And then Claremont Yeah good good paying you not to refer patients to hospital no they're not fortunately if they were would you take that cash and think fabulous Would you think something is deeply a ride with this idea I think it stinks I think it's an appalling idea it's a perversion of taxpayers' money it's unethical and I think most patients would be very concerned to think that their g.p. Was behaving because it was in their financial interest to do so I think that's dreadful because I'm a human Dr Reichman that when you are therapists into a specialist in a hospital you're not doing it for a lull and take the for a job you're doing it because you assume that that's what is the right course of action for that patient right well that's what you should assume and that's the case for most doctors and the idea that you now think there's something in it for me because I get money by missed referring you or not referring you is absolutely appalling sadly that's what the n.h.s. Is coming to but on a Friday most of us would have nothing to do with it and tell me about when a g.p. Makes the decision to refer a patient to hospital why do you do that on the basis of want. What either because they need something I can't. Somebody needs a heart transplant but we don't do that in the surgery so they have to go somewhere else or because I don't know what's wrong with them that's actually quite rare or because they need a technical facility that the hospital provides and we don't yeah all 'd because the patient. Appears to need a consultant opinion to satisfy them. That's actually quite unusual people think it's very common but actually it's quite unusual to refer only to an agreement between the patient and the g.p. It's not I decide it's the patient and I between us agree that that's the best course of action for them but once we've agreed I wouldn't want them thinking that I was then going to change my mind because somebody turned my head with cash. What is the purpose of the someone overseas to save money because the savings of easy accrues to the n.h.s. Not the savings are paid is cash back to the g.p. And are not going to add to that the pressure of anybody ever thinking that I was on the take as a result of not referring you for something let me also you know you say sometimes when you're with so you feel sort of uncomfortable because you obviously realise resources is stretched and you author the stretching them but surely that's not your job to have to feel uncomfortable you're not doing it's superfluously his league you're doing it because you think that person as you still needs an extra opinion or needs a procedure or an operational something that you yourself can provide Why would you have to feel uncomfortable because the patient will wait such a long time for a very inadequate service badly managed and we already have that where I practice in a variety of facilities particularly the community services which are pretty far in the pirates on it and it's not because the people doing it a bad it's because there's not enough of them so the service is so poor that to deliberately refer yet another person is going away a year something by also quite a wakeful seems to me to be cruel it's better to tell patients. You know I might refer you for this but you know going to get it so we you know maybe we should think of another strategy that they're giving you know you all if you would mind of a service that you might reluctantly refer someone for knowing that it would be at least a year and even then probably shambolic what sort of the thing physiotherapy would be the obvious one district nursing services are stretched so thin.
