I think it's important to understand that we as much as possible we have a transparent conversation around this with patients and we obviously do need to have more open conversations and say look these are medications that are great for short term benefits but perhaps not for long term however each patient is different so one size doesn't fit all so I certainly cannot say that medications for 20 days antidepressants are going to have desired effects in all people equally we need alternative therapies such as counseling such as exercise therapy which is something that we don't have as much as we need and the government needs to think about investment around these alternative therapies in making these provisions more accessible in the community and supporting G.P.'s but we also hear that G.P.'s are overworked not in any way saying G.P.'s are doing the wrong thing when they when they prescribe they obviously are there to help people but there is a feeling that econ go in if you're already on something perhaps it gets a repeat prescription more easily than you should be able to so some of the bad occasions such as cocoa Tamal which is an hour which is code in Paris a to move is available over the counter as well which makes it easily accessible patients to get access to these medications the high doses come to G.P.'s. And in relation to deprivation clearly you know people are living in more deprived areas they may have high rates of these conditions facing more problems and also may have less resources to be able and options to overcome the conditions without medication so but you know this is all a very complex thing we're very cane that there isn't one source single answer to this and you know G.P.S. On plane clearly clearly clearly has a role well we kind of sleepwalking into an opioid crisis in this country is it like America I don't think I mean we're very concerned about this and the secretary of state is as is very concerned about this and his comments today reflect that Parliament. Does not already happening in the system which I think is the broader point and what we could see from for example the opioids for the last 3 is the numbers have actually been slightly coming down so there's evidence that the system is beginning to grip this and as we were writing the review I was really struck by the wasn't a closed door to us everyone was keen to engage in the issues including G.P.S. And the colleges were all really came to get engaged with the issue and were already doing nothing what does that mean and if you can break it down a grip. Wholeness within the system what starts to reverse the trend well as I said some of these some of the numbers are already starting to go down we know about why that what what's doing it is I think it's more accountability more to more transparency more people are aware of the issue. Organizations like the Care Quality Commission is starting to think you know when they inspect and regulate primary care they're thinking about the state and this issue in the in those inspections really do more of that in the future there's more guidance coming out there's more guns coming out from the National Institute for Health Care Excellence .
