Now keep in mind, five million is the low estimate, due to asymptomatic carriers and limited Testing Health officials believe that the actual number could be ten times higher. I can understand why he is in favor of providing that because come january everyone in his family might need it. Trump also signed other executive orders including a payroll tax holiday, Student Loan Relief and Eviction Moratorium. Of course he wants that, come january everyone in his family might need it. A joke so nice we did it twice. Laugh. I will say this. Trump facing reelection and finally gave voters something that is not a virus. One problem, its against the law. Speaker nancy pelosi said this about trumps executive order. My constitutional advisors tell me theyre absurdly unconstitutional. In order to get the money trump is requiring states to pick up the tab for 25 of it and states must first agree to enter into this financial agreement with the federal government. Are you in the same digs, new digs, its been awhile. I think last time i was in new york city, im up in vermont now, remotely broadcasting. Stephen okay, my best to the cows. Doctor, sadly the United States just passed five million coronavirus cases. Where are we as a world, finding coronavirus and where is United States within that picture . Well ive been saying from the beginning this is going to have a beginning, a middle and an end and right now were in the thick of it we are in the middle of it. And i would like to think that we were doing better than we actually are. I mean i have been disappointed that we havent all been rowing in the same direction, there has been some mixed messaging. But on the positive side, youve seen this tremendous private, public, Academic Partnership that has done all sorts of really impressive things. So a mixed grade right now. Three viable vaccines are in testing right now. A lot of people think of a vaccine as a cureall. Lets say we get lucky and one of the vaccines turns out to be effective and safe at the same time. What does that look like . How does it roll out and when are we normal again . That say great question. And its one that tony fauci has been talking about, about not wanting people to have unrealistic expectations. So lets say everything worked out well at the end of this year, beginning of next year. Weve got a vaccine that we are ready to give to people. Stephen and thats it. That is as fast as you think, still even with all of the progress that has been made, that it ended this year, beginning of next year when it is efficacy and safety is proven . I think so. Are you seeing testing right now, there are big phase three trials so people are getting it. They are getting the vaccine. The question is how many months need to go by before we realize it is safe and effective. So far it has been safe and so far at least in the lab were seeing immunologic responses to peoples blood streams that look promising. The big question is is it going to really work in real life. And for that we need many thousands of people out there actually getting the vaccine in areas where coronavirus ask rampant. And sadly, coronavirus is wide spread enough in the United States and in the world that there are sites where we could adequately test it the unrealistic expectation thing that tony fawch si worried about is okay, its january, say, were rolling it out. And they are already betting they aric maaing, they are manufacturing hundreds of millions of doses of these now. They are not waiting to see whether the tests are successful. They are saying if it turns out it is successful, we will have all of these doses of vaccine. If it is not successful, we throw it out and all we have lost is money. But it is still going to take months to roll it out. It is going to take awhile to see is it effective. Do we need a booster. So people shouldnt think january, february, march it is back to normal. At the beginning there will be a crossover period where he where we still need to dot things we are doing now, wearing a mask, social distancing and things like improving ventilation indoors. Stephen a new gallup poll says one in three americans would not get the vaccine today even if it were fda a prfd. What does it mean to get in the country back on track if a third of us wont take the vaccine . It is a significant problem, stephen. Because think about this. You know the concept of herd immunity which is what percentage of the population needs to have immunity to protect people that arent immune. That number, is argued about, lets say it is 70 . Well, i 30 of the country is not taking the vaccine, then the other 70 of the country who take the vaccine, the vaccine has to be 100 percent effective in order for you to get that 70 herd i meurnity. It will be a big problem from 30 right off the top dont take it. Stephen a major concern for a lot of people. And in the news all the time rght now because of the kal den kal ender is getting children back in school. The president has said children dont really get it. But are we learning that children can not only get covid but spread covid. There is no doubt that children can get covid. Fortunately they dont get as ill, the delt rate is not nearly as high as people who are much older but there is to doubt they can get it. The big question is to what extent can they spread it. If they are at school and pick it up at school and bring it and infect grandma, you know, that is pretty bad. Stephen is there any reason why covid wouldnt spread as much as say a common cold because i have had children. The children are disease vectors. Like every parent once their kids start going to school, the whole family gets sick. Why would this be any different . Well, you know, the common cold is actually, this is a variety of coronavirus, which is in the family that causes the common cold. But you are absolutely right. Have i had kids too, they are now 24 and 28, when they were little they are all over each other. There is o no way you are keeping them away from each other. They are doing everything that you can imagine to infect each other. I think in terms of the school, the first thing that parents need to ask themselves is does it make sense, does it even make sense to try to go back, in the area of the country where i am. So if it is rampant it is one of these red zones t doesnt make sense to try to go back because you know you are just looking for trouble. If it is a green