Offering today is launching now more formally a scientific safety review. A workgroup of 11 individuals come experts in their field. These are Top Health Experts that will independently review any fda approved vaccines. A question i think it is, are you going to take someones word for . Of course, we dont take anybodys word for it here, we will do our own independently reviewed process with our world class experts that just happen to live in the state of california treasure trove of experts from ucla, ucsd san diego from partners that were able with Actual Experience in epidemiology from stanford university. People from uc berkeley and others that represent county and state health offices, all part and parcel of and independent group of Health Experts that will help us advance and closely monitor the protocols and procedures related to vexing trials. Let me just give you a sense. You can see this light of some of their expertise in biostatistics, epidemiology and pediatric Infectious Diseases and health equity. Care financing. Again, the spectrum of expertise. A diversity of expertise as well as a geographic diversity. And when we talk about health equity, thats the frame of cultural competency that is a big part of our commitment in the state of california. The proper distribution of resources particularly as it relates to vaccines and focusing on immunizations. That is Broad Strokes of the group that we are formally convening here in the state. We are very proud of the work that has been done by dr. Galli and his team to identify these experts and these experts again will independently review and monitor any vaccine trials to guarantee the safety, to guarantee the equity into guarantee the transportation are in c of the vaccines. Full disclosure, the transparency was put forward through Guiding Principles that we put out in a document. Vaccine plan that we submitted to the cdc last week. And return for cementing the vaccine plan, the state was afforded some 28. 86 million of federal support that we will utilize with our partners here throughout california. Meaning that 29 million will go to support efforts at the state level and also support local efforts in terms of advancing our Vaccine Planning efforts. The bottom line is the first phases of vaccine will likely be limited in terms of availability. Questions abound, when do we anticipate or expect the first vaccines to be made available . There are some stretch goals that you have been hearing that have been advanced out of washington dc. Those stretch goals have been organized around two companies in particular. That are asserting the availability of very limited supply of vaccines. As early as november and december of this year. We have different protocols and processes and procedures related to each company and their vaccines. They require different storage. They require different distribution strategies and plans and gives you a sense it is every Company Comes forward these happen to be from the cdc assertion that the two companies that are leading the space as it relates to the prospect of early limited availability. Moderna and for a pfizer and the number of vaccines will be very, very limited and require a protocol and a process of distribution focusing first on our essential Healthcare Workforce and our First Responders. Accordingly, the limited first phase supply and in a moment ill give you a sense of what i mean by limited. Also focuses on high risk groups. Clearly those 65 and over and those in Skilled Nursing facilities and longterm care facilities throughout the state not only Skilled Nursing the slaves but Residential Care facilities and those with disabilities. Those that are detained or otherwise incarcerated and obviously those that are often underserved that have become higher risk from an equity lens issues related to racial minorities in the state of california particularly the latino committee, African American committee. And never do we forget the Rural Communities in the state that often are neglected as it relates to the distribution of immunization strategies and vaccines. In this case they are front and center in terms of our planning. The bottom line though, with any appropriate vaccination plan is addressing some of the logistical challenges that we have come to long recognize as it relates to the challenges that have been imposed around covid19 transmission from the beginning. Viet ppe issues associated with testing and contact tracy. No different as it relates to a vaccine plan and getting under the hood and really looking at where the challenges may like. Lets start with supplies. Related to the distribution. Right now that supply needs are needles, surges, alcohol, pets, bandages, mask, pp ppe and the application of the vaccine itself pick to put it in perspective with h1n1 you didnt necessarily have the same symptomatic issues as you have as it relates to covered. As a consulting, many people may be asymptomatic and need to protect those distributing these vaccines with the appropriate masks and face coverings and the appropriate ppe. It at another layer of challenges. It relates to challenges and storage requirements. I mentioned storage a moment ago related to pfizer and moderna. Both pfizer and moderna that are talking about x number of millions of available vaccines for the United States total. By the way, the internal documents to give you a perspective and this is full transparency. By no means i dont want people to run with this number. But for planning purposes, we were provided an estimate. An estimate. The high end to put in perspective the high end estimate for planning purposes for vaccination a and vaccination b. Both pfizer and moderna. The total number of vaccinations for the nation for planning purposes on the high and through the end of the calendar year was about 45 million. Note that each vaccination needs to be used, rather each distribution of vaccination has to occur at a 21 day appeared. You actually have to have two shots, not one. From a logistics perspective, from a storage perspective, the requirement for notification, bring people back after the first position shot, you can take that number of 45 million in half cut in half and take a percentage of any given state including the state of california and youll get a sense that on the height and for a state like ours, it is 1 million2 Million People tops. And