But we need what we need. If a person comes in and needs a ventilator and you dont have a ventilator, the person dies. That is the blunt equation here. And right now we have a burn rate that would suggest we have about six days in the stockpile. All sortse are taking of extraordinary measures. I have spoken to help people across the nation, doctors, workers, all sorts of research, and we have extraordinary measures in place that can make a difference if we run into a real ventilator shortage. We know where all of ventilators are in the state of new york by hospital. If we have a problem in any hospital, we are going to take the ventilators that are not needed from the upstate hospitals and transport them to downstate new york, to the hospitals that do need them, which again, more and more are going to be on long island. We will return them or we will figure out the finances of it and make those hospitals whole. We are also increasing the number of ventilators by ending all elective surgeries. If you do not need an elective need an if you elective surgery, the hospital cannot perform it. If it is not critical, we will be freeing of ventilators. Anesthesiao be using , without the anesthesia, obviously, but using them as ventilators. Practicingpitals are that now, one ventilator, two set of tubes can do to patients two patients. We are also including bypass machines, which do not have the same force as a ventilator, but on an emergency basis, some research has been done that says they could be suitable. In thealso still business of looking for. Entilators to buy ventilators it is too late to ask the company to make them. In any way that would work for our timeframe. Our timeframe, seven days to 30 days no one is going to be able to make a ventilator for you and that period in that period of time. Nationwide, parts of the country that have a later curve, yes. If you give a company two months, three months, they can ramp up production. But not on our curve. So we have to find ventilators that we can buy, and we are still doing that. Again, the main place is china. We are converting the bipap machines. This has not been done before, but north well, one of our premier health care systems, has developed a protocol, and they are teaching the other hospitals now how to do it. We just bought 3000 bipap 750 came in yesterday. So yes, the burn rate of ventilators is troubling, and six days of ventilators in the stockpile is troubling. But we have all of these extraordinary measures that i believe, if push comes to shove, will put us in fairly good shape. I do not want to say yet i am confident, and it depends on how many we need, but i can say with confidence we have researched idea,possibility, every every measure you can possibly take to find ventilators. This state has done, that i can promise you. We are also going to open the Health Care Exchange Enrollment Period through may 15. We have about 96 of the people in the state covered with health insurance. If you are not covered, we are extending the Enrollment Period to may 15. Please get covered. Are you interested in mandatory curfews . It requires an executive order, would you be interested in placing curfews on those who are not interested in no social distancing rules. The question is what i be open to curfews, depends on what their problem is, their situation, and what they are trying to solve. To understand the theory of their action. Theres only a handful of people in the comfort, can you give us an update on how many people are in the ship and the Javits Center . I dont have those numbers but can get them for you. Those two facilities are operated by the federal government, they are for nonCovid Patients only. The theory is that the nonCovid Patients would come out of hospitals, bringing up more beds , and the javits and the comfort would be an overflow valve for that. The federal government determines whos in by their protocol. But i have to get their numbers. I dont have the numbers. [indiscernible] maybe not icd level ones . They could, you would need the federal government to allow you to have look, the Javits Center and comfort can do whatever the military allows them to. I understand on the comfort they dont want to bring in Covid Patients, because thats an entire ship. How do you disinfect it afterwards . Thats a complicated situation. Avits you could convert to covid facility. It would require the federal government to allow it. I dont know what kind of medical staff covid versus noncovid, maybe dr. Zucker would know. Depends on the positions, the Covid Patients who are recovering are in the convalescence phase and should be getting better, they should need some respiratory support and nursing care more than anything else. NonCovid Patients, obviously depends on their medical condition. But those individuals would have to be in relatively stable conditions. What about china, the cases and deaths might not be accurate , im wondering if thats true . Would that have any bearing or impact on the models and projections we are using in the United States . Question. Interesting housenot do the models in. We review the models, but we dont do the models. Mckinsey does models, etc. Its an art form unto itself. Modelers will tell you that the use wuhan data, especially when they are trying to calibrate the rate of spread, and the effect of social isolation. But they also have a lot of data post china that they are relying on. You have south korea, the rate of spread here. You raise a good point. If the china data was wrong, might it influence the model . Yes, but it is not solely dependent on that data. [indiscernible] the federal stockpiles have been anyid new york receive faulty ventilators from the federal government . The state overall, the city and the state have received 4400. We have distributed many of them. I have not heard of any faulty i have not heard of them. In the Los Angeles Times it was reported that some of the ventilator sent to them by the federal government were faulty. The exultation is that you apparently have to run ventilators every so often, its not unlikely not unlike a car battery. Thats what they chalks the issue up to. In new york when we saw the report, we are in the process of reviewing our stockpile, and we have not seen a problem so far. Are you expecting more ventilators from the federal government at this point . The federal government is very aware of our situation. I spoke to the president this morning, i work with the Vice President , the white house staff, i dont think the federal government is in the position to theide ventilators to extent the nation may need them. Question ofk its a whether the federal government has ventilators that they are not distributing. They may have ventilators they are not distributing now on the same theory that im not distributing all of our ventilators now, because you have a stockpile. The state has a stockpile, the federal government has the stockpile, the concept is that you stockpile until you need it. I dont know if they have more in the stockpile. I know that the ventilator a problemty is just for everybody. You have 50 states competing for it and the federal government trying to buy it. I dont know what the capacity is in the stockpile. I would not be surprised if they dont have enough. If they dont have enough to provide for the communities that need it. Thats why our