zone, one of those places where there are relatively few cases and the percentage of positive cases in surveillance examinations is relatively low, then you have to ask, is the School District add quaint adequately prepared. The devil is in the detail, one, is there a plan that says wear masks. Do physical distancing. And then this thing about ventilation which i think as it gets to be the cold weather, we know that outdoors is a lot safer than indoors and a lot of these schools may have old ventilation systems. Are they adequately addressing the question of indoors with relatively poor ventilation, is there adequate, are there adequate measures to make sure the air is healthy. Stephen okay, testing. President says weve done 55 million, 60 million tests, more tests than nibbles. Have we dong more tests than nibbles in the world . We actually have done more tests, but the problem is that even though we are doing all of these different tests, our death rate per 100,000 population, last time i looked it was number four in the world. So it is not just a matter of how many tests you are doing, it is what is going on in your country, is it a huge outbreak. We want to do more testing. But as you do mr and more testing, you would expect to be testing people who arent as sick. So you would think that the percentageofpeople who are testing positive would go down. Well, until recently it is going up. Have i to say in the last week or so it went to about 8 last i looked. No you it is starting to go down in some areas. But the problem is, stephen, that our 50 states t is like 50 different countries. So go try and figure out what is going on in what area depending on the politics. It is really hard. There are websites that will tell you what is going on in your neighborhood in terms of the schools, people, how are they going to keep track of this, what is going on in their neighborhoods and what is going on in terms of recommendation. Cbs news has this series called School Matters that will start next monday. And it is to try to help parents. It will be on the cbs evening news with Norah Odonnell who was a guest on your show recently. And we will have it on cbs this morning too. We will go over all kinds of things like ventilation, how do you know if it is safe in your school, what is the psychological issues you have to address. All sorts of things having to do with students. Lets talk about the festing. A lot of testing is taking four days, or a weak to get results back. Why is it so important to et go the results back 24 to 48 hours. You know, if you take a test today and you get the results back seven days from now, what good is that in terms of protecting the community. Because the whole idea is that if i am infectious, i want to foe about it quickly like within 24 hours or sooner so i can sequester myself. I can quarantine. And then somebody in Public Health department can figure out who my contacts are and isolate them. If it is happening seven days later, by then i have already infected who knows how many people. So we really, one person who i saw suggested if a lab gets a result back more than 72 hours later after you took it, dont pay them for it i think there has got to be some incentive to turn it around more quickly. Now lets talk about miscon semtions about testing negative and not being infectious. Because i was outside of the tri ds state area when i was doing my show for the five months and when i came back to do this we had to sequester for two weeks, basically stayed in the house, cleaned a lot of cabinets, you cannot believe how organized my sock drawer is right now. We tested negative immediately when we came back but it didnt matter. The rules in the tri state area are even if you test negative you have to quarantine for two weeks. Why is that important . Good rule. So if you your viewers hear nothing else in this discussion then this, i would be satisfied. Because this is the number one misconception, by far, there is not a close second in the entire pandemic for my patients in internal medicine and other people spheeking to me. You have to understand that the incubation period is 2 to 14 days. That means if i get infected today, infected today, i may not be infectious, i may not be able to be shedding virus or contaminate other people, infect other people from anywhere from two to 14 days. Tat means i am infected today, its in me but not yet shedding virus and able to infect other people. I could test negative five days from now. And then think oh, i can visit my grandma the next day. No. Because six days after i get infected, i could become infectious. The seven days, eight days, fine days, up until 14 days, so i have so many people say i just flew in from los angeles, who knows if i was infected, i will get tested today and see pie grandmother tomorrow or somebody else tomorrow, no, you have to assume you were infected. And if you really want to be careful you have to wait those 14 days. Now what are there any treatments out there are you hopeful about. I am, since last time you mentioned remdesivir recommend that has shown to be effect nif shortening hospital stay and who knows in a different setting t may actually show an effect lowering mortality. We dont know that for sure. Dex dex decks met doan has been shown to lower mortality for people who are inteu dated inteu baited and the trials of con va less ent plasma, you asked about it last time, if somebody gets infected, we know there are a lot of trials right now and the jury is in the in yet, you get infeblghted, you have antibodies you built up in your system, you take some of those and give them to a patient as various stages, maybe who was exposed. Stephen the example was can he go take tom hanks blood. Exactly. And the answer is no, because that not that we wouldnt want tom hanks blood, who wouldnt, what a nice guy, maybe we would all become nicer. Stephen i dont know. What they are doing, the vaccine actually challenges your immune system and makes it think it has been effected so it mowpts its own immune response, then are you ready to go. If i am giving you antibodies it is called passive immunity. While the antibodies are there, i may get some protection but once the antibodies are gone nay month or two that protection goes away. What is really interesting is something called monoclonal antibodies, there are a couple of trials. I am excited about that, a Technological Tour De force.
