Expression ive used what do you have to lose ok what do you have to lose you have taken it for about a week and a half and im still here. Also coming up 2 rivals are now sharing power in afghanistans new government one has promised to deal with the coronavirus threat the other has promised to deal with the telly bon threat to us i have proudly accepted this mission not as a privilege nor as a position but rather as an obligation and responsibility before god and people. To our viewers on p. B. S. In the United States and all around the world welcome we begin the day with the drug of choice of u. S. President donald trump yesterday trying startled reporters when he admitted that he is taking the antimalaria medication hydroxy chloroquine. In the hopes that it will prevent him from catching 19 there is no evidence suggesting the drug can ward off the corona virus but we do know that the drug can have serious side effects some coronavirus patients died after taking hydroxy clerking were trumps announcement drew immediate criticism from doctors and Public Health experts not only because the drug puts trumps wife in danger but also because of the example it sets to a worried public for months trump has pushed hydroxy coroutine going against the advice of his own coronavirus taskforce he started taking the drug 2 weeks ago when 2 white house aides tested positive for the corona virus publicly trump insists that he has prevailed over the pandemic and that the u. S. Economy must reopen privately with the viral threat so close he reaches for a drug convinced it will protect him knowing that doctors say it could kill him and as you know less saturday the f. D. A. Also gave emergency authorization for hydroxy a killer clerk when and. The hydroxide chloroquine is a. I hope its going to be a very important answer were having some very good things happening with it but i will tell you this i did speak with the president of honduras just a little while ago. And i did bring it up and brought it he said they used drugs chloroquine and he said the results are so incredible with a drug seeker when this happened an hour ago because i think its kind of heard a lot of good stories and if its not good ill tell you right doctor to get hurt by it its been around for 40 years from malaria for a loop as for other things i take it frontline workers take it a lot of doctors being a lot of doctors take it i take it. Hes taking it on our 1st guest tonight is dr david yearly a drug safety researcher and head of clinical pharmacology at the university of toronto he joins me tonight from toronto in canada dr your link its good to have you on the day you have been vocal in your warning about the drug hydroxy. And youve made headlines politico reporters asked you what you think about trump taking the drug and you were quoted as saying crazy thing to do now we want to be thorough here in case some of our viewers are not familiar with this drug why is it crazy for the u. S. President to take hydroxy chloroquine. Several concerns here in the 1st is the fact that we dont have and you made this point in the introduction we dont have any good studies at the moment to show us that this drug either prevents or treats coronavirus i mean we might one day have studies that tell us that you know over the last few months is the optimism 1st version in Southern France of the drug the limited data thats commode is not in her promise and so we dont have the data that it works we do know that it does have talks to cities and you touched on one of the hurt wrists of this drug are quite serious. My one of my worries is now the president has access to health care that the rest of us dont and he could have monitored very regularly for this problem. But if his pronouncements risk decision to take the drug please others to take it you know and perhaps in large numbers and its not heard to tell the story how somebody actually died is the result of sort of following leaders what are you worried about that i mean i know youre in canada but the influence of your southern neighbor is huge are you worried that people in canada or maybe even your patients will be influenced by what they see the u. S. President doing. Its funny you ask that question i need to talk to several 100 people at noon today doctors and pharmacists about the this drug and it became apparent to me that many doctors who look after patients especially those in Long Term Care homes which have been really hit hard in canada and the u. S. And i suspect in germany to. Have been hurt by this doctors it felt pressured by families to prescribe the drug figuring that it cant help but it might hurt and in addition to the cardiac side effects there are all kinds of other potential risks drug interactions. Complications low blood sugar that you know it really could hurt somebody and so you know the overall goal of prescribing a drug if youre a doctor or taking a drug if youre a patient is to is with the goal of the patient more benefits than harms and in this case we just have very little reason at the moment to think that theres a tangible benefit there and we do have a litany of harm some which are quite serious and so i think i think its a valid point that theres a lot of pressure to do something when you have nothing else. The white house physician he says that he had discussed the pros and cons of taking this drug in a Statement Released late on monday he says that we concluded the potential benefit for treatment outweighed the risks so what does the white house doctor seem to know that you and i dont know. Well the doctors done what we do all the time i mean you know we routinely prescribe patients medications that are to prove it is a daily event in every Doctors Office and hospital around the world. Ideally what you do though is you have a discussion with patients and you talk about the potential risks and benefits in that equation obviously wasnt there for the discussion between president his doctor. I suppose the doctor would have said to him listen maybe were not sure of the benefits but the but the most worrisome harm. The risk of sudden cardiac death which is rare but could happen could in the president s case being minimized by daily e. C. G. s for example you could see on the parity gram the thing we worry about that could set somebody up for sudden death i worry also in this is tucker but i worry about the nurse that you attributions here you know the president s not been shy about talking about his insomnia often be tweeting one or 2 morning. This is a person who controls Nuclear Launch codes and in this drug in some people can cause paranoia and hallucinations and and frank psychosis and so. I would if i was the president s position i would be very actively discouraging him from taking this drug and you know youre not his physician but what happens in the white house is is on show for the whole world and im wondering what kind of ethical questions are being raised here by a physician who prescribes a drug for a condition that