Dallas and federal throrts authorities overseeing the ebola virus. A second nurse tests positive, and flies on a commercial airplane. Amber vinson. A cdc spokesperson told al jazeera that the cdc did tell her she could do so because her temperature is below 100. 4. Authorities are now tracking down 132 people who were on board that flight with her. The cdc insists the risk to them is low. Vinson is being treated in the biocontainment unit of emory hospital. President obama urged cdc to step up protocols, under increasing doubt that cdc is not doing enough. Sheila macvicar has the story. Amber vinson is the second nurse. The first is nina pham, diagnosed with the virus and fighting for her life. Like nina pham this is a heroic person a person who dedicated her life and is dedicating her life the helping others and is a servantleader. This is a person with their life. Them. And a person who is dealing with this diagnosis with the grit and grace and determination. Vinson is one of 77 medical staff at Texas Health Presbyterian hospital dallas who cared for duncan in the 11 days before his death. Shared with the family and the Associated Press showed that she drew blood, changed catheters and was intimately involved with his karen. The centers for Disease Control say she should not have made the trip to cleveland. Because toop she wa because at that point she was in a group of individuals known to have exposure to ebola. Reporter on monday with a low grade fever of 99. 5, vinson flew back to dallas, moments later, eebility was confirmed. Out of an abandons of caution, the cdc will interview all of the foal passengers on the plane. We think there was an extremely low likelihood that passengers would be exposed. In scenes that are becoming familiar in dallas amber vinsons apartment where she lived alone has been cleaned and sealed. And her neighbors unnerved. It definitely takes my breath away. Like i said you know, its such a deadly disease. And you know, people are starting to get it more now. I dont think i dont know how, you know, from what they say its bodily fluids but i think if you wash your hands clean up enough you know it shouldnt be a good way of you getting it but reporter from the first days of ebola here in the United States the cdc has had one consistent message repeated again today. Do you know what protocols to use and how they work. Reporter but as error upon error has come to light that promise now feels much less reassuring. Thomas eric duncan the first ebola patient was left sitting in a public waiting room for hours even though he had a fever and his family warned he had exposure to the virus. Our investigations increasingly suggest that the first several days before the patient was diagnosed, appear to be the highest risk period. That was the 28th, 29th, and 30th. These two Health Care Workers both worked on those days. And on those days, nurses have said via their union they did not have proper protection. Impermeable gowns, hazmat suits, full care respirators, that it was three days before essential equipment arrived and all that time, dun can was vomiting and had diarrhea both teeming with virus. Protection control from another dallas hospital leaving neck and face exposed. Even the cdcs own guidelines on how to protect shows exposed hair and flesh, rest prawrt and respirator and two pairs of gloves option am. West africa, workers with Doctors Without Borders completely suit up to help patients. Biocontainment units, this one in nebraska currently caring for cameraman with ebola. There Health Care Workers have drilled for years. We have been practicing for this over nine years. Our nurses have been the ones who have been developing the protocols. They are very comfortable with them, they know them very well. Not so in other hospitals including Texas Health Presbyterian, whom nurses on a conference call, while treating duncan moved on to other patients. Hospital officials who interacted with mr. Duncan normal care duties, even though they didnt have personal protective care, while caring for mr. Duncan. All those who cared for duncan are still considered exposed. Dallass mayor had this warning today it may get worse before it gets better. But it will get better. America tonights Sheila Macvicar back with us. Talk us through the time line of things because it really helps us to understand how quickly this disease can spread. The time line is really important. Lets take a look. Duncan arrived in the u. S. On the 20th of september. He went to the hospital on the 25th and sent home. On the 29th he was suspected with ebola, not officially diagnosed until the next day, the 30th. That window of time, 28th to the 30th when he was in the hospital workers still wore regular gowns and scrubs not hazmat suits. We know he had contact with pham and vinson, the two that have the disease. 