So im going to start with the federal report and i want to say even though it seems like so long ago that we were advocating for the Older Americans act to pass, it actually did pass in march and that is good news even though were in a very different state of affairs right now, its really an important piece of funding for Older Americans act services. And so that passed in march. And since then, we have had two covidrelated stimulus bills that have put money into the Older Americans act programs and they are helping to fund some of the expansion that weve needed to do for services. The funding has specifically come out for nutrition, for family caregiver, for ombudsman and for aging and Disability Resource Centers and sorry, aging and resource connection and because california has received the kind of disaster designation by the president , we are able to actually use that funding in whatever way we want and so we are able to put it where we need it most, which now is mostly nutrition. At the state level, ive been doing a lot of work with the California Department of aging and they have really worked to provide support for triple as across the state. They have made the friendship line, which is our very own aging line for people who are lonely and need help or need somebody to talk to and they have made that a statewide program for the near future, which is pretty exciting. They hav are thinking about bang up some things that our communitybased providers are doing. And i really feel proud that our network, locally, has given the state so many ideas about things they can scale. And theyre thinking right now the longterm effects of covid19 on older adults and thinking about weighing that they can put new programs in place, thinking about opportunities for regioning senior centers, for instance, and thinking about community and thinking about social isolation and engagement and how we really think about using Digital Solutions and technology to really ensure people are not lonely. So there are a lot of ideas in the hop with th hopper with thet now and i think new programming will come out of it. At the same time, the govern has announced that there will definitely be cuts in the budget. Initially, the state was saying that they were waiting to see and they would put a new budget in august and they were going to move forward sooner with cuts that we will be hearing about soon. In addition well, something positive. That the state is doing right now and actually, the National Area agency on aging association is doing the same thing. May is Older Americans month and so, the state department of aging is running a campaign called make your mark and theyre asking local ceos and triple as to make your mark which means documenting things that are really positive, that are really engaging people and how were thinking about new models, thinking about volunteers who are older, who are calling other people, you know, really anything that were doing to respond and be positive, they are putting that on their website and asking us to engage in that. I know a lot of triple as have done that. I know were in the thick of things and hopefully well be able to do that sometime during the month of may. I promised we would do this even if it were may 31st. And so, thats something positive we can to. Do. We have so many great models were doing locally and i think well get some out there to the state and share all of our good practises. But locally, i just want to start by saying im really, really proud of our staff and all of the work theyve done. Our Management Team has been outstanding and really pivoted quickly to Emergency Response mode, really, really great thinking on the part of our managers in terms of thinking about how we develop new models on the fly and just so that we can be as responsive as possible to the need of older adults and people with disabilities in San Francisco. Im enormously proud of our new york and ceos that we work with because i think its just so hard for everybody to be in the midst of this pandemic and try to and be worried about family and be worried about children at home, be worried about parents who may be living in a nursing home and all at the same time be thinking about how do we best provide models, how do we best scale up the models we have and support the Current System and be able to do that effectively and with the grace that ive seen people do it. So i just want to start by saying that. Im really proud of everybody and just continuing to be amazed at our capacity to do amazing work. I want to start by talking about what were doing with our own staff and really the programs that we provide inhouse. So one of the things that we did right away is that we got staff out of the office and partly because some of our programs had worked really hard over the past couple of years to go mobile as much as we could with our staff, really to give them the opportunity not to drive in everyday didn and to be a littt able to work, you know, so they can work remotely and so we were able to get a lot of our staff out of the office. It was really important to us that they be safe and at home. So we did that across the board, for the most part. We still have staff coming into the office, but weve minimized that as much as we can. In ihss, in collaboration with the Public Authority, weve done a number of things that are aimed at protecting our Inhome Supportive Services workers. So one of the things that we did was to engage in covid19 testing for frontline