Trump and his top Health Advisers are at odds this morning over how long to keep the nation at home and to keep the economy frozen. President trump says he wants to reopen the country in weeks, not months. And that directly contradicts the concerns of the World Health Organization which warns the coronavirus pandemic is accelerating. A senior white house official tells cnn that if it were up to dr. Anthony fauci there would be no contact between humans until july. A Senior Health officials also tells cnn that public Health Officials are looking for ways for an orderly return to life, but they believe we could be in this precarious Health Situation for the next 12 to 18 months, john, despite whether or not were all getting restless. Thats right. The president s reversal on social distancing came on the deadliest day of the outbreak so far in the United States. More that 100 people died yesterday. In total, 542 people have died in the u. S. With more than 43,000 cases. Americans in 16 states and i stay at home order by midnight tonight, thats mo43 of the population. Army troops scheduled to ariv in new york city and seattle in the next few days. This morning, new york city is the area of greatest concern. Officials say the rate of people getting sick here is five times more severe than any other city. There was a spike of 3,000 cases yesterday alone. It was called the attack rate. Across the country governors and mayors are running out of times and patients, scrambling to fill gaps with medical equipment. To get by, medical workers are being asked to reuse supplies. One doctor says Health Care Workers feel like lambs being sent to a slaughterhouse. We want to bring our coverage with brynn who is live in new york city where we are getting the sense from officials that things here are bad and getting worse now at an alarming rate. Reporter yeah. Well, every 2 1 2 days here in new york the number of cases double. Thats the situation were facing here. I can tell you its 6 00 in the morning and right now theres about six people standing in line trying to get some sort of care, trying to get tested. New hospitals are being built. The governor is asking that all hospitals increase their capacity by 50 . This all coming as the president is pushing to restart the economy in the next couple of weeks, even against the warnings of his own Health Officials. New york citys now the epicenter of the coronavirus pandemic with more than 13,000 cases identified here. No one wants that distinction, not a single one of us, but it is true and thats why we so desperately need help, particularly from our federal government to get through it. Reporter the White House Coronavirus task force revealing the virus has an attack rate of close to 1 in a thousand in the new york metro area, and 28 of those tests are positive. This is the group that needs to absolutely social distance and selfisolate at this time. Reporter despite the climbing numbers in new york, President Trump still suggesting hes eager to end the virtual nationwide shutdown as soon as next week. Contradict his own health experts. Our country wasnt built to be shut down. This was not a country that was built for this. America will again and soon be open for business very soon. Reporter and countering many officials leading in states battling exponentially rising cases. We believe that social distancing, thats how you keep the pressure off the health care system. Reporter new York Governor Andrew Cuomo visited this center in manhattan where the National Guard is looking to transform it into a makeshift hospital. We can scramble and create beds. Well have a staff problem because staff are getting sick. Reporter even with the federal government stepping in to supply crucial medical equipment like masks and gloves, Officials Say thats just a temporary solution. Our National Stockpile allotment covers one shift at a hospital. Reporter across the country, businesses continue to close, and even parks and playgrounds are shut temporarily, all efforts to help flatten the curve. I think its necessary. I think an ounce of prevention is worth a pound of cure. Reporter over 142 million americans in at least 16 states will be under stay at home orders by wednesday. Its time to hunker down in order to win this fight. Reporter here in new york, the governor says he has his own task force to get people back to work, but it wont be at the cost of Public Health. A little bit of hope this morning, john. Were hearing that that drug that was fast tracked by the fda that was used to treat malaria, that will be arriving in new York Hospitals today and that drug their pier will begin immediately, john. Thats a good thing because dr. This was just before midnight that this happened and it was after day four, no deal on this 2 trillion economic stimulus package. But you have 15 hours of meetings that happened yesterday, you have back and forth offers, counteroffers, outrage spilling on the senate floor. The major players, chuck schumer, mitch mcconnell, the white house political director and treasury secretary Steve Mnuchin all meeting behind closed doors just trying to work through some of those details. Mnuchin saying late in the day that he had called President Trump ten times yesterday to see is this a bill that he would actually sign. He said that trump was happy with this, that they were very close and that mnuchin would come back here on capitol hill 9 30 this morning. Well see how all of this plays out. One of those Sticking Points, john, as you know, the 500 billion that democrats are calling a slush fund, this would be in the form of loans to businesses, but very