Coronavirus continues to dominate the news agenda with several European Countries announcing their first cases of the infection all appear to be linked to the growing outbreak in italy. In tenerife, three new cases of coronavirus have been reported at the hotel where britiish holidaymakers are among hundreds of tourists ordered to stay in their rooms since yesterday. An italian doctor, his wife and two friends who were staying in the hotel have tested positive for the virus. At least eight schools in england and at least one in Northern Ireland are known to have sent pupils and staff home after they returned from visiting northern italy over half term. Austria, croatia and switzerland now all have cases of the virus with spain also confirming their first mainland case following those in the Canary Islands. In south korea 284 new cases have been confirmed today, bringing the number of confirmed cases to 1261. And the first case of the illness in latin america has been recorded in brazil. A man in his 60s has tested positive after returning to sao paulo from a business trip to northern italy. Well go live to our correspondents in tenerife and italy in the next few minutes but first this report from andy moore. Its not what you would want to see from the balcony of your four star hotel. Instead of a fleet of taxis, theres a fleet of ambulances waiting outside the Costa Adeje Palace Hotel. Hundreds of holiday makers are on lockdown, though the spanish Authorities Say some who arrived after the first person became ill may be allowed to leave soon. That original case was an italian doctor. Translation this citizen arrived on february 17th on a plane. He got on a bus, he was part of a group of ten italians, and they went to the hotel. He has spent all his time in or around the hotel. The Authorities Say other guests at the hotel should be allowed to leave eventually if they test negative and have no symptoms of the disease. But thats a process that could take some time. If they do not have symptoms, according to our protocol, they ought to be free to do whatever they want. But we will need some days to test all these people. Coronavirus has now got a foothold in europe, and italy is the worst affected country, with more than 300 cases and 11 deaths. In milan, the cathedral and many museums have closed. In the uk, this private school in cheshire has shut down completely after pupils and staff returned from an italian ski resort. And this school at nantwich has closed its sixth form for a similar reason. The official advice from the government is that anybody returning from the italian towns in quarantine should self isolate. Anyone returning from the wider region of northern italy who feels ill should do the same. Andy moore, bbc news. 0ur correspondent fiona trott is outside the Costa Adeje Palace Hotel in tenerife. As you can see, the police here are still manning the huge cordon around the hotel complex. Most of them are wearing face masks. 0vernight a couple of ambulances have arrived and specialist emergency vehicles. Police tell me they are here on standby because as we now know, four people are infected. The doctor who is holidaying here with his wife, who is from italy, and two other people they were travelling with. I understand they are being treated at a hospital in the north of the island. There are a couple of tents outside the main entrance. They are being used by medical staff as a temporary station because they have a huge task at the moment. Looking at the size of the hotel, there are a thousand guests and staff staying here at the moment. From what i understand from guests i have spoken to inside the hotel, those medical teams are just going from door to door checking their temperatures and giving them advice, giving them a facemask. Thats the work thats going on inside the hotel to monitor patients. But its very tough for them. Some of them now have not left their rooms in 2a hours. They are beginning to wake up and i have spoken to some of them this morning on the telephone. They just want to go downstairs and have something to eat and drink. They cant. They have been told they have to stay inside. The spanish Health Minister is trying to ask all the guests inside the hotel to be patient. Since they were in the hotel, all the people that were in this hotel have been tested and examined by our health teams. If they have symptoms, minor symptoms, then we will apply our protocol. If they do not have symptoms, according to the protocol theyre going to be free to do what they want, but some days we need to test all the people. Some positive news this morning. The president of the Canary Islands has says he hopes around 100 guests from this hotel can leave in the next few hours because they arrived on monday and would not have come into contact with the infected people. Fiona trott in tenerife. Professor paul cosford is medical director for Public Health england and gave this advice. If you are in the hotel in tenerife then you should make sure you comply with what the spanish authorities ask you to do. Thats obvious for anywhere. At the moment we are monitoring the situation very carefully across anywhere where there is an outbreak of the infection to make decisions about whether places need to go on to the list of countries we are most concerned about. Our key advice about those countries is that if you have been in one of those countries and you are returning to the uk and you have symptoms of concern, you are coughing, you have a fever or shortness of breath, then telephone nhs iii and we can make sure you are assessed and tested if necessary. We are not advising a ban on travel to the Canary Islands or to tenerife. But what we do know is that although the risk of getting severe complications from this virus are much lower for children and healthy adults, for older people and