We also talked about an emergency brake based upon data and based upon conditions that would provide for a framework of flexibility to tighten restrictions. We have counties now that we are moving back based on the blueprint. Based upon the emergency break. Based upon the protocols we put into place when we announced the blueprint 11 weeks ago. It will allow us to move back, or for that matter, to the extent we city ceased ability to allow flexibility Going Forward. But counties to move back after one week and not just two weeks. As you may recall, we collect data. We adjudicate the debtor with our local health officers, with the state of california. We have a two week protocol process to do that pursuant to the announcement we are making today. The counties will move back after one week, not two weeks. Its because of the significant increases we have experienced and i will show you the map it a moment. Some counties will move multiple tiers and not just yellow or orange, or orange to red. You will see some examples of counties moving from orange to purple. We will not be moving simply one tear based upon the data. The significant increases you will see some dumps for multiple tiers accurately counties that move back also must make changes urgently and set up a 72 hour rate period, we want to see the application and implementation of these new tiered status occur in a 24 hour period. Let less in orange and left in yellow. Take a look at the map. You can just see all of the purple all up and down the state. I will go back so that you can get a real sense of where we were just a week ago and now the purple throughout the state of california represented in the tiered status update that we are now seeing here today. Not waiting until tuesday. Every age group, every demographic, racial, ethnic, and every part of the state. We are seeing case rates increase and Positivity Rate increase as well. No longer concentrated in just a handful counties. We are seeing Community Spread broadly now throughout the state of california. It is a consequence and we want to update you on preparedness for this moment. We have not been sitting back passively or idly. Quite the contrary. We have been preparing precisely for this moment. We have been monitoring this moment. We have been foreshadowing this moment for many months, since the beginning of the pandemic. You will recall a number of search facilities that we had advanced in the midst of this pandemic and to those difficult summer months. We worked with the federal government. We worked with the state and the regional as well as county agencies to set up sites in fairview and portable appearing in sacramento. The sleep train arena. The sites are what we refer to in the office of Emergency Service lingo we are now dusting off the status and we have the ability with 11 search facilities throughout california to stand them up within a day or then certainly a calendar week. We have the capacity if needed. Again, if needed of roughly 2000 beds, total capacity in the search status. 1872 to be precise. I should foreshadow the first site that will move from the orange status and we will start preparing it. It will tell be down in imperial county. That is just a preview over the course of the next week or so. We will be leaning in a little bit more aggressively. Specifically into tier county. That is above and beyond the existing inventory which as of today is ample and i will get to that in a moment. I do not want to create more anxiety than is necessary at this moment. I also want to give me an update on the inventory we have been able to accrue. Because of our large scale contract with b whitey and others, and are simply making california to find mr. Californiabased manufacturers to provide sanitizer and face shield, gowns and the like. Our inventory is substantially greater than it has ever been. Just take a look at the total number of and 95 mask circa 180 million are currently in the cash in our inventory in storage and to add to that, over 522 Million Masks total. Over half 1 billion masks are now in inventory if you include surgical masks procedures and surgical masks into the total. You can get a sense of the preparation. You can get a sense of the effort. Particularly on ppe as it relates to the needs of our First Responders and to address all of that anxiety that we had to address and what we refer to as the wild, wild west days early in the pandemic. We have not been passive. We have been active at building up supplies, including, again the surge facilities throughout the state. As it relates to our approach, i will remind you. We had a threepronged approach. I will add the vaccine commentary as a fourth prong the moment. We will talk about that. The threepronged approach the vaccine around prevention testing and isolation. On the prevention side i will remind you we were the first state in the country to do a stay at home order. We moved a number of months ago to the mask mandate. We have already distributed 600 Million Masks. Is not the 500 million in our inventory. We have already distributed 600 Million Masks and we put out 11 weeks ago this blueprint for a safer economy with the four tiered status strategies. The testing has significantly increased over 21 million tests have been conducted in this state. The larger number of test than any other state in the company. Country. Per capita we are not where we want to be and that is why we opened the new state lab a few weeks back, which will allow us additional Testing Capacity at the peak of about 150,000 pcr test a day. The lab is just getting up. It will take a number of months but it is a flywheel every day, and every week. More and more tests are being conducted and over the course of the next few months, tens of thousands a day and eventually over 100 or 150,000 test a day. The test turnaround is not 24 to 48 hours, which is much better than it has ever been. Certainly during those very difficult periods and days we had test results coming back 13, 14, 50 days. Was making the test utterly irrelevant. We are pushing the metrics part of the claimant on the Lab Test Results that are mandated within 48 hours are expected within 20 for hours. 