What sort of contact are you having with people who are concerned originally all those who are in that category, who are falling ill . We are seeing an increasing number of calls, an increasing number of calls, an increasing number of people testing positive and it seems to be more than the initial stages in march and april. Looking back then, things are very different this time around. There seemed to be relatively few cases at that time and now there are more positive places. People are testing positive with symptoms. A change from march and april. There has been concern about doctors surgeries as well, how you cope. 0n the one hand you want more people coming forward but at the same time there is concern about whether people in the nhs will get overwhelmed. What is the situation for you . Every day is a struggle. It really is. To make it clear that gps have been open throughout the pandemic, are still open. It might be very different that we do want people calling if they are concerned about medical problems. It is not just covid. If people have concerns about symptoms, for example unexplained weight loss, blood in there we, a pain that will not go away for example. We dont want people to be ignoring that. At the same time people can help. If they are simply concerned about covid related symptoms but you dont feel unwell, do you necessarily need to contact your gp about that . Could you find information from other sources . Hundred and 11 online, government sources. There is good information out there. 111. Nottingham, where you are, is one place that people say should be in the third tier, according to the figures. From a medical point of view, how do you look at it . It has been a very odd situation. With an eldest daughter who has the virus, and a younger daughter self isolating upstairs because of the virus, shes about to hear news. Ive just had a phone call from the hospital to say the results have been brought forward to today. Crazy situation theyre in. This is a roller coaster for the whole family. Libbs . When i informed my children, their first reaction was, were on our way, but they couldnt because we were on lockdown then. The thing is about the cancer, at least i was able to go out, whereas, with this virus, i was in lockdown 24 hours a day. But there is one person who saved him from the solitude of the virus. My nurse, hilary, she is in constant touch with me. Its not just to discuss treatment. She rings up, what are you up to . How are you getting on . She doesnt have to ring, but she does. Thats it as we film, the call comes through. Hi. Sorry, say it again. How do you ride a roller coaster of cancer and covid like this . How do you cope . You dont have a choice. You have no choice, you just have to get on with it. Yes. You take each thing as it comes. Have you really worried about your mum . Yeah. Its really, really worrying. She said youre good at hugs. We can speak now to professor david cunningham, from the association of cancer physicians. Good morning to you. Thank you very much for being with us this morning. Its wonderful to be able to get an insight into peoples stories, as we did there with catherine and with a positive result. But you have been talking about the necessity of treatment for months since the virus hit . Good morning, naga. Those are two great stories, illustrating different aspects of cancer care. But one of the key things moving forward , but one of the key things moving forward, and we are all trying to encourage people, is to seek attention and seek medical advice if they have symptoms or concerns they might have cancer. Its important that we reassure people that hospitals are a safe place now. We have made a lot of progress since the beginning of the pandemic. And we have systems in place that ensure people can come to hospital, get a diagnosis and, of course, get, as you heard, very effective treatment. What needs to be back in place in hospitals at this moment in time in order to further encourage people that the treatment is available . think everything is in place. I think everything is in place. I think its about getting the message out there that we can investigate patients. We do have the tools to make the diagnosis. And we certainly have the treatments to deliver effective therapies. You know, for example, when the pandemic first began we werent quite sure how safe it would be to get a patient a systemic enter anti Cancer Treatments. Although there are risks of immunosuppressant treatments. Although there are risks of immunosuppressa nt patients, treatments. Although there are risks of immunosuppressant patients, it would appear these risks are less than we initially anticipated. But immunotherapy, one of the new treatments were used to treat cancer, for example, the end week the nhs is a business. To see patients who have concerns. As we move into the second wave, which we move into the second wave, which we think will be much longer, a flatter curve, we have to make sure that we maintain all these other services for patients with cancer etc. Its very difficult to predict anything, isnt it . One thing that was always very clear when we spoke to various oncologist was the concern that people would be relu cta nt to concern that people would be reluctant to visit their gp, or relu cta nt to reluctant to visit their gp, or reluctant to getting to hospital as you have spoken about. What do you think the impact is in terms of cancer diagnosis in later stages now . Some of the patients turning up to our clinics. When you take the history often they may have had symptoms that just history often they may have had symptoms thatjust emerged around the time of the first lockdown. They decided to put things on the back burner. And then, you know, there are other priorities and eventually some medical attention. So we are seeing patients with more advanced disease. We know for virtually all cancers one of the key elements for success is actually making an early diagnosis. Dr david cunningham, a lwa ys diagnosis. Dr david cunningham, always good to talk to you. Businesses in parts of england under the highest tier of restrictions know they will get some support. The government pays 67 of workers wages up to a maximum of £2,100 per month. Too many hospitals supporting people with learning disabilities and autism are providing poor care which is at times undignified and inhumane. Thats according to the care regulatorfor england. The government, which commissioned the report, says its findings are deeply concerning. 