With a violent collision between what the president thinks and says and the more disciplined world of science and data. For two months President Trump has told that hydroxychloroquine is a game changer. It saves lives. The president himself is finishing up a round of hydroxychloroquine treatment. Sales of the drug has doubled driven by the president s praise. The lancet today said the drug did not help hospitalized coronavirus patients. Instead, that study said it made them more likely to die and it is likelihood that patients develop an irregular heart rhythm that puts them into cardiac arrest. Hospital data studied, take us inside the study. John, lets take a look at these numbers. The study that was done and published in the lancet today looked at patients in 671 hospitals with coronavirus on six continents, nearly 15,000 hospitalized patients who received hydroxychloroquine or chloroquine, sometimes in dom bination with other drugs as well as patients in a controlled group and what they found is those who took those drugs or some combination of them, 33 to 45 more likely to die, and that is even after adjusting for the fact that there were some differences between the groups in terms of how healthy the patients were. Also patients who took the drug, 2. 4 to 5 times more likely to develop heart arrhythmias. These are hospitalized patients. Trump said he is taking this preventatively. Those are two Different Things and we dont know what it does preventatively. Could it work . Its possible, but doctors ive been talking to say when you see these kinds of results who actually hurt patients in the hospital and there are other published studies that say the same thing, they wonder is it even worth studying as a preventative agent against covid19. A very important study and lets continue to discuss it with one of the lead authors, dr. Mundy mara is with the Brigham Center of advanced heart disease. Doctor, thank you for being with us today. You looked through the study and i want to read you a principal finding. We were unable to find a benefit with hydroxychloroquine or chloroquine for covid19. Each of these drug regimens was associated with a decreased inhospital survival and an increased frequency of ventricular a rith rrhythmias w used for treatment of covid19. Should hospitals stop using this as a treatment . That would be our strong recommendation, john. Our data has shown that across the world in the realworld population that this drug combination, whichever way you slice or dice it, does not show any evidence of benefit, and in fact, is i mutably showing a signal of grave harm. So there is a distinction in the sense that you are looking for hospitalizations of patients. This is not a live, Clinical Trial. That would be the Gold Standard of research. So for anyone out there now, for the hydroxychloroquine promoters who will say oh, wait a minute. Its elitist doctors trying to say the president of the United States is wrong, make the distinction between your research and some of the Clinical Trials that are under way now about the same drug. Yeah. So we looked at a very specific patients who were hospitalized with covid19. We all know that when you get hospitalized with covid19 its a very chaotic situation with the los of uncertainty. We know that patients who have underlying cardiovascular disease and underlying lung disease are particularly vulnerable to bad side effect of any medicine thats used in these patients at that time, and what our data is showing is in that chaotic circumstance of the hospitalized patients of covid19 and theres no question that there is a very clear signal of harm with these drug regimens. Caution is advised in interpreting these findings for people who have not yet had covid and are actually looking at trying to prevent covid. Its in that group of patients that Clinical Trials are to be conducted and we would endorse conducting Clinical Trials in those patient, but even in those patients, the use of this drug regimen in an offlabel capacity should be shunned and avoided notably because of these observed risks particularly in patients who have underlying cardiovascular illness and this is a very, very critical distinction to keep in mind. Well, so lets close on that point because the president of the United States is one of those people you just mentioned. Hes outside and not hospitalized. He wants to use this as a prophylactic. He keeps hearing despite studies like this from other people, and lots of people as he likes to say that this actually works. Heres one of the things you say in your research and writeup of this. Nevertheless, a cause and effect relationship between drug therapy and survival should not be inferred and this data should not be in the ambulatory out of hospital setting and randomized Clinical Trials can be used before any conclusion in covid19 patients. Thats in the out of hospital setting, but from what you learned inside the hospital do you believe this drug should be restricted to only disciplined, protected Clinical Trials and not the kind of use the president of the United States says he just went through . I do agree with that statement, john, and in fact, when i finished medical school one of the most important principles that i kept in mind was what i was taught at the time i was graduating was first do no harm, and when you see evidence of harm, youve got to be really, really careful using anything in any circumstance without a controlled Clinical Trial setting. What would you say, sir, to someone who says i believe the president . Well, its their belief, and i would personally disagree with that individual, but we all have a choice to make in our lives, and we cant push choice. A lot of doctors are taking it. A lot of people swear by it. I get a lot of tremendously positive news on the hydroxy. I really think its a great thing to try. What do you have to lose . Take it. I really think they should take it. The hydroxychloroquine. Try it. Kaitlan, repeatedly promoting a drug that this new study says is harmful, it doesnt help with covid and it is potentially harmful for people in the hospital. The lead author says he doesnt think it is helpful and until Clinical Trials it has been the most between the president and his doctor, of course, but that hes pushing other people to take it and hes saying the sentiment of what do you have to