After many years of hard work and commitment from numerous members of the community, we are happy to say that the dream of having a community health center site in Amherst is getting much closer to becoming reality.
Work on developing the John P.
Musante Health Center (JPMHC) which will be a satellite location of Hilltown Community Health Center (HCHC), has been in the works for many, many months.
With much cooperation and support from the Town of Amherst, we have secured a site on the lower floor of the Bangs Community Center in downtown Amherst and are developing architectural plans for the space that will allow us to meet the needs identified by the community..

HCHC was recently awarded $1 million by the Federal Health Resources and Services Administration (HRSA) for the development of the new health center site in Amherst..

JPMHC will be a Federally Qualified Health Center site, providing one-stop, affordable primary medical, oral health/dental, and behavioral health care to anyone who needs it: our neighbors and families who do not have insurance or adequate financial resources, have life challenges that limit their ability to access necessary health care, or are intimidated by the seemingly complex health care system..

Services will be provided through a patient-centered health home model, in which clinical teams collaborate and coordinate patient care in order to improve the overall health and well-being of patients.
Providers will offer medical, oral health/dental, and behavioral care in the context of the client’s broader needs, recognizing that social determinants of health can be as important as clinical services; factors such as poverty, education, language, transportation, and food security must be addressed as well as dental and medical needs.
JPMHC will therefore have strong collaborative and referral relationships with other established local service organizations.
This reciprocal relationship with other local providers and human service groups will allow JPMHC to connect clients with additional services, and will enhance our outreach efforts as we work through those organizations which have gained the trust of many members of our target populations..

We anticipate having on staff two primary medical care providers (MD and Nurse Practitioner), one dentist, and one hygienist, and will have the services of a behavioral health provider (LICSW or LMHC) available.
We will also have a Community Health Worker, who will help coordinate care, address non-clinical needs, and support patients’ health self-management efforts.
There will also be at least seven support staff members.
Whenever possible, staff will be bi-lingual.
We hope to eventually serve approximately 2,700 patients annually in the Pioneer Valley.
The JPMHC is projected to open and begin serving patients in mid-2017.
