A look at patient care. Number three they said we should retain the individual mandate, if we decide not to have something that is just as effective in getting young and Healthy People part of the insurance pool across the country. The legislation before this repeals or proposes to reveal the individual mandate. If that actually happens we will have a costsharing reduction. With having a tree like a stool, that is what it has been called by Health Insurance folks and providers. They all called for that three stool approach to reduce the cost of the exchange. Chairman,nt mr. Colleagues, focuses on reinsurance. A decade were around or so ago when we debated and voted on Medicaid Program establishing a drug program, Prescription Program within medicare, voted for a Reinsurance Program. Some of the folks will be and de pool under medicaid part they were really expensive. There needed to be a Reinsurance Program to make it work. We did that. ,edicare part d works very well it comes in almost every year under budget and has favorable ratings. Approval ratings that was exceed any of us in this room, 80 or 90 . Here is the way that would work. My proposal my amendment would create a pool that we call a stability fund. Stability fund of about 120 billion. The states could use that find to drive down to have the resources they need to set up a and visible high risk pool. A lot of our colleagues have said they need a better idea. Not toates that a lack do that, there would be a backstop. Here is how that would work did let me finish. And 2020nd 18, 2019, there would be a Reinsurance Program that sent four states that did not set up the invisible goals here is how it would work. Cost for a years betweenal in a year 50,000 and 500,000, 80 of the cost would be borne by the stability fund. Between 2021, 2022 and 2023 80 of the costs between those three years is between 100,000. The rest would be borne by the insurance company. All of that would be borne by the Insurance Companies. But we have heard with testimony in this room two months ago, by setting up a Reinsurance Program and giving the states the ability to have their own in invisible find along with preserving csrs and along with preserving the individual mandate or is something as effective. The cost of premiums and exchanges would be brought down by as much as 35 area the reason why is because Insurance Companies would get into the game. They would compete for business in the marketplace in the states where there is a lot of competition and their exchanges, what is happened is that they are not looking at 30 , 40 , 50 areease in premiums you looking at singledigit increases because of the competition. Again, colleagues, we know need to make it clear that they will not lose their shirts. We are not interested in offering them welfare. We want to make sure they the beauty of the program is that it does not think gives the state opportunities to set up their own high risk pool. We are all interested in a federal state partnership. This is a good example how how that may be done. I would ask my colleagues to consider it. If we are serious about stabilizing the exchange we need to do those three things. Ist the Reinsurance Program practical and the third thing would be to make clear the individual mandate is going to go away, what more would be effective in helping people participate. That is what we need to do that my amendment takes care of one of those. It is not within the scope of the hill. Would you like to say it is a good idea . I am not going to go that far. This would set back the corporate reductions if the secretary of treasury cannot certify that the coverage for Mental Health and Substance Abuse and Health Insurance coverage has not been diminished. The concern here to my colleagues is that we have the resources to deal with the Opioid Crisis that is affecting every community in our country. My concern is that, if you look at the underlining bill, the cbo has scored it in a joint Tax Committee at a 1. 5 trillion addition to the national debt. My concern is that we know the casualties from the Opioid Crisis. We know we need to have the resources in order to deal with it. We know that there has been an abuse of opioids in every community that has led to heroin addiction. Unfortunately, the use of fentanyl, which has caused significant overdoses and all of our communities. I have had town meetings and roundtable discussion in western maryland, eastern shore, cecil county, southern maryland, the baltimorewashington suburbs. The story is the same, a rise in the increase in the opioid addiction. We do know that having Mental Health and Substance Abuse coverage is critically important in dealing with this crisis. I would think all of us would make sure that we do maintain a coverage in dealing with this Opioid Crisis. That is the purpose of this amendment, to make sure we have coverage. I will ask consent that the Psychological Association as part of a record. I think this spells the issues we are concerned about, american Psychological Association and the American Association of organizations with a strong opposition to repealing the Health Insurance coverage mandate as part as the tax reform legislation. Strong, stable markets are finally important for the members and to millions of americans with Mental Health and Substance Use disorders organizations comprise newly 115,700 members and affiliates, technicians, researchers, educators, consultants, students. The reason is that in order to deal with the oprah crisis, we need to have coverage. I was listening to senator toomey as he talked about that nothing in this bill takes people off of medicaid or Health Insurance. I would point out to my colleagues that the joint Tax Committee has confirmed that 180 billion is saved in eliminating the mandate from the Medicaid Program and is spent in the chairmans mark because of will not be spent in medicaid and people who were covered under medicaid will not be covered. Let me remind my colleagues that medicaid includes Mental Health and addiction coverage. That could very well be lost. That is half the story. A 100 billion saved for subsidies not provided under the formal care act. That is spent by tax expenditures in the chairmans mark. That 180 billion of savings that is spent in the chairmans mark and added to the deficit means that there will be 13 Million People with medicaid and the formal care act that will not be covered Affordable Care act that will not be covered by Health Insurance, that will lose their Mental Health and addiction coverage under their Health Insurance policy. As pointed out by the american Psychological Association, many people, because of the 10 increase in insurance premiums under the individual marketplace will be priced out of coverage and not be able to get Mental Health and addiction coverage. For all of those reasons, we need to make sure we have the capacity to deal with the Opioid Crisis as a result of the underlining bill. My amendment will make sure that we have some resources available, if the secretary oftreasury cannot certify that we have maintained at least the current coverage for Mental Health and addiction. I would urge my colleagues to support the amendment. But you will spend the money that cbo says will be generated because of people who are not coming to the marketplace. In other words, what you say does not work. And therefore it is ok to get rid of it. We