Our correspondent Louisa Pilbeam reports. She was left needing a stoma bag after mistakes in her maternity care. She spoke to bbc breakfast earlier this year. The First Uk Inquiry Into Birth Trauma has heard what it calls harrowing evidence from more than 1,000 women.