10th duncan dice, pham is diagnosed a couple of days later, and vinson is diagnosed a week after duncan died. So rapid. So rapid. We know the incubation period is 21 days. That means theres still a remaining 14 days for those Health Care Workers who were in contact with duncan to potentially become infected or not. And its clear, in listening to what came out of the city of dallas this morning when they were having a press Conference Announcing this Second Health care worker who was infected that there is if not an expectation and anticipation that it is very likely we will see additional cases of ebola arising out of caring for mr. Duncan. America tonights Sheila Macvicar, thanks so much for being here. World Health Organization coordinates the International Response in a crisis like this Ebola Outbreak. In august that agency declared an International PublicHealth Emergency saying that the coordinated response was essential to stop the spread of the virus. The question this hour is who is canting the response in the United States who is coordinating the response in the United States . Joining us is dr. Gavin mcgreg gor mcgregor skinner. I will point out you were with us on our set just a couple of nights ago and at that time you talked about some of your very serious concerns about the mismanagement of information, wrong protocols being given. Youve been down at the cdc. Youve there. Do you see things changing . At the moment, they havent and i want to point out why. We dont have a National Communication plan for ebola and we need one. If you go to google, you you put in ebola, and youre taken to the cdc, website, its full of pdf documents. Also the ppe guidance there is not based on a Risk Assessment we do constantly in west africa and for u. S. Hospitals. Based on the symptoms, the likelihood of the outcome of the disease, based on the hazards and based on what we need to protect. And we need to protect u. S. Health care workers and that Risk Assessment for ebola is complete coverage of the body no part of the body exposed. You have said to me that really the front line Health Care Workers youre talking to the nurses are really quite terrified. They are asking for assistance, they are asking for training, they are asking for consistency. They are asking to use the tools in the tool box. Where is the web based training . Where are the tools of wearing the Gold Standard of putting on and taking off personal protective equipment . With these highly infectious environment, as the cdc director, we dont miss the protocols we dont make mistakes but we make sure we do what the protocols are asking us to do. You come back time and again to some type of coordination or lack thereof as the case may be. There needs to be ebola czar put in place and theyve suggested some names, even some former military leaders. Is that the way to go . A single ebola coordinator a czar and should that be a military leader . No, it should not. We need to look for Emergency Managers, we have the best Emergency Managers in the world, we train the rest of the world how to do Emergency Management and incident command decisions, how to look at the available capabilities and the available resources and how to manage them appropriately how to meet the strict protocols that are needed to prevent exposure to this highly infectious virus. At the moment we need the Emergency Managers and to go to the hospital and say you have seen the protocols what do you need to manage the protocols and create safe working environments. That will not be from the cdc . No, it will not. Dr. Skinner thanks for being with us again. Youre welcome. Well return to the Ebola Outbreak later in the program. With a second nurse infected and others under observation what is the best information that Health Care Workers about ebola is wrong . After the break, a violent reaction to a drug that is supposed to kick an unhealthy habit . I just snapped and went into an unoccupied building. Adam may first reported on this controversial drug, speaking to chantix patients, saying they experienced violent irnling urgd suicidal tendencies. The socalled black box the drug maker pfizer says that based on its latest warning is no longer needed. But as adam may reminds us not everyone is convinced. In huntsville, alabama, 28yearold James Mayhall pleads guilty to burglary a felony and receives a one year sentence in the state penitentiary. Prosecutors say after being fired from his job the truck driver broke into this warehouse at night armed with a rifle and then fled police. Mr. James how are you . James, its adam may, how are you . In an interview from jail, james said he was dilutional, suffering from the effect of chantix, a drug designed to help people stop smoking. I snapped, went through an empty door, into a building and set up with my forklift with a weapon and its just a haze and to this very day, i have no idea what i was doing in the building. Reporter hundreds of miles away in a suburban chicago neighborhood. It was just like all of a sudden, you know, boom something went wrong. Tina hurst and her selfdescribed perfect life as a wife, mother and business woman came unhinged after she started taking chantix. I started Walking Around the house screaming, yelling, crying. I was threatening suicide at that time. I threatened to jump out of a moving car and then the police came. And the paramedics. And they had to tie me up. Put restraints on me. Hurst wound up in this hospital, five days in a locked psychiatric ward. The experience prompted hurst to start a backlog about chantix. Where according to comments posted on her site it could have been a lot worst. This one was just posted august 31st. I lost my best friend and father to my children. He was taking chantix. He had become very angry. He hung himself in fronts of me. I could not in front of me. I could not save him. From 2006 when chantix first went on the market through 2013, 637 suicides. 