essential workers. I know the team worked really hard to ensure that the city understands that ihss workers are, indeed, frontline essential workers. I think that we tend to think of workers, Health Workers as those in hospitals and clinics, and yet, we all know here that ihss workers are really essential. So our frontline workers have Hotel Accommodations available to all essential workers and we have been able to provide, finally, protective equipment for workers which, of course, is extremely essential and we know that our ihss workers are working with the most vulnerable populations in the city and that we very important. Locally sciu is providing protective equipment to all independent providers, regardless of whether theyre caring for someone who is suspected or believed to have covid19, which is actually a fairly new development in the pandemic and also a big relief, because people are really concerned about infecting each other and infecting clients. Sick pay, the department of social services has authorized 80 hours of sick pay for independent providers to pay for covidrelated issues. And we just asked if authorizing and processing the claims for providers and again, this is really important. Ihss workers do not make a lot of money and we really want people to stay home if theyre ill. So this is a way to make sure they can do that. And then taxi rides home, this is the same as the essential worker Ride Home Program you may have read about in the paper. It is available for ihss workers, as well. And then weve done a number of outreach to independent providers. We have a web page dedicated to providing information to independent providers about safety and about their rights, things like that. So the Public Authority is also conducting emails and mail outreach. One of the things this is such a big workforce and we wanted to make sure people had consistent and correct information and we know that information sometimes changes. We get new information from department of Public Health fairly regularly, partly because dph is trying to follow the science of, i think, many of you heard dr. Kolfax say, were following the science and sometimes the information changes. Go on to this next one. In Adult Protective Services, staff are continuing to respond on an inperson basis. When health is safety risk is present, 50 of our cases, receive a facetoface response. This is something weve had a conversation about, but we feel conversation when somebody is self neglecting, they may need somebody to lay eyes on them. So our staff have been really, i think, courageous because people are scared and its hard to think about going to somebodys help and infecting that other person. So weve, obviously, are using protection whenever we can. And we continue to operate that program. So reports went down for a little bit and now theyre back up to the prepandemic rates. We were worried that this was not being reported even if it was present. And so our selfneglect is the most common type. Physical abuse numbers are back at the prepandemic levels which, again, is a good thing because we need to be responding to those and we want to make sure people are reporting. So Public Guardian and public conservator prohibit to inhibit visitors. Our staff are monitoring clients statuses virtually, utilizing zoom meetings to see clients face to face. The courts have continued all cases on the Public Guardian side, continued all cases except those that involve an intimate health and safety risk of the client and referrals to the guardian have declined quite a bit. In april, we only received three because that is, of course, the system knows that the courts arent operating at their normal levels. Public conservator, courts are continuing to operate virtually whenever possible and our referrals went down for awhile and now theyve gone back up again. We had 16 new referrals in april which is quite a few for us. And then covid status of clients, our licensed care facilities are notifying us whenever a client is diagnosed as positive or if an outbreak occurs. To date, weve had five public cases and no deaths. In a public conserv conservatord four clients with confirmed cases and one of those individuals passed away, but we believe that theres another individual who wasnt diagnosed with covid but had the symptoms and we believe that person died of covidrelated illness. Our seaour vulnerable branch is supporting the Operation Centers effort to house unsheltered individuals by providing key supportive individuals with high needs and we have a couple of models that weve stood up and one is our cert model. Thats caregiver Emergency Response teams and the home bridge, as you home provides contract mode, ihss and home bridges Specialized Team is providing shortterm special Care Services with clients with needs across the shelterinplace hotel site and there are 100 clients who received outreach from the ihss assessment team. Home bridge is currently recruiting staff to grow this team and we assume that the need for this will continue to grow as hsa continues to move more and more people into the hotels, to ensure their safety and safe distancing. Lastly for our inhouse staff, we have what is now called is the Community Living fund, home bridge Adult Protective Services partnership and what it does is bring key Supportive Services together to facilitate care coordination and services for individuals with complex need. What this means is twice weekly meetings, discuss to triage highstressed clients