A new case has emerged in california with no apparent links to china. There are two new cases in the uk. Financial markets are tumbling, again. And around the world, everyone is asking, how worried should we really be . According to donald trump, not very. He says america is ready and is downplaying the threat. That, by the way, was a similar to the position taken by president moon of south korea when he said, just two weeks ago, that the virus would soon disappear. His country now has the most serious outbreak outside china. Trying to get a reliable assesment of this threat isnt made any easier when leaders either arent on top of or perhaps transparent about what is going on. Heres what we do know from our correspondents around the world lets start injapan where the Prime Minister has called for all Public Schools to close from monday. The number of cases injapan is now well over 900. Now, most of those originated on the diamond princess cruise ship, which is still moored up down the coast of yokohama, some 700 or so cases originating there. What is worrying the japanese government are the separate cases which are springing up and growing all over japan, but mainly in the Northern Region of hokkaido, nearly 200 people have tested positive. Countries like kuwait, oman, bahrain and iraq have also reported a number of coronavirus cases in the last week have all placed those cases to iran. Traced those cases to iran. The citizens or the residents that were based in these countries have travelled to iran for religious travels, and that is the reason why they are seeing a sudden spike. Saudi arabia clearly does not want to take any chances, that is why they decided to suspend the religious travel at least for the time being. The 11 towns affected by this outbreak are still in quarantine. But places like milan, which are not in quarantine at all, are suffering. Hotels, cafes and restaurants are not doing big business, and the authorities here have issued a video saying that the city is open for business. Schools, universities and museums, however, remain closed. This is a time that is uncertain for everyone, and there are fears that italys economy could be tipped into recession because of this. So thats where we are today. Compared to other recent epidemics, the fatality rate of coronavirus Infectious Disease 19, is significantly lower than other global epidemics. The 2012 mers epidemic killed 34 of people who caught it. The 2002 sars outbreak killed 10 . The 1918 flu had a fatality rate of only 2. 5 . And theres covid 19 down atjust 2. 3 . Which sounds reassuring except the 1918 flu was the most dangerous it killed 50 million people. And thats because it spread further and faster than the others. Which is the big fear about todays epidemic because its spreading at the same as the 1918 flu strain. Joining us now for more is the former director of the us centers for Disease Control and prevention, tom frieden. Thanks very much forjoining us. Can you outline for us, because i think theres a lot of confusion everywhere around the world what you see as the best Case Scenario with this coronavirus, and what you see as the worst Case Scenario . Well we dont yet know how infectious it is and how dangerous it is. At the best Case Scenario, it is not that infectious and not that deadly. In that scenario, we would see clusters or outbreaks that could be stopped after weeks or months, then it would go away. We dont think that is likely any more, because we are seeing this continue to spread in many places, spreading explosively in many different situations, in a church, a shop, or health care settings. This looks pretty infectious. How far and wide it will spread . Only time can tell. But unfortunately we seem to be in the calm before the storm in most places of the world. In terms of how fatal or deadly it is, as you said, it is less deadly than sars, but we dont know how much less deadly. We are seeing case fatality rates, the portion of people who die from it, of less than 1 in parts of china, outside of hubei province. But even 196 outside of hubei province. But even 1 is quite high, if you have something that spreads to hundreds of millions of people. So it is too sooii of millions of people. So it is too soon to say how bad it will be. It is not too soon to prepare for the worst Case Scenarios, because by doing that we can reduce the impact it will have. So theres been some hope that this virus, flute type virus, by the spring when the weather gets warmer would proceed or disappear. At the 1918 flu, the spring outbreak, then it died away in the summer, then it re emerged just as deadly in the autumn of 1918 and into the winter. When will we know whether this is a virus that will go away when the warmer weather kicks in . We will know when it happens or doesnt happen. There is iio happens or doesnt happen. There is no way he predict with confidence what will happen in terms of the weather with this virus. There are other coronavirus strains that have a winter peak and a summer peak. With influenza, we know that in tropical climates, it continues all year long. It is notjust a winter virus. But this is unprecedented, we have never had a newly emerging respiratory virus spreading around the world. We continue to learn more every day and sometimes every hour. The more we learn and the more we know, the more we can do to protect people by encouraging people to wash their hands, covertheir people by encouraging people to wash their hands, cover their costs, people by encouraging people to wash their hands, covertheir costs, not go out if theyre sick, by preparing the Health Care System to search safely, to care for people, and by getting governments to invest. We will either pay out or pay a lot more later in lives and in money. When i listened to the British Health secretary, you get a sense of how difficult every government is finding