Presented by our dos executive director kelly dearman. Thank you. Good morning everyone. I just have a few things to report on. First, the administration for Community Living, acl has published its first ever proposed federal rule for Adult Protective Services. So, we are currently in the comment period, but this is great because it will build more infrastructure across the country, helps states build up their aps programs. Cwda and our own jill nielsen and [indiscernible] are working hard to read the rule and make sure it meets all our needs or making comments where necessary, but this is a exciting time and we are happy this is happening. Second, care court started on monday. Care cort started monday. It hasnt had a huge impact to us as of yet. We are waiting and seeing, but we should just all be aware that it has started and there is all kinds of rules and it is complicated. But it is happening and it should be a good thing. Alright. Last week i attended c4a Strategic Planning meeting, and for the First Time Ever we adopted a set of policy priorities for this year, which include maintaining a sustainable and robust aaa network, no wrong door coordinated systems, housing security. Doesnt mean building housing, but means we need to be in the conversation when ever we are talking about housing. Same with Economic Security and recognizing that many older adults would like to work and also people with disabilities are active and able to work and should be working. Affordable and sustainable transportation, which is beyond cars, but making more accessible transportation. Care giver support and emergency preparedness. Out of that, we had our legislative priorities for this year, which are going to be very sim lar to last year because we didnt get much traction, but it is around adding more funding for adrcs and caregiver support. One of the great things that happened is that susan damorris, the director of cdasorry, i get confusedshe said it is really important for us to ask for what we want, and start with the big numbers. Start with whatever it is you want and we can widdle down from there. So, we feel like we have a Great Partnership with cda and will work really hard and see if we can get some members in our legislature to work with us to recognize that our population is continuing to age and grow and that we are not ready. Lastly, i just want to give a shout out to maria mucardo. She and her team are administering flu shots at housing sites and also working with the Outreach Team so more folks know who we are, what we do and why we do it. And i also want to thank all my staff who were so great and gracious while i was on vacation and i did not get one call. Thank you very much. Thank you. Any questions . Do we offer flu shots . And it pretty much mirrors what they are trying to do at cda so our policies seem to be very much in accord with what they are trying to do. I will say these policies are not in the final final because we all recognize that there is nothing in here that talks about equity and so we are going to put it in thewho we are and what our focus is, so it is overarching Mission Statement for us. Any other commissioner comments . Commissioners, item 6 is the das employee recognition. Executive director dearman and i will present and honor jiaqi liu. Thank you. [applause] so jiaqi is social worker in das who empathy and understanding for all her clients and colleagues alike shines far and bright. Jiaqi is compassionate and caring and serves case load with unending patience and respect for everyone she works with. She has been with ihss since september 2021 and prior to joining ihss was a eligibility worker who worked with clients coming from diverse backgrounds and all sectors of life, which allowed her to transition seamlessly to ihss. Amazing communication skills with everyone she works with has made a stand out and role model in the department. As a ihss social worker, she continually displayed kindness and compassion for all of her clients and goes above and beyond powers making sure all the clients needs are met. Combined with thoughtfulness, she is diligent and never appears overwhelmed even when juggling a multitude of tasks and finds the time to help colleagues. She has exceptional Organizational Skills and High Performer and quick learner at ihss who can work independently and think outside the box. One of her best qualities is that she is a team player who welcomes the opportunity to develop new staff by allowing them to shadow her during home visits. She enjoys learning new processes and recently took the opportunity to accept an acting position with the Quality Assurance team to grow and develop new skills. Jiaqi hard work and dedication is greatly appreciated and we here at das and everyone at ihss would like to thank for all her hard work and dedication and how fortunate we are to commitment to clients and colleagues. [applause] commissioners, item 7 is the Advisory Council report presented by diane lawrence. Good morning commissioners rchl Deputy Director dearman. I have three reports combined here, but i think it was pretty detailed so ill give the highlights. I hope it was detailed. Just give the highlights. There was a action item from last months meeting on when i talked about the site visits and how we get more information out. So we are still working on that, but we did have discussion at the meeting and mike zogg brought the postcards in multilanguages for the hub and one of our members handed them out at a Farmer Market and grand parents day celebration, so what we are talking about is how to make the hubpeople know about it, so either some kind of card or whatever. Well continue to have those discussions and brainstorm. I did as i just told mike, give out the hub number the other day Assembly Member phil tings town hall. Somebody had questions so i carry it on my phone and hand it out. And the major piece of business at our last meeting, we had a lot of discussion was on the proposal to change the process for joint leg and i outlined the final priorities and i want to thank commissioners lum and bittner for their input, and support of that decision. It was both unanimous at the joint leg and with the Advisory Council. But what we will be doing over the next couple of months as we get ready for january is looking at executive director dearman just spoke on, c4a priorities, California Commission on aging priority, see how we can align so we as aaa are moving all in the same direction, so ill have more updates on that after the first of the year. We had a update on the disability Cultural Center. There was discussion around funding and the purpose and i think we got clarity, so i think we are in pretty good shape there. It was nice to see the rebar going up as i came up van ness