and 15 clients are currently enrolled in Case Management as seen by Community Living fund. Adult protective services is working with clients who may need services, but who are declining them. So thats whats going on and i would have to give a lot of credit to joel neilson, Deputy Director who oversees these programs. She and her teams have really been just working so hard and our staff, all of our staff have been working many, many hours during the week and working on weekends and hopefully, at some point, soon, well get back to normal hours, but this is because this is an emergency, its been really important for them to stand up some of these systems and get them going correctly and collecting data and all of the other things that we have to do and i just want to give a huge shoutout to them for the amazing work theyre doing. I guess im probably going to overstate it a little bit, but i cant tell you how much work theyve done and how great it is that our team has come together. Ive been down here at the Emergency Operations center and i keep hearing about how great our staff and i just want to make sure to overstate it because to me, thats just important and i have seen things working well here and i have seen things not working well here and our team is working really well. To to movso to move over to they city, cindys side of the house, in march, all of our meal sites stopped serving in a congregate fashion and most of them were able to immediately pivot to take out meals and make sure they were still reaching out to their clients, which is really great. And they did it quickly and efficiently. So overall meals provided in march to adults dropped by 24 given the changeover in service. But then our projected increase from march to april is 99,600 or 163 increase. Total numbers are fewer, the percentage of meals served to adult with disabilities is served with a 30 drop in march and 172 increase in april over march and that was homedelivered meals and obviously, when we first got the order, we just kind of stopped everything and, then, it took us a bit of time to say, oh, wait, there is a way to do this. You can serve meals for takeout or you can serve meals and have volunteers deliver them to peoples homes. Over the homedelivered meals and grocery side, hour homedelivered meals have been probably the largest request from the public. Our benefits and resources have done an excellent job in helping callers identify what their needs actually are. For example, asking if they have a support system like friends and family to provide for them and asking if theyre responding to shelterinplace, under quarantine, or if they have an urgent need. This information is gathered to identify the most appropriate resource for them. And many of the calls have been addressed through their own support systemming systems. And ill talk about that in a minute. Even with these options in place, theres been a modest overall increase in home delivered meals. One of the things about our homedelivered meals, providers ive spent a lot of time talking to the ceos of our homedelivered and actually congregate nutrition providers and i think theyve been under an enormous strain and really have had to do a lot of quick thinking in quick work to increase their capacity. Theyve had fear of things like losing drivers or having the kitchen go down because somebody tests positive. And there have been a lot of challenges and food is so important and theyre concerned about any one piece of their system going down and not being able to meet the need out there. The need is really incredible. One of the things im doing right now is the reason im doing at the eeoc is im movinged feeding unit. This means coordinating all of the feeding efforts across populations across the city and its just incredible, the number of people who are newly food insecure. From march to april, our homedelivered meal system saw 11 increase and in adults with disabilities, it was a slight decrease, 2 , so not super meaningful. And we were i think part of the reason we havent seen a huge increase, a much larger increase, actually in homedelivered meal request is because we quickly reached out to shauntee project and asked them to work with us to coordinate a volunteer program that could help meet the food and other needs of people who are outside of the Current System that we have for home delivered meals and congregate meals. So they very quickly expanded their capacity, their volunteers and they set up a new program in conjunction with our staff, mike and kate and cindy, to build a program called our covid1 covi9 Emergency Response program and this is to ensure that people get connected with volunteers when theres an urgent need, like food or medication and no other way to obtain it. If they cant obtain it through our regular channels. As of the time as of the 4th of may, theyve linked 524 people to services and food support is the most common request. Also, i was talking with the executive director of shauntee, and he said one of the things the volunteers are experiencing and able to provide is really talking to people about their concerns and their loneliness and their fear around covid19. He said theres been a real component of that to this service and that people are really stepping up and taking that part of this volunteer service very, very seriously. And its not just about dropping off the food but having a conversation with them on the phone and really listening to their needs and their concerns. We also