Too few hospital beds. Not enough gowns, masks; a desperate shortage of ventilators. Youve seen whats happened and what continues to happen in italy right now . Yeah. Dr. Maria raven is chief of emergecy medicine at the unversity of california San Francisco hospital youve got me on one gurney and youve got a 25yearold on another gurney. Youve only got one ventilator for the two of us, who gets the ventilator . Thats a very tough question. I mean, i think the answer is that we would use, if we were to follow principles from a specialty like transplant is that people that have more life years, might get the ventilator. But there are also many other sort of principles that could underlie the decisionmaking. You have to take into consideration underlying illnesses that people might have and you also have to take into consideration if there are other temporizing measures that you could use in one person. So that maybe you after all, you wouldnt have to need a ventilator immediately. Maria, maria, maria youre tapdancing. All right. I mean, i cant, this is a tough question. I mean i dont necessarily want to speak for u. C. S. F. On this, because they havent put out their guidelines yet. I mean, think, yeah i think. Actually the Clinical Working Group is working on them right now. These are terrible decisions to leave in the hands of doctors on the front lines , the ones actually treating patients. Well, there are certain situations, relatively few, thank goodness, that are catastrophic events that entirely overwhelm our ability to do whats right. Haiti was one of those. Dr. Thomas kirsch is an emergency physician sounding the alarm. Hes afraid that what he confronted in haiti in the aftermath of that countrys earthquake a decade ago desperate shortages of medical supplies equipment, personnel, could soon face us here. And that was the same thing i faced when i was in haiti. And its about the most difficult decision that you can make. Youre still choking up after all these years later. Why . Good question. You know, you think after ten years id be beyond that but its, again, its such a difficult decision. There are guidelines. Ny state put out a massive thoughtful set in 2015. The italians have drafted a set based on their current experience. But every hospital follows essentially its own guidelines. Were trying to save the greatest number of lives possible. So, in some cases, these guidelines would really shift radically the way we work and put both patients and doctors and families in the terrible position of saying, 11we cant do what you would want. Dr. Tia powell is director of bioethics at Montefiore Medical Center in ny. And the basic goal is to divide people into three large categories. One, the lucky group. Youll have a very mild case. You might feel miserable, but youre not in danger. The group that youre looking for in a hospital is the group that is very sick and that actually may die without actually may die withoutCritical Care resources. But if we can get them those resources they have a good chance of pulling through. The third group is the most unlucky. Thats people with a terrible prognosis, people for whom even with Critical Care resources the chance of surviving is terrible. Im sure not a term that you will happily use; but were talking about rationing. Its a form of rationing yeah. And i agree, everybody tried to avoid that word, but i would actually like people to understand what may be coming. What, what we fear will happen, what is most likely to happen isis that persons of, who are poor, persons who are racial minorities are less likely to get access to ventilators than those who are wealthy and well connected. David williams is a professor at the Harvard School of Public Health. And what makes it even more challenging is that the disadvantaged groups who we just talked about are the one who begin this process being more vulnerable. Because they have earlier onset of disease, more likely to face chronic, ongoing stressors in multiple domains of their lives. So we are looking at those most in need being most likely to be disadvantaged. We have to keep trying and doing things. So is it likely that people will die from lack of access to ventilators . Probably. Does that mean as an individual patient theyre likely to die . No. Because were not gonna quit trying. So, i understand why youre fighting me on this. You dont want to let anybody believe that the medical community is just gonna give up. And im not suggesting that you will. But i am suggesting in the strongest terms that we dont have adequate supplies to meet the likely demand. You dont think thats an unfair statement, do you . Given my knowledge of the situation thats not an unfair statement. But im not a supply guy. Im a doctor. Do you think the public is adequately informed about no no no. Im not, i dont think that the Health Care Community is adequately informed. And i think, you know, experts have been working on this but now, it is really a realistic possibility. And its a realistic possibility not in a year but soon. I never thought i would see this day. And were not quite there; but realistically, many, many Health Professionals are preparing to do just this. Managing type 2 diabetes . Dimitris on it. Eating right. And getting those steps in . But some of those people should probably get some roughage in their diet. The third thing ive realized is that i love the internet, thank god for the internet. The internet has been an invaluable tool for communication, and also for creative outlet. For not just me, but also my family. [ ] alicia keys so for the past few years the 39 year old married mother of two started doing some hard soulsearching, and the result is a book not a memoir, but a journal of selfdiscovery. She goes back to the very beginning, writing about how her mom, terry augello, who was then a single paralegal, had a relationship with Flight Attendant craig cook and got pregnant. Your mom talks about how she she nearly had an abortion. Right. And even her mother said, terry you never do anything easy you know . And she was making a really big choice and at the time, im sure she didnt even know why she was n2 makin that choice exactly but she knew it. She knew what she needed to do. Alone and financially struggling, terry raised her daughter in what was then one of new york citys toughest neighborhoods. Our interview with alicias mom was called off last week because of concerns about the coronavirus, but her accomplishments speak for themselves. She raised a girl in the middle of hells kitchen, which looked like it sounds, you know. It was rough. Hard, dangerous, difficult, scary. I cant even imagine, as a mother today, me, sendin my kid off into those streets. Because, but you had to. I mean, what whats she gonna do . She had to go to work, i had to go to school. We had to live. Thats how it is. Im so grateful to her for choosing me and i really appreciate here, you know . Did you appreciate it growing up . No. Hated it. You kiddin me . I was, like, could you give me some space . Music piano but the tough love worked when alicia keys graduated high school as valedictorian, she was offered a scholarship to Columbia University and somewhat poetically a contract with columbia records. She ultimately chose the music, and wrote the song that would change her life overnight. [ ] musyowere, i wanna make sure i have the right words, you were looking for that one song that would raise the hair on your arms. And you found it. I was actually writing that song already while i was at columbia. And that song was fallin. The song brought the rewards of sudden fame, and all the problems that go with it. Alicia says she wanted to please everyone, and that shed work herself to exhaustion trying to do it. You start to think that you have to do everything that you can possibly do to make sure that these doors remain open for you. Say yes to as much as you can, try to make people happy. Absolutely. But at the same time, are you making yourself happy . You think youre happy, cause right, thats what you were here workin for, right . Youre like this is the point, yeah. Its, like, oh, man, this is actually happenin, this is great, im so happy. And then i think little by little, you start to see how you are maybe goin too hard. Maybe you start to see that youre not gettin enough sleep. Youre just constantly ignoring your own feelings, your own emotions, your own intuition, your own instinct. I have to remind myself even today that, you know, you are valuable. Youre important. And you have to take care of yourself. You still are saying that so that to you today . Today. Today. At this point, ive created a habit that i have to remind myself, hey hey, guess what . You should go sit on that couch. You should just relax, because that is just as valuable as all of the work. Of course, the work is still important to her she has a new album,alicia,coming out soon. But it might be that alicia keys has captured where we all are at this moment pausing for a little selfreflection, and ready to come back Even Stronger on the other side. I think that this idea of bein even more accepting of just how i am as i am it, you know. Like, i might be in a bad mood and its okay. I can be there. Im good.
