Detailed description is The Community Connections Youth Project (CCYP) appeals directly to FosterAdopt Connect's focus on increasing the quality of life experienced by young adults impacted by the foster care system, by connecting participating clients to community-based resource providers. CCYP is designed to be voluntary, allowing clients to opt in or out without consequence. This service allows program staff to help clients in whatever ways they are willing to be assisted to meet their goals. . . CCYP's program model incorporates aspects of FosterAdopt Connect's successful Advocacy and Extreme Recruitment programs into a Strengths-Based model of voluntary peer case management for clients currently exiting or recently exited from the foster care system. This model of case management emphasizes the inherent strengths of the client, and builds upon existing (or recruited) resources to produce an increase in the quality of long term outcomes for former foster care alumni, who have historically tragic long term outcomes. This support, referral and advocacy program will be provided by Community Connections Specialist, whom are foster care alumni, to current or former foster youth. . . This program is focused on accomplishing a few primary goals: . . Identifying and collecting a wide range of tangible supports and resources, which may be beneficial to serving older clients within their respective communities. Such resources will help with transportation, clothing, food, utility support, health care, mental healthcare, child care, employment assistance, educational assistance, and more. . Partnering with the Division’s programs currently working with this population who are most likely to age out of foster care to establish a relationship with those clients prior to their exit. Seeking and finding older clients, who have exited foster care within the last seven years, and who could benefit from the services and supports this program offers. Providing targeted voluntary strengths-based case management, to include individualized goal setting, and tangible service provision to address areas where clients are struggling most, and to assist in completing the tasks necessary to achieve priority goals. . Creating a personalized family and natural supports ‘address’ contact book, which is done by working with FosterAdopt Connect’s Extreme Recruitment program model. This service will work with clients to identify and find family and natural supports, to engage with those supports to determine how each can be available to assist the clients with barriers he/she may face, and to create for that clients a personalized ‘safety net’ of familiar and concerned adults who can make sure that the client is not walking into adulthood alone. . .