developed a Wellness Check to ensure that client needs are being met and we rolled this out to the services first, which is the ihh and aps clients and then rolled it out to all Services Including the communitybased organizations. So 20,000 so over 20,000 calls, almost 21,00 21,000 wells calls have been completed by programs with 217 followup calls made with people with unmet needs. Before food support is the most common unmet need, 70 of people who have responded to the wellness calls said that food was their concern and home care was 16 fund mass distribution, we started distributing masks to community and cbos when it became a requirement to wear masks in public. Theyve distributed over 500 and then education and training. We sponsor weekly we webinars ad we had a variety of subjects we have covered, including covid19 education, including social distancing and Infection Control and this was done in partnership with the department of Public Health and weve done contractual expectations and training on Wellness Checks and we talked about transportation changes during covid19 in partnership with mta. We have distributed legal information and then weve had the department of Public Health response and updates for sros, assisted living facility is skilled nursing. And then our Quality Management service team, our nurses provided trainings to staff, as well as determinan cbos on socil distancing and proper types of protective equipment for delivery of services, as well as method for donning and removing that protective equipment. And they have done 24 training in total. And in addition to that, the nurse manager has work with the hsa Occupational Therapists provide n95 fittesting for aps, cls and family and children and employee who have close contact with clients. Im sure im speaking for the entire commission in saying that the department has performed with remarkable agility and competence and resource fullness in addressing this situation and please convey our sincere thanks and admiration to that staff that is not here today or participating in this meeting. Its astonishing it would be extraordinarily challenging if the caseload remained the same. But under the circumstances, with such dramatic increases in leads, it is even more remarkable. So thank you again and thank yoe entire staff. I have a couple of questions. You mentioned early in your report that the governor has indicated there may be some budget cuts, but do you think that if congress and the president are willing to increase funding for states and cities in the next package, that those cuts might be mitigated . Yes, thats a really good question. And that certainly has been a part of the conversation. I think that it is very possible that some of those might be mitigated for the populations that weve served. I mean, obviously, its a known given that there are so many unknowns right now. I also can say that because San Francisco overmatches our programs in so many ways, we have been able to do a lot of things here that other counties have not been able to do in normal times, but right now, well be much meeting dollars that comes from the state, any augmentation to the other americans act dollars, well need everyone of those. Ive been on heightened advocacy mode within my associations for trying to figure out how to get those dollars. The National Association of area aging, theyve been truly amazing in terms of really advocating in the administration. And capitol hill for adults adultolderadults and theyve jun amazing job and have taken the lead among the aging organizations that are national. So i know that they will continue that advocacy and that we will continue to support them to do that. Thank you. Just a couple more questions. Im so glad that you are able to get ihss workers computerred too be frontline workers and got the necessary protection for their work. Are we able to test their clients for covid19 in any uniform way . So thats a very good question that im not sure i have the answer to yet. Because part of the fact that people in San Francisco can be tested is very knew and so were still working out what that looks like and how to make it consistent, as you said. So we will be getting more information on that and possibly what we can do scenarios we get that information ias soon as wet out. People are making appointments, of course, and somebody has even if there asymptomatic and theyre in San Francisco, they can get tested, but its a question of how do we help them get to the appointment and all of that stuff, because people at this point will be going to the testing point for that particular program. And finally, regarding the huge increase in meals, are we still able to deliver culturally competitivcompetent meals for c. Good question. The governor recently announced restaurant meals, program for seniors and part of the point of that program, obviously, is to serve seniors who werent served in other ways, but its to really help support local restaurants and union shops who are providing or restaurants who have the capacity to provide meals and to get up and run quickly. And so that program, im hoping, we will launch next week. Im down here running the feeding unit and the feeding unit is a part of that process. We have a number of restaurants that have applied to be a part of the program and, in fact, there have been over 60 and were making that program as efficient as possible and, also, you know, make it simple enough to work well for staff. And so one of the programs that