An investigation into the death of a 23-week-old Queensland baby has highlighted âmissed opportunitiesâ by the Department of Child Safety to intervene with the family.
The deputy state coroner also raised concerns over the lack of a system at a state or national level to track children from birth through school to ensure they donât fall through the cracks.
Baby J was found unresponsive by his mother, with his cause of death ruled to be SIDS. (File image)