we have roughly that many people just in our Healthcare Delivery system alone in the state of california that will be prioritized. So if i havent lost you, i want to remind you that when we talk about vaccinations, dont anticipate or expect that you can go down to a local pharmacy in this calendar year and likely get a vaccination. We hope that is the case but based upon all the evidence, all of that data that we have been provided in the expertise that we have been able to glean it in advance or self in terms of protocols that we have advance we advance with fed cdc doing the five jurisdictions doing michael planning it simply unrealistic that the limited doses will be for a limited number of people. And we dont anticipate mass availability until 2021. The question for all of us is that in the first quarter, second or Third Quarter of 2021, might answer to my question is simply depends on who we talked to. Those who are more optimistic is q1, those that are more realistic is the middle of 2021. Those on the spectrum being a little more pessimistic are looking at q3 of 2021. It is 2021 for you and me that will likely have access and availability with these protocols and the vaccination planes we are putting out in terms of a vaccine. I just want to put a sober perspective on these issues and these challenges try to bring as much transparency to bear that we have internally as our want to make you aware what i and others are aware of so you get a sense of what we are up against. Challenges. Storage is a challenge pick let me show you why. You can see the storage requirements. These vaccinations or vaccines require ultra cold storage of 7 degrees celsius, below 7 degrees celsius so it requires dry ice. I can imagine you remember the transport media and imagine the swabs issues related and dont think that trice wont be a commodity in concern as it relates to vaccination strategies. Cold storage for other vaccines and we are looking at two at the moment for planning purposes. The moderna and the pfizer each required different storage and each required different distribution strategies. These are two examples and simply cold storage 20 degrees below celsius. That ultimately again require a unique challenge in terms of how you get them into urban centers and how you distribute them with ultracold storage considerations into rural areas and other parts of the state that are underserved. Again, all of these things are being worked through with our respective teams. The logistic teams and the team assembled that are looking at the efficacy and safety of the announcement we made today with 11 individuals as it relates to getting these vaccines out to people in a way where they know they are safe and are not concerned about their health. As it relates to Data Management is another big challenge pick the good news is that from an immunization and vexing perspective, we have an existing system and i am not nacve about existing systems in this state that cannot handle the kind of volumes that covid 19 has highlighted abundantly in the state. The good news is we do have a Data Management system called care. California immunization registry. Again, i remind you that the immunization that we are already doing are substantial in the state was significant. With the flu its a 19 million vaccinations pick p lack immunization that we are doing. They are all part of this existing infrastructure. I can assure you for those may have a question, what about cal ready and the other systems . Where we could purport similar competence. We never purported similar competence in the spaces. We have a little bit more with a little more with care but also have a team of people proactively as part of the work group on logistics really looking at the efficacy of the i. T. System itself. Because of so, Data Management is foundational. Notifications to get the second shot. Privacy considerations. Notifications between federal government and the state. No consideration is always in play. Of c notifications from the state to local Health Officers and local jurisdictions. If we include broadly and expand our points of access for the distribution of these vaccines to pharmacies as an example or blood banks as an example or other facilities, federal, state or nonprofit, nongovernmental, obviously the expansion of our registry becomes paramount in terms of making sure that is done with all of your Health Privacy concerns being addressed. As it relates to the broader issue of trust. The final bullet point is Community Education and engagement. This vexing plan will move at the speed of trust. You have to have confidence in the efficacy of the vaccine and covenants we not rushing to judgment in terms of its distribution and its access accessibility and making sure again thats we are a third set of eyes or second set of eyes as is the case here. This is what the workgroup is responsible for and this is what the Health Care Team is responsible for as well. Part of that outreach is warranting that trust by making sure that we are transparent and we are consistently engaged in a culturally competent way. Making sure you have trusted messengers pick making sure that we have appropriate psas and other educational material out there so people not only know where they can get the vaccines and when they are broadly available or whether or not they can lineup in q based on the privatization of distributions, high risk senior populations and just not First Responders in healthcare workers but all of that is part and parcel of a Community Education and engagement outreach strategy that is also part of our vaccination plan. I want to remind everybody that vaccines are not and this epidemic overnight. The bottom line is even if the answer to millions of americans, millions of californians get the vaccination and get that second shot over the 21 day time. Based on some of the early vaccines that we are aware will be made available, again there will be many different strategies presenting themselves of the next number of months. These are the first few that we are working for planning purposes, it will in this pandemic overnight. Thats why its absolutely essential that we maintain our vigilance and be reminded that the most impactful nonpharmaceutical interventions that remains as it relates to the issue of mitigating the spread and transmission of covid19 continues to be wearing a