were going to be beginning with that particular through that particular rfq process is the Restaurant Meals Program for seniors and so that will help with the issue. So right now, i think our staff are doing a very good job and our Community Providers are continuing do a good job of providing meals. One example of the way this is getting done is selfhelp through the ederly has contracted with the restaurant already that is helping them with the capacity issues and, so, that restaurant is getting out i wont give the number because i might get it wrong, but theyre really helping with of the capacity on getting out a number of meals to help augment what selfhelp was doing. The other thing that is happening here at the eoc is that we have Service Workers helping with delivery and so literally right out of here, at the mosconi center, we have a transit manager who takes requests from the Community Providers and helps with delivery in getting trucks out with delivery. So we have i cant remember exactly how many drivers we have right now but were helping with project hand deliveries and helping with deliveries with the food bank and deliveries for meals on wheels. Commissioner spears. I just want to echo our thanks for the work youre doing and i have one question specifically around elder abuse and whats happening there. I know in the life course, theres been a lot of uptick around that for children and i know that you said that the numbers have gone down and they have gone back to what they were around reporting of cases. Do you anticipate what do you think these longerterm impacts may be around elders who are maybe in situations where theyre not able to and we dont have have people that can go out and see them regularly . Its just an area of concern that i have been thinking about. So can you think to anything about that. There have been a lot of conversations state and federally, as well as locally. Certainly one of the biggest things were worried about is the scams that have popped up, covidrelated scams and scammers knowing people are at home alone and Financial Abuse is also a big concern right now. But i think families sequester together and that leads to a lot of strife and were concerned about the physical abuse, as well. So well just have to keep monitoring it. Certainly theres a concern across the violence organizations in general that those numbers are on the rise and that we cant actually capture what they really are. Again as mentioned by the president , these funds have been allocated to the city as a part of the federal family first Coronavirus Response act, which was passed and signed into law in march of this year. The total funds were allocated to each state and then in turn, they were based on a specific formula which considers the population over the age of 60 years of age an in related criteria. The legislation intends these funds to support congregate, nutrition and homedelivered meals. They do intend to use these funds for that purpose and youll see them appearing later in todays agenda when we seek approval for modification of current new Transition Program grants. And this item, before the commission is unique, as the emergency nature of the funds resulted in them being provided directly to the city without the usual process of us presented with a contract for the commission. I would sort of contrast that to the item up next, with the adrc, which is more in the regular course of business. And regardless, we still wanted to bring these funds to present them before the commission, for your acknowledgement and approval of their receipt. Good morning. Fellow commissioners, my name is kate and im the benefits and resource director for dos. The item before you is the department of aging, aging and disability resource connection infrastructure grant. And the San Francisco adrc is a partnership between dos who serve as the local triple a and the independent Living Resource Center of San Francisco and institute on aging Community Living fund. San francisco also has 14 extended partner sites which are called aging and Disability Resource Centers across the 11 districts across the city. The goal of the adrc connection is to develop longterm support services to divert adults with disabilities and older adults from unnecessary institutionalization. So in this 756,000 grant, the California Department of aging requires that this funding supports implementation and establishment of universal Access Points for benefits and resources for older adults and adults with disabilities. In San Francisco, were fortunate that we have created the dos hub and this serves as the onestop shop for services for these consumers. However, we rely with our partners across the city. Many of the adrcs are collocated at Community Centers to locate easy access to these. Weve created a system so regardless of where consumer reaches and who they talk to, theyll receive the same across the city. So they tend to utilize these funds to achieve two primary goals and the first goal is to develop an online resource directly. Theyre uniquely suited to launch a dynamic online director that will serve as a front door to services and information for consumers and partners alike. The directory is meant to be the first place an individual can go to get accurate, unbiaseddedbiad aspects of life to living with a disability. Making this available will keep resources current and accessible to individuals 24 hours a day, seven days a week. Theyre in the position of having the resources