A group of physicians formed a limited liability company (LLC) in the surgery center endeavor. The group includes physicians include Eric Arp, Harley Barrow, Daniel Decker, Jason Lindsay, Merwin Moore, John Neis, Paul Neis, Corey Smith and Kevin Steffan.