people with serious underlying illnesses, they are more at risk of those complications. So people will need to make their own individual decisions about whether to travel or not. A professor from public a professorfrom Public Health england. Doctor Margaret Harris is from the who and explains why it is not described as a pandemic. We talk about a pandemic to describe what is going on. And we talk about a pandemic what it means we have outbreaks in all parts of the globe but also sustained Community Transmission first up as you are effectively reporting, there have been cases reported in many countries around the world but remember, these are cases that have been identified as people arriving in the country and they have not shown onwards transmission. This is what we are seeing in the uk at the moment. There has been very good work from Health Authorities and by the community because people are saying, i have travelled, i am not feeling well, and they are identifying themselves as having a case perhaps. That means you are identifying people very early and it gives you a very good chance of stopping onward spread. Italy is a concerning outbreak but the authorities are doing everything they can do to stop further spread elsewhere by locking down the communities and asking people to self isolate and stay put. Naturally, its a place eve ryo ne everyone has been visiting, so when people return there is that concern that they need to be aware that there is the possibility. The highest number of cases currently in europe is in italy. That country is linked to other new cases in europe and the first case in brazil as well. We can talk now to our correspondent bethany bell who is in milan for us. Some more information is emerging today about why italy might be a hot spot and why the number of cases has grown rapidly there over the last few days. Those cases are now over 300 that have been recorded here. There are 11 people dead. Of the dead we understand they are elderly people, people with pre existing medical conditions. Its hitting them the hardest. Doctors are saying you see a similar pattern with things like influenza. The italian Prime Minister has suggested that a hospital in an area just south east of milan may have not followed the correct protocols when they handled the initial cases of this outbreak and there has been something of a backlash with local politicians about that, but the Prime Minister has said that might have been the reason we saw a spike in cases. There are communities south east of milan and a couple in veneto where people are in quarantine and people are asked to stay home. The police have erected roadblocks around there. The Prime Minister has said he is confident the contagion will be contained soon and he spread the message to the world that italy is safe to visit. You mentioned roadblocks around those 11 towns at the centre of the outbreak. Is italy doing anything more beyond that given that it seems to be exporting the virus to other parts of the world and other parts of europe, and brazil, as i mentioned. Are any other steps being taken . There are a lot of other steps being taken first appear in northern italy schools and universities are closed, museums are closed, and even the milan cathedral you see behind me, the duomo, one of the biggest Tourist Attractions here, has been closed because of fear of transmission. If you come into an airport here in italy then they will take your temperature as you arrive. You see a lot of people in the streets wearing face masks. There has been a run on things like hand gel. People are aware of the measures being ta ken hand gel. People are aware of the measures being taken and they are waiting to see what happens. Yesterday was the climax, or should have been the climax, of the venice carnival. Those celebrations were cancelled so things have been happening. Health ministers from a number of italys neighbouring countries met in rome yesterday and made the decision not to shut italys borders. They said that would be a disproportionate reaction. They promise to keep in close contact and would give information to travellers and they would also be in close contact about controlling how the best we are trying to control the infection. Im able to observe these patients and prioritise them and make doctors aware that i think these patients should be seen quicker than what they would normally be if they were left out in the corridorfor any length of time. A Royal College of nursing survey suggests these are commonly used now, with nine in ten emergency nurses in england reporting it exists where they work. Joining me now is Harry Quilter pinner, Senior Research fellow at the institute for Public Policy research, where he heads the health and better care programme. In a survey they have done they talk about 49 of respondents say the term corridor nursing is formally used in their workplace. Is that the new norm as far as you are concerned . I think increasingly it is, yes. This has been one of the worst performances for a e and access times on record. I think particularly concerning is the numbers we are seeing in the summer months. Last summer the performance we saw was equivalent to previous winters. What is that is suggesting is we are in a permanent winter crisis in the nhs. We talk about winter pressures a lot but more and more that pattern and pressure is seen right through the year and not just in the coldest months. Exactly, and that suggests to us there is a structural and systematic mismatch between the amount of demand, the number of people who need care, and the capacity of the nhs to provide that care in a timely manner. What difference is that making to the quality of care, do you think, and to the stress placed on health care professionals, nurses and doctors . 