little oversight. President trump saying that he would be the oversight. So that is a big problem. They said that theyre going to try to work through that. Republicans also accusing democrats of adding pet projects, things that have nothing to do with an emergency. And finally House Speaker nancy pelosi yesterday outlining and unveiling her own plan. Its called the take responsibility for workers and families act. I asked her about that, what it entailed, and she said that title is meant for trump. A message to him to take responsibility, john. All right. We really are standing by for any news on this because an announcement one way or the other you might think would come before the markets open later this morning. Thank you. And, doctor, i want to talk to you because youre spending the time, youre not on with us, actually on the front lines battling this pandemic. How ready is this battle against the pandemic . Howed ha how ready is it for the end or phasing out of social distancing . Well, were still at the very beginning of this exponential case of increase growth of cases in new york city. Think were quite far away from that, quite frankly if the was really strange to be at the hospital on sunday. We had basically cleared out every patient who has anything other than coronavirus. And, you know, its a really strange feeling. You know, the hallways are really quiet because everybodys keeping to their cornsers and then every hour or so you get a page that a patient needs to be put in the icu and on a ventilator. What would happen if in a couple weeks the president relaxes some of these guidelines . People can go back to work . They can gather in groups of more than ten . Maybe not in new york city, but around the country. What would happen to your efforts . Well, i think your numbers of patients would continue to shoot up. And, you know, right now were already out at some hospitals, were out of the n 95 respirator masks lot running low on the face shields, low on the gowns. So doctors are basically reusing wearing the same thing over and over for the course of the day. Even then were running low. So youre going to create situations that are extremely dangerous for Health Care Providers that are also not safe for patients because, you know, you can only staff so many patients safely. Mayor landrieu, how should citizens in the United States or People Living in this country take what appear to be mixed messages coming from mayors and governors in some cities who are now just instituting some of these orders . Massachusetts the stay at home order goes into effect today. You have some states saying stay at home now where the president s saying in two weeks i may tell you you dont have to stay at home anymore. So how does this messaging affect people . Well, the first order of business when you have a major disaster like this or pandemic is to do everything you can to protect peoples lives. The organizational structure is there has to be clear command and control and clear organization. We have the opposite of that with the president. The president is going to come to regret think there terribly, his Statement Last night because this week in america what we all have to do three things, stay away from each other, get all the medical supplies to the ground and to the First Responders like the doctor said and see if congress could pass a recovery package so we could figure out thousand get our way out of this. But when the president asks us to choose between life and our pocketbook, thats a dangerous equation to talk about to the american public. It was very unfortunate that he started talking about it last night. All of the health proechblds professiona professionals, theres a reason why dr. Fauch chip wasnt there last night, that would good him the right advice and that was informant. Unfortunate. We have to stay focused on the a, b, and cs and its too soon to talk about anything else. You were watching this News Conference last night and you reacted by saying did i miss something here . Is the United States all of a sudden winning this battle against coronavirus . And what about the masks . Yeah, i mean, im just im with mayor landrieu. The disconnect in what were hearing where weve got this attack rate on new york, we still have another week of the president s own order of the 15 days of something of a shutdown and now the president s talking about pulling out of all of this. You had to listen closely, i think, to what officials were saying. Dr. Burks was saying something different, which is lets be led by the data. Lets see where we are. The president was saying that as well. Theres no question that hes being lobbied hard by people who are saying, look, you cant continue to shut down forever. You cant fight the virus at the expense completely of the u. S. Economy. Thats why the stimulus package is so important. And what i heard Health Officials say, because i think that the mayor is right, his Health Officials and the president acknowledges it by saying if you leave it up to them wed be shut down for two years. Which was this kind of dismissive caricature of their advice. But i think what hes saying is can we get to a point where we can think about isolation and quarantine in some cases in more isolated parts of the country . How you go about doing that is not at all clear, but think thats an even on cnn last night there was an expert out of yale who was saying something very similar and perhaps a less jarring way than the president was saying it. But, i think the scientists would argue youve got to be led by the data. You have to see where you are in a week before you can contemplate that kind of patchwork approach at a time when we dont have complete buyin around the country around what you have to do to isolate. Doctor, talk about this attack rate in new york city, if you will. Its something that dr. Burks brought up in this News Conference with the president last night saying that 1 in 1,000 people in new york city are now with coronavirus. 