Luckily, nobody was injured, cause there were no passengers on that train, but it led to widespread delays that day and has now further eroded the publics confidence in the transit system. They told us that somebody had just overlooked it. It turns out to be more complicateded than that. Reporter august 6th derailment was caused by a broken fastener that holds the rail in place. A month before the derailment, a track inspector discovered the faulty fastener but mistakenly delete it had from the inspection report, so it was never fixed.