140, 160 for thousand average daily test now. We are conducting in the state. You will see the fight in a moment. Over 200,000 test were conducted or came into the result in our data collecting yesterday isolation is profoundly important. Also supplemented by the partnerships advanced through ucla ucsf and the Contact Tracing support with a number of state employees that have been deployed to help support and augment the local county efforts. Now with the vaccine. The bottom line is we are moving in the north to a sprint. This is important. This is the point of some optimism in the midst of all of this overwhelming data not only coming from this presentation in the state of california, but obviously all across this country with increased case rate. That is the good news that we all heard today around modern. This is coming on the heels of pfizer and the efficacy of their vexing coming site of 95 . Better than the efficacy that pfizer had an ounce. You will recall a number of weeks back, with socialized reclamation plan. We were one of five jurisdictions working with the cdc and the department of defense working with the federal government what we refer to as microplanning for our vaccine distribution. Rather than criticizing the administration, and rather than lamenting about what could or should be done, we engaged proactively and continued to engage in proactively to make sure that we have a plan for safe and equitable distribution of the vaccine and i will remind you, the focus on equitable. The critical nature of making sure that those that are most in need are prioritized in terms of the access and distribution of the vaccine and to make sure that it is safe. And we put together experts on committees, the Safety Committee. In fact the Biden Administration widely took one of our Safety Committee advisers as part of National Covid Recovery Plan on good news for california. There were a number of other californians that were brought in by the Biden Administration, was only will reinforce our capacity of partnership with the new administration moving forward. Experts in safety as well as experts looking at equity and looking at the distribution and looking at is to present again along the spectrum making sure a black and brown communities disproportionately, are in part because of the impact they felt disproportionately because of covid19. We have been preparing for that. Meaning and other network that is a four prong approach in terms of what we are doing to spread through the next number of months until we say widespread distribution of the vaccine. Not just limited 20 or so million americans that are likely to be vaccinated late in december. Again, those primarily will be First Responders and health workers. Doing the math, back of the envelope 12 or 50 of those would be californians. We have a long way to go to get to a vaccine that is widely distributed. Good news, nonetheless. Pfizer and dharna. By the way, those were the two companies we were working and microplanning to advance protocols. The good news is pursuant to the early plans, expectations are being met. The timeline is a little bit delayed. Not substantial but the efficacy is much greater that some of the early indications suggested. The net result is some positive double limits and some encouraging signs in terms of the proverbial light at the end of the very long and dark tunnel. As it relates to the Community Spread and the current tiered status, i want to just turn it back overcome a or turn it over to dr. Galli to talk a little bit more in specific terms about some of the county movement and what has been occurring. He will turn it back over to me and i will update you on the total case numbers and compare and contrast a little bit about what is happening across the rest of the country, and then we will get to some other specific comments and announcements, and questions in just a moment. Thank you, governor. I just want to take a moment before i present a little bit more about where we are with the tears today and some important reminders about what is ahead. Just to say, just three weeks ago we were talking about how we only had nine counties in purple and many of you, together with your neighbors and communities, did a tremendous job to get trent transition transmission done. All of those approaches are going to carry us through the weeks to come today. There is a move up now 41 counties in total in purple. That exactly underscores what the governor shared which is the rapid rise in cases. There is significant change in transmission. Not just in a couple of counties. Not just in one or two regions across the state, but statewide it is absolutely essential for us to immediately take control of and manage so that get out of this difficult period. We have the benefit of the great degree of preparation that we did as a state, as Healthcare Delivery systems across the state to prepare for this. Together we need to continue working person by person, family, community, household by household, and we can certainly make a difference here. Why area, today, sounding the alarm, using our emergency brake. Because we must keep transition low. That is to avoid flooding the Healthcare Delivery system. We know a few things today that we did not know early on. The days when we were talking about bringing on additional search beds, recruiting clinical healthcare staff to work the beds for this moment. Certainly but also tracking the data over many, many