Ofsted chief apologises to the family of Ruth Perry and says 'we will do more' to address concerns after inquest found inspection 'likely contributed' to her death - as headteacher's despair is revealed in heartbreaking handwritten notes

A coroner today ruled Mrs Perry, 53, took her own life after the watchdog downgraded her Caversham Primary School in Berkshire from 'outstanding' to 'inadequate'.
