That translates to in real life that is approximately 40 hours a week now when I was working full time in a hospital because I go from allowed to do 40 hours a week there are no place to prevent to keep a face there is nothing to limit my workplace as a general practitioner and I think this irresponsible irresponsible phrasing and wedding by the g.m.c. And then all the reports that come out in the eighty's Since then don't seem to call come to the fact that 3 days does not mean. 3 times 8 and a half hours it's a lot more. So as the b.m.j. On paper when we're working out how many sessions we're covering how many days of appointments where offering all patients we have enough G.P.'s we are all very unusual in that respect nice practices struggle to recruit doctors they struggle to recruit masses they are using other health care professionals and they are an asset to us but they can't do the same thing so seeing a paramedic for a condition may be completely appropriate relevant but it's not the same a seeing a g.p. That paramedic won't be able to do everything that a general practitioner candy which leads to more pointman being generated there is a big push for us to use a wider workforce there is some recognition that G.P.'s are betting out on to think of hundreds of colleagues who have have to reduce their sessions in order to protect their own health and be able to actually go to work I've not been in. General practice for that long and I get worried on some days about the number of decisions I've had to make and the number of decisions I've got to make while going through my letters my reports and making sure I don't make a mistake because the results of that could be devastating and we ask what kind of patients are at risk risk is loaded less outside what kind of patients with what kind of sort of presentations are at risk of forming you know affording between the gaps here Adrian it is every single person I encounter because a final 20 fatigued from decision making and that's different from being physically tired I think most of us understand what it means to be physically fit see the decision making city make sheep out everything whether you've called the patient by the right name I mean that's a big deal actually. But all the way through to my assessment if I see a 5 year old with a sore throat I still need to be confident and happy that sending that child with the without medication if it's needed is going to mean that that child is still say and that child is going to get better and still be had more of it and that's a sore throat if I have an older person with a complex medical history if they've got 5 or 6 big medical problems I need to take every single one of those into account with every decision I make even if it's something that's relatively simple like giving them antibiotics for a year an infection and all of those decisions we do we're making hundreds of these decisions every hour and it's exhausting and once you start doubting yourself once you feel unable to get the appropriate amount of attention to each one of these decisions it takes an emotion it takes a physical toll on you but it's the emotional toll which means that we are recognizing that it's not safe for us to be doing. I think we need to cut back fortunately a lot of G.P.'s are in a position to be able to cut back about the end result is that we have to view a g.p. But the time I got seeing me as an undergrad I was drinking very heavily out a degree of tolerance and probably dependence and that led some health problems and a kind of white cop call when I kind of had those health problems so I didn't drink for most of my twenty's and in my thirty's I kind of went back and said You know I can probably drink moderately and have been able to stay there for a while so I mean there are many people I know as I'm sure you know will shake their head and you couldn't yourself if you can do it you can do it but I think you can do it ha. It's easier to stop than to moderate definitely you've got good you've got one decision to make and hard though it is you've made it so if you're moderating and trying to drink less then you you've got a 1000 decisions to make you know every day every you know every week but if you got it takes on this what your thoughts on this do text I 5 o 5 I would like to like to hear from you so if you got any advice through and everyone's got a slightly different drinking story right now yours is particular but so how do you drink less Well I certainly agree that for most people particularly if they're kind of early stage in reflecting on their drinking or thinking about or making changes to it you know there's so many things that you're going to be used to doing or triggers or problems or expectations from other people that might make changing that kind of drinking behavior difficult. I think one thing to know is whatever your goal or changes over time it does become easier sorry but sticky at the start if you're not drinking at all that is a very clear boundaries know where is where I feel whereas if you're trying to moderate and not kind kind of can kind of open up all kinds of tricky aspens I think one of the key things is if you are attempting to Mordor right to be really clear about what moderation actually means to you is it within the recommended guidelines is it to have this amount of to have a certain amount of drink per occasion how many days awake I think is good evidence can probably back me up on less than having a clear kind of goal or or limits or and strategies to achieve is is one of the most helpful things you can probably Day So do you decide to have days off for example Yes absolutely so when I decided I would you know attempt to kind of moderate drinking relationship in direct contrast to the very heavy drinking I deliberately proceed on a set up a number of kind of rose for what that was and and Yes certainly 3 or 4 alcohol free days a week was is is. So I did what you know I thought was an expected thing and that was just to draw a keyboard on the paper and start practicing from there but how do you know if you're hitting the wrong notes Well I used to relate the. Notes that I was you know the shapes of my my hand positions on a paper keyboard and relate them to music theory 2 Tauriel cells watching on online. And I was just beginning to associate the sound of what the instructors were doing on the computer with the shape and the movement of my hands on the paper and just as I was beginning to do that I would do it just by repetition be listening to the same video you know over and over again until I could begin to hear you know the sounds that would be created in the cable just on my head. And yeah it was just over time I began you know what was just an association really began to become an ingrained part of practice so it's not so did you get your hands on a proper keyboard then to do You can't turn up your grade one exam with a paper piano well if you got a very sympathetic examiner. Yeah it was 2 years later so I did my my 1st my 1st 3 exams I would park as a home on piece of paper and I would go to my my piano lessons eventually when I started taking them on our part to some on a real piano. But after you know mom saw it really committed committed to this she borrowed friends you know money from friends and family and. You know it is really you know an effort you know I have a lot to thank a for. What was the inspiration what made you want it in the 1st place. Well really it's a bit dull to begin with you know as a long show you do a long we don't. Just prog on you know I had to go to secular school. And everyone seemed to have their thing you know people or small or tea or theatrical I didn't really have a thing and it was just drawn to you know my party school was. Really really lucky to have a fine teacher there who led music in assemblies and she played very well you know on a piano in the corner of the Assembly Hall I think I was just drawn to that. But it was only after I really started listening to music online and you know getting into the pieces of plane I really really fell in love with it and let's just listen to you play a bit of Schubert. Made any mistakes can you and I think you get the hang of this if you got your grade 3 yes. It sounds like you had so did you gain anything from learning the why you did what if you'd been presented with a grand piano on the day you on the day you decide you wanted to play would you be any better of a player now you know what I'm really really thankful actually you know I started the way I did because looking back it meant that you have to think so much harder about each of the notes you're playing each of the movements that you're making and you have to make real conscious efforts and in decisions that otherwise you know really easy to take for granted but it means now it's something that I think a lot what I practice now and you know real concerted. Decision making really. But also from a theme theoretical point of view and thinking you know because I began by just looking at each of the building blocks of pieces of music and that's now something I take forward into into anything I play now really so now you're a concert pianist you earn money doing what you do you play that right so yeah yeah nice to drink before you know what I'm trying to allow you to operate your story a little brandy or something I really can't I really talk about after Yeah yeah so as though you'd be careful you download Claire's app and you'll only be having one I promise to be honest that's the thing often when I play you get given a glass of champagne you know one entry but it means I'm just.