its not supposed to be prescribed for and he also prescribed the drug as a prophylactic since knowing that the drug is not approved for that. Well its not wrong for him to prescribe a drug to his patient after undertaking with the discussion of the risk to benefit but i think your is an important point here. Theres the possibility of others using the drug on mass as the president has done that will you know. Expose those people you know in an even greater risk multiplied by hundreds of thousands of people become something consequential it diverts people from Clinical Trials that actually could be done to study the drug and it also runs the risk and this is happened in the us already of using up the supply of the drug for people. Who who have good reasons to be on it theres no the other you know we dont use a lot of this drug for malaria in north america but it is used quite a lot for patients with lupus and router uprightness and other diseases and it helps keep their disease under control so you know in if the drug is suddenly vacuumed out by the public in response to what the president s position has done for him and made public theres a very real risk of harm to people who need the drug people with chronic diseases who are reliant on it. Let me just ask you the president he refuses to wear a face mask but he demands to take this drug as a physician what message do you think this sends to the public. Since a terrible message i mean. We were a face mask to protect those around us in the event that were you know carrying the virus and without knowing it everyone should be encouraged to do that but heres a president who is. Who is not wearing a face mask despite the pronouncement of ransoms of experts in the fields of epidemiology and Infectious Disease saying that thats the right thing to do and hes doing i think most doctors in the know in may of 2020 would not do which is which is take a drug thats not evidence based and has potential harms dr david. The Research Clinical pharmacology at the university of toronto fascinating discussion with you dr we appreciate your insights tonight thank you. And. The kobe 1000 pandemic has had a devastating effect on the Global Economy in the United States alone tens of millions of people have lost their jobs unemployment is at a level not seen since the Great Depression europes largest economy germany has slipped into recession governments on both sides of the atlantic are spending trillions to save jobs and to resuscitate entire sectors before its too late the damage done can be measured in lives lost and jobs axed but what about the long term risks the World Economic forum has just released a study looking at a post pandemic world the pitfalls and opportunities the report lists the 10 most likely falwells now the top 3 include a prolonged global recession followed by a surge in bankruptcies of small the middle sized businesses and the failure of entire sectors to ever fully recover interestingly enough at the bottom of the top 10 the least likely of the most likely is a 2nd outbreak of covert 19 or another infection disease. For more lets bring in one of the authors of that report john scott is head of sustainability risk at Zurich Insurance group he is in london tonight john its good to have you on the day i want to start with the number 10 on the top 10 there the most likely fallout based on what we hear from Public Health officials and doctors i would think that another coven 1000 outbreak would be right near the top why do the 350 senior risk professionals that you consulted for this report why do they think differently. Well i think its a shame brant that it is as he said bottom of the top 10 where shells still read 30 to. Unlikely and in fact then if we asked another different version the question was what whats the most concerns that an especially is the number 3rd risk from the concerned only some people are definitely worried about it and its hardly surprising and i think in terms of likelihood the credible efforts that governments around the world. Taken to shut down economy to contain the transmission disease and avoid more people catching the current virus and managing the impact on the Health Care System that that session worked in a lot of countries and i dont want to minimize the Health Impacts and more people are. Being aware of. The same time i think people feel especially when you think about the response this Business People they were thinking well ok thats quite well managed my real concern now the economic groups which. Are in the top 10 by what you. Your report is clear that Economic Risks are the most likely fallout especially a prolonged global recession do you see governments and Central Banks doing enough i mean we are now talking about stimulus packages in the u. S. In the trillions of dollars. Yes insulated europe trillions and euro so i think i think theres been an amazing response. Central banks and governments and realize the tremendous importance of not holding back in providing those monetary and fiscal support. To provide Money Financial Services system to. Give moments of interest loans to to the real economy to companies that need to continue their employees for example and then the physical responses being incredible to support those employees who not have much work because of the self employed. And prices and thats really not i think this last point about a fiscal stimulus is it is really that because this falls are really great to use this terrible crisis as a platform to really change the way the world looks as we recover and by that what i mean is particularly in europe with the new green deal is to focus on an issue that other really have comparable business been running for several years bitches Climate Change and being able to drive the transition through these this can stimulus packages from high incomes a low Carbon Economy even really good. It would be would be an excellent outcome and we hope that will be the outcome i want to ask you about something in your report that caught my eye you write about a fault strike cottony a Public Health and economic well being i want to ask you is it a false dichotomy at the moment Public Health can only be assured by economic paralysis as long as there are no vaccine right so is it really a false dichotomy. Well i think its in this case its not either or its we want healthy population and we want a healthy economy and of course you know its weve done the right thing as i said earlier about you know controlling the transmission of disease and places further outbreaks and social distancing and containment measures and thats been really hard and its described in this economic crisis and and thats not to get the reduction in the moment its also urgent an Energy Crisis as well im seeing oil prices negative thoughts never seen up before and in emerging and developing countries were seeing ash in emerging markets very process especially in those countries where. That search for Health Care Systems that actually its a struggle