this morning. Lgbtq updates, open house is building a 15 story lgbtq housing, which will be the largest in the city and that will be on market around the corner from the laguna street site. We are excited about that and will get continued updates. Our membership we still struggle. Our bord of supervisors vacancies continue and i want to thank executive director dearman for her efforts with some of the supervisors. We have senti did do a update at the Dignity Fund Coalition meeting to ask for suggestions if they know both for Commission Vacancies and supervisorial vacancies. We did send supervisor chan a resume, and we also a member offrom district 5. Someone was recommended from the tenderloin Development Network and lisa came, so he sent her resume on and she came to the last meeting and would love to have her as a council. She comes as a client so provides really wonderful insight. We still are workingwe have five Commission Openings and we may look if we dontdepending what happens with the supervisors may possibly ask the two people we have resumes on to serve to present them to your nominating committee for approval, because they would add a lot to the council. We got a couple updates as outlined here on legislation. The one i wanted to stress is one of our members, stephanie dignum works with arch and will be holding affordable connectivity programs, so our guest who has one of those programs and stephanie were able to connect and do additional work and we referred her to senior and disability action for their at t program, so well get updates on that. As i mentioned before, and not in here, one of our council members, her group senior power sponsored a grand parents day at the outer sunset Farmer Market on september 10 and handed out information on the hub, information on sidewalk repair, information on being prepared in emergencies and had a lot ofi happen to be there, a lot of Cross Generational interactions, so it was really i think very successful, plus a nice day in the sunset. Commissioners, item 8 is joint legislative report presented by diane lawrence. There isi dont think there is much here to point out. We summarized the bills we focused on. I will have a thorough report next month when well meet after the governor has met the deadline, so he has until october 14. I put some definitions at the bottom. Mainly for me, because i keep reading these terms and wanted to make sure i was clear, so i thought i would just add them to the report. And then again, we did the first round of the action item to change how we meet going forward. So, our first in person meeting for the joint leg is march next year, but we will be in communication as we get the bills and if we need to call a meeting sooner or need to add a meeting during the year, thats all part of this and we are pretty sure we dont need to make any changes to ourcouncil bylaws, and there is nothing really formally written about joint leg we would need to change in your bylaws or charter piece either. Executive director dearman, pleasure to see you all today. Ill first start with the case programming. In september we had kathleen sullivan, executive director of open house and the out Reach Coalition manager of open house present the plans on a research that they are doing in collaboration with citrus at uc berkeley. The university of california San Francisco, and the California Department of aging that aims to [indiscernible] the aging experience of lgbtq plus californian sism the purpose of the study is develop baseline information of economic wellbeing of older lgbtq plus resident of california throughout a statewide survey. The study aims to provide state wide data to support the master plan on aging, and goals and objectives and inform California Public policy positions and improve the care and service of the lgbtq plus older adults and give attention to all state regions and the population diversity. Jupiter plans to promote the need for Community Members of the case affiliated agencies to complete assessment regarding the lgbtq plus older adults and person with disabilities to include those findings into this research project. So, we are happy to oblige and do that. In october, susan stadler, the executive director of [indiscernible] with growing older and shes an architect is giving a presentation on the aging 360 workshop which is developed by at home with growing older and aging 360 if engaging in simple low cost or no cost age frndly home adjustments that could be executed with or without and Minimal Services from a handyman or friend and certainly would address the needs of many older adults in the bay area who live in older homes that need adaptations to help them live at home. Looking forward to that. And lastly, on our calendar is a presentation from jamie godord, director at sf reserve, Community Living program and jamie is going to give a overview of sf reserve as a match talented and skilled older adults and people with disabilities with parttime and project based work at local non profits and Community Based organizations. Thats the case programming for the next couple months. In december we are making plans for our holiday party. Case activity and you heard this from both fiona and i in the last couple months cases supporting the provider working group on trying to salvage some funding for the emerging needs happening with our older adults and people with disabilities here in San Francisco. We all know at this point that the 3 million contribution to the dignity fund was suspended this year, and then we did not receive any money through the addback process, so we are left with looking for one time only funds at this point to the department. Service provider working group has met several times to identify several needs that are listed here and i think fiona went through this in the last meeting with you and we are looking at the november oversight and Advisory Committee to make recommendations for 3 or 4 of the top primary needs. We just dont know what the funding looks like at this point and wont know that until the november meeting with Oversight Advisory Committee so we can have that discussion at that point. Item 12a and b are informational only. Item 12cg are action items that require a vote by the commission. The first item is 12a, the Community Living fund 6 month report january through june 2023. Melissa mcgee will present this item. Good morning president spears, commissioners and executive director dearman. My name is melissa mcgee, a Program Manager at the office of Community Partnerships at das. Here today to present the Community Living fund 6 month report covering january to june 2023. The Community Living fund or cls was created in 2006 through San Francisco ordinance to support aging in place and Community Placement