Further attacks on Ukraine Today with three children among seven people killed by a russian missile and a Drone Attack on the western city. Yesterday, people were killed by a strike on a ukrainian Army Training Camp and a hospital. It was the single deadliest attack on the war this year. Our Europe Correspondent is there. The mother and her three daughters were all killed in their apartment in the early hours of this morning. The dad survived and some very harrowing images have emerged of him bloodied and the welded. The conclusions are damning that the fire in 2017 was the result of decades of failure by successive governments and by what the chair called the systematic dishonesty of Planning And Insulation companies and by a lack of strategy from fire service. Weve been struck by some of the phrases weve had and i think one of the key ones was, a path to disaster that started years, if not decades before the fire itself in that very tower in 2017. What it spoke about in many different ways, many organisations in many key players. Firstly its the role of the government from the early 90s up to 2017, the report found that there were multiple opportunities for the government to realise that there were inherent issues with safety on high Rise Buildings and that there had been similar fires that could have been acted on and could have had lessons learned. There was also this change in the approach to regulation and this Regulation Issue from 2010, from the conservative led Coalition Government in which there was a tend to challenge Red Tape and deregulate. That led to the idea that safety had been ignored, delayed or indeed disregarded, which the report found was a contributing factor to this feeling and the approach to safety. We had an awful lot about the companies involved in the refurbishment of the tower, particularly about those manufactures of Cladding And Insulation and how they were guilty of a systematic and dishonest approach to the safety of those products. A deliberate and sustained strategy to mislead the industry about the safety of those projects. And indeed, so have those organisations who are meant to be certifying their safety, are meant to be certifying theirsafety, how are meant to be certifying their safety, how in effect, they missed opportunities to deem these products are unsafe. Then we had about the refurbishment itself with the inquiry singling out the architects and contractors involved as well as the tenant management organisation. Again, all of the Amana Role to play frankly were often, in the words of the inquiry, either dishonest or in some ways entirely incompetent. Degrees of guilt, the inquiry said, these contributing factors were so significant. All these multiple levels of issues. 0bviously, bringing onto the idea of what happened on the night in the days and years leading up.
