Cause of tragic plane crash revealed

Toxic gas slowly filling the cabin of a seaplane flying over a river in Sydney s north was the likely cause of a tragic crash that killed six people, a final report into the incident has found. Gareth Morgan was carrying five passengers on the Sydney Seaplanes DHC-2 Beaver when it took off at Cottage Point and crashed in Jerusalem Bay, in the Ku-ring-gai National Park, on New Year s Eve in 2017. Elevated levels of carbon monoxide seeping into the plane left him confused, disoriented and unable to safely fly the aircraft, the Australian Transport Safety Bureau has ruled. On board the flight bound for Rose Bay was the chief executive of catering giant Compass, Richard Cousins, his sons Edward and William, fiancee Emma Bowden and her 11-year-old daughter Heather.

CO Poisoning led to Beaver Crash: ATSB 29 January 2021 The Australian Transport Safety Bureau (ATSB) has determined that a carbon monoxide (CO) leak into the cabin of a de Havilland Beaver floatplane contributed to a fatal crash on the Hawkesbury River, killing all five on board. Beaver VH-NOO had just taken off from Cottage Point with four passengers on board on New Year s Eve 2017 when it circled back from Cowan Water into the narrow Jerusalem Bay. During an attempt to perform a high bank-angle low-level 180-degree turn, the pilot stalled the aircraft into the water. In the investigation report released today, the ATSB found that cracks in the exhaust collector ring on the engine allowed CO gas to escape into the cabin via holes in the main firewall where three bolts were missing.

Coffs Harbour plane crash: ATSB investigation reveals the cause of Jeffrey and Matthew Hills' deaths

Pilot Jeffrey Hills, 59, and his son Matthew, 25, died in a plane crash in 2019 Mr Hills did not have a valid licence for the flight, according to an investigation The pilot encountered poor weather and requested to increase his altitude  A trainee controller declined the request another then gave limited information   This resulted in Mr Hills lowering his altitude and crashing into high terrain
