There are a few simple steps you will need to take before the process of obtaining medication for you can begin.. ◾The first step is to mail, fax, e-mail, or private message us your name, telephone number and a list of your medications. The list must include the name, strength and number of times a day you take each medication.. ◾List the name(s) of your doctor(s) that prescribe each medication. It is very important that we know which doctor prescribes which medication.. ◾Within 48 business hours of the time we receive your communication a representative from our office will call you and let you know which medications we can assist you with. At this time we will also obtain some additional information from you to determine qualification to the program. This will include your date of birth, social security number and househould income information. . . FEES. . There is a processing fee of $12.00 for each Brand name and $10.00 for each Generic medication requested for each 3 month supply. The pharmacy may charge a small dispensing fee. You will be advised ahead of time if any other fee applies.. .
