Theres, of course, this is fema. They brought in the resources. The items that are needed as this location begins to take in patients. The other thing i wanted to show you. Theres stethoscope, gauze pads, scissors, other materials. Theres protective gear in here. All sorts of sterilizers. A lot of this is stroewn along the line here in whats a makeshift hospital. The other thing inside these, some of these rooms, this is what theyre now designed to look like is some of the things to perhaps maybe make some of the patients who are going to come here feel better. Theres lip balm. Theres a honey lip balm that the japhet center, jacob javad center. Theres things to help them feel better as they come in. Theres Hand Sanitizer as well. All of this, obviously, is going to play a big role to try and relieve some of the stress that the hospitals we keep hearing about, the hospital in the city, the hospital in the state are going to start to feel, once they start accepting patients here. The governor finished speaking here and continued the work and this should probably be up and running some time next week, brianna. I wanted to ask you about something, shimon. The president on twitter attacked Governor Cuomo, claimed thousands of federal government ventilators found in a new york Storage Facility and the state must distribute them now. You were talking to Governor Cuomo just a short time ago and you asked him about this. He essentially gave you a fact check. What did he say . Reporter he did. He said it was ignorant for the president to Say Something like that. The fact that there are these ventilators. Of course, theyre there. Theyre there so that they could be distributed when they are needed. Heres exactly what the governor said, brianna. That is incorrect and grossly uninformed. The point is, we have ventilators in a stockpile and we didnt send them to the hospitals yet. Of course, we didnt. Thats the whole point. The hospitals dont need them yet. The hospitals arent at the apex. The hospitals have enough ventilators today, but the numbers are going up. We are planning for an apex, a high point and by 21 days, thats when we need the 31,000 ventilators. Not today. Right now, we put them in a stockpile. So the point is, theyre in the stockpile, you must not need them is just ignorant. Of course you dont need them today. You need them when you hit the apex which is 30,000. Were not there yet. Reporter brianna, this is what the governor has been talking about so they have access to them to move around the state and new york city. Were hearing stories from hospitals saying theyre going to need more help, theyre feeling the burden of this virus, that there are too many people at some of these hospitals and they do anticipate things to get worse. And the governor is saying, we need these ventilators to move them around when the time comes. The governor says he still wants more ventilators. Hes hoping to get that. The apex, as you said, 21 days and theyre still working to make sure the hospitals have everything they need, protective gear and obviously, the ventilators, brianna. President trump publicly pushes to reopen parts of the country, claiming some states are at a low risk, microsoft cofounder and philanthropist bill gates is warning against that, saying social distancing must be the countrys top goal nationwide until the number of cases goes down. So heres what he told Cnns Anderson Cooper and dr. Sanjay gupta last night. So were entering into a tough period that if we do it right, well only have to do it once for six to ten weeks, but it has to be the whole country. We have to raise the level of testing and the prioritization of that testing quite dramatically to make sure we go through one shutdown, so that we take the medical problem really stop it before theres a large number of deaths. We do then get an economic problem which is why you want to minimize the amount of time and having states go to Different Things and thinking you can do it county by county, that will not work. The cases will exponentially be growing anywhere you dont have a shutdown. An epidemiologist and Infectious Disease expert joins us now with elizabeth cohen, our cnn medical correspondent. Elizabeth, to you first. Cnn has learned that these plans to reopen states, theyre not yet finalized but walk us through what the president is proposing. Well, you know, hes being very vague in what hes proposing. Hes saying maybe divide the country into quadrants, go county by county. Being very vague about what hes thinking of. The general feeling is not Everyone Needs to shut down. The term county by county, the experts ive been talking to, that makes them really bristle. People go from county to county. Why would you shut down one county but not the adjoining county . People travel from county to county, they could easily spread it that way. That certainly seems too microscopic. Whether or not to do that on a larger scale, as mr. Gates said, if you shut down the whole country, you may actually get the economy up and going earlier because you can stop this thing now. But i think the underlying thing im hearing from people is this, brianna. They want to hear the president talk about science. They want to hear him say, according to the numbers were seeing, we should do x. According to the testing data, according to the number of hospitalizations, we should do this. And thats not what hes saying. It doesnt seem like hes making