alternatives for people who otherwise require care within a institution. A section in the San Francisco administrative code requires das report clsb Level Service of care and cost every 6 months. My report will be fulfilling this requirement. Cls provide home and Community Based Services Using a 2 prong approach of coordinated Case Management and purchase goods and services. The institute on aging is currently the contracted Service Provider for these services. The program serves adult San Francisco residents with incomes up to 300 percent of the federal poverty level. The individuals must be willing and able to live in the community with appropriate supports and they have to have a demonstrated need for a service or resource that will enable them to live in the community and prevent institutionalization. Target population includes individuals ready for discharge from laguna honda hospital, zuckerberg San Francisco general and other San Francisco Skilled Nursing facilities or individuals at imminent ringe of institutional placement but willing and able to remain in the community with wrap around services. The Community Living fund Program Provides housing subsidy support through brilliant corners our contracted Community Partner that administers rental subsidy and helps acquire scattered site Housing Units throughout San Francisco. The fund also includes a Public Guardian housing fund that is specifically provides housing subsidy and assistance to individuals by Public Guardian and meet clf criteria. I are want to identify some highlights from the report. In the 6 month report covering january to june 2023, cls received 93 new referrals during the period. About 54 percent of those were eligible and 50 percent of those eligible were approved to receive services. Served 245 participants in total with 62 percent receiving intensive Case Management service through ioa. 100 also received rental subsidy through brilliant corners. [indiscernible] remain consistent with prior periods with some miner changes. Referrals for white individuals decreased slightly and remain the Largest Group at 38 percent. Black African American referrals at 30 percent, which is slight increase over the last period, latinx remains steady at 15 percent and Asian Pacific islander referrals increased slightly to 13 percent from last period but remains consistent with overall trends to date. Those identifying as other or unknown race remain steady about 3 percent. During this reporting period, continued to support the das Public Guardian also through the pg housing fund by providing housing subsidies and assistance to 6 par ticipants. The most Common Service requested at intake remain consistent with prior periods. These include Case Management, 69 percent. Inhome support at 69 percent. Notably, requests for support with assisted devices, 46 percent and food, 42 percent appeared more frequently then in recent periods while request for Housing Related Services declined. As for program cost, this reported period shows a net increase of 383. 957 in clf program cost over the prior 6 month period. With increase cost in Case Management, scattered site rehousing and one Time Investments in data systems. This home care cost and rental subsidies that average 3047 per month. Excluding cost for home care and rental,b average cost for clf participants who received any Purchase Service was 129 per month in the latest reporting period, which is decrease of 52 from the previous 6 month period. Finally, as of september 2023, the clf wait list had 24 referrals waiting to be assigned. Average these individuals have been waiting for 125 days. Approximately 92 percent are waiting for intensive Case Management. The others are referred for purchase of service only and have a separate community Case Management. This wait list is slightly longer then the wait list in the prior period, but lower then historic wait list trends. Consistent with the prior period and in reversal of historic trends, individuals waiting for purchase of service spent less time waiting then those waiting for intensive Case Management, 75 days waiting compared to 129 days. One final thing and important new addition to the clf program is relation ship with the San Francisco health plan. Towards the end of this reporting period that process has been implemented and referrals done, so this will be a interesting addition to the next 6 month report. Andthalities thats it. Any questions or comments . I is have a question. Can you explain moreremind you [indiscernible] San Francisco health plan . Right. Do you mind putting your mic . You mentioned new addition is San Francisco health plan will be involved. Can you remind me what that means . I think a month or two, or cant remember exactly ago, i brought the contract with the health plan to the commission. We have das has relationship, a contract with San Francisco health plan and they refer clients to us who meet clf criteria for enhanced Care Management they call it and so those clients will our contract is with the health plan but clients are served by the ila clf program. Thank you. I have one other miner question and a comment. My question is, i dont know why i pick this up, under demographics, i noticed there is a High Percentage of zip codes that coded 40 percent. Just wondering why there is such a gap . That would be on your report on page 3, the top of it. It says, the proportion of referred clients missing unknown zip code increase sharply accounting for 40 percent of all referrals. I cant hear, but the bottom of page 2 . Yes, bottom of page 2 and talks about zip code. Some reason the zip code coding is 40 percent unknown. Previous report was 12 percent. Are they not coding the zip codes . I suspect. That gives a good idea exactly. Good morning commissioners i work with the Planning Team and help prepare some of the Data Analysis that supports the 6 month report. As you may know and think we also written about in the Program Update section of the report, das recently ungone a important data base transition that effected the data entry for program intakes. This is a area that we suspect but still digging into that may be not a function of business process, but instead a technical challenge that we are still resolving with our data base center. That is our suspicion at the moment, but well hopefully resolve this by the time we come around for the next 6 month report. Thank you. My one comment under systemic changes, i just want to comment that im pleased to see that in this period of time that now open house is involved. They are part of the clf Advisory Committee and thats a great addition. I agree. Thank you. Thank you. Other questions from commissioners . If you dont mind. The report notes there is a lower incidents of referrals and indicates that