Lets start with cnns athena jones. Shes outside of new yorks elmhurst hospital. This week 13 patients died in just 24 hours there. Athena, good morning to you. Whats happening where you are. Describe as you said in your last report with us the epicenter of the epicenter. Reporter thats exactly what its been called. This is a hospital thats done a lot of coronavirus patients. In fact, you can already see a line beginning to form here over my shoulder, maybe a dozen or so people waiting in line to be tested. Some of them wearing masks. Mostly keeping that social distancing that everyone has been asked to do, six feet apart from one another. New york state accounts for almost half of the coronavirus cases in the entire country and new york city, of course, accounts for the vast majority of the cases in new york state. This hospital has seen overcrowding, quite frankly. We heard the report from that er doctor who shared some video with the New York Times last week, talking about the lack of supplies, talking about feeling like they dont that have weapons they need to go to war. What some doctors are describing as a war saying they dont have the ammunition they need. One physician inside this hospital said that theres so much crowding, so many patients, so many stretchers that they are lined up three deep and doctors can barely get across the room at times for instance to deal with a deteriorating patient. Folks, still officials are surging more supplies. They sent medical personnel and sending more supplies. So this is clearly a hospital thats very much on their radar. We understand that they are also going to begin to divert noncoronavirus patients to other hospitals. At least at this hospital this is just one view into whats going on around the entire city, hospitals being fwasd lack of what they need. So, i want to ask you about this. Some people are pretty hopeful. What have you learned regarding this fda approving this 15 minute test . What are experts saying about it . Well, first, this is a good thing. 15 mint point of care test, go to a medical professional, get this test and get results very quickly. Fda authorized it on an emergency basis which basically means that they believe it benefits outweigh its risks and the idea here is that the laboratory, abbott say it expects to deliver 50,000 tests per day beginning next week. But heres what experts are saying. Looking at the situation and what theyve been saying this week you still need protective gear, still need proper gloves, masks, gowns, etfs youre the medical professional taking that swab. So that hold up or that lack of supply, that shortage, severe shortage all across the country is going to possibly affect the ability to administer these tests. The company said they are still going making same amount and the timeline is still the same. We have to keep one thing in mind. This comes after a week of intense scrutiny by the nations governors essentially begging for needed medical supplies saying they were only getting a fraction of what they requested. We know that new yorks governor, andrew cuomo, among others was pleading with President Trump to use the defense production act to get these states what they needed. Now, i do want to note one thing. One of the big things are these ventilators. Last night President Trump seem to cast doubt on the number of ventilators that cuomo said he needed. Take a listen. I think their estimates are high. I hope they are high. They could be extremely high. Were doing even hospitals based on pretty high estimates. Doing them anyway. If we do not need them that would be wonderful, we can help a lot of great people all over the world, we can help them live. But i think his estimates are going to be very high. Well see. But despite the fact these governors have expressed frustration with President Trump, americans as a whole are happy with his response. I want to pull up a poll here. This is a recent poll of President Trumps handling of coronavirus, an Approval Rating of 52 to disApproval Rating of 45 . Thats one part of the poll. Thats hard to say because testing, a test thats created this quickly, its hard to know how well it will perform. Sometimes you have to do that work in the field. However, im happy that were going to have a rapid test. Having more numbers from an epidemiologic view to see how many people have the disease will be very helpful to the country to trap the disease and understand the true mortality or severity of the disease. To be honest from the emergency Medicine Point of view if i have a rapid test sometimes these rapid tests, sometimes they help me and sometimes they dont. In our department were seeing sick patients coming in with respiratory symptoms. Xrays look bad. Bilateral problems in their lungs. If we have the test or didnt have the test, if we recognize the patient, we can still treat them the same way. You say what is new that youve noticed recently the type of respiratory illness youre seeing that seems to be different, that the escalation of the distress that patients go through is different. How so . Yes. Thats right. So, in the Emergency Department right now what were seeing are patients who are coming in often very short breath, breathing fas fast. Often when we have these kinds of patients we can apply to them breathing adjunct like a facemask we can apply. A mask that you might use for sleep apnea. When you apply those masks we have predictable increase in improvement in the way that patient is getting oxygen in their blood. Were encountering two problems with these patients. Their blood oxygen level comes in low and it can dropoff quickly if we dont act fast. Number two, these devices we traditionally use, having trouble using them because they create dangerous aerosols when we use them and it increases the risk for much in the room or someone taking care of the patient. New say they create aerosols. Help us understand what that means. Sure, sure. So right now the common mode of transmission for the coronavirus, everyone is pretty sure its through droplet spread. Droplet is like a large, a large drop of water you might cough out. This might go six feet or so. Then it might dropoff on to the floor or clothing. Aerosol, however, is a much smaller particle. When someone coughs, or if they have a high flow of air traveling through their airway it can create very fine small particles less than five micros. When you use Something