Luckily, nobody was injured, cause there were no passengers on that train, but it led to widespread delays that day and has now further eroded the publics confidence in the transit system. They told us that somebody had just overlooked it. It turns out to be more complicateded than that. Reporter august 6th derailment was caused by a broken fastener that holds the rail in place. A month before the derailment, a track inspector discovered the faulty fastener but mistakenly delete it had from the inspection report, so it was never fixed. I really dont know what to say, like i use it every day and i pay a lot for it, so, it is very hard to hear about. Reporter metro officials declined to be interviewed about the report but this a written statement, acknowledge the inspectors mistake was only part of the problem. That such a serious error went undetected congresswoman Eleanor Holmes norton says the inspect he were is not to blame. If you want to ascribe this to human error, then we are going to keep seeing this happen. Sure, it was human error. Whose responsible was it to find that error . It was the responsibility of his supervisor to say why did he delete this . Reporter now, metro insists the system is safe and that they are beefing up their inspections as well as the checks and balances on those inspections. Those closest to Timothy Holden say most everyone knew he was a kind and loving man. He was a brother, best friend, great dad to five children. This according to Prince Georges County police and the states attorney, they do believe that this was a case of murder and is sometime under investigation. You never know, people have a different life outside of Reporter Police say the 20yearold killed his girlfriends 1yearold son inside their riverdale apartment while she was at work. What we snow this baby didnt die as a result of an accident. We know that this baby was, in fact, murdered. The iteam told you how employees with the u. S. Department of home lands security were swapping their taxpayerfunded credited cards for tense of thousands of dollars in starbucks drinks. Some of those cards have been stolen. The Us Department of home lands security protects the home lands, protects the country but they couldnt protect their own credit cards. It was last fall, the news4 iteam found some federal employees with the u. S. Department of home lands security made dozens of starbucks purchases with their governmentissued credit card. I have a short steamed soy milk. Reporter 30,000 worth of coffee in just one year, a bad use of taxpayer money, says form he were federal prosecutor and government auditor brian miller. You want wise and careful planning and you want things can done so that everyone benefits. Reporter turns out the coffee wasnt the only nipping brewing. So, too, was an internal investigation inside agency headquarters. Members of congress and Homeland Securitys Inspector General saw our story and investigated who was buying the coffee and why. Internal investigators 15i 5 of the purchases we reviewed and that they reviewed involved stolen cards and we asked the department of Homeland Security how often their cards or card information had been stolen and they declined to answer that information.