28 of the people being tested are positive with it. And then Governor Cuomo was suggesting that the number of cases could double every 2 1 2 days. Put this all in perspective for us. 1 in 1,000 may not sound like a lot, but once you have a doubling rate of every two days, we have one of the highest doubling rates in the world right now, that means that this is really going to grow quite quickly. And so, you know, if i was covering 30 patients on sunday, i would be looking at covering 60 parkts the ne 60 patients the next day and 120 and 240, you can see where that quickly is not sustainable. And when that happens, thats when you see more talitality ra shoot up. I dont want to make it seem like the president say lone on this and a complicated discussion with republicans and democrats weighing in. But listen to the president on that issue. We have a very active flu season, more active than most. Its looking like its heading to 50,000 or more deaths. And you look at automobile accidents, which are far greater than any numbers were talking about, that doesnt mean were going to tell everybody no more driving of cars. Car accidents, is this just an issue of Car Accidents, we dont shut down the roads, mayor . I would make a couple of points. First of all, the doubling rate in new orleans is almost exactly the same as new yorks. So this is really spreading across the country fairly aggressively. Secondly, when the president says it like that, its very dismissive, its like why we are doing any of this anyway . If thats how many people die from Car Accidents, why are we even bother something aing . And giving him the benefit of the doubt, if we have to find a more precise way to deal with this, last night was the wrong time to talk about this. This country this week needs to do three very simple but very hard things. One, stay at home. Two, get the medical supplies to our First Responders so that they can be safe and treat the thing. And thirdly, for congress to get that package passed. If they did that alls a country, all unified, we would make significant head way and at some point in time begin to talk about more precise ways to fight the virus is. Last night he caused confusion and anxiety and took our eye off the ball. I think that was unfortunate. But, david, we are hearing it from the New York Times you talked about the yale med dean who you heard from last night when has written about this. Economists, the former head of Goldman Sachs saying theres got to be a way to open up parts of the economy here. Is it possible or how is it possible . Well, i think that, first of all, you have to be driven by data. What i heard dr. Katz say last night with anderson and others have made the point, is that it you have a reliable set of data you can perhaps project out and surveil the disease. Where is it going . What i dont like about the comparison with the flu even or Car Accidents is that you have so much ambiguity about this virus, where is it headed . Where does it stop . We dont have a way to stop it because we dont have a vaccine. Given that, Health Professionals have to be able to evaluate data on demographic sets for one thing. You know, who is least likely to be the most affected by this virus . And i think theres no question that if you are vulnerable in any way or if youre an older american, youre going to have to continue this isolation and this separation for a long time. And i havent heard anybody say differently. Then how you go about doing that is not clear to me. You can look at a map of the United States and see that new york is far more affected than other parts of the country. Do you have an ability for Small Businesses even for schools to reopen in parts of the country . I think thats what the economists are looking at right now. Theres really no question that public Health Officials almoare saying were not ready to end this. So we asked them to film themselves. Throughout the day as they try to fight this virus. Heres what its like in their own voices. I work at Massachusetts General Hospital in boston. And its currently about 6 30 in the morning, im about to head into work to start my 7 00 a. M. Shift. And the thing thats concerning us most right now, i think, for the majority of physicians at least is the decreasing supplies of ppe or personal protective equipment. Im going to finish my cup of tea and head out. I slept about five hours last night. I would have slept more but one of our colleagues had a medical emergency coming into work this morning. I think the thought on my mind is some combination of anticipation or apprehension mixed together with a strong sense of comradery and a strong sense of cold determination. I just arrived at the hospital, starting my shift this morning. And immediately theres a difference. The face mask is now required of all employees in the hospital and is to remain on for the entire shift. The other thing thats particularly striking is the atmosphere within the hospital. Its markedly different. Its very, very quiet. Usually on that monday morning this hospital would be bustling with activity and theres just its almost like a ghost town. So im about to go off on a round with my patients and well see how the day progresses from here. Hi. Its about 3 30 in the afternoon right now and i