mask, physically distancing, avoiding mixing with people that are outside of your households. Maintaining vigilance as it relates to any modification of the stayathome orders as we move here to tear into these reopening strategies and areas and industries to do so by being accountable. All of us as consumers, not just as this leaders as well as those that are here to protect and educate our kids that we are stringent in our reopening strategies and maintain our sanitation which again means simple things as is flu season will. Just washing your hands and doing the things that your grandmother reminded you of as you are raised. Another thing that is critical is the issue of testing. On monday, just to give you a preview, next monday we will be announcing this remarkable effort. I am incredibly proud of this effort. We announced a few months back, just a few months back a partnership with kirk and elmer to develop a facility in california from scratch to significantly increase our Testing Capacity here in the state of when you pick at a time when your sink testing being pulled back, we are accelerating our testing efforts. We announced last week and updated you on some of the isolation and quarantine strategies. The Contact Tracing work that we are doing. Again, testing a big part of the isolation and the containment of this disease. More testing pcr tests available across this state and that will only help in advance our efforts to reopen and Quality Support corrente and isolation. And we are not walking away from treatment at the said counseling is abundant with biotech and biotherapeutics. No one does it better than we do. The credible partnerships with the uc and csus in the work being done at stanford and other universities with translational quality of research and development from federally funded Research Institutions and not just independent and private institutions. Again, therapeutics dominantly therapeutics coming in and emanating out of this state. And we continue as we have a therapist task force and i look forward to updating you on that task force shortly. Ive been hesitant to want to over from us in the space pick a few elected officials have been on the therapeutic side. We have been more cautious but i think it would be good to highlight you on some of those available therapeutics that are being tested here in advance trust. Uniquely and disproportionately impact in the state of california. All of these approaches, forgive me for being long winded. All these approaches are foundational in terms of efforts and approach to getting us pass so we can turn the page on covid19. Vaccines are one part of that approach. We have all of these other modalities and strategies and prevention containment and treatment that are also part of our approach to get back to work in getting our kids back into school and getting past this pandemic. One thing i cant resist is i was out this weekend and saw a number of people that had their masks on but not the correct way. I want to remind people that when your nose is exposed, it may be more comfortable but it is not necessarily the right way of wearing a mask. I dont malign anybody or begrudge anybody for momentary lapses. We all, including me, and all of that is part and parcel pick its not about browbeating anyone. A quick reminder that this is not the way to where this. And we had to put this up because i think a lot of us are experiencing with friends and family members and perhaps those looking at us in the mirror with the incorrect use and want to encourage you with that correct use. Here is the slides of some caution and consideration. We have been previewing this for a number of weeks. You may recall the hospitalization chart that many of you are familiar with. We have seen decreases over 14 day time. In double digits consistently over the last 67 weeks. You see here of the last 14 day and the last two weeks a 4 decrease in total hospitalizations. We are seeing a decline in the rate of decline in hospitalizations. And we are getting to a point where if you just extend that trend that we are beginning to flatten out and plateau was to hospitalizations. Even more interesting and concerning to the extent it is obvious point of concern for all of us and that is icu admissions and state. Nursing commensurate 4 on hospitalization decrease just repeat percent decrease in admissions related issues over 14 day. But over sevenday period, if i had a sevenday slight, youd actually see a slight increase in icu emissions. And still aid decrease over about what more of a reminder you need in seeing these numbers plateau and you can see just from that low we experience, we are seeing a number of new patients. Still doesnt but nonetheless in the icus, each and every icu admission is precious and obviously a point of real concern as it relates to the importance of quality care and timely care. And obvious concern around mortality and morbidity. So a point of caution, and again what more do you need than this slide and the fact that i think it was roughly 38 states in this country that have experienced a 10 increase over the last seven days of total number of cases. This is the dreaded third wave that some had projected. For california, this may be better termed as a second wave since we never experienced that first wave like many did in the north east part of the United States. We really had our first wave extended around july button on the last the second or third, this is what all the epidemiologists in the scientist and anyone who followed any history and data from 1918 and 1919, the spanish flu endemic this is what we anticipate moving into the colder season and moving to where most of us are moving inside and into a season warm of us begin to mix and come back with extended family members for the holidays. This is an area of obvious and concern and being sober and stubborn about some industries in the state that i know are eager to get guidelines. Tomorrow dr. Galli will update you on some of those industries and guidelines including sports and some of these theme parks where we will break up the theme parks and not just one or two brands its many different parts that are part of the theme and part industry but dr. Galli will update you tomorrow on those guidelines and again, i hope one recognizes our stubbornness on a health first data driven decisionmaking process. Its done with eyes