But three received a C grade, meaning patients are more likely to die a preventable death at those facilities than those graded higher. The Leapfrog Group, a national nonprofit organization committed to improving the quality and safety of American health care, assesses roughly 2,500 hospitals nationwide twice a year. Leapfrog uses up to 28 national performance measures from the Centers for Medicare & Medicaid Services, the Leapfrog Hospital Survey and information from other supplemental data sources.
