เกาะกูดเริ่มปรากฏหลักฐานตั้งแต่รัชสมัยพระบาทสมเด็จพระพุทธยอดฟ้าจุฬาโลกมหาราช เมื่อองเชียงสือ เจ้าเมืองญวนที่เข้ามาพึ่งพระบรมโพธิสมภารของพระองค์ หนีออกจากกรุงเทพมหานครมาลงเรือที่เกาะสีชัง จากนั้นแล่นเรือชักใบในอ่าวไทยมา 7 วันจึงถึงเกาะกูดที่ไม่มีคนอยู่เลย.
ชนพื้นถิ่นดั้งเดิมของเกาะกูดส่วนใหญ่เป็นคนไทยที่อพยพมาจากเมืองปัจจันตคีรีเขตร์ (เกาะกง) ที่ตกเป็นเมืองขึ้นของฝรั่งเศสเมื่อปี พ.ศ.
2447 มีหมู่บ้านคลองมาดเป็นหมู่บ้านที่เก่าแก่ที่สุด.
เดิมเกาะกูดเป็นหมู่บ้านขึ้นกับตำบลเกาะหมาก อำเภอแหลมงอบ ต่อมาได้ยกฐานะขึ้นเป็นตำบลเกาะกูด ในปี พ.ศ.
2523 ตัวเกาะกูดมีระยะทางห่างจากอำเภอคลองใหญ่ประมาณ 40 กิโลเมตร ประชาชนชาวเกาะกูดมีความยากลำบากในการเดินทางไปติดต่อราชการ ประกอบกับเกาะกูดอยู่ใกล้ดินแดนประเทศกัมพูชาด้านเกาะกงมากกว่าฝั่งไทย ทางราชการจึงมีนโยบายเพื่อความมั่นคงและอำนวยความสะดวกแก่ประชาชน โดยได้ยกฐานะเกาะกูดและเกาะข้างเคียงขึ้นเป็น กิ่งอำเภอเกาะกูด ตามประกาศกระทรวงมหาดไทยลงวันที่ 19 มกราคม พ.ศ.
2533 โดยมีผลบังคับตั้งแต่วันที่ 1 เมษายน ปีเดียวกัน และต่อมาได้มีพระราชกฤษฎีกาลงวันที่ 24 สิงหาคม พ.ศ.
2550 ยกฐานะขึ้นเป็น อำเภอเกาะกูด โดยมีผลบังคับตั้งแต่วันที่ 8 กันยายน ปีเดียวกัน.

เกาะกูดเป็นเกาะที่ใหญ่เป็นอันดับที่ 4 ของประเทศไทย มีเนื้อที่ 105 ตารางกิโลเมตรหรือประมาณ 65,625 ไร่ ความยาวของเกาะ 25 กิโลเมตร ความกว้าง 12 กิโลเมตร เกาะยังมีสภาพความเป็นธรรมชาติอย่างสมบูรณ์ โดยมีภูเขาและที่ราบสันเขาซึ่งเป็นต้นกำเนิดลำธาร ทำให้เกาะกูดมีน้ำตกที่ขึ้นชื่อคือ น้ำตกคลองเจ้า ที่มีน้ำไหลตลอดทั้งปี ทางฝั่งตะวันตกของเกาะ นับตั้งแต่อ่าวยายกี๋ หาดคลองเจ้า หาดอ่าวพร้าว อ่าวง่ามโข่ หาดอ่าวเบ้า หาดคลองหิน อ่าวพร้าวจนสุดปลายแหลมเทียน ล้วนแต่เป็นหาดที่มีทรายสวยงามน้ำทะเลใส ประกอบกับธรรมชาติสงบเงียบ ร่มรื่นด้วยทิวมะพร้าวริมหาด นอกจากนี้บนเกาะกูดยังมีป่าชายเลนที่สมบูรณ์และแนวปะการังหลากชนิด รวมทั้งเกาะแรดและเกาะไม้ชี้ซึ่งอยู่ใกล้กับเกาะกูด.
อำเภอเกาะกูดประกอบด้วยเกาะใหญ่น้อยรวม 24 เกาะ โดยแบ่งเป็น 3 หมู่เกาะ.
หมู่เกาะกูด มี 3 เกาะ ได้แก่ เกาะกูด เกาะแรด และเกาะไม้ชี้.
หมู่เกาะหมาก มี 9 เกาะ ได้แก่ เกาะหมาก เกาะยะรั้งใน เกาะยะรั้งนอก เกาะผี เกาะขาม เกาะกระดาด เกาะนก เกาะนอก และเกาะใน.
หมู่เกาะรัง มี 12 เกาะ ได้แก่ เกาะรังใหญ่ เกาะรังเล็ก เกาะตุ๊น เกาะกระ เกาะเทียน เกาะทองหลาง เกาะมะปริง เกาะยักษ์ เกาะกำปั่น เกาะใหญ่ เกาะกลาง และเกาะเล็ก.