say, wait a minute, it works pretty well because it will be 180 we will not have to spend in medicated because people will not sign up for medicaid because they do not have to anymore. We will save 185 billion in a ca subsidies aca because they will not sign up anymore because they do not have to. What you say that does not work, you cannot wait to spend the money because cbo says it does work. Just a minute or two. Senator brown wants time . Lets let me say this. The bill which includes all of the insurance mandate and essential health benefits. One of obamacare written in the Public Health service act. Not the Internal Revenue code. This amendment is not germane. I cannot allow 10 minutes for every nongermane amendment. I will do it in this case. Senator stabenow. Discussion and debate will impact the entire economy of our country. This is a critical issue for our veterans returning home who have served and to keep our country safe. The opioid addiction issue is touch with every community i know. I think it is a urgent we take its and and say we will not allow people to lose critical services. Chairman, i, mr. Think about the opioid in my state, i think about a man sitting with his daughter in he said if it was not for medicaid his daughter would not be alive. There are 200,000 people in my state who have been getting opioid treatment. We need to put a human face on these. Mr. Chairman . If i may be heard on that. Since this amends the Internal Revenue code it is not amending the health code. You have drafted the amendment consistent with your instructions and i would urge the chair to be fair the this amends the irs code. It does not amount to health code. Can i ask the reason why it would be nongermane . How do younongermane amend the code to deal with Health Care Issues on mandatory coverage . Lets have some fairness here . Lets put it to a vote. The public is not going to get any time to see what Congressional Republicans cash out in secret discussions between the house and the senate if they manage to pass this bill. Because the public is really not going to get a chance to see where Congressional Republicans come down because of all the secrecy we have seen. There is going to be more of the same the conference. I think this is important, being on record. This is not something i offer lightly, i dont think the public will see what the republicans had shot. We understand that and respect that. Four years they have this incredibly legitimate concern about health care. We should have listened to them and come together to address the issue for rural america. We should have repealed the Affordable Care act and replaced it with proposals to health care for millions of americans and provide tax cuts for those making millions in income. All your we had tax cuts masquerading as a Health Care Plan. Now we have a Health Care Plan masquerading as a tax bill. The plan to the cbo would force 30 million americans to lose coverage and would raise premiums to percent on the individual market. Told that it is going to lead to 25 million in medicare cuts. This is the opposite of what we need from world colorado. There is not enough coverage. Rates have gone up, there is not enough coverage. My a memo provides a backstop for Rural Communities in my state and the country, rural areas lose coverage and receive premium hikes as a result of this land which is predicted by those that have looked at this plan. I think that if the majority is so confident in the merits of their proposal as they are with deficit reduction patient support this amendment and provide peace of mind to smalltown colorado and the country. Passing this bill are going to be felt hardest in areas where there is little competition already. Where people have a hard time avoiding insurance and where they have Health Insurance. Colleagues, this would be amendment to 28. It deals with veterans. Panel are us on this veterans. I served in reserve duty. One of my favorite day of the year is veterans day. Homet all of us were back thenbly on friday and saturday we know those of us who served that it is the 11 month of the year and the 11th day. That i spokeings with back in delaware as we went around the state is talking about the benefits that we when i went there back in 1973. With the v. A. Health care, at the time we had 250 per month. Today receivedme a bill and they want to go to a state university. Folks paid for tuition. Then they get a Housing Allowance. Looking around seen if we have anybody there . The Housing Allowance for 36 a housingterans allowance and idaho of 1100 per month in i dont know about you but that is a fair amount of money. Delaware, the Housing Allowance is 2100. If we have anybody from new york state that is 4000 per month. For three years that is a great benefit. Had a got to delaware we Veterans Hospital that was it relic of a hospital. Not great quality care. Clinics orutpatient veteran homes. At that world war ii relic of a hospital it is regarded the best place for health care delivery. We now have Outpatient Clinics in every county of delaware. I suspect we are the only state in the country that can make that claim. Gets theireteran Health Care Coverage through the v. A. Of people get their coverage through medicaid. About 2 million veterans. As it turns out when the Affordable Care act went into of fact there were veterans who have had this decrease. I will say this the number of working veterans who are uninsured has decreased not by just a couple Percentage Points but by 42 . They are expanded medicaid. Mr. Chairman and colleagues, the amendment that is before us what a chore that veterans can see a reduction in access to their health care as a result of this bill. Let me say that again. But if you are living in cleveland and you move to salt job, you cant deduct the cost of your own move. This fixes to problems. Youone, fixes inequity if move to take a job. The other thing it does is say why should win Courage Companies to move overseas . It has become too often the Business Plan for too Many Companies to shut down production in, you know, Washington State or michigan, and move overseas, deduct the cost of the move, and sell the product back in the United States. We will lose 11 ohioans to this epidemic by the end of the day. More funding is needed to address the situation. The senate has acted. The house has acted. Moneye not put much behind our actions. We also played defense when people were trying to cut medicaid. I stand with governor kasich. Hes a republican. A mid to regret. I stand with him on defending the cuts to medicaid that would have hurt people getting treatment for open rates today because they have the aca. This bill makes it harder for people to donate money to charitable institutions. Surely that is something we can support, mr. Chairman. Senator hatch thank you. Any rebuttal or comments . We wrap up, i want to make it clear we reject on our side the idea that you cannot have a partisanship from the victim bipartisanship from the very beginning. What we said was both sides ought to come together to work for sensible policy that in effect will lay the foundation for arguing bipartisan tax reform now, but we have shown the last couple of years, and you remember that, nobody thought we could get a bill with sensible policies on both sides. When we were charged to do a proposal on savings and investment was the beginning of our working relationship.