it. In how to advise people in their own countries, overreaction has its costs. It costs on livelihood and the economy, but you look at the example of china where these extreme measures appear to be working . They are working so far, but what happens when china releases its clamp down on people staying at home . There are hundreds of millions of people who have stayed at home for the last month. You cannot continue that forever. That still has value because it buys time. It buys time to prepare Health Care Systems to invest in new programmes, to train people and educate the public. But that is only useful if you use the time youre buying to prepare more. That is why i think it is so important that we invest in preparedness now not only in things we can do today, but also the us, there is a supplemental budget appropriation. That is very close to important to watch. Globally, we need to think about why we are so underprepared. It is inevitable that this will be a pandemic and that there will be future epidemics and pandemics. What is not inevitable is that we continue to be so underprepared. With this, you dont have that because kids are recovering. It is sort of like not a u shape, why do you think that is . We dont know, there is some speculation in china that older people, particularly older men who smoke a lot, have compromised lungs already by the age of 50, and that might be why you are seeing that higher mortality rate related to china. In china, women smoke less which is why they are getting killed less by it as well. We will have more on the coronavirus later in the programme. For decades, uk industries could sell their products to countries within the eu with no restrictions. In return they accepted eu standards and laws as well as Free Movement of people. Once brexit takes effect at the end of this year, that will change. But by how much will depend on trade negotations that start next week between both sides. Today, the uk published its strategy following the eus earlier this week. There is some broad agreement. Both the eu and uk want to do a deal which would continue trade without tariffs. But there are significant disagreements, too the uk says it wont continue abiding by eu rules on support for industry. The eu meanwhile has rejected the uks demand to have the same kind of access as canada does to european markets. Lets speak now tojoe owen, Brexit Programme director at the institute for government. He joins us from westminster. Ive had a quick look at what is in this publication today, and the Prime Minister is pretty explicit that if it doesnt go in the right direction or he doesnt get what he wa nts, direction or he doesnt get what he wants, hes ready to walk . There wasnt a lot in the content of the mandate that was new, it had been direct fared over the last few weeks, via his europe adviser, david frost. But it was saying if we havent made progress byjune, the uk would be prepared to walk. I think it is slightly over blowing to say the uk is therefore really dialling up the pressure on the eu for that meeting because the reality is they would need a deal done by september just to finish is they would need a deal done by septemberjust to finish the ratification before the end of the year. So it is kind of a statement of fa ct year. So it is kind of a statement of fact to say that if they havent progressed through the noncontroversial issues by that point, it looks like we are probably heading towards that direction. Sol think theres a lot of posturing here because the uk governments reveal preference has been to say they are prepared to walk away. But a lot of it actually is probably just stating the obvious. Just to clarify, do we know if a no deal at the end of 2020 would be the same as ano the end of 2020 would be the same as a no deal in the autumn of last year . Would they look the same and impact the same . The short answer is probably yes, there will be similar. There will be areas covered by the Withdrawal Agreement that will be the exception. The rights of brits in europe and eu citizens in the uk would be protected, the uk would continue to pay their outstanding budget constitutions and their debts that was agreed in the financial settlement. The big difference really is what happens on the island of ireland where no deal wouldve been felt most acutely. That shifts the checks that wouldve sat on ireland undera no the checks that wouldve sat on ireland under a no deal last year which now exist in the irish sea. What happens to dover and calais, law firms, Financial Service firms, all those implications would be the same in december largely as they we re same in december largely as they were for october last year. The difference being theyve had a little bit more time to prepare. The 1. I little bit more time to prepare. The 1. 1 would make is that actually, a lot of what is proposed in this deal is not much more of an advantage on no deal the one point. Areas where the uk say they are happy to pay for this on wto rules in this area. The Withdrawal Agreement is legally binding, we signed up for that, but the political declaration was more ofan the political declaration was more of an aspiration . Thats right. I think the political declaration was always seen more in the eu as a tool to help the uk sell the Withdrawal Agreement domestically. If you rememberat the agreement domestically. If you remember at the beginning there was this big argument about sequencing and the uk paying its money, but what would he get in terms of the trade agreement . Thats what the political declaration was before, sketching out the future, it was never meant to be a solid framework. In a rare officially approved interview with this doctor, as well as videos provided to us, the bbc has been given unique access to a man on the front line of a war. One he says is being won. The patients were pouring to the