a data or sciencebased situation. Hes just making a decision, i want to get the economy going again, thats not databased. Thats not sciencebased. Because you have to know what the problem is before you can solve it. Its pretty basic, and dr. Mcgregor skinner. I wonder if when we hear bill gates say this, shutting down for so many weeks, do you agree with that . Whats happening at the neighborhood level, the community level, even the county level, brianna, weve created resilient zones that are trying to keep the virus out. We know the only way the virus moves across the country into different towns, different neighborhoods is by people movement, and we know its not everywhere. We use lots of maps. Right now, were reporting where people are showing any type of sickness and put dots on the maps and letting everyone know this is what you got or havent got patients. We try to create resilient zones to keep the virus out, not just focus on flattening the curve. But would you think of a shutdown, a nationwide shutdown for that long . Do you think thats totally reasonable . Right now, my focus is on training in hospitals. Ive been in a lot of hospitals in the last two weeks, where im focusing on provider safety. Front line Health Worker safety, and what im seeing though is that all the communities, all the neighbors are trying to protect the functionality of the hospitals. Again, shutdowns, thats not my area of expertise. I focus on hospital safety, but what im saying here is that as people are not going to work, they they come ascome together the amount of virus transmitted in their community. So let me ask you then, dr. Skinner about, if youre talking about protecting hospitals and were looking at them becoming overwhelmed, and theres going to be staffing issues, what do you want to see done when it comes to medical students who are supposed to be graduating soon, right . And what do you want to see in terms of folks being trained on how to use respirators because that may be something thats in shortage . It is, brianna, and we are training every day. Even during the outbreak now, again, i teach and train in disaster medicine. Ive done that for over 25 years. Even during the disaster, you still train people, and as you get volunteers, you get people within the Hospital System, as well as those retired, as well as new graduates coming from universities, we train them in the proper ways of safe ly putting ppe on and taking them off and other medical equipment usage. That training is going on every day in every hospital in the country. We do it facetoface. Large paper documents produced by the cdc, hhs and turn them into youtube videos so people cannen watch them in their own time and everyone learns by doing and instead of reading, learning by doing, and putting people in the thousands to fill in the needs necessary gaps on the front lines. Right now, i want to bring in a guest, shauna pavel, home nurse in the cleveland area and yesterday, shauna, the company you worked for received first coronavirus patient who was being discharged from the hospital and needed some home care. Shauna, thank you so much for being with us and just explain what do you do in a situation like that where youre caring for someone with coronavirus and you have concerns that they may be either undiagnosed or they may still be contagious as they recover . Thank you for this opportunity. So home care is so incredibly important, especially right now. We are providing a number of services in the home that not only keeps people out of the hospital, but also its helping create space for the hospital as they prepare for this big wave of covid19 patients. In the home, what were trying to try and protect not only the home care workers, but the patients we see as well is were trying to conserve the ppes we do have and coming up with new policies to try to do the most safe practice that we possibly can. And so how many masks would you have on a day to use . Currently, with all of our patients regardless, we use one surgical mask, the paper mask every day. Were reusing it. Those are obviously in short supply, and right now, only a few nurses that have been properly trained and tested are able to get a mask and they will be the ones to take care of the covid19 patients as they come home from the hospital. Okay, and so youre seeing multiple patients a day, right . So that is part of the issue is you have one mask. Some of your patients are really the most vulnerable here, including immunosuppressed patients. Whats your biggest concern as youre going about your work . So were not going to stop seeing our patients because of whats going on. We have a job. We have a duty. Were going to continue to do it. The scary thing is how can we prevent exposure and transmission to all of our patients while also trying to conserve the ppes we have . We really cant do both. So you have to make a choice, right . So then what do you do . So were being creative. I work with some of the most amazing nurses who are creating their own masks with air filters and Household Items to keep themselves safe. We are working together coming up with new ideas and the company very open with their communication and keep everybody safe. Our main priority and with taking care of these patients. You and your colleagues, shauna, and home care nurses are doing gods work across the country and trying to keep the hospitals from becoming clogged with other patients as