Philip Evan Moss entered a guilty plea when he appeared before magistrates in Hereford on April 28.
The court heard Moss had failed to provide a blood specimen to police when suspected of having driven a vehicle in Leominster on April 13.
The 34-year-old also admitted driving a Kia Sedona in Leominster's Hawthorn Place without insurance or a licence on the same date.
Moss, of Garden Close, Kington, was disqualified from driving for three years and fined £120. He must also pay costs of £135 and a £34 victim surcharge.