hospital. We do not have enough beds that are available for everyone. I think things are holding up better right now. Wuhan shows what the virus can do to a health system. The thousands of extra beds have begun to make a difference, but there are still major challenges. Im not satisfied with the mortality, its too high, for patients with severe symptoms, the mortality is even higher than sars virus. That is what we should address right now. This is dr xiejiang dealing with one of those cases. With the virus attacking the lungs, no city in the world he says would have enough specialist ventilators to keep thousands of such patients alive. Even doctors have died, including this man, one of the first to try to warn about the dangers of the virus, only to be silenced by the police. I was very sad when i heard this kind of news. It was really a big blow for us, for the whole medical staff. Do you think china should learn lessons as a result of his death . Absolutely. Information disclosure is really important. I lost five patients in one night, can you imagine that . I should warn the rest of the world that you guys should take care. Dont neglect this disease. At the end of another night shift, this time dr xiejiang sings a patriotic song. He believes china will win this fight but it is when the rest of the world may have only just begun. John sudworth, bbc news, beijing. Now for the first time, a patient in california has contracted coronavirus with no clue as to where it came from. The patient didnt travel anywhere known to have the virus and the patient wasnt exposed to anyone known to be infected. The governor of california, gavin newsom was just speaking about the situation. We have been in Constant Contact with federal agencies. We have history and expertise in this space. We are not overreacting, nor are we under reacting. To the understandable anxiety many people have as it relates to this novel virus. Lets cross now to capitol hill and speak to democratic congressman from california, raul ruiz, who is also a doctor. Well, were looking at this as something important because most of the cases that have been diagnosed here in the United States are from travellers from outside of the country. The fact that this is to an unknown transition which very well may have been a person to Person Transition where we have no clue where they got it from is a new episode of the virus spreading within the United States. So thats why its important that they do the Contact Tracing as rapidly and thoroughly as possible. I guess this is why the who said they are in this decisive phase of the virus and government have to act swiftly. Do you think the Us Government is acting swiftly enough to try to contain the outbreak here in the states . Contain the outbreak here in the states . Well, heres the good news. The good news is weve only had 16 cases. Theres been no death and theres possibly three person to person transmissions, so right now, the damage is very, very low. However, what concerning is that its not a matter of if, its a matter of when this virus spreads. When you hear makes messages and discrepancies within the administration from the leaders were supposed to be in charge, it doesnt bring much confidence in their ability to efficiently prepare for this. Mixed messages. If we dont explain to the American People what to do in case this get out of control, then were not really preparing the American People. So what we want to do is to ensure that people understand that this virus, we can prevent this virus from spreading if you wash your hands repeatedly. Dont sneeze or cough on people and make sure you keep an eye on the most vulnerable, which are those with illnesses, those that are the elderly, and start making local plans within your homes, within your schools, within your counties, preparing for the worst but working for the best. Thats the motto, thats what we should be aiming for, and lets get it done. This is becoming intensely political now. We have Mike Bloomberg out with a new attack ad. Hes criticising the president. The president in turn, he said democrats were trying to use this to attack him. At some point in the next few weeks, theyre gonna have to agree a budget in congress. Can they do that . Can they get the president to sign it was green yes, i believe he will sign it. This my prayer was to help them get their act together for the health and safety for the American People. Whats important is the health and safety of the American People. However, it is important to point out the facts that president trumps proposed budget cuts 700 million in the cdc. Hes fired experts in this realm, hes left very important pandemic experts within the National SecurityCouncil Positions a very vacant, and he expanded the office of pandemic response. These are decisions that have not helped in this case. What we need to do is we need to be straightforward, we need to provide a robust funding package, but you cant just find this function. You need to find a functional system thats coordinated, thats efficient, that educates the American People, is honest, consistent, that is not alarming but at the same time as smart in preparation, because when you try to figure it out during an epidemic, youre too late. Right now, we havent reached that threshold, but we very well could, so lets take the time to prepare in our homes, our schools, our governments, our states, to make sure that were prepared to minimise or innovate the amount of harm this will do to the American People. Its good to have these conversations so that we are prepared. Thats where i am at right now. Looking around to see the vulnerable individuals in the communities that will need special attention. Its time to talk to the Health Care Systems in the community to determine if they have a plan for quarantine, for identification, isolation and containment within your location.
