You must provide
evidence of identity for any party (and the person lodging the application if different) who is not
legally represented.
The evidence must be in form ID1 or form ID2 and must be completed by each individual person who is a party to the above transactions who is not
legally represented. This requirement applies to both private individuals and corporate bodies such as companies.
Both forms can be downloaded free of charge from our website or by phoning customer support.
Both parts of form ID1 or form ID2 lodged in support of an application must be dated and signed no more than three months before the time of lodgement.

As major reforms to mental health legislation have been unveiled,
Nursing Times has spoken to nurse leaders to get their views on the proposals and the implications they have for the profession.
The government published a new white paper in January outlining suggested changes to the Mental Health Act 1983 (MHA) that seek to give patients more control over their care and treatment.
The white paper paves the way for the introduction of a new mental health bill next year.

This case concerned an elderly woman, GA, who has dementia and was represented by the Official Solicitor. She was cared for at home by her adult son TA. The court was concerned about the negative impact on the proceedings by the actions and conduct of TA and examined two issues:
• If TA should be permitted to record the court hearings
• the extent to which the court had power to restrict his communications with the court office.

In March 2019 the local authority responsible for meeting GA s needs under the Care Act 2014 brought proceedings under the Mental Capacity Act 2005.