lose . Based on this study people could have a lot to lose, so i think thats been the concern, and of course as elizabeth noted on how the president is using it which he believes as a preventative even though the fda says there is no evidence that it actually works to prevent you from getting coronavirus, but we know that what the president has said matters. People like senator rubio have said they dont believe people are taking prescriptions based on what the president is pushing, but we know after the president started pushing hydroxychloroquine in march there was an uptick for prescriptions because people were asking private pharmacies and retail pharmacies were seeing a higher interest to use hydroxychloroquine. Is he encouraging other people to where they feel more comfortable. We should note, john, that today is the last day for the president to be taking his own prescription of hydroxychloroquine that he told us about two days ago. This is back on the 15th. Restrictions are lifted in both states. People moving around more than 45 . 45 here. 40 here. Look at the places that are sheltering the least. Theyre all in the south. Arkansas, oklahoma, alabama, mississippi, places where people are sheltering the least meaning theyre getting out and interacting as the economy starts to reopen. Alabama versus mississippi, these are the cases. You see more activity and be careful, you cant directly correlate these things, but this is what you have to watch as people get out and start to move about more. The question is some stress especially in montgomery of the hospital system. We need to watch this. You look at the sevenday track here and its the lighter line and flat, trending a little bit and alabama has had a spike in cases and so the scientist will look at the cases and the mobility and try to make a connection. Are we pushing it or are states fighting away to manage it and thats in weeks and week ahead. Joining us to talk about the data trend is ryan from data graph. When you look at this data, you can get overwhelmed and what jumped out at you the most when youre seeing the big changes to now as the States Reopened. What jumped out most . Yeah. Hi. Thanks for having me back, john. As you know, the focus right now is how to save as many lives as possible without killing the economy. Every state and local government is working to figure out those policies and were seeing different states try different strategies and the important point is you cant figure out these best approximately sees if y you cant measure physical distancing and visiting businesses and thats why its a valuable piece of the puzzle. We are seeing that thousands of individuals from governments, industry and academia are using this including the cdc to figure out the right policies to reopen. You mentioned earlier data yeah . I want to go over it with you. Lets take a look at urban versus rural. People in urban areas tended to be staying home more and even as they get about more so than rural areas and the blue is distracting here. You see in rural areas they werent staying home as much to begin with and theyre out and about now. Is this in conservative areas of the country or more space in rural . Can we answer those questions . Yeah. Exactly. Were showing how much people keep staying home and theyre 25 most urban and 25 more Rural Counties and as you see urban counties are staying home at a much higher rate and thats generally been the trend throughout the pandemic and urban counties are staying home around 35 , and theres a number of reasons to explain the difference. First is that urban centers were hit first and hardest by covid19 so the policies have been higher there and urban centers lend themselves to shelter in place because they have more structure around delivery and things like that. Lets look at something else. This is foot traffic. So you can monitor not only are more people getting out and about, but what are they doing . This is just mississippi foot traffic and you see a heavy increase in people going to the gym and the yellow line is music stores and then you have department stores, shoe stores and beauty salons and people getting supplies that they couldnt get, shoe stores and things they wanted when they couldnt get out of the house and some people are deciding its time to get back to a more normal life, if you will, things that are more important to them personally. Yeah. I think thats one of the most important takeaways is that not all businesses are affected equally by the pandemic. Some are surging up fast as economies start to reopen. I think the cat gore oegory on t that jump out at me is gyms and exercise classes. Places like gyms are particularly dangerous when it comes to spreading covid. People are eager to get back to their gyms, but hopefully everyone is staying safe and maintaining physical distancing. The last one i want to show you is the biggest foot traffic increase since may 1 and there are states in the northeast and you see New Hampshire jumping off the charts in recent days and particularly in new jersey and rhode island and these are among the states hardest hit and New Hampshire hasnt had as many cases and theres the beginning of the reopening and those are states that are coming out of the tunnel, if you will and mor more. I think that the states are but overall people are eager to get out and i think thats a struggle that Public Policy will have to deal with. Hopefully the counts stay down. You know, obviously no one knows exactly what the future holds and we know when the School Reopens in late august and Early September for most of the schools that the students will come back to temperature checks and wearing a mask and sanitizing our classrooms and maintaining social distance and were having those conversations to guide our thousand School Districts about how to approach opening with social distancing conditions. Every piece of this is complicated. Let me pick up on one of the pieces you just mentioned. You have 6. 