Here in the middle of kutupalong camp it is very dusty and smugly at night and hotter during the day. All the people are crammed into tiny shelters behind me. You can see the shock of people the sort of sheltered people are living in. These are ideal breeding grounds for disease. I have been along to one clinic run by an aid agency where they have experienced a big rise in measles cases because of the conditions. The middle of one of the rohingya camps where people are crammed in, living in basic shelters with poor sanitation. Ideal conditions for the spread of disease. Emergency clinics have been coping with a significant rise in the number of cases of measles with over 600 in two months, the vast majority of them children. This is a population especially vulnerable to measles and additionally what we have also realised is that there are a few of these people have been vaccinated before, so when they arrived here they had no protection at all and so its a perfect environment for very rapid spread of the disease. This clinic has seen such a significant rise in the number of measles patients that its had to open its own unit specifically for that. I havent had measles but my colleague annie has and shes going to take you inside. Shes going into the isolation area, where the worst affected patients are kept. Everyone here is underfive. Juberas11 month old baby is not taking foods, since yesterday she said she hasnt even breast fed. They are given medication, vitamin a and in the worst cases, oxygen. Some have died but so far most have pulled through. With this crisis still being described as an acute emergency, the fear remains that in these conditions a more serious disease could take hold. The story of how one clinic is dealing with this outbreak and to discuss more conditions in the camp, iamjoined by discuss more conditions in the camp, i am joined by shelley from the world food programme. You have been here for several weeks. How do you feel the conditions are now compared to ten weeks ago . As you can imagine, we are still seeing new arrivals every day and one new arrivals every day and one new arrivals said to me that he looked around, look at this camp. We call ita around, look at this camp. We call it a mega camp and we are already dealing with alarming cases of malnutrition. He said he is worried about, where do i get my food . My shelter . My clean water . When you are dealing with Something Like one in five children dealing with malnutrition, food alone is not going to be able to treat that. We need to look at the soros, better health, better hygiene. We need to basically realise that these people have spent Something Like 6 7 days walking from myanmar to bangladesh. They have suffered a huge amount of stress and trauma, very little access to food and that is something we are dealing with on a daily basis. How do we get life saving food to children and mothers, Breast Feeding mothers, and we have special centres where we try and give them very special porridge. Also, to be able to tell them why it is important and why they need to supplement their diet. Baz i have spent time Walking Around various bits of these counts and people are really living in very poor conditions. They are living very close together, sanitation is bad, there is more evidence of my shift oil is being made but still a lots of dirty water around. How can people live like this for a long time . You will also have met refugees who have been here for 20 yea rs refugees who have been here for 20 years so this is not only grown from a registered refugee camp to new families that are coming in. One of the things we are trying to do is look at a livelihood, how we can empower people and give them dignity and improve the quality of their life. One example would be to give a cash card so you have Something Like 10 on a card and the choice to select your food. But we need to think of the long term and look at the site planning, where are people going to live and how they are going to live and what sustainable ways of how these families may live for the next two months or maybe one year. Its not really about training for knowledge so much as training for using the right system. I always say, why not make the right way the easy way . Why dont we make it easy for staff to do the right things . But across the nhs, a shortage of staff to provide safe care remains a problem. We have got a real concern about Staffing Levels. We have been saying there isnt enough midwives for some time. We need more staff and more capacity to safely care for mums and babies. Progress is being made, but there are concerns that difficult lessons are not being learned. Joining me now is professor timothy draycott, senior clinical lecturer in social and Community Medicine at the university of bristol. A lot of people will be surprised at the relative power figures for england in terms of stillbirths. Why are we amongst the worst in western europe . The good news is that stillbirth rates have halved since the 50s but we are definitely one of the 50s but we are definitely one of the worst rates in europe. I think the worst rates in europe. I think the exact reason is not quite clear and there is a lot of Research Going on now to try and work out what the causes of this height stillbirth rate. One of the things, when there has been a difficult outcome like a stillbirth, hypoxic injury for babies, we need the system and the local system has to investigate that and really understand what is going on. That is why we are keen to welcome what the secretary of state said today, to investigate these incidents and share that knowledge right across the service. Where do the parents comment on this . Grieving parents who havejust the parents comment on this . Grieving parents who have just gone through the most traumatic experience of their life, many say they dont feel they are part of that process, that they are kept out and that actually adds to the pain. Thats a really big issue and one of the things we are really looking to address with this new incident investigation. One of my senior lecturers is doing exactly that, asking parents how they want to be involved. Not all parents want to be involved. Not all parents want to be involved in the same way and one of the things we have to do is be flexible enough to encourage patients, women and their families, into the investigation, both to contribute some understand the issues and also so we cant understand the issues and share those right across the service. know its not the case at Southmead Hospital but the perception is that in some hospitals there just are not of the staff. Mothers are coming on and there is no midwife or qualified doctor to look after them. In most hospitals there is qualified midwives and doctors. We definitely haveissues midwives and doctors. We definitely have issues with staffing and as with everyone across the service. We have to think that it is just not numbers of staff, although that is important, its how we train staff to provide the best possible care. That care is for the best possible outcomes but also the best possible experience. Sometimes that gets missed in the safety argument. One of the things we argue for us, lets do some research to work out what staff you needed to make a good birth but for both safety and experience. What about the threat of litigation . How much of the cloud is that when you are dealing with what should be a perfectly natural process when it goes wrong, the pressure may be just to perform a ca esa rea n pressure may be just to perform a caesarean so pressure may be just to perform a caesa rean so you pressure may be just to perform a caesarean so