months. It shows us that cases today went up at the hospital beds in 2 to 3 weeks. Consistently at a rate of about 12 . We can, with todays data, and looking at todays case numbers, yesterday and tomorrow, begin to understand what is coming in the Healthcare Delivery system. Additionally, do not just do what we have been talking about for yourself, for your familys, really to protect our Healthcare Delivery system and the workers and their families in the systems as we see winter right around the corner. Cases started to increase with covid. You need to be prepared for the flu protecting workers facilities is key to a success in the state. What are the simple reminders that is boiled down to three sample things. The masking, the limiting nixing and the reducing our movements as much as we can. Upon masking we have talked about this for many months. Wearing a mask whenever you are in public. But i wanted to add two other important aspects. We talked about gathering. We talked about household gatherings but we know people gather with people they do not live with, often are Close Friends and family members, we think it is okay to put your guard down. We think it is okay to take off your mask, even for a little bit to enjoy a drink or a meal. But it is exactly those moments that might create a high transmission risk. We urge you to consider how you engage with friends and family over the weeks to come to keep transmission rates low. Additionally, out on this flight but an important aspect. We have been tracking household transmission and what do we mean by that. A little bit different than gatherings and transmission at events and gettogethers. Household transmission where somebody who may be works outside of the home is exposed and in fact, infected. They come home. They do what many of us do. They take their guard down. They spend time to with their 10th family appropriately. It ends up turning out they transmit covid to many members of their family. Rates based different studies across the globe and since the beginning have sort of pinned that at a third or even a half of family members that get infected. This becomes another important opportunity especially if you live with the vulnerable individuals, older californians. People with underlying conditions to consider how you interact with them even if you are not symptomatic because we know you can spread pretty easily and simply even within your own household. To underscore this, we put out gathering guidance again last week. We updated it and we want to just underscore some of the most important parts of that. Always keep your gatherings short. We know that is not always easy, but we hope you can find that way. Red and purple tiered counties are strongly urging, do not together indoors with other households. This is the type of gathering that is highest risk. We know the purple and red counties transmission is quite high. Without any knowledge, it is easy to have basic a person who is covered positive spread to a number of other individuals who are gathering with indoors. The orange and yellow counties that keep the windows and doors open. Something we have been seeing for the last couple of weeks to keep outdoor air circulating, wearing a mask and keeping your distance. And a special callout for high risk individuals who should avoid these gatherings if they can. But if they cant avoid it and they do attend, consider having a surgical mask or even an and 95 that fits tightly for those individuals so they are not at increased risk while indoors. Last thing i wanted to underscore it last week we talked about travel advisory. We along with our neighbors to the north, oregon and washington put out a strong travel advisory. This is not a mandate. This is not a trouble band. This is not restriction. This is really to discourage nonessential out of straight travel. We are asking people who do a go out of state and come home or who have guessed from outof state to do a 14 day self quarantine in the state for the mix with people outside of their household. This allows us to be confident and certain we are not having an address can additional risk of transmission. Especially with visitors coming to the state or you are returning to california from places with really high levels of transmission like we are seeing in the middle part of the country. And overall, possible, stay local. Stay within your communities. Certainly, short drives come in 2 to 3 hours to see others. That keeps the level of transmission and the fact that some communities have higher than other levels of transmission. That gives us some confidence that those movements will not create an increased risk for yourselves and for those you are seeing and for the communities at large. I will turn it back over to the governor to run through todays data numbers. Thank you, doctor. As you can see from the case numbers the numbers have substantially increased. 9090 is the total number we are reporting today, november 15th, of new cases. You see the sevenday average is over 8081 98. Lets take a look at the next slide. For. 6 Positivity Rate over a 14 day period. Beside, familiar to many of you as a 14 day slide. For. 6 . The sevenday is 5 . For. 6 , you may say sounds pretty good. It is only if you contrast it to the 9. 8 . It is difficult to see this on this Johns Hopkins flight. This is the new National Test Positivity Rate that came out over a sevenday period. 9. 