We usually get information in a matter of hours or certainly 24 hours back to the patient and provider. We would like to test more widely. Thing i mentioned several times from this podium is to clarify about the timeline for vaccines. And with that have pretty real impact on what we would call a rebound and cycling in season. Certainly a vaccine will not help us now or next month or the month after. Trialt into a phase one and i keep referring to one vaccine. There is more than one. There are a couple at different stages of development, but they are all following the same course, and that is you first go into a phase one trial to see if it is safe, and you have very few people, but if i people, in age group, all healthy and not at great risk of getting infected. The reason you do that is you want to make sure that it is safe. The next thing you do, and it takes three months or more, which brings us to the middle of summer, you go to a phase two trial. We put a lot of people in there. We hope that there arent a lot of people getting infected. But it is like a really there will it is likely there will be somewhere that is going on and we will get an efficacy signal. We will know whether or not it works. If it works, we hope to rush it to be able to have some impact on recycling in the next season. Like i said, could be a year to a year and a half. I am not changing any of the dates mentioned. One thing we are going to do that we need to understand that has been a stumbling block for previous developing of vaccines, and that is even before you know something works, at risk, you have to start producing it because once you know it works, you cant say, great, it works, no give me another six months to produce it. We are working with a variety of companies to take that risk. We didnt take it with the zika. Same with sars. We are really going to push on that is to be able to have it ready if it works. Your thoughts after listening to dr. Fauci . Guest he did Vaccine Development 101 and if i may, because i know there is confusion, let me unpack some of that and explain what tony was talking about. The first thing in development of a vaccine. It is not for treatment, it is for prevention. We would like to prevent this disease, likely prevent measles, by giving a vaccine so you dont get the disease at all. The first thing you have to do is work in the laboratory to create a candidate vaccine. Then, and this is what dr. Fauci was talking about, why does it take so long after youve got this candidate vaccine in the laboratory to develop it before it is ready to be licensed for use . Two things are very important. You have to make sure that the vaccine is safe and that it doesnt make you ill in and of itself. There have been vaccine candidates in the past that have done that. So we dont want to do that. We dont want to harm the American People. The other thing is, you have to be sure it works. I will walk you through that. The first stage that dr. Fauci talked about was that you get a small number of volunteers, people who are recruited themselves, usually young adults who are otherwise healthy. About 40 or 50 of them get the vaccine. You check to make sure nothing adverse, other than a sore arm goes on, and you draw blood periodically to test whether the vaccine has worked in the body. , by thef that goes well time you recruit those people and give injections and follow them up, that can take three months, surprisingly long. Then you open it up and get a much larger group of people who volunteer. Some of them will get the vaccine and some of them will get a placebo, a salt solution, for example. Nobody knows who has gotten what come and then in the context of outgoing break outbreak an outbreak, you will be able to discover whether the vaccine prevents the disease and giving it to a larger number of people, you have a better assessment of how safe the vaccine is. Once you know exactly how well , how effective it is and it is safe, then you can go to licensing. The government will make an initial investment, probably a public, private cooperation, working with manufacturers. If dr. Just like anyone for an illness we are giving intensive care, we watch that patient very carefully, hoping they respond. Some patients, the most severely ill, will die. Others will recover and we can gradually wean them off of the respirator, and some of those people will leave the intensive care unit and go back to life and recover. Capacity to provide intensive care is so much better today than it was 20 years ago. We would expect some of those gravely ill people to actually print the corner and come back to life and go back into the community. Host we will go to bill in new jersey, a medical professional. The vehicle is thoroughly sanitized. Base, thek to our unit is sanitized again. And then the unit is left outside with the air conditioning on and the vents on for approximately an hour. We have the option to do that because we are not really a busy outfit. We have 1500 calls a year. These other units that are out there, which are doing runs things after runs have to be thoroughly sanitized. I wonder how they are doing that. Thank you for your service. We have long known that unless the n95 respirator is in some way gotten soiled or distorted, it can be used more than once. You are getting double protection because in addition to the respirator, you are putting on a face shield in front of that, so you have double protection. You can sanitize that face shield very readily with a wipe. So good for you. It sounds like you are doing a good thing. Guest i am sorry if ive given you the wrong impression. There is no doubt we are going to get many infections and causes a spectrum of infections. All the way from people who have no symptoms to people who have very mild symptoms. We see this every day in nashville. People have milder symptoms and dont get serious involvement of their lungs. There is that notable, serious component. It is thought that about 70 to ,0 will have milder symptoms not need hospitalizations. Have about 15 ou to 20 of people who have symptoms severe enough to need hospitalization, some of whom need intensive care, if we get widespread infections, that is still going to be a large number of people, even though they percent is relatively small, even though it is a minority. That large number, if it comes all at once, is really going to put a strain on the health care system. We are not saying that all over the country, not yet. But in new york, new orleans, growing in chicago, now in detroit, parts of florida, they are the hot zones. The other areas are warming up. That is what is happening in nashville. We are seeing a steady stream of patients being admitted to our institutions in the middle part of the state. I am afraid we anticipate more. There are two stories, but most of us who are wearing the white coats are concerned about the folks who are being admitted to the hospital and it is great that many other people are getting milder infections and dont need hospitalization. But remember, they can be spreaders and they can distribute the virus more widely in the population, sometimes affecting people who are older and have underlying illnesses who will get the more severe infections. I would say that if any of those people are sick, they can get tested, because interestingly enough, the financial barrier is not the issue here. If they are sick enough to qualify for a test, tests can be provided for them. I know there are a variety of psychiatric aspects to this illness. There is a terrific epidemic of anxiety about coronavirus out there, and there are people who have been quarantined for 14 days and there is an element of cabin fever out there also, particularly among older people who can feel very isolated. There are many volunteer groups and professional groups working with the agent helping them get through this period of time where people feel removed from each other and very lonely. My concern is adequacy of total testing and in particular the surveillance testing we need to do to get a real sense of how widespread and deep this illnesses. Doubt. There isnt any we all know about this and have heard about it, we have come to testing efficiency rather late and it is still gearing up around the country. From both a clinical and a Public Health point of view, we still havent tested sufficiently. I actually think there are three reasons to be tested. The first is a very clinical reason. So both the patient and the doctor can define whether an individual has coronavirus. That helps the doctor and the patient and if the patient is well enough to go back to their family, it makes it more real for that individual. The second thing is that we need likeublic Health Purposes the listener has said, we would like to define better the extent of the infection in the United States and see where it is focused and where it is getting worse. We need much more widespread testing. Then i would say there is even a third reason because our National Leaders have told people that we ought to be able to be tested if they care to be tested. There are a lot of worries out there. We have heard about infection without symptoms. They are concerned about themselves and their families. They would like to know whether they have the infection. When we get infected, our body responds and creates proteins that circulate in the blood that actually work against the virus. The virus is eliminated from our bodies and we recover. But those proteins continue and they will protect us in the future if we get reexposed to the virus. So that is interesting. We could take some of those peoples blood and kind of purified, if you will, y it, if you will, and use it for other people. It needs to be evaluated. In the past, some of that has ed, kind of. I was wondering what the supply, shelflife, and success rate has been and also the percentage of those cured with hydroxychloroquine. Bob, we are only in the early stages of using the immune plasma to try to treat patients. I havent seen any data about that. Hat is very early days hydroxychloroquine is another story. That is a drug we have known about for a long time. It is not a completely safe drug. It has all kinds of side effects and has to be monitored carefully. He has been used in the past to treat malaria and certain rheumatologic diseases, such as lupus.