think one of the thoughts that comes to mind right now is just how much covid19 has just completely defined everything that were doing here. The care of every single patient is impacted because of it. Patients who need to go to Nursing Homes who cant because its considered too risky to transport them right now. So its the end of my shift now, its just after 7 00, ive signed out my last patient to the night team. Just because we have this this surge, this virus going on doesnt mean that people dont get the usual illnesses, dont get the need for admission for other reasons, and that doesnt stop. We still need to continue to care for those patients. We have had quite a few patients today who are concerned that theyre going to be exposed and they feel like theyre probably safer at home than in the hospital and have actually decided to leave because of that. And thats thats a difficult situation to reconcile, especially if somebody is not necessarily ready to leave the hospital. Back with us now is our cnn medical analysts and Infectious Diseases specialist. He know that their stories are your stories. You relate to everything theyre saying. You go right from your location at our broadcast back to the emergency room and to the hospital, so what are your days like as you try to treat patients . Well, alisyn, it was really striking to me the doctor who said it was eerie and quiet. Thats what ive been describing what its like to be at the hospital. Because on the one hand its full, its full of patients, but we dont have families coming to visit. We dont have the other staff you usually see around, the physical therapists, the social workers, the pharmacists, et cetera, in the hallways, at the nurses stations with us. Its really very its very unsettling and, you know, the quiet is essentially punctuated every hour or so by an overhead page saying theres a patient in distress who needs to be transferred to the icu and put on a ventilator. And thats sort of whats marking our hours these days. I mean, look, for somebody, a lay person whos not in your field and who didnt take your oath, it sounds like a horror movie. It just sounds it sounds eerie, as you say, unsettling. How many hours are you spending at the hospital a day . Sunday i pulled a 13hour shift. You know, i mean, thats pretty typical. I wouldnt say thats necessarily out of the ordinary for doctors in general. But, you know, it is very concerning because if we truly have a doubling rate as we were talking about earlier of every two days, you know, i was covering 30 patients on sunday, but if every two days that doubled, that means you go to 60, 120, 240, i mean, thats thats not possible to take care of patients well. And thats when youre mortality rates, your death rates not just from coronavirus but anything that theyre there for start to shoot up. Thats why in certain situations like wuhan, they had much higher case fatality rates. And weve put ourselves in a situation to replicate that if not worse. And, i mean, look, obviously youre trained for this. You had to work lots of hours in, you know, medical school and once you graduated. But how are you and your colleagues keeping from cracking during these stressful times . I think a lot of my colleagues are really having a hard time. You know, one, its the stress of the job. Its the long hours, yes. Its also the stress of feeling like were not being looked out for, that were basically being asked to jump off a cliff, you know, without appropriate protective equipment. And so thats thats really hard. Its, i think thats probably whats more stressful than the actual hard work of it itself. That makes perfect sense. I mean, the idea that not other you having to dive into this pandemic, but youre doing so without the right equipment and that so many doctors are getting sick. Do you know doctors who have become ill . We definitely have a number of staff right now who are not able to come to work because they are sick. You know, that also hurts us because then we have that many fewer people available to take care of patients. So that sort of exacerbates the original problem of too many patients, not enough providers. President trump has used the metaphor for the economy, which i think is an apt one, that you dont want the cure to kilt patient kill the patient. I think we all understand that. But if the country opens up again in some way where people start going back to work in bigger numbers, what do you think will happen at the hospitals that youre seeing . Well, you know, you have to be alive to be able to take a cure, i would extend the metaphor in that way. Thats what we need focus on first. And then, you know, approach it the way that china in wuhan did. They did social distancing measures for about a month, but they did so extremely strictly, which were not doing really anywhere to that degree of strictness. And thats what then allowed them to compress the time during which they had to do that and then start to lift measures. So the more aggressive we are up front, then that really buys us the luxury of being able to reopen things down the line and do so with some of the more targeted approaches. But the sloppier we are right now, the slower we are, thats actually going to draw out this pain and the economic pain as well. Treasury secretary has said, alisyn, maybe a couple weeks. They want to get that money in your hands as quickly as possible for a family of four its going to be Something Like 3 grand and they want to get a bunch of non Small Businesses so they can keep people on their pay rolls and not have to fire them. So one Sticking Point is this 500 billion fund to give to