wide open on whats happening now around the world. Not just whats happening across United States. Not yet here in the state of california but that is only because we are being vigilant and have to maintain that vigilance. We need to avoid any further increase in transmission. Just a final few slides and open ourselves to any questions. Here is an updated slide from last week. A new tier status. You can see 10 counties in the purple status which is the lower tier as it relates to the most restrictive guidelines related to covid19. Now some 48 counties at of that purple status with 27 in red. You see 13 in orange. And eight counties now in yellow status. Not just in Northern California but also parts of Central California. There are parts of Southern California though in particular thats remain very concerned about pick dr. Galli will update you tomorrow. The new press conference around some of the interventions. We hope to advance in a number of different counties not dissimilar to interventions we advanced in the Central Valley and number of months ago. As it relates to increase and areas of concern. Those will be again reviewed and he will transmit some of the guidelines and recommendations this time tomorrow in that space. Let me just briefly remind everybody that we continue to battle these historic wildfires and making progress on the spread of the largest wildfires in the state. Some 4. 8 million or 4. 1 million acres have burned. Year to date. Throughout the state of company pick i think 3. 8 million since august 15. 12 major complexes right now. We are down 7400 firefighters. Thats down significantly from the number of firefighters working the lines just a few weeks back. Heroic work and extraordinary work by cal fire. Our mutual aid locally in federal. They have done a magnificat job with these larger complexes. Including some strategies on prepositioning and prevention that went into place last week with these high winds that we experienced with higher heat or humidity. In the northern part of the state. I should just note that we anticipate those conditions to return again later this week. As early as tomorrow evening and start to see some winds whipping back up in Northern California. Southern Central California bearing a little bit better from the wind perspective but that could change quickly. The next eight days based upon our briefing this morning, the next eight days around the northern part of the state in and around where im north of where we are. Up into the border, expect temperatures to not be as high as they have been in the past. But the humidity levels are concerning and the winds whipping back up will put a lot of pressure on doubling down on prepositioning strategies and prevention strategies. Just a brief overview. 31 lives that have been lost in this wildfire season. Almost 10,000 structures with 9282 structures destroyed. Those are destroyed and not damage. The damage number is substantially higher. Just a reminder of the challenges we continue to battle as it relates to the hottest and not just august in recorded history but the hottest september in recorded history. A reminder of what we say often is that Climate Change is here and it is real and it exacerbates and challenges us. It is not alone exclusively. The reason why we have wildfires as we have had a fires historically but the ferocity and the devastation of these megafires is such that it is only made more challenging worse in the conditions that Mother Nature has provided us. Again, she has joined the climate conversation. I will remind you what has been said by many others in past. She bats last. She bats 1000 pick she is chemistry. She is biology. And she is physics. Shes not a political point of view or an ideology. As it relates to objective truth, the august complex, 1 million acres. The largest fire in our states history. What more proof do you need of the concerns we have around the hot getting hotter and the dry skinning to pick 75 contained. Last week we were 82 contained and holding the line and working with the partners at the u. S. Forest service on the complex. A brief of rupert the final creek fire in fresno and it is 55 contained. The for Service Working these lines as well pick 333,000 acres last week pickett is grown a little bit over the last seven days. 61 contained today with 350,000 acres. There doing a wonderful job on that fire. Some areas of the creek by that they are just allowing to burn safely burn. But nonetheless, containment beginning to improve on these two very large and what now i term iconic wildfires here in the state of california. Speaking of iconic, we have a unique strategy here in the state of california that is now being replicated in other parts of state. Of course we took the cue from the five state that do mailin ballots. We have a deadline today. Its the last day to register to vote to receive your mailin ballot before election day. Its not the last day to register to vote but its the last day to register to vote to receive your mailin ballot before election date. If you want the convenience that so many of us are afforded, please register so we can send that mailin ballot to you so you can get the opportunity and the privilege to be an active citizen and not inert in this election process. But to think about it, the logistical challenges we heard the governor talk about is not just the storage of what will be tens of millions of vaccines at some point one day down the road, but also the supplies. We will need all the needles and the bandages and the ppe as well. And the governor said that when it comes to mass availability of a covid19 vaccine, we shouldnt expect that until 221. Whether thats the first or second or Third Quarter of next year. The governor said he could not answer that pic theres been a lot of talk about who would get the first doses of the covid19 vaccine. There has been some agreement that certainly frontline healthcare workers would get the first and also those who are most susceptible to coronavirus and those with Underlying Health issues. But again there is no plan laid out just yet but a lot of people are concerned because the people are hinging their hopes on that covid19 vaccine. Sing was that vaccine is widely distributed, then perhaps life can start getting back to something close to normal like a year ago. We will keep monitoring the question is responding to.
