Dr. Rivera, looking at cdc guidance warning Antibody Tests may be wrong half the time, should we take stock in these tests when it comes to determining whether its safe to return to work . Something important to recognize, its not that these tests in particular are horrible or anything like that. This is a problem any time youre testing broadly, it has to do with specificity. 95 specificity means 95 of people will test negative, 5 will test positive. In 100 negative people, 5 will show up positive. If a disease is rare and only 5 people in that group have the condition, 5 false positives, 5 real positives, and you get this 50 50 situation that were in here now. The cdcs guidance is good. You might need a second test to confirm the first test. That increases the accuracy a lot. Perhaps doctors might want to consider only testing those who have a high probability of actually being positive. Or use a test that has very, very high specificity to prevent this problem. One of the takeaways is definitely, if youve not had a positive coronavirus test, all youve got is this one positive serum antibody, you should not be confident that youve got immunity. So much attention on a vaccine. Merck announced its working on an antiviral drug that eventually could be a treatment. Your thoughts on a timeline for antiviral treatments . Are they our best hope at this point for fighting this thing . Antivirals basically attack the virus directly or they reduce its ability to replicate, in contrast to a vaccine, which basically mimics a virus, gives your body the ability to fight it off itself. Vaccines in general are much better. So the problem with antivirals, unlike antibiotics, they tend not to be very effective. Viruses are very simple which makes them harder to kill. They mutate a lot faster. Youll see the antivirals used in medicine like for hiv, shingles, flu, tend to reduce the severity of symptoms or barely contain the virus rather than cure it. But i think in general, vaccines are much more likely to get us out of this. Doctor, we know because of this pandemic other Health Issues have popped up or been ignored. Concerns about a rise in Heart Disease or cancer cases because people are not getting routine checkups or theyre afraid to go into hospitals. Now the cdc in canada is warning about a surge in stds because people are not getting tested. Should people still be concerned about going to the doctors office, especially if that doctor is in a hospital . So absolutely, positively, if you need medical care, you should go. I will say, honestly, i feel in some respects safer going to the hospital than i do going to the grocery store. Because at the hospital we really know how to disinfect things. Everybodys wearing a mask. Everybodys social distancing. But especially if you have a chronic condition that needs care, probably the risks to you of not seeking care are much higher at this point than the risks of seeking it. Our thanks to dr.