8 again. California for. 6 . Do not misled by that. The rate of increase is alarming and while we have fared better than the national average, we do not compare ourselves to the average. We can do more and we can do better. You have done more, and you have done better. And that is testament again to sum total acts of 40 Million People that have really done justice to this very difficult period of time. You will see the average number of daily test. 160 for thousand. I noted a moment ago to 2000 tests that came in over the last reporting period. Really, as always, ashamed of the top headline numbers for no date generate headlines the total number of cases. We want to go after and find those cases. The Positivity Rate because that is a real sense of how we are doing from a Community Spread perspective from a surveillance perspective, really understanding the magnitude of what we are up against. And you can see the increase. You can see it here in particular looking back just to where we were on november 2nd 2014 day Positivity Rate november 2nd was just 3. 2 . I said just. It is still too high. 3. 2 . For. 6 today. You see the trajectory. Not surprisingly you will see similar trajectory and total number of hospitalized. 48 increase in hospitalizations over the last 14 days. You can see the trendline reflected not just in hospitalizations. I want to put that in a bracket. Before i go to i see you. Take a look at the total system capacity. This should for the moment you just heard from dr. Galli previewing things to come. That is why i updated you on the surge beds outside of the Healthcare System capacity because we are not waiting around. We want to be proactive. Look at the existing Healthcare System capacity. 5 for the current patients with covid19. Just 5 . Not 95 . Not 70. Some states that that is the case with the plus the present. In some cases even higher. The total number of people in icu at the hospital. At california it is 5 . In the icu it is a little higher. You will see 38. 8 increase in the last 14 days. And i see patients or take a look at the slide. 13 of the icu patients are currently in our Critical Care systems. Still 20 thousand. Almost 21,000 ventilators available. Ppe. Ventilators. Search capacity. The ability to move quickly with a warm beds and making them available, if necessary, if the system gets overwhelmed. 30 is a little bit higher than it was a week or so ago. It was 11 in the presentation last week. Critical care capacity, we monitor very closely. We will continue to monitor both hospitalizations going up, the icu going up, as case rates are going up and Positivity Rate are going up. Will not to be deterred in terms of our proactive stance and again, emergency brake that we announced here today allows us to move pursuant to this blueprint we put out. The blueprint is working as it was designed to work. The progress was being made when we saw stabilized rates and we thought a lot of movement to less restrictive tears. The orange tear. The yellow tear. Now, when we see case rates go back up, we are seeing Less Movement forward. In fact, now we are only seeing movement and 40 counties out of 58 that a moving backwards, pursuant to the tiered status. And so, that is the surgical approach based on conditions on the ground, county by county. We announced that 11 weeks ago the tiered system is working as designed, and all we are announcing here today is not only the 40 that are moving backwards, but not awaiting the extra day and not waiting Going Forward the extra week, in terms of making a subsequent announcement as it relates to tiered status. Two weeks, now just one week, and not waiting every tuesday to make an assessment and an announcement. We are making them as necessary today. We will make them not tomorrow. And as i noted a moment ago we are very opened and i anticipate by friday, dr. Galli will be back up making the presentation on the prospect of even more tiered restrictions based upon the conditions that exist today at this moment. That is the update in terms of where we are. As always, remind you credible importance of wearing the face coverings and masks. In where we put out Additional Guidance in the space i fiscally distancing, we are also augmenting and putting out more guidance as well was more activity moves back indoors based upon the more restrictive tears as well as the likelihood was moving back indoors as it gets colder, even and less restrictive tears people moving from Outdoor Activities back indoors. We wanted to tighten up the efforts. We are also considering full disclosure. A little bit of a preview. The notion of a curfew. Before you jump in terms of your mindset or weather or not it is a good or bad idea, we are assessing that as well. Have, on my desk, quite literally three studies from france, germany, and saudi arabia that have done comprehensive studies on the efficacy of their strategies as it relates to curfews. We know in the United States there are two states in particular that have statewide curfews of interest, at least pick that is massachusetts and alcoholbased curfew in the state of virginia. There are a lot of cities large and small all across the country also promoting certain kinds of curfews. All of that is being assessed. We want to socialize that. We have a lot of questions about what it looks like and what it doesnt look like. Who does it impact . Doesnt it impact. What is real curfew mean in terms of certain kinds of industry and Business Activities. That is what we are referring to this space but we really want the data to bear out and we want the information to not be anecdotal. We want to take a look at the studies. And also learn from portugal which is well known, at least come up with the following internationally. Some of the protocols as it relates to curfews. Always wash your hands and minimize mixing. On the minimize mixing i would be remiss if i did not acknowledge something just before we go into the q a, and that is, knowledge that a few weeks ago i was asked to go to a friends 50th birthday. My wife and i am. Woman ive known for almost 20 years and put a lot of time and energy into his 50th birthday. He was in napa and the orange status. Relatively loose compared to some other