Its monday, march 30th, 8 00 in the east. The United States shut down through at least april. The president extended the social distancing guidelines through the end of next month, which is a change from his earlier suggestion or as he might suggest aspiration of easter. The president received a sobering presentation from his Top Health Officials and conceded that the u. S. Death toll could reach 100,000. This is what dr. Anthony fauci told us just moments ago on new day. And we felt if we prematurely pulled back, we would only form an acceleration or a rebound of something, which would have put you behind where you were before, and thats the reason why we argued strongly with the president that he not withdraw those guidelines after 15 days, but that he extend them, and he did listen. I would not be surprised i dont want to see it, id like to avoid it, but i wouldnt be surprised if we saw 100,000 deaths. The death toll in the United States is now nearly 2,500. Were told the peaknationwide b dr. Fauci warns that millions of americans could become infected. John, the death toll in new york alone is nearing 1,000 people. New york city mayor bill de blasio says hospitals could run out of supplies this week. They will run out of ventilators, he says, even sooner. Health care workers are reporting that Emergency Rooms resemble war zones, with shortages of protective gear, and overcrowding. Mayor bill de blasio will join us with a status report, just minutes from now. The Naval Hospital ship comfort arrives in new york today t will treat noncoronavirus patients to free up space in hospitals. Field hospitals have been set up in the huge Javits Convention center and in central park and governors across the country are now bracing for a similar crisis. Joining us now is dr. Sanjay gupta, cnn chief medical correspondent and dr. Lena nguyen, Emergency Room Physician and former Baltimore City health commissioner. Great to see both of you. Dr. Nguyen, give us a status report of whats happening in Emergency Rooms in baltimore right now. Well, here in baltimore in the d. C. virginia region, we are waiting for the storm. This is still the relative calm before the storm but we know that storm is not far away. We hear and see the reports of our colleagues in new york. Were seeing whats coming their way in michigan, louisiana and we know that our met poll tar area juice likes areas across the country are not that far behind and this is why. These are things that we were predicting for a while. We know we urgently need supplies. We need personal protective equipment and we need ventilators. We know what numbers we need and we really need the federal government to be pulling out all the stops now, because of now is not the time to be delivering us supplies we need. What are we waiting for . Are we really waiting for things to become so bad that they resemble new york and it becomes a true emergency or should we be heeding the words and following the preparation that is already happening here . Sanjay, at the moment, this morning, u. S. Deaths are close to 2,500, and President Trump said yesterday that he believes the peak of death rates is likely to hit two weeks from now. Does that comport with the models that youre seeing, too . Well, you know, the country is sort of hard to sort of describe in one fell swoop because so many different places around the country reacted to this differently. In new york, for example, if you look at some of the modeling and again this is modeling. Some would say this is conservative models, it is around the middle of april where youd see the peak and sadly, alisyn, around that time the projections in new york around 2,000 deaths a day around midapril, so obviously pretty significant, pretty sad. Florida they think the peak could be sort of middle of may, so in miami potentially being one of these hot spots as theyre being called. You obviously have new orleans, detroit, you have florida and georgia overall, which have been increasing significantly over the past several days, so its a little bit hard to project, but yes, thats around the time period, but i think alisyn, well see what happens over the month of april. These projections of 100,000 to 200,000 deaths dr. Fauci was given was if the plans stay in place, if we do everything that were still doing and dont pull back, we still keep the stayathome policies in place. Hopefully people are reminded of that. Theres a lot of people who have been at home the last two weeks and done a really good job. If they had no symptoms, because the incubation is typically two weeks, theyre starting to get around the point where they think im free and clear. Dont let the gas off the pedal yet. Theres still some work to be done to keep these death rates as low as possible. And sanjay, on all of that, just one more point, and that is dr. Fauci said when he showed the president this weekend the models that 200,000 people in america could die in the next month, that that was what really got the president s attention, and made him realize that easter was not a realistic date, and that the u. S. Has to be shut down through at least april. Thats right, alisyn, but one thing, alisyn, as we follow the story along now for close to three months, i think one thing has become clear is that people like dr. Fauci and others, they are