2064 attempted suicides. Reported to the u. S. Food and Drug Administration as adverse events associated with chantix. Thats according to fda documents obtained by america tonight under the freedom of information act. And a british study look at all prescription drugs in the u. K. Over a 14 year period found chantix resulted in the highest rates of reported suicide, suicidal behavior and depression. We should point out these Adverse Event Reports dont prove chantix cause people taking the drug to become suicidal or experience any other side effect for that matter. Chantix is designed to help people quit smoking by partially blocking the nicotine receptor in the brain, reducing the urge to smoke. About one in five smokers who take chantix is able to quit under ideal conditions during Clinical Trials. That is slightly better than counseling or nicotine therapy or a placebo. But some chantix users have reported anger depression hallucinations and other serious side effects. Dr. Michael segal, a doctor at the Chicago University of public health, has studied smoking and tobacco for 25 years. We know that smoking affects saiserotonin levels, serotonin s known to be involved in Mental Illnesses especially in depression. So its actually not surprising that a drug like chantix could have effects on personality. And could lead to depression and suicidality. A probe by the food and Drug Administration in 2006, chantix users began reporting troubling side effects almost immediately. Some of the cases detailed in this report are bizarre, others are down right chilling. A 24yearold woman woke up her sleeping boyfriend and started beating him because he looked too peaceful. A 42yearold man walked up to a stranger in a Bowling Alley and suddenly punched him. Then another man choked his wife, then hung himself. Thomas moore is a researcher in the institute for safe medication practices. He studied these acts reportedly linked to chantix. These cases had three striking characteristics. First, the violence was absolutely unpredictable and senseless. Second, the victim was just anybody who happened to be nearby. Could have been a fiancee, could have been mother, could have been a police officer. And thirdly these people had no previous history of any kind of violence, were unlikely prospects for a violent act. Moore looked at all violence acts reported in a five year period for all drugs sold in the u. S. How did chantix rate . It was by far the worst, 18 times more reports that were unexpected, serious psychiatric effects, effects that made it unsafe for people in critical situations, because there were seizures blackouts, temporary blindless. In may 2008 the institution for public practice, among persons operatining aircraft trains bus he and other vehicles. That report prompted the faa and the Defense Department to ban chantix use among pilots and air traffic controllers. The federal agency that governs truckers, the federal Motor Carrier Safety Administration said chantix may adversely affect the droifers ability to safe the drivers ability to safely operate a motor vehicle. And the department of transportation seventh a memo to the federal transportation administration. Moore the thinks those restrictions dont go far enough. People who carry weapons for a living such as police officers, military but others, should not take a drug which is now well documented can cause uncontrollable rage. The food and Drug Administration in a statement to america tonight says, animals of controlled Clinical Trials found an analysis of controlled Clinical Trials than placebo treated patients. Even so in 2009 the fda citing the small but potentially risk of newer he psychiatric events required whats known as a black box warning label. The most serious warning possible. Alerting possible chantix users and their doctors that chantix has been associated with changes in behavior such as hostility, agitation, depressed mood and suicidal thoughts or action he. Actions. Dr. He meubl michael segal. I would argue that death is a side effect that youre warning about, thats the case with chantix because the black box warning says look out for suicide. The patient may commit suicide. If thats what the side effect is then the warning is ineffective, theres no point to having a warning, because if the patient commits death and the physician monitors for it and find out the patient died, theres nothing you can do about it. The patients died. Tina hurst. Besides this break down, anything else in the weeks or months leading up to this psychiatric episode . No changes, no hardships in your personal life . No, absolutely not. How convinced are you that this drug caused the outlook . Without a doubt, i went off the deep end. It changed the chemical in my brain and it caused this absolutely. Im absolutely apositive that it was the chantix. Pfizer the drug company that manufactures chantix in 2013 paid close to 300 million to settle most of the 2700 lawsuits filed against the company over chantix suicides and other serious side effects. Herb quit smoking with chantix and support. At the same time the company also stepped up its advertising of the drug. The federal judge overseeing the chantix lawsuits, in inga johnson says the black box warning is