attitude is that we are on our own. I will ask the federal government if we get to that point. I will ask them, i will plead, i will do whatever i have to do to get ventilators from the federal government. But thats why we are taking those extraordinary measures ourselves. Just assume you are on your own in life. Thats the operating assumption. Having said that, im sure the federal government would do anything they can do to help. There is nothing in our relationship that would suggest that they would not do what they could do to help. The question is what can they do. I dont know the answer to that. Are are there any buying consortiums . Where the states are working together . No, i think your past that because you cant find the product. Use ofyou compare the the bilevel machines to the regular ventilators . Question. Great i would love to respond to it, except i have no idea how a bipap relates to a ventilator, besides the generic comment that the bipap does not have the same level of force. It does not pump as much air, beyond that i dont know but dr. Zucker may know. That was a good excellent nation. The bipap cyst good explanation. The bipap system does not have as much energy to push the air in. The ventilators have many more capabilities of doing different types of respiratory support. But the bipap machine will help many individuals once you convert over. And these are relatively for minor cases . This is a condition which changes as somebody gets better or worse. I think sometimes people think you put them on one machine and they stay on that machine but you can move things around. You can put some buddy on a ventilator, they get better and then you switch them to a bipap machine. And that may provide the support that they need and you can move it to the next phase. Situation. Stagnant a ventilator has more variability. This is not an ideal situation. This is not a standard medical protocol. Everybody has made that clear to me. In the Perfect World everyone would have a ventilator. That is our first goal. Ive been talking about ventilators since this started. If we hit the apex and we run out of ventilators, then on the theory that you do whatever you can do, then we have those measures. Take the ventilators from hospitals that dont need them, splitting the ventilator so you double the capacity and used bipap machines on those who you can. Those measures go into effect because you have no option. Wherell be in a situation if a person does not have a ventilator they die. If you dont have a ventilator you come up with plan b, c, d, i. E. , f, g, h. That is what we have done. We even have a plan for the ventilator,ber which is just a bladder that is manually operated. We purchased 7000 of those. President trump said that this new york had gotten a late stop late start. And then in the next brat breath said cuomo is working hard. How do you make sense of the criticism of the state and praise of you. I dont know. You dont know. Know. Ont i dont know how you square those two statements. Intense w the intents of those two statement. We started purchasing and probably will have wound up purchasing more than any state in the country. I will bet you dollars to donuts that we wind up buying more ventilators than any state in the country. Dayere very aggressive from one. Thats on the assumption that each state is supposed to be managing the situation. But i thank him for his good words that he thinks im doing a good job. Canre doing the best we with all of the special populations, in prisons, nursing homes, and homeless shelters. Whats the moratorium on exceptions for the next 90 days, what happens after that if tenants are not paying their rent . Should landlords evict . Lets look after 90 days. A lot of these situations, what do we do about rent, income, infrastructure after 90 days. So much of this is changing, its fluid. I dont think anyone can sit here and tell you what the right plan is for any of these areas 90 days from now. I just want to get to 90 days from now and get their healthy get there healthy. The number of people who come in per night who need a ventilator is somewhere in the range, its a little different every night, but if you wanted to be safe, you would say 350 people come in every night who need a ventilator. 2200, it disappears very quickly. What do you do . Andurchase more ventilators then plan b . Plan c . There is no state and no country that has done anything to provide ventilators that we are not doing. We have looked at literally everything that italy did, china did, south korea did, every other state. We are doing more than any other state or country has ever tried to do in terms of backup contingency plans. Beyond that, every day we try to find more, purchase more, be more creative. You said yesterday at the end this changesk that people might, this changes people. Have you changed . Have i changed . No. I defy change. My kids will tell you that, caro will tell you that, they have tried to get me to change for many years, they try to get me i have tried to teach me for many years. After this is over we will have to get back to work and have an economy that functions and have infrastructure is an essential part of that. Airports are still functioning in planes are still coming in. Laguardia is a whole configuration of physical routess and alternative pending construction. Would be like that very difficult to freeze and nonfreeze. Anyone who has not asked a question . [indiscernible] the emergency bond act how will you make the decision on whether or not to pull the plug on that . Gov. Cuomo it is on finances. We are going to look at access to the market, access to the market is limited. We will borrow to deal with shortterm revenue. By next october we will look at the state foss finances and look at our access to the market, then make a determination. [indiscernible] howt is hard to tell, likely the economy will return to where it is next october. What is the total of shortterm borrowing . We are not sure. We will look at what the market will allow us to borrow and make that determination. April andall between july 15 is 10 billion. Between that number, we have some access to the reserves we have to to make the determination. That the legislature and the executive got this budget or are getting this budget done with all of the initiatives that are in this extraordinary feat of government accomplishment. I praise the speaker and the Senate Leader and every member of the legislature. It would be easy to say this is an extraordinary year, lets do the bare minimum and go home. Needsaid we have a lot of , there are a lot of issues that need to be addressed, and they stepped up to the plate and did it. If you look at the list of initiatives in the budget you can put that budget against any budget i have done in any normal year and it would be a great budget. That it was done this year is really extraordinary. I praise them for that. Is wrong, said that incorrect, misstated, do not be shy, now is the time. While there are additional crimes that are bail eligible, that is what they are, bail eligible. It does not necessarily mean it will translate to more people. The effect will be 90 days from now. It takes into consideration the current circumstance. There are 115 times we moved to be alternate to bail eligible, things like remote tracking and treatment and other things.
