Lorik Tool & Automation Inc., a small business and manufacturer of custom automated machinery located at 19 Copernicus Boulevard, failed to ensure that there was a procedure in place to prevent material from injuring workers by tipping or falling.

On June 19, 2018, the two men were trying to load a five- by 2.5-metre electrical panel that weighed more than 1,800 kilograms at Lorik Tool and Automation Inc.