Like cpap, again these masks people might use for sleep apnea or other high flow oxygen devices, you can generate or a patient will generate these very small particles. Unlike a droplet that falls down quickly, the aerosol might float in the air for a longer per of time and travel some distance. Unclear how far in my travel. Its hard for me to say. Because they are so small it makes them more dangerous and more troubling. Doctor, i just cannot emphasize enough how much people are thinking of all of you. I was standing in line a long line in a supermarket separated by six feet per person, and a woman who came out of the store turned to me, shouted a profanity in my direction and when i looked up pulled down her mask and purposely coughed in my direction before walking away and getting into her car and driving off. I didnt even know how to process it. I couldnt respond until afterwards. And it took me a while to even recognize that i was the only asian person in line and that was the likely reason why this occurred. Its one of those did that just happen moments when it happens you dont expect it. Obviously the vitriol is disp k the world should be reaching out to wuhan not just in a humanitarian way, but to learn something. Yeah. Wuhan, my family and friends are all under lockdown, have been under lockdown for the past, more than two months now. People there need supports, not just aid support but emotional support. When you give people emotional support their immune system will go up and have a better chance in beating this virus. And thats why we have been working on this campaign and going around the world now to hopefully breach that gap to bringing these emotional supports for wuhan. Its a lot of process of essentially putting what were seeing in individual incidents like i encountered that many other people are facing and putting it into policy, institutionalizing this calling out of china. I think weve seen historically consistently that when government leads people follow. And it inflames the feeling of hate, of xenophobia, of targeting towards people who look like they may be the soufrt disease and that includes people who are right here in the United States. You told one of my producers that theres fear and ignorance as well that is happening in asia that when they hear an italian accent or they see a person using your characterization that european looking, that people kind of recoil. Is there conversation happening in china much like the one were having this morning . Yeah, for sure. Theres actually regional discrimination, so to speak, as well thats going on inside china. If youre like me whose hometown is wuhan, if you speak with a wuhan accent you need to be very ware and need to be careful when walking on the streets in beijing and shanghai, for example. Not everyone is that understanding and not everyone is ready to greet and embrace a wuhaner. Its fascinating to see how all these hate and discrimination evolve through the course of this virus through the world because initially it was and still right now asian looking people than potentially people with italian accent or in asia if you see like you said, like i said earlier people with european looks that would bring out panic in some people. I dont personally think that race is the fundamental issue here. I think its fear and ignorance. Fear really is there to save and protect us. Ignorance is the problem. Ignorance is what causes to carry out irrational beharvey, carry out unhelpful behavior while everyone is dealing with this crisis together. I look forward to getting them in the Emergency Department. Thats what i care about. When will they be accessible. The other thing to tell people we dont know the exact accuracy of these tests. Most of them still can be 60 to 70 accurate. Even if you get a negative test if you have symptoms of covid i tell people to act like they have it and selfisolate. People who say Young Healthy people are immune, who arent immune compromised dont have to worry. Theres this new report of a man, 25 years old, twotime all american lacrosse player, hes in medically induced coma right now, critical condition. He had no symptoms beforehand. He hasnt been out of the country. Does that change . Does that case change what we know about this for young people . Im really glad that youre asking that. Because actually what were seeing is that even information from china, china largely recorded on the death rates which is why some people are saying are we seeing more illness in young people. Its not the case. In the United States were reporting more information. Rear not just reporting on death rates were reporting people who are severer and critically ill and what were finding is that this disease does not discriminate Critical Illness by your age. Were seeing that in many icus, im talking to many of my colleagues anywhere 12 to 30 of their patients in icu are younger ages. The death rate is still higher when youre older but people of all ages can be critically ill. Okay. Lets get to some questions from twitter and from instagram as well. First question up here on what surfaces can the virus survive . Metical, plastic, paper, skin. Whats the answer . So, victor, we get a lot of these questions. What should i do with my grocery . Theres been a number of tests and things like copper the virus stays about four hours. Maybe your pennies. Also on cardboard around 24 hours. On plastic and steel around two to three days. So what does that mean for the average person . It means when you bring home your groceries if they are plastic or in something thats metal if youre worried wipe them down otherwise it would be on there for two to three days. Is it necessary . Is it infectious. We dont know that. If you want to be on the safe side. Be careful with door knobs and things like that. I want to ask you this. Someone tweeted me coul could covid19 if i sanitize my hands before i touch Something Like a gas pump could that kill the virus . Thats a good question. You want to sanitize your hand before you touch a gas pump to prevent illness or spreading to the next person but for you, after you touch that gas pump, before you touch your hand or face or anything like that, wash your hand or sanitize with something thats 70 alcohol or higher. Right now were primarily testing people who are in the hospitals. Thats our only data point. When do we see fewer patients coming into the hospital.