continuously looking at the same data that were looking at, that we all have access to now and we have been able to see that not only have the numbers gone up, but the pace at which these numbers have gone up has also increased, and my point is that theyve known this for some time. I think theyve been telling this to the president for some time. I think he knew some of these numbers even before he talked about easter, and some ways i think this is sort of maybe trying to ease the country into accepting this, so instead of saying right at the beginning, were going to be shut down until the end of april, theyre saying lets do this every couple of weeks and sort of see how it goes. Now its going to be an additional month at the end of april. Could it be a little bit longer . Perhaps, alisyn, thats one thing obviously well have to wait and see. If you look at other countries around the world, it has been more of a tenweek, eight to tenweek sort of time frame and we started later, so it may be a little bit longer for us, because of that. We dont know. But its not surprising to me at all, and i think probably to dr. Wen and a lot of other people that this had to be extended. You knew this i think at least almost since the 15day pause sort of started. Dr. Wen, we just have a bit of a breaking headline here, dr. Deborah birx of course very visible on the president s task force gone on television said the u. S. Implemented social distancing too late. I know that wont come as a surprise to you but to hear her say it, what does that tell you . It tells us that she is following the science, following what Public Health experts have been saying all along. Ive certainly heard sanjay say this for weeks now, that social distancing, we know from the research to be the most effective has to be implemented early, and consistently. That its not something that we can do piecemeal. We know from past pandemics, past outbreaks that the earlier these restrictions could be put into place, the more effective that theyre going to be. And also that everybody has to take them seriously that, we cant treat this as, well, school is out but we can still see our friends or were not going to work but were still going to hang out. This has to be something that we really understand as our best way to reduce the transmission person to person, and im glad that dr. Birx is acknowledging it. Now we need the entire country to do even more than we have. So many people have made substantial sacrifices in this time to change their daily behavior at great risk to themselves, or great sacrifice to their families. We need to honor that sacrifice and continue to do our part and as sanjay says to do this early, consistently and honestly. Dr. Wen, one more question for you and what youre hearing from your fellow emergency room colleagues. President trump said yesterday that he felt there must be some sort of hoarding or worse, he actually said, because he couldnt understand the amount of equipment that is needed in hospitals in terms of its masks, in terms of the ventilators. Ive had doctor sources texting me this morning saying they found those statements bizarre. What do you believe is it possible that theres hoarding going on . No. We can look at the federal governments own projections about how much equipment is needed during a yearlong pandemic and they estimate something on the order of 3. 5 billion masks are needed. Were looking at the numbers of patients coming in, and question easily see how much equipment is needed, and not only that, but its not just the equipment that were using right now, we also have to be prepared for the equipment that were going to be needing in the months to come, because this is not a onetime event. Its not as if we get through today and were done. We need to be preparing for months or years, and frankly, its insulting to suggest that hospitals are somehow keeping equipment away from Health Care Workers, and jeopardizing their lives. Actually all of us should be doing everything we can to supply the basic equipment that Front Line Health Care workers need to protect themselves and protect all of us. Sanjay, youve been educating us on how many people need these masks. They are the people who draw the blood, the technicians, the nurses, the doctors, the respiratorier this piy er thith anesthesiologists. New york presbyterian goes through roughly 50,000, thats their estimate, a day. These numbers are hard to get your head around. I mean look, theres Hospital Systems that have gone through their entire sort of personnel protective equipment if stores for the season, theyve gone through this in a week. So you have to treat every patient that comes in, even if theyre not coming in with specific coronavirus covidlike symptoms. You have to treat them like they might have it especially in communities where theis circulatin circulating. Every time patients are seen by the doctors, medical students, residents, therapists, nurses, whatever, they need to wear protective gear. Youll go through a lot of protective gear as a result. I talk to my colleagues all the time, i know leana does as well. The idea theyre being told theyre hoarding this stuff is a little insulting. They are showing up to work not sure theyll have the personal protective gear, theyre risking their lives and im not