. Ive been here, this is the third week staying in the same place and im probably going to be here for at least another four, maybe six weeks. Me and one of the other Emergency Department physicians both are trying to keep our families safe. Theres so much we dont know about the disease process itself and the last thing we want to do is bring it home to our loved ones. Many people arent symptomatic carriers. Were around so many patients who are suspected of having covid19 or do have covid19 and the last thing we want to do is affect your loved ones. I hope that sinks in for people. Youre willingly separating yourself from your family for up to six, eight weeks maybe so you can do your job to save lives. Youve been in disaster zones all over the world, including haiti and other places. You say this is a culmination of all that work. How so . I say that because its the level of intensity that happens taking care of sick people. Its a whole different kind of intensity when everybody is fearful of the same thing. There was a disaster everyone was afraid not just of what they were experiencing. They were wondering was it going to come back. Thats the same thing at coronavirus. The only difference in this case is that coronavirus is almost a silent killer. In haiti, you saw the rubble, you saw the buildings that collapsed, you saw the cars overturned. But with coronavirus, you dont necessarily see that. The streets look clean and calm. Its almost as if every day is like a sunday morning. But when you go in the Emergency Departments, you see all the people being impacted and youre standing outside of the e. R. S in the waiting rooms, you see some of the family members waiting to get word back from their loved ones. Only thing is, they cant come in there. You talk through facetime or skype to let the loved ones talk to each other. Have you been using hydroxychloroquine with your patients and how has it been working for you . I used it last night for the first time. Its been some of the internal medicine docs on the floors are starting to recommend that. But its too early for me to determine if theres going to be major Outcomes Using it. Like with every drug, every time you use it, there can be consequences. With hydroxychloroquine, you can get nausea, vomiting, hair loss, also even cardiac dysrhythmias or unusual heart rhythms. You were a cnn hero for the work to battle violence. Kings against Violence Initiative. Are you able to do that work at the same time . One of the things were dealing with is so many nonprofits, causes are being neglected because of the need to focus on covid19. Can you do both at once . So i have an Incredible Team that allows me to do both of the work. One thing that we know, any kind of disaster situation brings up triggers and traumas. Even though we dont see the same rates of violence right now coming into the Emergency Departments, it doesnt mean people still arent suffering. A lot of people have acute distress disorder, ptsd, theyve been dealing with tragedies. Our team is still reaching out to them, both within the hospitalbased program, as well as our schoolbased initiatives. Dr. Rob gore, you prove again and again, you are a true hero. Thomas hospital where Prime Minister Boris Johnson is likely to be the downing street and the hospital are not confirming that yet. This is just five minutes away from 10 downing street at the exit we can see increased we see photographers waiting presumably for the possibility that the Prime Minister may leave at some point today. But so far, alisyn, we just dont know when that could be. All weve been told by downing street is that the Prime Minister was admitted here last night for precautionary tests because of, quote, persistent symptoms ten days after contracting the coronavirus. As you probably know, alisyn, when you continue to have those symptoms for ten days, thats when it starts to get worrying. Were being told that there is no cause to worry yet. That the Prime Minister is still in charge and able to essentially run the countrys business from the hospital here. Of course, many are concerned not just for the possibility that the Prime Minister may be making himself even more sick by continuing to try to go about government business while fighting this vicious, vicious virus, but also concerned potentially that he might not be able to bring all the normal energy that he would in ordinary circumstances. So people waiting and watching to see when and if the Prime Minister will leave the hospital today and when we might learn more about his condition. Alisyn . Okay. You see him with friends and siblings and he looks great. So have they given you any clue as to why this virus hit him so hard, though he was only 31 years old . Did he have underlying Health Conditions . Riley had kind of got bronchitis quite a bit. Never on asthmatic drugs. Even as a child, i mean, he was the bronchial guy, you know. Of course, he was always large. He was big boned. Big kid. But generally healthy. He wasnt really in bad shape, although when he went to the hospital in boston, of course, he had the fever and the coughing and shortness of breath. But i think that was symptomatic of the virus, not necessarily his Current Health issues. I mean, i think his health was okay. He was generally healthy, yes. Yes. So on saturday, march 21st, he went to boston Medical Center with 104 degree fever. As you said, shortness of breath. I know that you said that you believe that the doctors did try the antimalaria treatment on him. Did you mean that hydroxychloroquine . Do you know if he got that . I understand from my son who was bedside with him, he also lives in boston, told us that they were giving him that drug. We have we didnt know. Of course, the world didnt know much about whats going on. We have people in california, my other son called out to california and asked what they were doing out there. They agreed, theyre doing the malaria drug. He was given the malaria drug, some type of antibiotic with it i think. He goes in sunday morning, 2 00 or 3 00 in the morning. By tuesday, 10 00, 11 00 maybe, because i talked to him about 9 00 just texting. He was admitted, sedated and we never got to talk to him again. Tuesday all the way through the following sunday morning at 2 00 a. M. Is when he actually passed away. Gosh. Were so sorry. Were your other kids, sons able to give him messages from you . Yeah. You heard the cell phone, the iphone. They actually dressed my oldest son up and he went into the room. He looked like an astronaut, but he went into the room and he was able to talk to riley. Riley couldnt talk back. We were also able to talk through the iphone so riley could hear our voices and hoping that hearing us tell him to fight that maybe he could beat this thing. He was still doing good all through saturday. Thats when the turn of events got really dramatic. This was huge. To christines point, the banks by their own admission werent ready for prime time. There are all sorts of differences between the sorts of loans that the banks themselves could give and the then the protesters saying giving that information to the Small Business association that guarantees them. Last year the Small Business Association Gave out about 58,000 loans. Bank of america from the date of on friday alone processed 99,000 loans or more to the tune of 25 billion. The problem is giving all that information to the Small Business association. Now, the good news on that front from what im hearing, apparently Amazon Web Services has been working with them, Small Business association, since last week to try and build out the technology and the infrastructure and theyre saying that they believe the money will start flowing by wednesday and thursday this week. But the infrastructure here is being built out to try and allow some of the loans to be processed. Thats critical here. It will have to be more bailouts for Small Business and maybe more direct payments to consumers. They havent received their check yet, their 1200 check yet. The money hasnt gone out the door. This is to get through weeks and mornts, julia. I dont know if people are digesting the time frame were talking about. I dont think they are. There is recognition that theyre trying to do their best to get money out to people. Whether its checks, were talking weeks, it seems. The functionality problems with the direct deposit. The Labor Department as far as this extra 600 a week in Unemployment Benefits is concerned. That money will be available to the individual states as of this week. But then the warning comes again that it could be up to two or four weeks before the states then have the infrastructure in place to start giving that money out to people.
