The report also ruled out any issues with weather, birds or other external factors. The investigation concluded there should be no disciplinary actions or administrative actions against any Marines.
Despite the findings the military continues to fly the V-22 with no firm understanding of the cause or any definitive mechanical fix for the problem in sight.
“The investigation revealed no error on the part of the pilots and aircrew, and confirmed no maintenance errors led to the mishap," the V-22 Joint Program Office said.