exaggerating when i say that and then theyre going home and potentially exposing their families to this. I mean, we owe them a huge debt of gratitude and we have to take them at their word in terms of what theyre saying. To suggest theyre hoarding it, stealing it, taking out the back door, i think that was the rest of that quote, i think that is insulting to these workers who are risking their lives. Dr. Gupta, dr. Com cnn learned the u. S. Navy is preparing to send a 450person medical team to texas and new orleans to help the coronavirus outbreak there. Louisianas governor says hospitals in new orleans could run out of ventilators by this week. That state has more than 3,500 coronavirus cases, and 151 deaths. Cnns ed lavandera is live in new orleans for us with more. Whats it like on the ground there, ed . Reporter good morning, alisyn. Here this is a significant week in the days ahead will be crucial, according to the governor and medical experts here who continue to see the cases of coronavirus here jumping by the hundreds every day. As you mentioned now 3,540 cases across the state, almost 1,400 of those are here in new orleans alone, and that is putting great strain on the medical system throughout the city and the state. There is a makeshift hospital that is being set up at the convention center, that should have, were told, a thousand beds by this coming weekend, and the governor says by this weekend, there will be an incredible lack of resources on the ventilator front. The governor requested some 12,000 ventilators to date so far. Theyve only received 192, and according to the governor, none of those have come from the national stockpile, and that the state continues to request those ventilators, but here, theres also a great deal of concern, just about how people in this state and new orleans in particular are behaving. They say state Officials Say that not enough social distancing is happening, in fact an arrest warrant had to be issued over the weekend for one man who led a second line parade. What is what the mayor had to say about all of this yesterday. We will have to enforce the laws every step of the way. We need law and order. And superintendent ferguson the eninjury new Orleans Department here will take action as were called to do if we find those who consistent ly violate the law. Reporter the message here is clear as this week continues, more social distancing and people here in the city and the state need to take it a lot more seriously. John . All right, ed, ed lavandera in new orleans. Thanks so much for that report. Joining us, dr. Robert hart, chief medical officer of oxnard health in new orleans. Dr. Hart, thank you for being with us. You heard eds report, you heard governor John Bell Edwards say the state could run out of ventilators by this weekend. What are you seeing . Well, it is concerning to hear that type of a discussion, and i know we have been struggling and working hard to get ventilators in. We have been working with our supply chain constantly, as weve continued to add icu beds. The state has been able to help us get several vents, which have been very useful to us. Were able to use those in different ways. Some of the vents are the full best type made that we normally use. Some are transport vents. They do Different Things. Different vents do Different Things for us, and so getting creative about how we use the vents on Different Levels of severity of illness for our patients. I think id like to talk about the patients, if we can. I think sometimes in something this big we lose sight of how theyre actually presenting and what they are suffering. So what are you seeing in terms of people . What types of people are showing up . What are the symptoms in what a are their needs . The symptoms center around respiratory illness. Keep in mind, we are probably admitting at least 50 patients a day with the covid or covid ruleout type symptoms. So you can imagine as you do this day after day, the capacit. We had to add over 100 icu beds, anticipate having to add more. Many of the patients that do seek care wind up having to be intubated once they come in. Some are lucky enough to come in and go home, but these patients get very sick and frequently get sick very quickly. One of the things that were increasingly concerned about around the country is its not just older people. Initially weve been told its people over 65 or people with underlying conditions. But a lot of hospitals are telling us around the country theyre seeing people in their 40s show up and not necessarily underlying conditions. What are you seeing there . Right, we actually here in new orleans have begun to see almost a bimodal type spike. We see it in the olders like you said but around 40 there seems to be a little bit of a spike as well. And so those patients come in. We have to be concerned about them getting just as sick as the others. So we are seeing all ages. Certainly the preponderance of those that are older but it is certainly affecting younger patients. The idea theyre safe is not true. And were trying to get that message out. Talk to me about your supplies, masks, gowns, everything you need. How well are you set up at this point for the peak, whenever that may come and when do you think it might come . So we certainly know we have probably another couple of weeks