They are known as what is known as quiney opaque as twins, their skull is one long tube. Its incredibly rare. The 21 month old girls have separate brains, but are misshapen. The surgery is so complex, it will happen in three stages over several months. The twins wont be physically separate until the final operation. So we can see the artery work to do anything to it will use the microscope. Each twin is supplying the others brain with blood. Cutting these connections is dangerous and will take two operations to complete. So the artery has been clamped. The twins have been in the theatre now for more than seven hours, and there are still several hours of this operation to go. So far, everything is going to plan, and both girls are doing well. The central segment will be our richard keel. The frame is made from pieces of the twinss skull, which can be detached in later operations. Being older, they are later operations. Ideally, the surgery should have been done a year earlier, when healing ability is strongest, but there were delays finding a donor to pay the medical costs. Despite the risks, the family and doctors believe it is right to go ahead. And i think the whole team feel there is an excellent chance of a successful separation here. Models of the twinss brains and shared skull have been created using a 3d printer. For surgeons, it is massively helpful, so actually being able to touch and hold things makes it so much difference to understanding how things are. This hemisphere, the right hemisphere of the brain, is standing up, so this is actually projecting into the other childs skull. What we need to achieve is effectively untwisted the brains, and thats difficult, pretty difficult to dojust and thats difficult, pretty difficult to do just in your head. And this is surgery without scalpels. Absolutely incredible, this is exactly what we wanted. Using virtual reality. It works, it really works. So now we can see a whole lot more information, a level of detail we have not been able to access previously. This is clearly the way of the future. A month after the way of the future. A month after the first operation, the twins are backin the first operation, the twins are back in theatre. Surgeons have to finish separating their shared blood vessels. But ours heart begins to fail. They fear losing her. We are not stable but we are less unstable. They re good we are not stable but we are less unstable. Theyre good enough for me. The crisis passes, because one is that we could win, they give her a major blood vessel to increase her chances of survival. But it disadvantages safa shortly after the 20 oh operation, she has a stroke. We we re very 20 oh operation, she has a stroke. We were very close to losing her. She stayed in that critical state for 48, 72 hours after the surgery. It was a very difficult time for the girls, theirfamilies and it was a very difficult time for the girls, their families and the entire team looking after them. But after a lengthy time in intensive care, both twins pulled through. The last two months after their last operation on the brain has been a little bit of a stormy time for safa and marwa, but they are hanging in there, and both reasonably well. Throughout history, how can we separate them and try to create when we have this unusual situation of people join together, how can we create two separate individuals . Sometimes it is straight forward and sometimes it isnt, and we have to ask ourselves that even technically we can separate them, is it something we should pursue . And this is a very challenging situation where the children arejoined challenging situation where the children are joined together at the head with a lot of potential neurologic comp locations. When you say the question is should we proceed with this, what are the arguments against proceeding with this . Is your story mentioned, one of the biggest arguments is this is a big undertaking, 50 hours of surgery. A big undertaking, 50 hours of surgery. You need multiple surgeons and other caregivers to make this a success. So you are asking much time in the operating room, big expense, a lot of peoples time and effort, sometimes at the expense of other things going on in the hospital. So it is important to think about that before going forward. Is this something that is going to be successful or is putting all that effort forward going to likely end up effort forward going to likely end up in effort forward going to likely end upina effort forward going to likely end up in a bad outcome . So we often need to talk about that and be very honest with parents and their caregivers about what is the likelihood of our success. I was told this series of operations cost close to £1 million, but it has been paid for by a private donor. As part of the point that as well as benefiting the girls it is actually benefiting the girls it is actually benefiting medical science that we are learning, and the surgeons are learning much more about incredible brain surgery like this, and making real medical advances . Oh, and that is certainly an excellent point. I am not advocating we should not separate these children, its just we have to be thoughtful. If you go back 50 years ago this was not even possible. We have been able to separate most types of conjoined twins that are connected at the level of the chest, the abdomen, the pelvis, and now we can actually do it at the level of the brain. It is still a very risky surgery. But each case we learn more and more about what is possible, and this very stepwise approach is what has shown to in other cases. One of the other ethical considerations is judging between the two patients, the two girls in this case, because as we heard of that report, they switched the blood from the stronger of the girls to the weaker, and the stronger one then suffered a stroke asa stronger one then suffered a stroke as a result. So you are kind of making ethical choices all the time. Yes, and this is something you have to make choices in the operation itself and you cant really predict whats going to happen. The biggest risk of this operation is they can have a blood clot and a stroke so you have to make choices about which twins will get which blood vessel. You try to do that in the most even mannered way going forward, because you want to look going into surgery which child has the best chance of survival, based on anatomy. How is the brain structured, so you can make these choices going in so you are not surprised and try to do a Fair Distribution of each of the blood vessels. Go to talk to you, fascinating.
