While there is no question that in too many instances this underfunded and under-resourced sector has indeed failed, I wonder if perhaps we should be looking at things from a different perspective.  While many people refer to the need for a fence at the top of the cliff instead of ambulances at the bottom, even that is a tired, old cliché, when the reality is that there is so much more that we could be doing to prevent people from ever getting near the cliff edge.