2 Million Students, 360,000 teachers, masks. School districts provide the masks . These are children. Masks are going to get lost. Masks are going to get soiled. What are you talking about . Youre in the hospital business, you are in the pp ebusiness as you try to reopen schools . Unfortunately, thats our reality, and i believe that as you said schools will have to be prepared to have masks on hand that, you know, students forget things. We all forget things and so, you know, our department is working closely with the Governments Office and the office of Emergency Services to find ways to make sure there are lots of masks and personal protective equipment. This is going to be the challenge for everyone everywhere, every sector, but this is the order of the day and weve got to make sure that we have lots of equipment and supplies, otherwise we cant do this. The only way we can do this safely and we ensure we are going to ensure that we can only do this safely is if we have access to all of the personal protective equipment. Where are you, and again, it will be different in every district and we started talking about this in the last conversation about the urban districts that dont have large classrooms, that dont have enough teachers and you have to deal with the social distancing question and are you running simulations, and running data to see what makes the most sense and one week some students are in the classroom and the others are doing Distance Learning at home and how will you do that if you have some at school and some at home and if you split the day you have a cleaning challenge. Youre right. These are complicated factors and were having conversations and School Districts are being very creative. Some are thinking about smith t splitting the day in the morning session and afternoon session. Right you in we have good weather and some schools are having class time outside. That gives a little bit more flexibility for schools that are hindered by space. Were seeing this in our child care sector where theyre using the outdoor space to broaden the campus and what we can do around social distancing, and ive got to say a lot of schools are preparing to offer a kind of blended approach of inclass instruction and some Distance Learning and the Distance Learning might actually help us to keep our class sizes smaller. We know that families are asking for Distance Learning. So were anticipating that there will be a balance of inclass instruction with stuntdents Wearing Masks and social distancing and Distance Learning as the order of the day as we go forward in california. More states and local governments have had a giant hole blown in their budgets because of the economic shutdown of the coronavirus. So whether its ppe supplies and whether its uncreasing cleaning at schools, and whether its getting the technology for the students that cant afford it, for those that still have to do distance or Remote Learning and where are you in addressing that challenge which i just have to assume is overwhelming. All of this has huge fiscal implications and theres no question that we can do this. We cannot do this without the help of the federal government and i want to, you know, appreciate those in congress who have talked about providing an additional federal stimulus and relief package. It absolutely is needed in california and every state. I speak regularly with state superintendents and state schools and chiefs all across this country, almost to a state. We know we need Additional Support from the federal government to support, you know, more personnel so we have smaller class sizes and so we can support more equipment and supplies to keep our schools safe. We have to have that revenue from the federal government, as you noted. The coronaviruss impact on the economy has been devastating and this is the time more than ever that we need for congress and the president to send another relief package to support all of our states. Reverend curtis young is the Minnesota Youth council and he support the governments continued restrictions. Now you have a split in the faith community. I am the head of the Minnesota Council of churches which represents africanamerican denominations, the mainline church and the Greek Orthodox church. A central part of our concern is that as we listen to communities of color, many of our congregations and congregations that represent refugees and immigrants, africanamerican, latinos and even seniors theyre saying why the urgency . Theyre so directly affected theyre actually afraid in many cases to go into Group Gatherings and so we dont see the urgency. We feel that we need to make our decisions based on good science and the recommendations of our Health Department and weve been in an ongoing conversation with our department of health, with the department of Public Safety and the minnesota governor. One of the complaints from people who disagree with you is they feel a sense of discrimination. I can show you some of the reopenings, elective surgeries allowed to resume, bars and restaurants on june 1st, barbershop, salons and mall of america, and the governor looking at whats allowed to reopen says this. Its not permissible for an unspecified number of people to enter shopping malls and go to stores. Big box stores have hundreds of people inside at any one time and it prohibits of a gathering of 11 people with a Seating Capacity of several thousand defies reason. Do you understand the complaint that if you have 50 to 100 people wandering around a mall why cant you put a hundred people in a church . Sure. We understand how that sounds and how that looks. I will say the business perspective comes from the need to make profit, and see the economy to get back on course. While that also affects us our central concerns have to do with the health of our people, and so we are not feeling that our facilities are ready. We do know the governor and the department of Public Safety are preparing plans for how to open up. We just dont see the urgency of why we cant wait a few more weeks before we do this. To that point, youre in touch with the governors team. Are they telling you, is part of the reason youre able to go to your people and say lets hang in there, they tell you what . Two more weeks, three more weeks. Did they give you a better time line . Theyre working on that kind of a time line. Unfortunately, its not public yet that would help our process and i do ask for our governor to become more public in the kind of timeline required, but we are in those kinds of conversations and its not just the churches and the Jewish Community and the buddhist, and Hindu Community and we are behind a more cautious, healthdriven approach that the governor is taking.