0verall, what we have seen is that outcomes for most areas of care have been improving in the last decade. But access has deteriorated so people is level of stress, the amount of time they wait in corridors has got a lot worse. And to get out of ambulances into a e as well. The latest figures are pretty bad. Exactly. You make the point about nurses and when you speak to staff they say it is getting harder and harder to deliver the care they wa nt to and harder to deliver the care they want to give and they feel increasingly under pressure. The problem is, thats a self fulfilling crisis because nurses and doctors then say, i dont want to work full time, i dont want to work in these conditions, so they leave the sector and we have an even bigger crisis and we have an even bigger crisis and the capacity problem gets worse. The Royal College of nursing is talking about a need for an in depth examination of the primary care sector, of creating more of what they call the neighbourhood nhs with more focus on gps, pharmacies and so on. You talk about structural problems. Is that where the answer lies . I think there are two parts to the solution, two priorities. The first is exactly as you mention, the primary care crisis. If you cant see a primary care crisis. If you cant seea gp primary care crisis. If you cant see a gp then you are likely to turn up see a gp then you are likely to turn up at a e instead and we know patients are doing that. Particularly if you have a long term conditions, if you are not supported by the gp and the team of professionals around the gp, to support yourself properly in the community, Long Term Care plans and supporting the community, then you end up at a e, so part of it is about dealing better with that in the community. The people who should not be in hospital ending up there because of issues with the primary ca re because of issues with the primary care its mismanaged care, effectively. The second point is about discharge. We are seeing the length of stay in hospital going up. When you are in hospital, the length of stay is going up and the root cause of that is there is not enough capacity in the Community Sector and social care system so when people move out of hospital they are looked after. A survey has shown that 30 of beds in some acute hospitals have been taken up by people who are medically fit. They could be in the community and back in their home but u nless we community and back in their home but unless we put a package of care around them and support them with a social care system, we cant discharge them from hospital and that creates problems when youre trying to move people into hospital through a e. So you have the situation of hospitals trying to deal with patients who we shall call medically unfit but they are being squeezed at either end. Exactly. We need investment and reform in both primary care, to prevent people turning up at a e in the first place, but also investment in capacity in social care so we can help you move back into the community when they are well. And finally, what will the government say about its plans for the nhs, social care and so on, are they in any way matching what the sector saysis any way matching what the sector says is needed 7 any way matching what the sector says is needed . We have what is the nhs long term plan, put together by the nhs and the government, which is a very good plan, setting out all the measures we have talked about. There is a big question about whether they they can deliver that and in particular workforce constraints and delivering the staff to deliver that. In social care, we have had decades of promises to solve the social care problem and get a proper long term funding settle m e nt get a proper long term funding settlement and we still dont have a solution. The government has committed to cross party talks and they say they want to make it a priority but we have heard this before from a succession of governments. Until we get real evidence that there will be more money put into social care and a long term solution, we will not solve this problem. Two severe danger to life flood warnings are in place in shropshire for the river severn at shrewsbury and ironbridge. The indian Prime Minister Narendra Modi has appealed for calm after the capital delhi has suffered some of the most violent sectarian clashes in decades. The violence which erupted on monday during clashes between supporters and opponents of the new citizenship law has killed at least 20 people, and more than 200 others have been wounded. The Prime Minister Narendra Modi in the last hour or so tweeted, appealing for calm and saying he has taken stock of the security situation with officials. But a lot of criticism of how all this has come a bit too late. There has been a lot of criticism on how this was not addressed by either the police or the government authorities much more quickly and they did not make any attempt to quell the violence immediately. But we are not putting advice out to say dont travel to italy. John holland kay is the chief executive of Heathrow Airport where more than 200,000 people arrive or depart from every day. He says the airport is taking extreme precautions, but has to stay open to passengers. We have to be responsible with this whole coronavirus outbreak. Were monitoring the situation very closely. We have to balance between keeping people safe and of course, keeping the economy moving and keeping heathrow flowing. But we will always start with taking the advice of Public Health england. We need to listen to the experts on this. And so we have been implementing our plans according to the guidelines, and that has worked very well so far. Weve also gone beyond thatjust to make sure that we make it as easy as possible for people to say safe. We are deep cleaning the airport so that handrails and things that people might touch will be as clean as possible.