sufficient warning. Making further lawsuits unlikely. What would happen if the fda disagrees with the judge . Five advocacy groups say the agency should not do away with the warning, instead they should strengthen it. They need to take chantix off the market. They need to take it off the market. Tina hurst says her life is finally back to normal. She turned down a settlement offer from pfizer so she could keep speaking out against chantix. Very forfei fortunate that im okay and im living a great life. It could have been much different, like many other people who took chantix that killed themselves or hurt somebody else. Pfizer declined to sit down with us in this story. In a statement the company said chantix is an important treatment option for adult smokers who want to quit. Pfizer has had more than 9 million prescriptions of the drug in the ultimates. Worldwide sales for the drug top 4 billion. James mee han wishe han wishes Everything Else would have turned out differently. Ive had only two speeding tickets in my life. In one week, i changed my personality, lost my job. Just about ruined everything. James me mehal is now out of prison after doing hard times he blames on chantix. Adam may, al jazeera. The fda has no plans to pull chantix from the market. The label for chantix has already been updated to include, quote, results from a pfizer metaanalysis from Clinical Trials that showed no increase in the incidence of suicidal id yaitio ideation. The statement continues, pfizer is committed to the ongoing safety of its medicines, including chantix, to provide Health Care Professionals and patients with the most up to date information. Later in our program. We return to a growing health fear, ebola and oview from scientists who warn, what we now know about the virus may not be enough to stop it. If you have ever been to new york city you probably have noticed those rats. Now for the first time researchers have tested and cataloged the virus and bacteria the little fellows carry and it is as bad as you think. The rats carry food borne illnesses like salmonella as well as fever borne illnesses. As far as ebola, government Officials Say ebola can only be spread by the bodily fluids of an infected person. The president reiterated that today and is meant to be reassuring as cdc sets out protocols for patient care. What if the information is just wrong that officials have offered so far . On a crisp new york city afternoon, it is hard to imagine the risk of a mutated virus. One that could be spread far more easily than Health Workers have so far thought. Through a breath, a cough, in the air. To be clear, Scientists Say there is so far no evidence that it has happened. But several have stepped up to warn it could. The possibility for airborne transmission is not zero. And the longer the epidemic proceeds, the greater the chance is for airborne transmission. With his team at purdue university, biologist David Sanders has been working with ebola for more than a decade. Initially, he only meant ebola to be as a carrier. What he learned confirms his caution that the ebola virus can attach itself to the lungs. What we were able to show is that the virus enters into the lung cells from the airway site. We actually have the data. Thats second point. The receptors are on the lung cells and they are on the side of the lung cells, the same side influenza uses to get into the lung. The implication is that ebola is easier to spread than Health Officials realize. That is possible, the other possibility is the virus may have to change in order for it to be less subject to desiccation or able to survive on surfaces easier. Sanders isnt a lone woofl in raising th in woofl in wolf in saying. National institutes of health the cdc and the united nations, Sanders Point out that very few scientific researchers have focused on ebola as he has. And there could be a disconnect between what scientists believe and what researchers see in the library. They may have been familiar with the epidemiology and the medical literate, but i think they were largely unfamiliar with the scientific literate which says that this is a real possibility. Where there is consensus the urgent need for a vaccine. Sanders work allowed him to patent a vaccine model similar to the one being tested by the nih but it cant come quick enough. Nih director said it was a matter of money. The ebola virus had been developed by now had the money bin there. Is that the case . There is no doubt, the biological evidence in the United States has been harmed by the sequester, by the lack of growth in the nih budget absolutely. We know that the treatments is been successful in animal models we dont have the human trials so we simply dont know whether the vaccines would or would not have worked at this time. There is no question that research on ebola has been damaged by insufficient amount of money. There is no question about that. By politics. Im sorry . By politics . I hate to say that but it may be expensive and controversial but investing in new research is getting more and more urgent. With 9,000 cases and rising all the time, the spread of the disease is too fast to contemplate. For example if you have 10,000 cases there is a certain probability. But if we have a million cases that probability is multiplied by 100. But we cant say its wrong to tell