The OPD was twice a week and during those early days of Sleep medicine we had a few patients only.
Bit the repeated theme that came through was that the patients were not aware of any problems, while their bed partners were very anxious about their snoring and the stopping of breathing which took place every night..

Few people did come to us with daytime sleepiness and tiredness which is classical of sleep Apnea..

After a diagnosis the patient was called back to the hospital and given the diagnosis and more importantly treament was discussed with them..

Then, as now, the first treatment modality was using a machine during sleep.
Since they had to wear it at night while sleeping.
It could be claustrophobic at first and we had to call them in and use the machine first in the hospital and then only could they take it home..

During those days the machine was not available in the NHS and so had to be bought by the patients.
Since they were expensive so a lot of counseling had to be done too..

Before the machine was prescribed another night had to be spent in the Sleep Lab for titration purposes.
The second night the patient slept with the machine on.
In the night when he has apneas the technician used to first add pressure and then increase it gradually till the apneas disappeared..
This was recorded by the technician so that when we prescribed the machine the pressures were mentioned as well.
This reminds fixed till we decided to change it for some reason..

This was required occasionally but had to be confirmed by a repeat titration study each time..

The early experience with these patients was mixed.
Some came back and said- "Doc I am a new person after night of use of the machine".
And they were so grateful for it.
Others couldn’t use the machine properly due to various reasons like claustrophobia, dry mouth , leakage, machine blowing too hard sometimes stopped them from getting to sleep etc..

Their spouses however we’re very happy as the snoring disappeared straightaway and for the first time in years they could sleep well.
