It is not ideal. No one should have to worry about switching jobs because of their health, but it is the system we have right now. On top of that, the insurance or pharmacy plans will say to their broker, if you keep this employer with us, we are looking at giving you a bonus this year 30,000, 40,000, or 50,000. That is how our services as doctors and nurses are being sold on the market like energy on a grade. There are so much wasted there. Most businesses are getting ripped off. She was 90. I have personal experience with monitoring her medicare bills. She was a dementia alzheimers patient. As pretty much everyone knows, i do not know what the statistics are, but endoflife patients are the ones who get something 80 of medicare. My mother did not have to pay these bills, but they were paid by medicare to various providers. Once i saw the costs of some of these things, it rankles me. I figured out where that 60 it is theyear fraud and theft and mismanagement of medicare. This is something that medicare for all people never discussed. 60 billion a year, which is 1 5 of the entire medicare budget. I saw that personally. To want that for the entire country is just insane. Until we can fix that like dr. Makary has talked about, getting a handle on this corruption, this fraud, this waste, and abuse the other thing i want to ask about getting these costs under control is why isnt Health Insurance structured and priced similar to my Car Insurance or my Homeowners Insurance . Lets go with Car Insurance. My Car Insurance does not cover new batteries, windshield wipers, tires, oil changes. If it did my premium would be unaffordable. That is why in my humble opinion Health Insurance is unaffordable. As adults we should be responsible for our basic needs, just like we are responsible for we should pay for basic tests, xrays. The meat and potatoes. The really mundane stuff of health care. That is our responsibility as a citizen and as an adult, especially if we are working. You look at systems 10 and 20 years down the road and they are often underfunded and sometimes even dilapidated. I do not think we want that. Look at our own medicare funding system. I get the appeal. We want to cut to the waist, cuts the middleman, cut through the price gouging and there is an appeal there with medicare for all, but we have to be fiscally responsible and we cannot do that right now. We have businesses spending more money on starbucks. Spending more money starbucks is spending more money on health care than they are on coffee beans. He took those prices and correlated them with the outcomes in our National Database of heart surgery outcomes and guess what . There was no correlation between price and quality. None whatsoever. We took those prices and compared them to the charity care at those hospitals. There was no correlation. High prices are due to highquality or high prices are due to charity care is by and large a myth and it is a reflex out there when people in health care do not have basic Health Care Literacy. People are hungry for honest prices right now. Proxy shoppers of health care if you are not paying for health shop proxy shoppers will on your behalf and they are just figuring out what hospitals are delivering babies for 4000 and what hospitals are delivering babies for 70,000. They are creating preferred networks and doing steering. People are hungry for honest pricing. And i go to court on behalf of patients who are being sued to have their wages garnished, a that we exposed this summer, all of these stories that came out were from us calling reporters and saying this is going on, we want to meet with you and talk about it. This has been the story of the year. Hospitals will go after patients to garnish their wages even though they are nonprofit institutions. Many hospitals have stopped suing patients. Ill for these are walmart workers, postal workers, these are not people who have done anything wrong. They work and have insurance and they have been terrorized financially. Billing quality is medical quality and financial toxicity is a medical complication. We really feel that we have forgotten in this country and maybe just in general that its ok to disagree with one another. That something thats part of a healthy democracy. But when we do that, theres a way to have those conversations thats civil and respectful and can be really insightful and helpful to democracy. Host what do you think is causing people to forget that we are able to have those conversations . I do think we are becoming more siloed whether we want to or not. The way that digital algorithms work, we just end up kind of in our own bubble sometimes. It requires of individuals now in todays times is to conversationsout with other people who dont share the same viewpoint that they do. And i say that acknowledging that thats a very difficult thing to do. Its not the type of thing you wake up in the morning and think i want to go out and seek a fight with somebody who thinks differently than i do. I think our perspective is that it doesnt need to be a fight if you are equipped with the right and that seeking out those conversations can help educate and make youru case stronger when you have those dialogues. We are talking about the political divide in the u. S. Some practical advice where people are going to encounter a friend or Family Member with radically different political views and they will share the same space together. What are your thoughts on that . Brain toain your respond to dissent. When we hear something that we inherently disagree with, our body very naturally goes into fight, flight or freeze. Thats a very natural response. Often times what that means especially around a holiday that we fight with the people we are talking to and we go right into battle mode. That ispens when we do there is absolutely no active listening happening. Not only are you not able to hear what the other person is saying but they are certainly not able to reciprocate that and hear what you are saying. So train your brain to know that when you hear something you disagree with, your initial reaction is going to be to maybe respond in a way that escalates the situation and if you can rather be in the practice of taking a beat, maybe even repeating back what you heard them say, not in a cheesy way. Youre saying this. This is what im understanding. This is why i think differently than that. It can really change how that conversation goes. About you hear something that you find is offensive or absolutely factually incorrect . How do you in a polite or civil way respond to that . One of the things that we find that in trenches people the most in their views is when they feel attacked or isolated. When you hear something that you factualoffensive or not , it is so natural to want to respond to them in a way that really is attacking. And in a sense, thats a good thing especially when you hear something offensive and you are a bystander in a situation and want to stand up for someone else. Thater, will you change persons behavior in the future . Probably not using the attack method. Addressing it head on is really important. Absolutely doing something about it is important. But doing it through more of a dialogue approach and a way that , i usually love your jokes. What you just said crossed the line for me. I want to understand more of what you meant by that. Thansounds a lot different , i cant believe he said that. What are you thinking. Just approach the conversation giving individuals the opportunity to engage with you rather than feel shut down, attacked. Do that, reason you you dont want individuals to become more entrenched in their position. You want to work with them and try to bring to the surface the issues that need to be worked out. Into 2020, your thoughts for how media organizations and College Campuses can encourage a broader array of views to be presented. I think part of the solution is to start right now. Intonk it the more we go 2020 the more heated we are all going to become and so being able to have some of those conversations now while yes, things are tense but we are not in october. Theres not a big election pending. Is really important and part of training ourselves. I think things like town halls, dialogues where people are encouraged to come together that have different viewpoints. And just discuss in a way thats really effectively moderated. Neutrally moderated where individuals can share their viewpoints, be heard and also facilitator or have the folks in the room be able to acknowledge different viewpoints before moving onto the next one. What we tend to do right now is we hear something we dont like and maybe we are quiet and assume that means we are being polite. Its a good start not interrupting. Usually what we are doing is while the other person is talking, we are just reloading in our head the next thing that we want to say. We are not actively hearing them. Being able to stop and make sure really givesard to feel an opportunity like they can express themselves, be heard and share viewpoints. There are several very good statements that have been shared so far about how deeply entrenched it has become as well as people commenting about how there are counter efforts offerng which i think to a possibility of a more positive picture going forward. Its a matter of where you want to put your emphasis. Previous thing is the doctor from Johns Hopkins had many worthwhile statements about the value of transparency. Having lived overseas for several years in singapore, i had to have a major operation which was not covered by my u. S. Health insurance in their hospital system. Before they admitted me, they gave me a detailed economic picture of what it would cost to go through the procedure right down to the detail to have me fully informed before i got the procedure. I think thats very unusual in american hospitals to do that and commit to a cost. I thought that was great. The big thing marty left out that is equally important, the american wellness. The selfcare issue is so tremendous. I think this also comes from living abroad and coming back to america after several years. The issues of obesity and of course the opioids.