is because of the change at laguna honda. It all made sense and answering my questions, but then you also still have a waiting list, and is it because thats apples and oranges . I assume peopleit wasnt a opportunity to address more people on the e waiting list. The waiting list is consistently have a waiting list and often the process of conducting outreach before enrolling the clients and that sort of thing, as well as in this specific reporting period, some staffing issues, which have been fixed and are full staffed. Thanks. That does explain that. Okay. Thank you. I have one question. This isthere is a significantthis is on page chart 1, page 5 or 6. The significant decline in ioa serving clients in this processing from 206 i have to ask you to speak up a bit. Sorry, never been told to speak up. [laughter] i guess it is a point. This is regarding the number of clf clients served by ioa that declined in this particular Service Period from 206 to 153. Thats a significant dropoff and just wondering the clients served is in year chart number 1. On page 6. I think i read that correctly. Yeah, on chart number 1. Im not exactly sure of the answer for that. I think part is the referral process from laguna honda and challenges there, as well as the staffing issues and that sort of thing, but happy to look into it further and then send ravi a note. The other seems to be the Scattered Sites seems to be consistent. Just curious about that. Okay. Any other questions from commissioners . The next item is item 12b presentation das fiscal year 2324 schedule of requests for proposals which is also information only. This would be steve kim and mike zaugg to present the item. Good morning commissioners and exective director dearman. Im steve kim and with me is mike zaugg. We are here today this morning to present [indiscernible] along with fiscal year 2324 request for proposal schedule. Every year, contract and Program Staff Work Together to develop the schedule. The schedule gives a overview of what we are working on and programs and contracts will be bringing to Das Commission for approval. I will pass to colleague mike to start the overview. Good morning commissioners. So this first slide, dont know if you are familiar with this, it is themed throughout many documents. This is the four Year Contract fund cycle we use and this helps filter what contracts are up each fiscal year. We have taken our appreciately 250 total grants that we utilize and group them to three different categories. We alternate on different years into four year grants. This is a way to ease Administrative Burden and the amount of contracts bringing before the commission. It puts our grantees into longer term grants that they know are there for 4 years to create stability and allow for growth and development of their programs. It also allows us to focus on specific areas each year. Lets see if mymy little circle there is where we are right now. Thatswe are right now in the circle rfp contracts that are sent to expire in june 30 of next year in 2024. We are getting at that right now so that it takes a while to do that, so we get at them over the next couple months and present them throughout the spring to this commission so everything is set up and ready to go for july 1 of 2024. Generally everything you will see before you today iswith a couple exceptions will start july 1 of 2024 and run for that four year grant period. You got fancy and [indiscernible] there are 25 items this year and 81 grants that are expiring this fiscal year, june 30, 2024. For last fiscal year, 2223, we had 33 generating 125 new grants and contracts. 80 were just on the july commission alone. In addition there were 87 modifications total of 125 plus 87, 212 action items generated for last fiscal year mpt modifications are generated by addbacks, oto or [indiscernible] im going to quickly run through what is on the calendar for the upcoming year. Starting out we have a couple that are actually going into effect in the immediate future. We have Disability Community Cultural Center, which is coming very momentarily to this commission as well as district 6adr services. These are things we bid out and procured. We have our creative aging pilot program, which is lested as a september release, it is actually come out october 10, but that will give us time to get to january commission. Aging and disability aging Resource Centers, 13 of them located throughout the city. We are going out to bid on new grants for those programs, so that will be started shortly. Moving to the next one, our Adult Day Program including Adult Day Programs and alzheimers Dementia Care and Resource Centers are going out for bid. Our Education Advocacy peer mentor programs have been in effect a couple years now and will see those rfp this year. Moving along. Our housing subsidies program, our reserve Employment Program which Dan Gallagher alluded to in his program earlier, which are our programs which are up this year. Our suite of legal and naturalization services, there are a number of grants in that area, but 10 different providers help us with providing Legal Support for clients as well as support for permanent residents naturalizing to become United States citizens. Last slide of our laundry list, we have sf connected programs. Our Digital Technology programs, the Largest Group of them up this year. We have something supportive services. This is new programming. This is new funding, which we received from the California Department of aging. It is intended in a broad category of aging in place programs. We are currently in the process of still researching kind of how broad that is and understanding needs in the community and various ideas that are out there, with the idea that we will come up with a more focused procurement for service in that area in next year. You will see that seeing that later in mid2024. Okay. Kick it back over to you, steve. This slide shows two items that have for Source Program this year. Source is a option where we determine these are the only grantees the agency that are able to provide these service. Source doesinate go through a normal rfp process, we must go through the approval process [indiscernible] so, the items on here is first is the Legal Assistance for veterans with Mental Health claims. The reason this is a source is because this was awarded by the California Department of veteran affairs, specifically with specific vendor as part of the grantee. Namely, source plow share. The next item [indiscernible] data base platform migration. We anticipate this as a may potentially because we are looking to migrate the platform that is used since late 90, 2000 for the Public Guardianship Case Management services we provide. We currently are early adapters of this program and we in doing so