Woman with skin infection died after receiving substandard care

A home care provider has been found in breach of the Health and Disability Code after failing in its care of a woman with a skin condition. Opportunities were missed for a woman to get the clinical care she needed, the deputy health and disability commissioner says. The woman in her 60s suffered from health conditions including a history of skin infections and had not left her house in over a year. Access Community Health Limited workers raised concerns over her deteriorating skin several times in the months before her death, but they were never escalated or actioned.