even to get food and water so so i think these are all highly interconnected its fair that we cant solve unlike previous Economic Crises we cant solve the crisis and we solve the Health Care Crisis and we confront in effect an exit not exit strategy that most people agree is in effect to involve all. Of a more than billions of doses vaccine that takes time to develop and also probably arrange a therapeutic drugs that have fallen on for more discussion and. Corcoran and so you know thats a search for all sorts of therapeutic drugs and the decision got there are going to be really important to get people to confidence to come back and have some. Version of them out of the i mean i dont think they get to 0 so far as the 19 pandemic of the many things changed in peoples behavior when they travel the work of you know youre talking to me on video from our home. Office i think people and Consumer Behavior is change and still want to travel on vacations and so on i think the last. I think that means you know we can really. Only when this is all true its a mission expect to see some distance things and future weeks and months does that mean mission that you know absolutely. All right john scott head of sustainability risk Zurich Insurance group john we appreciate your time tonight and your insights we appreciate you talking with us from home as well thank you you know. The coronavirus threat is a global one but one with varying levels of impact depending on where you are consider Afghanistan Taliban militants on tuesday launched a failed attempt to capture the city of can the scene leaving dozens of people dead or wounded theres been a spike in attacks by militants on the government despite a u. S. Brokered cease fire and until this week 2 political rivals were both claiming victory in the president ial election theyve now agreed to share power but these shared burdens are immense with or without a pandemic. Signing the deal was the easy part now rivals ashraf ghani and Abdullah Abdullah have to find a way to work together. We shared the burden. And god willing our shoulders will be lighter weight. Trial confident that you have the competence capacity and capability for this great desire of the Afghan People and we will be working with you. Which the burden is tremendous despite billions of dollars in International Aid afghans are almost twice as poor now as they were in 2012 and the taliban remains a potent threat to stability. Who claimed fraud in september election after losing to danny is now responsible for ending the countrys decades long standoff with the taliban. Into us. Proudly accepted this mission not as a privilege nor as a position but rather as an obligation and responsibility before god and people are used to us struck a deal with the taliban in february although the Afghan Government was excluded from the talks the deal commits them to a prisoner swap but violence still remains a negotiating tactic afghan Officials Say the taliban has carried out 3800. 00 attacks since the u. S. Deal killing hundreds of civilians and Security Forces the group has also taken credit for an attack on monday that left 9 afghan troops dead while the taliban keeps busy the president ghani faces his own challenges foremost the coronavirus pandemic after 4 decades of relentless war the impoverished country will find it almost impossible to control. Lets bring in Phyllis Bennis now she is with the institute for policy studies in washington d. C. Shes written extensively on afghanistan feel it is good to have you on the program but were always seems trying to find a positive in the narrative about afghanistan now that we have this power sharing agreement with the 2 president s if you will do you think thats a reason for hope moving forward. Well its certainly better than the alternative which seem to be fighting between the 2 contending groupings but the reality is that the Afghan Government does not control most of the country whoever is the president or in this case copresident s or whatever theyre going to call themselves i think that were dealing with the legacy of 40 years or more of Foreign Occupation of sanctions of more of an already impoverished country and if we look at what the impact has been its its been horrific you know the notion of debating should the u. S. Pull out what if the u. S. Troops pull out the problem is that the u. S. Troops being there and the Coalition Troops supporting them being there has not made life better for the vast majority of asking its you know certainly there have been some gains in the cities in kandahar kabul but the vast majority of people in the country dont live in the cities they live in very scattered small villages throughout the country and if you look for example at the question of of womens lives in afghanistan during when when the taliban were in power afghanistan was the worst place in the world for in terms of childrens mortality into mortality rates were the highest in the world after 20 years more than 20 years now of of u. S. Occupation where where is it now afghanistan is now the 3rd worst place in the world only because sierra leone and gola got a lot worse so do you think then that President Trump is by saying troops have to come out the u. S. Has to get out of it is he going about it the right way. But this is hardly President Trumps idea to pull out this has been a demand of many afghans and plenty of people across this country for much longer than than. President truck called for the withdrawal of troops just last month in a new poll 75 percent of afghan veterans in the u. S. And 7075 percent sorry 75 percent of afghans veterans and 70 percent of military families agree all of the u. S. Troops should be pulled out they do need to be called out is that going to solve the problem no the problem is far greater than that but its a prerequisite for having any chance of negotiations between the various sides in afghanistan and and ending the long standing war there with the u. S. Troops pulling out it means that fewer afghans are likely to die because we wont be having the kind of airstrikes and drone strikes that have characterized the u. S. War in afghanistan for at what about the last 12 years. Those are going to 30 seconds what about the danger there from the taliban once the u. S. Is gone there is a danger of the taliban theres also a danger of the of the Afghan Government which isnt a whole lot better than the taliban and all of the issues that the us claims to care about the u. S. Has claimed that were only theory to protect the posset to prevent the possibility of another 911. 00 style attack on the United States that isnt going to happen regardless of whether the u. S. Troops are there or not thats not whats going to protect from from this kind of terrorism military force does not prevent terrorism ok so as a virgin were out of time Phyllis Bennis with the institute for policy studies in washington those we appreciate your insights and your time tonight thank you thank you thanks very much.