are about two Technology Platforms behind the latest version. Those who have gone through your laptops and pc in the past, [indiscernible] you dont get the support and security support you get, so this will be something we are working on this year and placemarked for may potentially if we need additional funding for the migration platform transition. I will be available for any questions and concerns you may have. Questions. First of all, thank you for the power point. My heart leapt when i saw power point because so used to looking at this. I did have a couple small questions about acronyms because im new to a lot of this stuff and on the first four year dignity fund cycle you have cna and sap and are might have defined those but didnt catch it. Cna is Community Needs assessment. That is process by which we go through a variety of data and focus groups and other surveys and things to understand the needs in the community. Sap is serving allocation plan. That is a result ofborn out of the Community Needs assessment that is the department distilling the Community Needs assessment into our planning as we look forward to the 4 years. Fantastic. And then i wanted to comment. I was happy to see a number of initiatives around technology and providing access for elders and platforms and so thats really good to see. And then one thing that might be helpful if you want to consider a rev next time around would be general size of the rfp for these categories. Just because numbers will give me a sense of scope for each of the initiatives. I have acronym question, what is moca. I didnt say because i couldnt think of it. It is modernizing older californians act. They were trying to return the dollars from the 2008 budget cuts. Okay. I think you are saying it is the second cycle for this process where you got it all mapped out around the contracts and when to expect them and rfp and all that. From our standpoint, it is great because i feel like we havent had huge meetings where they are very long with lots of contracts. It helps to make that go more smoothly over the course of the year, from the standpoint of the staff and people who apply, has there been an improvement or in the sense was this change and has you gotten feedback on doing all this . I think this has brought a level of organization to things that have calmed some of the chaos. I think it made things simpler. We are doing less renewals of grants, so i think that has eased workload somewhat. Okay. Steve would be an i think the fact we did 212 action items last fiscal year it is pretty significant and the fact we are able to break up into three phases and target and have the terms of the contracts 4 years at a timeprior to this we were having various different years of terms so standardsize term and structure allows to be more efficiency. We are limited resources. You see on the last year was quite a bit Case Management, a lot of activities and this year you see focus advocacy and such and next year see quite a bit of renewal. Nutrition programs. Being able to focus in the 4 year cycles allows us to have thebe efficient and have consistency throughout. I have a question. Any of youdoes any of the data reflect a loss of personnel in the agencies to provide services, which seems to be happening all kinds of Services Since covid and so sometimes you want to do everything you put out here and then you have nobody to do the work. What does that look like . It is notit is a thing that we are working through right now. People are really putting in their time to get all this stuff done. It is not a reflection in this right at this moment. I would also like tofrom cbo perspective that is very touch market because very high competition. Not only the agency, but our staff Program Staff and contract staff are down quite a bit. Both of the groups are probably down a third of the regular staff headcount so that is quite a bit of challenge. Thank you. Commissioner lum. Under the Legal Assistance for veterans, contracted to source of plow share and i know it is a very very efficient and excellent organization. The count veteran Service Office retain control and oversees these cases . Yes. That is a county veteran Service Organization who is the prime contact who is working with the plow share to coordinate the program and this was part of the Mental Health service act under the proposition 63 in 2020. Mike will present. Good morning. Mike zaugg. The item before you today is presenting a current policy in place with department. The office of Community Partnerships has provide services in the community of yes vote from you today adopts this policy you as the governing board adopting the policy. We are bringing today in request to California Department of aging who brought to the attention this needs to be adopted by the commission. This document before you is not a new document. It has been in effect since at least 2001 in this format with revisions over the years. This document is as you look through this Program Memorandum, it is a highly technical document as it is something that is kind of incorporating all the various regulations and requirements in order to create baseline for what our grievance policy looks like. I kind of included some additional documents to try to give a little context to whats going on with all this. We are committed to giving our clients the ability to raise their concerns about services. We ask it start at the provider level, but they have appeal rights that we respect and provide timely response to them along the way. I have this little chart that i built in there that summarizes the three levels. Again, level one is at the provider level. Level two, if a client is not satisfied with the result of their grievance at that provider, they are given the opportunity to appeal it to the department. At the Department Level it is handled by our clinical and Quality Improvement team. They are independent, they are part of das but different from Office Community partnership so they bring level of independence to the process and oversight. If that process does not satisfy the client, they have the ability to appeal to a third level, which is a panel of Advisory Council members who conduct a sort of formal but informal hearing on the matter and issue a decision. The additional document i got in here is that as part of this policy we ask our providers to develop their own policy and include the ability and require them to give notice to the clients they have this right. Here we have this is sample sign that we give as a template. It does not have to be used exactly like thish, but it is to provide some template for them. When our staff go out for our annual programming monitoring visit, we check to make sure that it is clearly marked and clearly visible for people