the public this is not a possibility we dont need to worry about it at all. And im especially concerned as the number of cases grow. And we will continue to keep close watch as the four people still being treated in the United States for ebola infection continue their treatment. And dozens more remain under observation for any sign that they were exposed. When we return a healthy bite but a dirty past . New york is a city that prides itself on its food scene. It already considers itself a leader in the farm otable movement. But if farmers knew what got their food to the table, they might think twice , america tonight correspondent christof putzel. On going local. Behind the idyllic barns. We try to do the work well, its not easy to be working sometimes 16 hours continuously so they can have a fresh glass of milk. It took jose coyote perez two weeks to cross the land from his home in mexico. He is now a worker in upstate new york. He is working long grueling hours to support his family of six but hes one of the lucky ones. Translator now, in this job its very different because we receive overtime, vacations and two paid sick days. Reporter many workers are not so lucky. What i found was, there are labor abuses everywhere. But outside just being labor abuses athe way the law is structured in new york state, workers dont have the right to overtime pay, they dont have a right to a day of rest and they dont have collective pawrging protections. So bargaining protections. So what this means is you could work a 90 hour work week seven days a week and get straight minimum wage. Maggie says conditions here are defined by an 80 year old labor law. One of the new deal legislations that gave us the weekend, minimum wage overtime pay, that secluded farm workers and domestic workers. The politics is really that so many of the domestic and agricultural jobs were filled by blacks and they didnt want blacks to be able to have the same benefits on the job. Reporter the builtin disparities are now being felt by a new crop of workers. So we have a large category of workers who are undocumented. Who are really ripe for exploitation because they are afraid to speak up about their concerns. And this means we have an underclass of workers. People talk about a new jim crow or new juan crow because it affects so many latino workers. Organizing for political change is too risky for most. But for workers like coyote perez its a chance worth taking. What would you like to see changes . Translator for it to be more humane, for a fair wage but above all that this country would consider us as people that are helping to produce life and propel it forward and that they would allow us to stay here helping farms to produce milk. Even people who options us drink a daily glass of milk. Reporter on a dairy farm just north of buffalo, jose canas prepares for his day of work. He was too afraid. So we met him offsite. We have a fear because of immigration, because of police. That doesnt allow us to go to the store freely go shopping. It seeps us a lot more private. It keeps United States a lot more private. Jose canas and others through organization he like the Workers Justice Center are just now starting to speak out. Translator i worked in a farm where we worked in bad conditions unsafe jobs. There was a lot of insecurity. And that motivated us to organize our colleagues. They start to promote a program that was related to safety and health in the workplace. Reporter do you think it will work . Translator i hope so, it is my dream that this works. Reporter one of their main goals to pass the farm worker fair labor practices act. For overtime pay and a day of rest to collective bargaining rights. The bill before the new York State Senate has yet to pass. Canya says it is what keeps many undocumented workers to speak out. Translator we are here in this country to harvest fruit, apples pears and other things. We are working here day after day. We are not criminals, we are workers. One undocumented woman was so afraid she would only speak if we hid her identity. One apple picker seven days a week, ten to 12 hours a day with no paid overtime. We feel that people take advantage of us because of the immigration situation were in but we dont have any option but to accept the work how it is. Its very stressful. Tiring. It hurts. A lot of apple pickers in the morning when they wake up their hands are numb. They cant move their fingers. Reporter for ten years shes worked in the same farm in the same conditions. Now shes joined a campaign thats demanding more than just Legal Protection for farm workers. For them its also about respect. And what would you like americans to think about the next time that they go and buy apples . That they remember the workers, those that bring these fruits to their table. That they remember the conditions that we work in each time they eat an apple. New york has the secondmost profitable local farm economy in the country. The farm to table has been a boon to farmers. The local Food Movement has really helped New York Farm dealers a great deal. You cut out the middle man. Selling direct to restaurants and that means a lot more money in your pocket. But unfortunately, those gains have not been passed on to the workers. The campaign for farm workers rights have been resisted by New York Farm bureau which resists