businesses. Democrats have been insisting on some oversight of this. Who would provide that . Well, the president says he would be the oversight, which of course does not harden any democrats here. But this is sort of like a stabilization fund. Its necessary. You need to get in money, have it there so that companies have this to rely on, super critical. But what democrats dont want is they dont want something thats just discretion only of the treasury secretary and the president. They want to make sure theres some congressional oversight of how that money is spent. Theres other springs they want too. They dont want to necessarily be bailing out airlines or industries and then in two years those industries and companies are paying their ceos a ton of money and using their new profits to buyback their own stock, which is good for shareholders, not for workers. Weve seen this movie before. This is why democrats are insisting on getting the details hammered out beforehand, because there was all sorts of human cry about this in the past stimulus things. That was recent history. Theres still a public mistrust of this process of bailouts because of what happened in 2008 and 2009. I do think, alisyn, one bit of breathing space that congress has right now is the fed came in yesterday with the big guns and said, we will support the economy endlessly. The Financial System endlessly. So you do have the fed backdrop here that allows the democrats a little bit of breathing space to try to get what they want. One of the problems is that when President Trump says ill be the oversight, i think thats his exact quote, the problem is that he is the selfproclaimed king of debt. He likes debt. So its hard for democrats to trust what he would do with oversight. And hes also in the hotel business, his family is in the hotel business, right . And he could personally profit or could potentially need we dont know what his business arrangements are at the moment, but theres mistrust among democrats that the president wouldnt be selfdealing in some way here. Hes been asked about it and not really answered that question, sort of diverting and saying he doesnt take a salary so he doesnt get enough congratulations for not taking a salary. But, you know, this is 500 billion. We have never pushed this much money out the door, big 2 trillion package. So im actually amazed that its grown from a trillion to 2 trillion in a matter of days. It shows that both parties are willing to spend some money to prevent the economy from falling off a cliff. But im glad that you point out that the president is not a totally disinterested player here. In fact, and i think that it needs to be noted, his resorts and golf clubs are suffering during this time. I think that the six highest revenueproducing clubs of his are obviously suffering. Theyve been closed or at least shut down temporarily. And so when hes growing restless and impatient with the speed of turning the economy back on, you know, obviously this affects him closely. Theres also a contradiction here. If hes saying if he is, you know, getting restless about the economy shutting down, but then at the same time saying we need this money, we need 2 trillion, we need democrats to approve this, isnt that kind of a contradiction to say we need to have the economy up and running but it has to have this emergency spending too . I mean, you do need that spending, you do need this is a 20 trillion economy, you probably need more than 2 trillion to stabilize it especially if this is a matter of weeks or months, you need that money, both parties now it, theyre just hammering out the details. In particular, a malaria drug called chloroquine. This has been around for many years, strong, powerful drug for malaria. But we think it might work on this, based on evidence. Based on very strong evidence. Its true, themation has b t been around for many years and has few side effects. Now, enthusiasm for the possibility of treating the Novel Coronavirus largely centers on one study out of france which used a derivative of chloroquine used with an antibiotic commonly known as the z pack. The study was small and the patients were followed for six days. The study that looked at that drug and showed activity was a study that involved about 20 patients and only six showed the benefit. The benefit that she showed was that they decreased the amount of virus in their noses when you did nasal swabs in those patients. So it could very well be that the drug is reducing viral shedding but having no impact on the clinical course of those patients. So the data on that is very preliminary. We took a closer look at the study and heres what we found from the were originally 26 patients in the study who were treated. 20 completed the trial. One left of the hospital before the trial ended. One couldnt tolerate the medication. Three went to the intensive care unit. Thats an 11 Critical Care rate. And one died. A 4 mortality rate. Now, those numbers are higher Critical Care and mortality rates than the general population of infected. Keep in mind again its a small study. There was another study from 2011 which found that while chloroquine was effective in the lab against the flu, it ultimately wasnt effective in humans. Look, thats why trials are needed. And thats cey can be done quic many labs in the World Health Organizations had been studying these drugs to help us find an answer for a disease that currently has no known cure. Using untested medicines without the right evidence could raise false hope. And even do more harm than good and cause a shortage of essential medicines that are needed to treat other diseases.