and wealth of connections across the city that are needed to develop a comprehensive directory for the San Francisco community. Our second goal to put in place, a phonebased Satisfaction Survey who call the dos hub. Since they serve as a phone center and plan to create a phone survey. And this will allow staff to allow someone when they call the hub if theyre willing to participate in the survey and transfer them to an Automated System which will collect feedback. This allows us to assess whether consumers needs were met and whether they experienced positive outcome. By creating both an online derogatory andirector, they gain understanding of consumer demands along with the ability to assist for consumer gaps. Having live, accurate data will prioritize and advocate for services across San Francisco. We intend to put out an rfq to get a couple of vendors to provide a bit more of a timeline for us. I do think the development of the online resource database could take many months, if not a year, to get something truly dynamic and accessible to the community. I wanted to add this item as an informational item to speak more broadly about the grouping of the subsequent items to give you kind of an overview of how weve packaged them this way, provide you some brief discussion on each of them. And then either give you the opportunity to ask questions now about this process or when we go through each one of them individually, ill b i can answr questions about the individual item at that time, as well. I think that due to the unique circumstances that we are in right now, we have a lot of staff on the frontlines responding working as part of the city effort and shereen has done a great job highlighting a lot of those. This is our contracting unit which a very high percentage, i believe over 75 of them, have been deployed to work either at the hsa, department Operations Center or the Emergency Operations center and the work that theyre doing is primarily logistics work which means sourcing of resources needed to support city efforts. So this will be things like the hotel rooms, the equipment needed for response, ppe, personal protective equipment and other items that are needed. As a result of that, that has left us short staffed and we are presenting these items to you in this distilled overview format. I want to highlight even with this presentation, the funds that we are seeking approval for today to allocate will still follow city practises related to billing and development of measurements to determine performance and impact of programs. And those are being worked on currently at just a slightly lower pace due to the reduced staff. Six of the next seven items do actually, rather all items require immediate action as they will support funding to continued currently existing dos programming and expand programming. Six of the seven items cover immediate Coronavirus Response need and the seventh item that well see at the end is a holdover from the canceled april Commission Meeting and is needed to continue programming already in place. Shereen did a great job highlighting the changes weve experienced in or congregate in our Nutrition Program and the funds here today are going to be what support those changes. I wanted to just i did want to also bring up some of the food banks who were mentioned today and highlight, in addition, some of the things they are doing. They have seen a massive increase in demand, as well, and the funding today will help support those efforts as they expand pantries and set up new models to include popup pantries to serve throughout the community and they are also committing to move all clients meeting certain criteria, including clients over the age of 65, clients with Underlying Health conditions that may make them more susceptible to covid19 illeffect, as well as adults with disabilities that make it difficult for them to access pantry services. They will be working to move all of the of those clients to a new temporary program theyre calling pantry at home which will work to deliver those groceries to them at their home. I would note another thing i would note with regards tor our nutrition items today, in preparation, theres a process in preparing things for commission and some of the information used to present these things are almost two weeks old at this point. And the situation has been changinchanging rapidly, and wek the demand for Nutrition Services likely outpieces the oe funding levels today. We will be talking about this at the next Committee Mission meeting. The shauntee item has been discussed and the importance of that and the new shelterinplace and Public Health requirements have left older adults and adults with disabilities each more isolated, harder to reach and disconnected from Everyday Services so that model has been great in trying to meet that gap. The last item well talk about today, this is support of shauntees social Isolation Program and this is a holdover from april commissions and this was intended as a continuation of enhancement of programming, which actually started last fiscal year, fiscal year 1819, which focused on expanding Programming Options for new activities being offered with a focus on wellness and healthrelated programs. And with the changing conditions, those programs are being shifted to online and other virtual models which continue to engage with clients in that program, with respect to shelterinplace and Public Health orders. And so thats kind of my investooverview of the coming i.