farm workers and farm payroll across new york state. Our workday is whatever it takes to get the job done. Local farm owners like maueree, say paying overtime would make her less competitive. Were price takers not price makers and we have to compete. I also have the highest input cost of any other state in the country, to farm in. And to add this would just put a nail in the coffin. Reporter maureen, an 11th generation farmer provides benefits many other farmers dont but paying overtime is something she says she simply cant afford. We would have to limit hours so we could compete. Time and a half should be a national issue. If it was all across the country then fine. But a state by state, no. Reporter right now only seven states mandate overtime pay for farm workers. On his home coyote perez has other fears as he finds time to be with his wife and four sons. Translator the biggest challenge is that i will be separated from my family. Thats my fear and thats what ive always been afraid of. Reporter what is it like living with that fear every day . Translator its something that tires you. It schools you. I. T. Consumes you, it consumes you. It blocks part of your mind because its always present inside of you. Translator he is anxiously awaiting an Immigration Court date next year, one that will decide his future and that of his immigrant country men. Christof putzel, al jazeera, new york. We have an exclusive story tonight, and we go live. Finally from us this hour, weve heard about the effort to get more young people into Science Technology engineering and math. Stem. A cattle list for its robotics team, teaching Chinese Students how its done. Kostas grammatis has the story. [ singing ] there may be high level rivalries between United States and china but in day in china, thanks to students, there is nothing of such thing. They taught how a good thing to work. Its about growing together, teaching everyone to the best of their abilities. Young people from china and the United States working side by side, sharing a common interest in robots. The team from las vegas, the 987 high rollers were cited t invito share their expertise with Chinese Students. You would think they would be more advanced than us in technology but this isnt a thing that they have at their high school level. We went over there and showed them what they would need to run teams like this and what it would take to run a competition. Very cool to bring something that we know so well and to bring it here and introduce them to it. You can see their faces when we introduce something new that they can do in the coming year. Back in las vegas, at the cimarron high school, it is the love of technology, that have helped turn the lives of some american teets teens around. This isnt some top Tour High School . No we are a title 1, 50 are on free or reduced lunch. This isnt top echelon for sure. What do you mean turn around . Finding out why Graduation Rate is falling. Whether i came here it was 51 . Now its already up to 70 . And its baw of these programs . Absolutely. These are the great kids. It destroyed our bit so it wasnt a very good moment but definitely a learning one. Team 987 high rollers home base is this 4,000 square footwork shop where kids often spend 12 hour days can sometimes seven days a week building bots, countless car washes support the team and also bought them a brandnew 50,000 haas cnc machine for building robots. What is our feed rate . Right now were going conservative on this machine. What on the pockets . 14 inch per minute. Its the team, and this is every bit as popular and every bit as important as any other team on campus. The football and the basketball. So its on par with athletics. You turned down magnet schools to come to this school. And i think it was definitely worth it. Why . Because this team is unique in so many different ways. It has machining equipment that no other school has access to. Its dedicated during school and after school and its got the best students all around the valley. Stem education, science, technology, engineering and math that fosters this kind of learning and in a discipline that is traditionally maledominated the high rollers are different. I wouldnt say its intimidating. We work as hard as they do. What happens if you use the wrong g code . It will crash. Girls do everything. I started coming around the shop and being around the guys and i started really liking being around the mechanical part of it. Thats how i got to wanting to be a engineer. Okay well do the cnc machine, girls only. And then they know more than the guys and its not intimidating. Ladies at the machine too. Exactly. What may be intimidating to the uninitiated are the robots themselves. More sophisticated han the cars. The level of these machines are amazing. Hit it brandon and theyre built like tanks. Theyre programmed to complete a competitive task. In this case, for fun, throwing frisbees at a certain techknow contributor. What did you notice about this generation, this class of students, are they more sophisticated in their technological understanding and are they appreciated or the that, what is the cultural phenomenon going on . Shift. When i was on the team there wasnt that. Students involved in this program are graduating high school with honors, many getting full rides to major universities.