leading up to the peak. This has been a strain on all of our resources throughout the city, throughout the country, as youve mentioned several times. Right now, were fighting all the time to get the ppe we need, to get the testing we need, to get the beds, to get the staff. The biggest thing is the staff and ppe. Weve got to have, again, i said were adding 50 patients a day into our hospitals, and many of those are critical care, and we are trying every day to get more staff in here, to help us take care of these patients. They get very sick. One of the things weve heard from the front lines is, remember, its not just the equipment. Its the people wearing the equipment, that we have to be most concerned about, their wellbeing, and keeping them there and the numbers they need to be there. So again, what will you need over the next two weeks in terms of medical personnel, and do you feel confident that you have it . Well, right now, i feel confident that we are working hard to get those medical personnel here. We have a lot of people working to bring in icu staff. It is going to be a stretch, as you heard from the governor. We know that all of our facilities here are going to be stretched, trying to find the staff to take care of these patients, and over the next two weeks, we anticipate those numbers will continue to grow. Weve got to find a way to offload our hospitals, once these people get well, and i do want you to know, many of these people are getting well. We are being able to extubate people, take the breathing tube out, were seeing improvement but this is difficult for our nurses taking care of these patients that are ill for so long. On thursday, march 5th, Heather Bauer went to a casual dinner party in her town of westbrook, connecticut. Four days later she felt achy and feverish, a temperature of 101. Five days after that she was in the coronavirus ward at yale hospital fearing that she would die. But heather survived, and she joins us now to tell us the rest of her story. Heather, it is great to see you. Were so happy that youre feeling better. Your story is incredible and you look great but i no he that that belies what you just lived through, and so lets just start at the beginning, heather, and just tell us, this dinner party that you went to on a thursday night in westport, now when you look back at that event, because you got sick and so many people from that dinner got sick, and theres so much question about social distancing, was there anything unusual or anything that should have been done differently about that party . No, it was just a simple party, you know, wasnt that many people there, it was nothing out of the ordinary, and there was multiple parties after that, by the way, with hundreds and hundreds of people on thursday and then on friday and saturday. Nobody was social distancing. Any party i went to, by the way, people were not hugging and were already thinking about this a little bit, but you know, there were maybe 50 people at the party that i was at. And you know its such a cautionary tale for everybody, because if it can happen there, it can happen everywhere. Lets talk about what your symptoms were. So after that, you are as weve said a very healthy woman. Youre a Long Distance runner, a marathon runner. So a few days later, just describe what your symptoms were, over the course of the next week. Right, so it started with a fever, and i thought, you know, maybe im getting the flu, even though i had a flu shot, and you know, i didnt know that anybody else of the people i had been with were not feeling well, so i thought maybe i had the flu. I took some advil, and the fever went away, because for the first few days when you get sick, it seems to come the fever at night and in the morning it goes away, and then in the morning it was gone and the fever came back, but then it was this relentless fever that just continued, and i did have my chest but i didnt realize it was shortness of breath. It seemed i was breathing differently. I kept my breath very short breath just to not get that pain in my chest bone. Very shallow. So you were having like a shallowness of breath, and then yes. It got progressively worse. Your fever didnt go away, the aches and intestinal symptoms. Were seeing a picture of you looking desperate in the hospital right now, looking so sick. What was the point at which you realized you had to go to the hospital . I mean, i was very fortunate because i was giving the number to yale covid, they had a hotline and they had Nurse Practitioners that were calling and checking in on me, and every day, actually, so they had actually they were the ones that recommended i got tested, because of the exposure and because of my fever. The test results took five days, which is just unbelievable, and every day they checked in, and when one of the Nurse Practitioners called me, she didnt like the way i sounded and she had a doctor come on the phone, and they asked me to take a breath, and when i did, i coughed very quickly, and so they recommended that i come to the hospital right away. But they they were like do you have a sore throat, a runny nose . I didnt have those symptoms. So, sorry, what was it . So you spent nine days in the hospital. Yes. And those pictures of you look horrible. I mean, frankly, you look so sick. Were