accessing services. We also ask about clients who dont come into a center and how are they notified of these rights . The answer is usually through a welcome packet of information when they join the program. I think thats all i got for you today, but happy to answer any questions about this. Any questions from the commissioners . I do have a question and comment i guess. As far as the document itself, it says draft, but pretty much what is used since this has been in place since 2003 and so forth . Yeah. Okay. I will say technically this most recent round in addition to bringing before the commission they asked us to scrub the term complaints from it and to only use the term grievance which i have done. I opted not to bring this to you in red line because it would become a mess and didnt know how material it would be. Anywhere it used to say complaint, which might be a more easily understood term was removed by request for the term grievance. Main reason i am asking, i dont want to rock any boats but when i was reviewing it, i found the timeframes to file grievance were a little confusing, so you might want to just take a look at that. Okay. I got the big picture described on the chart is very clear, but it is the in between stuff, like when can a person file a grievance. Within a certain day from the time the action took place . Thats not defined and thentake a look at that if it needs to beit has been working then its fine. Okay. My last comment has to do the sign. It is suggested format not like a final thing . Correct. Okay. My suggestion is insure that the first step that the organization does indicate exactly the name of the person they need to file the contact with. You is a right to pursue the next step and says the Program Director but doesnt give the name or contact information. Contact. Right. Okay. Also the second step, we are using the information hub number as the source. Perhaps if theres ability to offer another way of contact rather then just telephone. Some individuals may have issues and difficulty to make that grievance. Okay. Could it be email or other way to contact das . Any other comments or questions from the commission . Can you give us athis is essentially in place for a long time. What is the volume ofand from at each tier . Like how often does it occur that units needs to confirm and how many complaints are there . The vast majority happen and are resolved at the provider level. We do not keep a close track on that, so we dont have the volume on that. We do keep track at that second level when it comes to das. You think it is typically in the 1 to 2 grievances per month, so10 to 20 per year. With not reallynot necessarily a pattern or consistency to what they are about or how they are resolved. How often does it go . Yeah. Out of that lets say there are 10 in a year, a couple will beour clinical Quality Improvement will make recommendations to ocp and the provider. I think we had things about where the provider was not clear enough or timely enough responding to the person on their concerns. We have some where there is no action that could happen and then there is always one or two that are abandoned along the way. The person withdraws. We have not in recent memory had one go to the third level. Thank you for asking my question. It sounds like a robust process. People can file grievances correct. Handling them and our clinical Quality Improvement team does a pretty good job. Their document it and come and talk to us and say whats going on and they give guidance when it is appropriate. One more question or another question. Just because numbers mean something and just wondering if the low level of numbers of grievances is possibly because people are not aware of the program exists . Right. Right. I dont think im saying everything is great because they are so low. But, i think we are going out and making sure that signage is there, making sure clients are aware of these things, so i think we are doing our best to make sure people are aware of these rights. I dont know we need to takeget morei dont know that we are informed we need to be more aggressive getting it out there. Thank you. Sandra teixeira, the Program Manager at the office of public conservator and been with the office since 2007. The office works with individuals that are deemed gravely disabled due to Mental Illness and or chronic alcoholism because they cannot provide for their basic necessities such as food clothing and shelter. The Adult Protective Service program insures the safety of older adults and adults with disabilities by investigating reports of suspected abuse and or neglect. As a routine function, the public conservator and adult Productive Services programs often transport high risk clients who have high levels of need, including those with the potential for unpredictable and aggressive behaviors, including inlopement. Use high risk Transportation Services on average of 6 times per month on annual basis. The clients are transported from hospital or jail to treatment facilities. Other examples are transporting clients to Legal Proceedings and or medical aopponentments. In the Adult Protective Service context, transportation is used to assist clients to meet with Legal Services providers to attend medical appointments and or to obtain benefits. So, it would potentially then you may need to modify it, because i imagine it doesnt seem like a lot of money, but may not be a big volume now . Comfortable with this budget, but we will be monitoring the care Court Petitions as they come in and we may be back to ask for an amendment, but at this point we are comfortable with this budget. The people we transfer, high risk transportation is a small group. Our office serves all most 700 people so it is a very particular group that we monitor extra closely by this mode of transport. Im the special project manager at das Community Partnership and i want to take a moment emily smith is in the audience with us. And part of the dccc. And commissioners, we are finally here. It is big moment. With this contract, San Francisco and department of disability and aging services will be taking historic action to address the impact of ableism on deaf and disability communities. This grant will create nation first funded Community Cultural center lead by and centering deaf and disabled community adding another chapter to San Francisco prolific disability history. Deaf and Disability Community have been Global Leaders in social justice movalment. 