0ur social Affairs Correspondent alison holt has the details. Through a hatch in the door of a bare seclusion room, bethany is reaching out to hold herfathers hand. Shes a teenager with autism and learning disabilities. For two years, bethany was living in long stay hospitals segregated from others, because her behaviour was said to be so challenging. This is bethany now. The 19 year old is thriving, living in a flat in the community with services built around her. It was her case which led to todays report. Her dad, who had to fight to get her moved, says hospital was always the wrong place for her. The environments beth was in in the hospitals, they were incredibly bright, they were full of fluorescent lighting. That fact in itself hurts beths sensory requirements. The noise in those places, with alarms going off, people screaming, the doors constantly slamming, theyre not therapeutic at all. Somebody like beth, with her sensory needs, is never going to get better in an environment like that. Despite promises for change following abuse scandals, the regulator says there is still too much poor care in long stay hospital wards. In some places patients were restrained on a daily basis. And the time people were secluded or segregated for, ranged from two days to 13 years. We found that people, the staff were looking after people in hospitals, often didnt have the and knowledge to understand how properly to use restraint, segregation, seclusion. And i think that often it was a case of not knowing how to meet peoples needs, not understanding autism, not really being clear about what are the triggers and the reasons behind peoples behaviour. So if you change the focus of training for staff, you get the right specialist staff in place, then it should both reduce the need to use any form of restrictive practices, and make sure that people have their needs properly met. The report says there needs to be more investment in the sort of Community Services that bethany now gets. And a minister to ensure a change happens. Alison holt, bbc news. What might have the chance an ounce lighter . Speculation that in tier 2 there will be more than that of 22 support . Is it enough for all businesses . If you take away the pubs and restaurants, we have still got to have the driver, the fuel, all that vehicle for one or two deliveries, does make commercial sense. Explain how tiny your margins are . If there is a small consolation for some of these fruits, for example, in a restaurant in manchester, what happens to you guys . Well, we are stuck with the product. We cant sell it on. We have reached a whole new level in the Pandemic Response and its such a shame. Marcus rashford and all those who have been inspired by his campaign will be so disappointed. And children will be left going without and parents getting even more anxious in the run up to christmas. Can you give us a sense of what it will mean in practice, in terms of some families and their lives . Well, the cost of a school meal per child, £2 30. Over the summer, meal per child, £2 30. Over the summer, one of the things that pa rents were summer, one of the things that parents were telling us in their research was how much they appreciated it. Ithink research was how much they appreciated it. I think they had a voucher for £15. But for some of those families it made a massive difference. I think the real question weve got here is, why are we looking to this measure in the first place . As the government was saying, we dont normally provide Free School Meals in the holidays. There are two reasons. One, the pandemic. We are not in normal times. And the other reason is family incomes are not high enough for some families to be able to get by without this extra support. Can i put to you the government response . They have said, we have been giving substantial support to families and children by providing Free School Meals when schools are partially closed, increasing welfare support, giving councils a £63 million for families facing financial difficulty. The picture theyre painting is that there is sufficient provision already existing . painting is that there is sufficient provision already existing . I mean, the answer is that a lot has been done. And that is really appreciated. The extra money into universal credit has made a big difference to many families. But what was missing right from the beginning of this Pandemic Response was any Financial Support targeted directly at families and children. Without that extra support, families have been struggling. We have been recommending that actually you solve this problem completely by increasing child benefit. That is traditionally what families have, a sum of money to meet the needs of children. Im sure youve followed the debate with interest last night. There were times when it became very heated. And we know how the vote ended up. They were comments by the mp Brendan Clark smith who said that pa rents mp Brendan Clark smith who said that parents should take some responsibility for their children. He said he did not believe in nationalising children. And there will be people who have that thought process in relation to this issue, notwithstanding the campaign by Marcus Rashford, that there is a danger that responsibility here, there is some part of it missing in this equation . Yeah, i heard some of the debate. Such a shame to see it politicised in this way, really, when we are looking at helping some of the poorest kids in the country. For context, to be eligible for a Free School Meal a child has to be inafamily Free School Meal a child