you dont go down a road when lawyers will be hovering . Is that a factor . Defensive medicine isa is that a factor . Defensive medicine is a problem. In maternity care, we are dealing with normality, what should be a normal process. With healthy women on the whole. Clearly thatis healthy women on the whole. Clearly that is not the whole service and for some women a scissors caesarean section is a life saving process. We need to work at the right circumstances. Litigation is a massive issue in the uk and it is clearly money no one wants to either receive or spend. The way of dealing with that is not by worrying about defensive medicine, we need to do the right thing more often. If we could reduce the preventable harm, the litigation costs would look after themselves. Thank you very much for your time. Remember that the loss of this work was done in whitehall because the government was not being prepared for brexit and not thinking about the conference line consequences. The government refused, saying it would undermine their negotiating positions and then in november there was a fault in the house of commons forcing the government to hand over the documents to this support committee. That has been done, late last night the government came forward with a big pile of papers, 850 pages. That has been distributed to those on the committee who will look at it. But many of them have a problem because they say it is not right that the government has decided to take some bits out and that the government has decided which bits to take out. The select committee this morning has decided to ask david davis to appear before us. What has happened in the Lake District that the mountain rescue teams are feeling so much pressure . Basically there are almost record numbers of people being rescued from the mountainside. The rescuers are volu nteers the mountainside. The rescuers are volunteers and they are looking at the possibility of a record year this year. There are ten teams covering the Lake District. In 2010 there were 600 call outs, they are closing in on that already before we have reached december. One has had 140 this year. They are saying, please, if you go to the hills make sure you are prepared and you follow cautions because you are putting a lot of strain on people who are not fully paid up, not salaried members of mountain rescue teams, they are volu nteers of mountain rescue teams, they are volunteers and cannot keep leaving theirjobs to rescue you from the hills. What are the consequences of this increase in pressure . It isnt easy, if we were members we couldnt just walk out it isnt easy, if we were members we couldntjust walk out now, you have a job to do. Lots of people are in that position so its making life difficult for lots of people. They are saying morale is good, theres noissue are saying morale is good, theres no issue with retaining existing volu nteers no issue with retaining existing volunteers that they already have, they do it as a passion, but if you are being called out sometimes as many as five times in a working week to go upa many as five times in a working week to go up a mountain and help out somebody who may well have been legitimately injured and it may have been an unavoidable accident that could have happened anyway, but if its been caused because you are ill prepared, you dont have the right equipment, you dont have the right equipment, you havent told someone where you are going, you cannot read a map properly, they are saying please dont put other people out through your own stupidity and that is the big cry from the mountain rescue teams. I go there every year and every year there are more and more people because it is such a wonderful area. What are they saying in readiness for next year . How can they prepare for it . Its a beautiful part of the world, spectacular. There are more people in the summer although they tend to gravitate towards places like windermere, towards the water. Yes, they go up the hills and you can get caught up quickly because the weather can change in an instant, but also this time of year it is more extreme and a lot of the people who go at this time of year are hardened, experienced and know what they are doing but for those that dont, even at this time of year they are putting pressure on people. Joanna dixon is one of them, she was on a night shift in pita bread when she was called to someone who had fallen offa was called to someone who had fallen off a bicycle. She was treating him in the back of an ambulance and he attacked her. It was a Sexual Assaults but she wanted to share her story with us. She said she felt degraded. It is very challenging because you are in a confined space and there is nowhere to run away too. Our priority is looking after the patient and making sure they are safe. We are doing ourjob to look after them so there is no escape from it. So what happened to the man who attacked her . He was convicted. Hes been given a suspended prison sentence and hes now on the sex offenders register, but he is a minority. We know that criminal charges are only brought against around the of these attackers and the boss of the Ambulance Service says even when these offenders are caught the penalties are not tough enough so even though they are working closely with the police to they have made it clear these assaults will not be tolerated. Do we know why theres been this increase in assaults . It is largely because ambulance crews a re it is largely because ambulance crews are having to attend more drink and drug related call outs and those are the patients who tend to be uninhibited, violent or simply lost a ny be uninhibited, violent or simply lost any sense of what decent behaviour is. In terms of what it means the staff are facing, we have heard from a student paramedic who was head butted, another who was jabbed with a needle, and we have the Sexual Assaults we have just heard. They are having flashbacks, not working on the front line any more, they are having counselling for ptsd and all because they went into a profession to help people. Its crazy, isnt it . He arrived in the uk from india in the 19505 to work in a factory and began taken photos of the citys Asian Community over a 30 year period from the 19505 to the 19805. But they only really came to light last year. Now his photographs have finally been published and his book is being feted around the world. Phil mackie reports. After more than half a lifetime as a photographer masterji has never had a higher profile. His fame came late. Hello, masterji would you like to have a look at your book . Yes, thank you. Its a very nice photo. He was only discovered when he was 94, now a year later his work is being published for the first time. His work will documents a time of change in his home city of coventry, he photographed the families who like him had moved here from india and pakistan but his pictures also evoke a moment in modern british history and also show a wry sense of humour. When i came here in this country i got only one camera, start from there. I was a passport photographer in coventry. In the Asian Community. So know me everybody. Everybody knows you. In coventry is a sense of familiarity and people were telling stories that they are building on the collection that masterji has himself, people coming up with their own family photographs and stories about their own family photographs and a real depth of meaning and feeling for the communities in coventry masterji recorded. I can see my father in another light, not just as my dad but as a photographer that had skill and talent and knew what he was doing. Its a wonderful way to remember his legacy as well.