1977 leaders organized 504 occupation of the Federal Building. Stage occupation because no regulations to the new civil rights protections passed just a few years earlier. There was a [indiscernible] community occupied Federal Building for 26 days to win critical regulations to enforce the law. And this struggle served as the foundation to passing the ada. Establishing the Disability Community Cultural Center builds on San Francisco rich legacy and provide virtual and physical space to honor history and advance disability justice. The center will be a space full of life where people are able to access referrals to critical services, explore and express identity through art and education, be able to gather and advocate for change and connect to each other. You can see it over there. It is just across the street. There are structures already happening and over the next couple years you will be able to see the building itself be constructed. It will be at 165 grove street the corner of van ness and grove. It has a view towards this very room probably. It will be located on the ground floor of the civic center. Disability forward and Affordable Housing community built by [indiscernible] mercy houses with 112 beautiful accessible new Housing Units. Haven of home and [indiscernible] disability of sf state have a track record of providing service and cultural programming lead by and for people with disabilities. The services delivered under this grant will be accomplished in three phases. A planning phase, a virtual phase and brick and mortar phase. The timeline fl these phases will be depending on building development, however the first phase will start now and go through june of 2024 and thats the planning phase. In that phase, the grantee will work with das, hsa, kelsey and mercy housing on the buildout of the space and also deliver a project plan that includes some timelines and outreach plans, service deliverables, method of evaluation for brick and mortar space. The virtual phase launch july 2024 and last to around spring 2025 when the estimate opening of the building is kboeing to happen. The virtual launch will start before the brick and mortar opens and include creative and cultural programs and information and referral resources and development of Advisory Council. Collaboration dedication and advocacy, spring 2025 will culminate the grand opening and operation of the first municipally funded Disability Center in the nation. It is with enthusiasm disability and aging request approval for this contract with lc and lily cox haven of hope. Im happy to do my best to answer questions you may center have and thank you so much. As a member of the lgbtq community, i know how important the culture and strong identity are to the politics of something, and when you become confident about pushing forward with an idea and so im so pleased to see first of all, the history recognized and how this study made that connection between the importance of cultural awareness, since of history to your identity and pushing that forward to a new world and new vision. I get emotional, but im allowed because this is such a historic think so i want to congratulate you for this excellent presentation and looking forward to seeing it come from an idea. I love the fact there is going to be a virtualyeah, we can do this before it is built kind of idea, and then this building,x which is really going to happen very soon. I am very excited about it. Make a historical comment in the 70s and cant remember exactly what year, director silverman of the Health Department called the group of people to look at Health Cultural diversity and services were going in the country and one of the members of the Gay Community at that time said, we need our own cultural space and i look and said, i thought we were all in this together and he said no, there are different issues. I was a brand new to San Francisco and didnt know a lot, and to me this was one thing that started that going. It was essential to have that identify recognized in terms of healthcare at the time, and so i feel like i was there and this is very very exciting to me, so thank you very much. Thank you. I have to say, i was reviewing thes grant and report at home and i went yes we did it we are here we are taking the big step, so i share the other commissioners excitement we are at this place. I ju just want to also comment, i feel this is onea well thought out contract in terms of the three different stages. I appreciate the fact we are bringing in a Advisory Committee or aforgetthe steering committee. I think it is important and that is great. Only thing just a comment, not necessarily a concern. All the different pieces going on with mercy housing, with the housing part, with our contract here, the services with the Cultural Center, with the buildout of the building to how can we insure all the dots keep connected and there are not gaps or miscommunication through the process and will you be the one that is the point person for das . Yes, i will be the point person for das and now we have been meeting weekly with the kelsey and mercy housing and soon [indiscernible] grantees of the dccc will join us while making sure the movement and the construction of the building moves with the plans for the dccc. Aging and disability Resource Centers administer information referral assistance and Translation Services at locations throughout San Francisco. Providing support for many older adults, adults with disabilities veteran jz care givers. Services are structured to insure availability and accessibility throughout San Francisco with at least one adrc in each of the supervisor destricate,er providing services in the community where the client lives. April of 2022, the San Francisco supervisor dist rth lines were redrawn leaving the new district 6 without a adrc. In order to address the gap, the das office of Community Partnership al kateed a portion of existing California Department of aging adrc infrastructure grant to Fund Additional adrc in new district 6. The current grant term for all existing adrc ends june 30, 2024 and the new Grant Agreement will provide services in district 6 33 the end of that term. As mike noted in the rfp calendar presentation, rfp for the next grant term will be issued this fall. The independent Living Resource Center of San Francisco uniquely qualified to hit the ground running as district 6. They have been serving seniors and adults with disabilities since 1977. Offers Information Support and advocacy around a wide variety of issues which are vital and establishing and maintaining independent Community Based living. These includes adult day healthcare, care giver assistance and support, community services, health and wellness, education, emergency preparedness, employment, financial assistance, food and nutrition assisting, housing and are shelter, inhome care, vocational training, wheelchair care [indiscernible] well established relationships with providers across San Francisco and has robust outreach built into all the work they do. In addition to designated staff