counties. It was to be an outdoor restaurant and we started the program at 4 00. It is was one of those early reservations. I got there a little bit late at 4 30. As soon as i said on i realized it was a larger group than i had anticipated. I made a bad mistake. Instead of sitting down, i should have stood up and walked back and got in my car and drove back to my house. Instead i chose to sit there with my wife and a number of other couples that were outside of the household. You can quibble about the guidelines, etc. But the spirit of one i am preaching all of the time was contradicted and i got i got to own that. I want to apologize because i need to preach and practice and not just preach and not to practice. I have done my best to do that. We are all human. We all fall short sometimes. We have been out and i think three times since that. I know it has been three times because i remember all of the dinners very vividly since february. Just three times. Twice with my wife, by myself outdoors and then this one occasion with a larger group and there were just a few extra people there than the spirit of what i am promoting. So if youre going to minimize mixing you have to own up to that. I just want folks to know that. The paid for dinner and we had an early dinner but it didnt matter. I should not have done that. I should have turned back around. When that happens, we paid the price but you also own the mistake and you never make it again. And you have my word on that. Just know this has not been a practice that i have indulged in the past. I take it seriously what we are promoting and have been doing so for many months. And you should expect nothing less of me and i expecting more from myself. You have that commitment in my resolve and that respect as well. When i say in mice mixing, i mean it, but again, a covered fatigue is exhausting and im empathetic beyond words. Im not here to browbeat anyone. Im here to find the higher angels and the spirit that is this moment to try to work through and get there they stand to get to the vaccines for the light is at the end of the tunnel but to work through what we had all anticipated. That is what has occurred in the last 10 days, or so. That is a rate of increase the makes this perhaps the most difficult and challenging time throughout this year and certainly since the beginning of this pandemic. They will touch businesses there is no distance between a business operating went responsibly and one that isnt. What do you tell these businesses that are playing by the rules and are frustrated and angry by this announcement. What is the benefit to them of doing what they state says the must do if they will still be penalized. Inspect the bottom line is, we have been very effective with the surgical approach to mitigating the spread of the virus. The original stay at home order was determinative in california, avoiding the first wave that many of the states of the nation have avoided. That was the consequence of efforts to really restrict movement, but also restrict activities. Sex activities in certain sectors of the economy. We put our guidelines and we have rules and we wanted to enforce the rules for individuals as well as businesses. As you know well, i come from a private sector. I have a number of businesses in the state of california. It is in a blind trust but i know a thing or two about Small Businesses and i use i know the hard work that is being done to not only address the state guidelines but also to figure out and navigate some of the local guidelines that are coming out, least of which of the guidelines coming out of the cdc as well. Dr. Galli has not been shy im turns of his admonition for us to be careful and cautious as it relates to people mixing in our household to encourage those activity to be outdoors. He made updates in terms of the comments about any activities that are indoor and the importance of wearing a mask people outside your household are indeed guests in your household. I want dr. Galli to come back up and he will can talk a little bit more specifically about the epidemiology. The fundamental point that you are making and the question you are framing is where are we seeing an increase in the spread. It is not just household. It is also businesses. But indeed, dr. Galli can clarify the point that i think you are making as well. Thank you for the question, governor. Absolutely right. We know that household gatherings are a part of the challenge of transmission. Cases do occur we talked about why. If they happen indoors, covid likes the indoors. If they happen pretty close by with people, covid likes to interact when people are close. It likes it when we take off our mask. All of these things which are features of highrisk activities that we often see small private gatherings with people we know and mess. With that said, we now also from a number of our counties, different issues in different counties and a number of our business sectors, least of which healthcare sectors we have been talking to you about the spread and some of our healthcare facilities, important. It also many of the businesses where masks come down and people spent a long time there. The office is one. We talked about some of the challenges with dining and drinking establishments, where people sit at tables. Even if outside, pretty close to folks who they are not with all of the time with the mask down. Those are the ideal situations for covid to spread and the situations that we know we want to try to avoid. The data to support that. Not just the local data, but the national evidence. It studies that are demonstrating the same. We are doing as much as we can. Employing every californian of how they mix the private settings and how to keep their guard up even when they are with the people that they love and have not seen in a while.