well. Well, were seeing the scenes from new york and we know that whats happening in new york is probably, almost certainly going to happen in many other parts in our country, and it just is untenable. And really, unimaginable. I mean, i could not have imagined two months ago the idea of Health Care Workers going to work and not having personal protective equipment. Making their own masks and trying to find gowns online. I also could not have imagined the situation in this country where doctors and nurses would be forced to think about rationing resources of having one ventilator and three or four patients who may need the ventilator in order to survive. Our Health Care Capacity and system is stretched already, and were seeing what happens in new york when its stretched beyond that capacity. We know that other places are soon to follow and the key is now, how can we get everybody else prepared in time so that were not, we dont keep on being behind because thats whats been going on in our response so far. We are always weeks behind where we should be, and now we know how to anticipate. We know whats going to come our way. Can we get those supplies in time . The beds, the equipment, and the staffing, so that we can avoid a real Public Health ka cacatastr where tens of thousands of people die unnecessarily. You were actually more than 39 weeks pregnant, right . Due on tuesday, and someone close to me is due on monday. I am sure tons of people out there have someone close to them whos pregnant right now, and you are looking at giving birth on the upswing of this thing and i was wondering what your concerns are and what you would say to other people in the same position. Yeah, this is my second child. The first time, i had a lot of anxieties and a lot of nervousness about pregnancy and delivery but its very different now to be pregnant in the middle of a pandemic where things around you are changing so much. I mean, hospital guidelines are changing. It used to be in my hospital that we allowed the entire family to be present at birth. Now we allow just one visitor and they are hospitals in new york allowing no visitors at all, which i think a lot of us cant even imagine, the idea of being in labor and not having someone with us, but this is whats happening. I mean, health care is happening every single day. It doesnt stop in the middle of a pandemic and patients and physicians and nurses, Health Care Workers, everybody is making enormous sacrifices and in times of uncertainty that we have to embrace that uncertainty because it is what it is, but we also have to recognize our collective selflessness and the sacrifices everyone is making in order to get through this time and to add another note about social distancing, theres a lot of people who cannot avoid the Health Care System right now. Theres a lot of people who cant help but go out and go to work because they have to. So for those who can stay home, please do, because your selfless act is helping others to save lives too. Today it has wrapped up. You can see the lanes set up behind me. The tents set up. When we got here, that was full of cars waiting to go through to get swabbed by the Health Care Workers wearing these protective gear, but today, it took a little while longer for them to reach capacity of 250 but bottom line, a lot of folks are coming. Today though, anyone who was a new jersey resident who has symptoms. Tomorrow, at this site, they will be testing Health Care Workers and First Responders only. Symptomatic Health Care Workers and First Responders and we know thats important because these are folks who are on the front lines dealing with patients. Were out here on the front lines trying to take care of our patients, working hard with our governors in the states and all you can worry about is when youre going to get people back to work and back to church and trying to make yourself look good and all this instead of taking the leadership role youve needed to take for so long. For him to fully enact the defense production act to get the equipment we need. And so far we have not seen that from the federal government. The four Hospital System there in michigan is acknowledging that it drafted this memo that came out which outlines end of life protocol at the facilities there. It is not an effect we should mention. But clearly the hospital is getting prepared to make serious life and death decisions. Are we headed to the point where there is going to be a rationing of medical equipment and treatment. I think the big answer is we dont know but weve seen it happen in italy. And we know that beaumont system in detroit area is nearing capacity of ventilators. If we dont have a an adequat number of supplies that will happen. Hospitals are having this communication and giving some directions to the doctors that have to make the decision because just contemplating that is gutwrenching for us that want to save lives that we might have to tell a struggling patient and a griefstricken family that for a lack of resource, the richest country in on earth, we just cant save their loved one. Its abobbinable. I think it is criminal that it isnt happening yet. Im sure youve had the tough discussion before where you have to give someone bad news. But i wonder if youve ever had to give someone bad news and say it was because you knew there was something that could save her life, perhaps, but you couldnt true it and didnt have the resources.