person who specializes in information and referral, all ilrc staff are able to provide information referral and application assistance. Majority of staff have disability and majority of the staff identify as lgbtq plus. Language is used by staff includes english, spanish, several dialects of chinese, farcy and asl or american sign language. The many years of experience combined with diversity of staff make naturally suited to operate as the new district 6adrc. Im seeking approval to modify the existing Grant Agreement with selfhelp for the elderly for the senior escorts program. The purpose of this modification is to provide additional funding that enable the program to increase provision of Escort Services to older adults and adults with disabilities for activities to insure safety on San Francisco streets for the remaindser of the fiscal year and term of the contract cht the funding providing specifically for this service by mayoral initiative. The funding will enable the program to provide an additional 750 Escort Services in this current year and thousand additional Escort Service in the following 3 years of the contract term. It will also provide some funding for operational costs. Selfhelp for the elder ly will provide Escort Services to older adults and adults with disability city wide in San Francisco. The Escort Service is one to one basis or in groups. Program staff will connect with consumers who have Escort Service needs once a referral is received, staff will confirm date, time, Meeting Place type of activities and provide the Escort Service. Staff will meet client and accompany them to their activity or wherever they need to go. I request approval to enter into this contract. Any questions or comments . Thank you. Is part of the reason we need the Escort Servicei certainly understand part of it is hate against the Asian Community and difficulties on the street and getting around and and certainly understand that. Is another aspect the general awareness how unsafe it on the muni and does thatis that a part of the reason . Yes. Im very aware of that. Initially at the very beginning the first time it came it was specifically about the rise in hate crimes against api community, but at that time when we first started i think our goal was to provide 200 Escort Services in chinatown, and it has grown to include where ever people need to go and how ever they need to go there and acknowledging it isnt just the api community but community in general both on the streets and muni, in lines waiting at the bank and various other places. [meeting adjourned] shared spaces have transformed San Franciscos adjacent sidewalks, local business communities are more resilient and their Neighborhood Centers are more vibrant and mildly. Sidewalks and parking lanes can be used for outdoor seating, dining, merchandising, and other community activities. Were counting on operators of shared spaces to ensure their sites are safe and accessible for all. People with disabilities enjoy all types of spaces. Please provide at least 8 feet of open uninterrupted sidewalk so everyone can get through. Sidewalk diverter let those who have low vision navigate through dining and other activity areas on the sidewalk. These devices are rectangular planters or boxes that are placed on the sidewalk at the ends of each shared space and need to be at least 12 inches wide and 24 inches long and 30 inches tall. They can be on wheels to make it easy to bring in and out at the start and the end of each day. But during business hours, they should be stationary and secure. Please provide at least one Wheelchair Accessible dining table in your shared space so the disability people can patronize your business. To ensure that wheelchair users can get to the Wheelchair Accessible area in the park area, provide an adequate ramp or parklet ramps are even with the curb. Nobody wants to trip or get stuck. Cable covers or cable ramps can create tripping hazards and difficulties for wheelchair users so they are not permitted on sidewalks. Instead, electrical cables should run overhead at least ten feet above sidewalk. These updates to the shared Spaces Program will help to ensure safety and accessibility for everyone, so that we can all enjoy these public spaces. More information is available at sf. Govt shared spaces. Once i got the hang of it a little bit, you know, like the first time, i never left the court. I just fell in love with it and any opportunity i had to get out there, you know, they didnt have to ask twice. You can always find me on the court. [ ] we have been able to participate in 12 athletics wheelchairs. They provide what is an expensive tool to facilitate basketball specifically. Behind me are the amazing golden state road warriors, which are one of the most competitive adaptive basketball teams in the state led by its captain, chuck hill, who was a National Paralympic and, and is now an assistant coach on the national big team. It is great to have this opportunity here in San Francisco. We are the main hub of the bay area, which, you know, we should definitely have resources here. Now that that is happening, you know, i im looking forward to that growing and spreading and helping spread the word that needs that these people are here for everyone. I think it is important for people with disabilities, as well as ablebodied, to be able to see and to try different sports, and to appreciate trying different things. People can come and check out this chairs and use them. But then also friday evening, from 6 00 p. M. Until 8 00 p. M. , it will be wheelchair basketball we will make sure it is available, and that way people can no that people will be coming to play at the same time. Our transgender Nonbinary Community advocates were really letting our government know that we needed to be heard. We needed to be considered and policy and budget decision and so, then the mayor lee and founding director of spark created officeof initiative that allow us to advocate for equity for transgender and nonbinary communitiful we focus on 4 areas. Training, education for the city employees. We focus on civic and Community Engagement making sure our leaders have a voice and are heard by our elected officials. We work on policies and programs to make sure our city is responsive to transand Nonbinary Community and add voice to departments to integrate transinclusion in policies, procedures and practice. We still have, lot of work to do to improve and address equity in san fran for our Community Upon i feel that we are on the right track and seeing how peoples lives are improving thanks to those changes. I do think it is unique that our local government is sponsive to transgender communities so i hope that people can remember that despite the work we had, we seat progress.