has to be in a family that is earning less than around £7,000. So youre earning less than 17 hours a week at minimum wage. So we are looking at really the poorest of the poor here. When they talk about parents taking responsibility, of course it is for pa rents to responsibility, of course it is for parents to provide food for their children. And thats one of the most important part of parenting, choosing, shopping for, making family meals, but when family incomes are too low, what the secretary of state did not say was that over the last ten years we have lost almost £a0 billion from the Social Security budget. So putting a bit back is appreciated, but when you see an elected families have been, while there has been money found for all sorts of schemes to support business, then you think, well actually, something is going wrong here when you cant look after the most Vulnerable Children and fulfil our moral responsibility. Louise, people often talk about disconnect between politics and people because mcgree lives. When you look at the numbers. 322 votes to 261. Is that what you think as well . There is a disconnect between what the politicians now and see and wa nt to what the politicians now and see and want to act on, and the reality of some peoples situation . I do understand that between two particular areas that might be in tier3, there might be different variations and that makes it even more important for people to go and inform themselves. The idea of three clear tiers that people could understand would have been discussed with them. We have helped people with rents and given money to local authorities to help with hardship funding where families have particular difficulties. While we recognise the concern and the places concern is coming from, we think there is a different route by which we can assist those families and that is what we are trying to do. We dont decry the motivations and we hope our motivations wont be decried either in return. These are really tough decisions that are having to be made across the whole economy and across the whole population and we think this is the best route to get money to the low paid. People from black and minority ethnic backgrounds will get workplace risk assessments to offer extra protection from coronavirus thats one of the measures in a government report intended to address the higher prevalence of the virus among bame communities. But some medical experts say focusing on ethnicity isnt the right approach. Our community Affairs CorrespondentAdina Campbell reports. As a family we were all devastated for many months. And as a family we think a hospital or government could do a bit more after my brother passed away. This doctor was a specialist in neurological rehabilitation at the Royal Berkshire hospital in reading. His family says he had concerns about the lack of ppe while at work. He died in april after catching the virus. An investigation into his death is ongoing, and Royal BerkshireNhs Foundation trust says it is waiting for the results of the report. Today the government has published new findings on covid disparities and is giving more details about how it is going to improve Health Outcomes for people from ethnic minority backgrounds. They include recording ethnicity on death certificates in england as a way of tracking which groups are more at risk. Its also going to spend more than £25 million on a new Community Champion scheme to help protect the most vulnerable. And workplace risk assessments for everyone to improve Health Conditions and reduce the number of deaths. So why are people from ethnic minorities more at risk of dying from coronavirus . They are more likely to live in poorer areas, with limited access to the health care they need, and because of this they may be in shared housing, meaning there is a greater chance of catching the virus. And we know people from disadvantaged backgrounds are more likely to have underlying Health Conditions linked to higher coronavirus death rates. But one government adviser who has just been appointed to look at this issue more closely says we shouldnt solely concentrate on ethnicity. So my suggestion Going Forward is that instead of using ethnicity as the criteria by which you see whether someone needs extra testing or extra attention, is look at the actual risk factor thats causing it. Look at overcrowded housing, their deprivation, their occupation etc. And then you will capture everybody. Those behind the report though say it will lead to a clearer understanding of how much people from ethnic minorities are affected by coronavirus, and help protect those most at risk. Deprivation is one of the risk factors we have taken into account. We are doing this across the population for those people that we believe are vulnerable. I think that we have a responsibility to show that they are not being left behind. And as a black woman, thats something that i have personally taken an interest in. But for some Health Workers who are seeing first hand the impact of this virus in local communities, todays report doesnt go far enough. Chris is a gp and also caught coronavirus earlier this year. This seems to be a proposaljust to gather more data and give the impression of doing something rather than actually doing something. Businesses in parts of england under the highest tier of restrictions know they will get some support. The government pays 67 of workers wages up to a maximum of £2,100 per month. But firms in areas under tier 2, where pubs and bars can stay open until 10pm, are facing what some call the worst of all worlds a fall in takings without enough support to stay afloat. Greater manchester moves into tier 3, the very high covid alert level, from tomorrow, South Yorkshire on saturday. Opinions there are divided. I think its a good thing. In west yorkshire, parts of the north east of england, nottinghamshire and other areas in tier 2 which are in discussions about tighter restrictions, the chancellor has been under pressure from unions and business groups, to plug what they see as a gap in