those moments you thought you would survive this . Yes, it kept getting worse. To be vomiting in your bed in a pan is a horrible feeling. I had migraines. You lose your taste. You lose your smell. I ended up developing hepatitis. There was a day where they told me they may have to do a spinal tap. They were concerned about me developing meningitis. Actually, that was one week ago today. It was terrifying, and you just dont know if youre going to be that person, and im 42. Im a nutritionist. Ive dedicated my entire life to being healthy. Ive run 15 marathons, and so i was thinking im going to be that one. Im going to be that one that this is taking down, and i just felt like i was completely out of it was taking over my body. I had a rash all over my body, like every area of my body, this disease had taken over. And i wasnt getting better. And i was fortunate enough to be, i was at yale, i was part of their trial, i was placed on the hydroxychloroqui hydroxychloroquine. Thats interesting. You were able to get that sort of experimental drug, and do you think that that changed the course of your recovery . I do, because i had a fiveday treatment and i believe that was when it started to turn for me. You know, that was when its possible the fever stopped and its interesting the fever stopped, the migraines started and im not someone who gets migraines, and they can only give you a certain amount, theyre not giving advil because advil can aggravate it. Its hard. Heather, we only have ten seconds left. I know you want to thank your doctors and one interesting thing for people to know you were alone for most of the time in the hospital room because they would appear via screen. Theyre trying to protect themselves. So sometimes you would only deal with people in this kind of futuristic way but in the final ten seconds, what do you want us to know about your care there . My care was amazing. My nurses, i had the most amazing, amazing nurses and im so grateful to the care from my nurses and my doctors, they put themselves on the line, and thank you so much to yale for your amazing care and for saving my life, truly. Im forever grateful. Heather, thank you. And for getting me through this and the country. Thats a great message and thank you for sharing your story. So weve been asking to you send us your questions about coronavirus. You have been sending some great ones and cnns chief medical correspondent dr. Sanjay gupta, he has been providing answers, so sanjay, lets jump right in. Whats the time frame for the potential 100,000 deaths in the United States . Is that within the next 12 months or could it be even less . Well, sadly, i think all the projections show that time frame to be less, sometime between april and august, which is still a wide time frame, john, but these are all models and you know, you heard dr. Fauci talk about it this morning with your interview with him some 100,000, 200,000 deaths, so its also a range of deaths and a range of time frame. Keep in mind, john, thats also with all these mechanisms staying in place, at least until the end of april, staying home from work, schools, no nonessential travel, all that sort of stuff. Is it just us . Is alisyn with us right now . I dont see alisyn so ill jump into the next question. This is from michael, can we truly say that the United States is in a flattened curve state right now . No. We are not in a flattened curve state right now. There will be some indication when we get to that point. Two things to look out for. Youre going to continue to see numbers, again, sadly, going up in terms of numbers of people who are infected and numbers of people who have died. What youre really looking for is how are those numbers changing day to day, the trend, the doubling time. Theres all these Different Things people will apply to it to try and figure that out. The flattening of the curve, the numbers may still be going up at that point but the pace at which theyre going up starts to slow down and eventually you get to that apex, that peak and hopefully start to come down after that. All right, this one is from rob in kentucky who writes are any Antibody Tests being researched in order to determine if someone has had coronavirus in the past . Yes, this is a big deal, and the answer is yes, and in some krisz arou countries around the world theyre doing Antibody Test or sirology tests, youll hear both terms. Important for two reasons. One is something that you guys have been talking about on new day quite a bit with peter hotez, the idea of convalescent serum. Simply put, someone had the infection, they recover. Their body has these antibodies which can fight off the infection. If you take their plasma, their blood, and give it to somebody else who is dealing with the infection, you might be able to help that person. Very simple idea. Its been used for other inn x infections called convalescent serum. And to sort of get a better idea of how many people have been impacted by this infection and how many people are likely to already be immunized against this infection as a result. Here comes one from kerry in rochester, new york. I know so many people are asking this. How many patients who end up on a ventilator end up dying, and what is the typical length of time on a ventilator for those that recover and those that dont . Yes, well, keri, ther can ke tough situation from someone ends up on a ventilator. Statistically their chances of surviving and recovering are still higher than not. The time course is really interesting. Typically for most things on average, if you end up on a ventilator for something other than coronavirus, its typically three to four days. Here some of the data theyre seeing out of new york, it could be 11 to 20 days so much longer on the ventilator. People who are critically ill on the ventilator. Overall mortality rates overall fatality rates still 1 to 2 , obviously much higher if you end up on the ventilator but still then, you know, in the 20 to 30 range. Youre still more likely to survive. It will be a tough road, no question, but still more likely to survive than not. The fact the navy is here, the military is here to help new york city in our hour of need, that is amazingly important to this city. This is like an additional hospital, just floated right up to our shores, and now its going to help to save lives and alisyn, also for the people in the city have gone through so much the last two weeks. What a morale boost, what a shot in the arm to be able to see our military here to help us in such a powerful way. Its very poignant and moving for all of us. We need the help, let me tell you that. I agree. It sort of chokes you up, watching this ship, you know, slowly make its way here to new york. Meanwhile, mayor, i want to ask you about something that you said this weekend that got so much attention. It was a warning, and you said basically that new york only has supplies through this coming sunday. What does that mean . Then what happens . A. M. Slisyn, right now we ha help the people in this country understand whats happening in new york is going to happen elsewhere as well. They get supplies, they go out to the hospitals used with a huge increase in cases daily that our Health Care Workers, doctors, nurses are dealing with more and more every day. We send out the ventilators immediately, people need them. D and they need another one and another one, because all the work theyre doing. We set a deadline if we dont get reinforced particularly with ventilators and more medical personnel, after this coming sunday, were really going to have a problem ensuring that everyone who needs care gets it. Look, im really troubled way in, alisyn. I find that insulting to our Health Care Workers. I found it just incredibly insensitive. The stories of our doctors, our nurses, all our Front Line Health Care workers are going through hell right now. They are seeing people die around them all the time, no matter their best efforts. Theyre saving lives but theyre also losing lives that they did everything they possibly could, and couldnt save, and what theyre asking for is to have ive been saying this is a war and the ventilators are ammunition. Theyre just asking for the ammunition to fight with, and ive explained this directly to the president , that when someone needs a ventilator, its literally a life and death moment. A doctor cannot wait. You cant say hey, doctor, could you hold on an hour until that person gets a ventilator . No, they need it right then. If a patient doesnt get a ventilator right then, they literally can suffocate. So ive tried to explain this to the president. This is an entirely different reality than this country has faced before. This is battlefield medicine, huge number of cases, life and death decisions, all the time. We need what we need to keep going and what the president should be doing is praising our Health Care Workers, not suggesting somehow theyre doing something wrong with the supplies that have been sent. Its insensitive and unhelpful. What he should focus on as commander in chief is get us the support we need right now. That should be the only thing hes thinking about. And will you be getting more ventilators this week . I have asked the president , asked the federal government to get us 400 right away so we can even get through to sunday and then were going to need more after that. Im going to constantly update the public about exactly where we stand, but i need everyone in washington starting with the president to understand, weve given them fair warning about the situation in the nations largest city. We are the epicenter of this crisis, one in every four cases in america, right here, and theyve had warning and theyve had time and they have to get it, they have to make sure every company that can produce one is producing them. They have to make sure the military is getting them here. It is within the president s power to do that. Im trying to work with him, but im being very clear, we need a lot of help by sunday just to get through next week. Ive talked to the military about the need for more military personnel, the doctors and nurses that the military have to immediately provide relief for our Health Care Workers who are just extremely overwhelmed and stressed right now everything theyre dealing with, and i believe the military is sincere and the fact the comfort is arriving gives us real hope but this going to be daytoday, weektoweek. I need results constantly to be able to make sure we can save the lives that can be saved in this city. Ive gotten no new assurances. Ill keep demanding them and put down a marker, sunday is dday.