the governments approach. Just weeks ago, rishi sunak announced his winter economic plan, including wage support for workers in what are deemed viable jobs, whose hours have been cut. But there are warnings it may need a rethink as the virus has continued to spread and the economy faces further strain. Jonathan blake, bbc news. Lets speak now to our Political Correspondent helen catt. There will be a lot of individuals and businesses as well listening very carefully to what rishi sunak is saying and, from what we know, i know we dont know the detail, there is going to be something put forward. Yes, what we are expecting is that there will be some more support for those businesses you heard about in the peace there in tier 2, who are being particularly hit by rules like the ban on household mixing indoors, but havent yet been told to shut by the government. So we are expecting that is where the support is going to be tailored, in areas currently under those sort of restrictions in places like york, birmingham and essex. The treasury is always really tight lipped about the sort of statements, so we dont know exactly what the chancellor is going to announce but options we understand that have been discussed are grants issued through local councils and also perhaps the government paying more of workers wages in tier 2. At the moment, they pay 22 of workers who have been asked to go part time, the government pays 22 of their wages so one the government pays 22 of their wages so one option discussed is a thing about. Here is what the policing minister Kit Malthouse had to say. There are lots of areas where, you know, people are filling the inbox of their mps with distress and concern, that is all being fed in and hopefully this afternoon we will see the chancellor trimmed the sails to make sure we are getting the right balance in terms of the support we are offering across the economy. So i think one of the important things we are looking out for is what is the scale of the support that is offered and im so sure labour will be looking for quite a large uplift bear, and, also, weve heard a lot about the chancellor being agile in his approach to this and downing street has said as circumstances change, they have to be able to adapt but they have to be able to adapt but the chancellor has been in the common several times tweaking his plans that were only announced last month, so it is a tricky balance between being agile and adapting that looking like it has a strategy. That was definitely the lowest point. It was bad hearing it, and dealing with it, but coming home and telling the children. I asked the doctor, will i be here in 12 months time . He said, well, maybe five, six months. And to be told news that you werent expecting, bad news, and i had all of that information out of the blue and. On your own. It was hideous, actually. Gabriel and kathryn are both facing the double whammy of cancer in the time of covid. But, for catherine, covid is far too close for comfort. With an eldest daughter who has the virus, and a younger daughter self isolating upstairs because of the virus, shes about to hear news. Ive just had a phone call from the hospital to say the results have been brought forward to today. Crazy situation theyre in. This is a roller coaster for the whole family. Libbs . Itsjust kind of a precautionary measure. Ive had seven months of thinking, maybe im going to be the one that passes it on to her. Your nerves must be shredded. Its horrible, its awful yeah. For gabriel, a terminal diagnosis during lockdown, farfrom family, has been almost entirely solitary. When i informed my children, their first reaction was, were on our way, but they couldnt, because we were on lockdown then. The thing is about the cancer, at least i was able to go out, whereas, with this virus, i was in lockdown 24 hours a day. But there is one person who saved him from the solitude of the virus. My nurse, hilary, she is in constant touch with me. Its not just to discuss treatment. She rings up, what are you up to . How are you getting on . She doesnt have to ring, but she does. Thats it as we film, the call comes through. Hi. Sorry, say it again. How do you ride a roller coaster of cancer and covid like this . How do you cope . You dont have a choice. You have no choice, you just have to get on with it. Yes. You take each thing as it comes. Have you really worried about your mum . Yeah. Its really, really worrying. She said youre good at hugs. Yeah. She smiles a lot. Just because you couldnt write it. You just couldnt write a year like this. Oh, yes it sounds good. All good. No more cancer. There you go. And do you know, i did almost cry once talking to hilary, because she asked me a similar question, and i thought, you know what im looking forward to . Her ringing me and saying. Excuse me. Term has been watching this morning, because jane got in touch, sharing his story, and he was really pleased, because he wanted to tell his four sons that he loves them and hopes to see them again soon once it is safe to do so. It is so lovely seeing his relationship with the nurse, which is clearly so important for him. And thank you to kathryn as well. We should update you about her situation, because that phone call that she wasnt expecting, the wonderful news, sharing it with us as well. To update you on her circumstances, she had her Reconstructive Surgery only yesterday, and we are pleased to say, this has come directly from her, it has all gone well. Were joined now by professor pat price, who is a clinical oncologist. Shes in cheshire. Good morning to you. We have heard two stories there that are very, very moving, one in a very different place to another at the moment, but highlighting a real problem right now, real concern that people are not necessarily bringing themselves forward and as a result not getting the treatment. Yes. They were very heartening stories, though, werent they . Showing how amazing people can be and the front line staff, and yes, it is worrying, and as the doctor said this morning, and professor cunningham, we do need people to come forward. And i know it is frightening, there are lots of things to be frightened about at the moment, but really dont, you can act. If you have got a lump or pain or you are passing blood, anything thatis or you are passing blood, anything that is not right, breathing, phone yourgp. That is not right, breathing, phone your gp. Remember, even if it is a telephone call, they can help you, and remember, most patients it will be nothing and you can be reassured. And if you do need an urgent cancer referral, that will be made, and just remembered that nine out of ten people with those urgent referrals are reassured and they are fine. So remember, there is very positive here. A lot of the time you will be com pletely here. A lot of the time you will be completely reassured, and if you are not, you have caught it early and usually we will be able to do a lot about it. Please, dont be frightened, you can come forward, you can do your bit. Professor price, such an Important Message and you put it across very well. You have first hand experience, and we know there are practical problems in relation to health care, and it is affecting Cancer Treatments sometimes. What do you see in terms of the units you work in . Yes, i think we see whatjeremy hunt alluded to in your piece yesterday, is we see that the services have been very much disrupted, completely disrupted in march and april time, and they have unfortunately been very slow to get back. As jeremy hunt said, Cancer Services are getting back up, but they are not 100 . But as he so clearly said, to deal with the backlog, they are going to have to be increased by 20 over the 100 for two years, so that means we have got to do so much more. And the problem is, the front line staff are doing herculean tasks, we have seen in your programme. We cant just tasks, we have seen in your programme. We cantjust say to them, worked 20 more, we have got to find solutions, which is why we set up the catch up to cancer campaign. On that note, we know that eve ryo ne campaign. On that note, we know that everyone is looking at these figures, and as the infection rate grows, there is this pause, isnt it . We have a two or three week period, and then we see hospital admissions. How concerned are you about the time period coming up now, going into november and december, about the impact of how the infection rate is now, what we know is coming . We are concerned, obviously, but i think everybody knows, if they play their cards and follow the guidelines and keep their distance in, we can keep the numbers going into hospital at a level that we can manage. As you have heard today, the hospitals, although they are working so hard, they are going to try to keep up the cancer operations as well. We learned a lot from the first time, we can treat Cancer Patients better now, and we know that we must continue, and we have to continue with the services, and we can do. One of the other issues that certainly cropped up early on, in april and may, was nhs staff themselves, and those people who either contracted covid or maybe had a family member, so they were self isolating there are real issues for. I dont know your own personal circumstances or people you work with, if there is a problem in the community, there is a knock on effect about staffing levels. Absolutely. Although may i say, everybody has done so well on the front line, and i think that this is true, remember, we went into this covid crisis on the back of austerity, the nhs had a lack of funding, we had some International Studies out just before funding, we had some International Studies outjust before we went into lockdown. The uk yet again were at the bottom of the league for cancer survival. We were hoping to put this right and then covid has come. There is an absolute workforce crisis, and that cant be solved overnight. But our message is there are Smart Solutions to solve this problem. Just in one example, radiology for diagnosis, i know there is a plan for professor richards with digital technology, from our end in the treatment site, we have set forward a six point plan to boost radiotherapy. The great Technical Solutions that you can solve a lot of these problems, we need Smart Solutions, we can do this, but we have to be given the tools to do it, and that means investment. Professor price, really good to talk to today, thank you so much. An oncologist, some practical experience there, a subject we have talked about before and we will no doubt revisit again, so and we will no doubt revisit again, so important, that first point she made about coming forward in the first place. But, this year, it will be different. With so many remembrance events cancelled, the Royal British legion wants us to remember in our homes and gardens, in our windows and on our doorsteps. Bill is a 95 year old royal navy veteran of d day and the Burma Campaign in world war ii. Most years, he would be out collecting for the poppy appeal. You know, we cant go anywhere, i cant go out. Youre just locked up. It is a bit annoying. So this year many Remembrance Services wont be taking place. What about the idea of remembering in our own homes instead, or on our doorsteps . Yeah, that would be ok, just to remember. I mean, both of the world wars, i think, is something that we should not forget. To mark todays launch of the poppy appeal, the Royal British legion has released these images of volunteers, veterans and serving members of the armed forces. Captain sir tom moore has been an army officer, a successful businessman, a chart topping singer and of course a champion nhs fundraiser during his 100 years on earth. Now he can add another occupation to that impressive cv artists muse. Hundreds of people have felt inspired to paint pictures of sir tom. Before theyre auctioned off for charity, hes been casting his eye over them with the help of our arts correspondent david sillito. Music i was absolutely overwhelmed. So many of them and such good quality. People have so put such great effort into producing them.
