We had a will the of holes line up this time. We had a pilot that was new on this airplane. We had an instructor who was new as an instructor, we had fatigue. The pilots relied too much on the automation and were flying too low and too slow when a plane slammed into the sea wall. Though the ntsb made 15 recommendations in the wake of the sfo asiana flight which ones will be adopted and by whom is still very much up in the air. And we go to tom vacar who looks into what went wrong. Reporter ntsb recommendations to asiana are just that, recommendations. Even the federal Aviation Administration can only hope asiana complies. Asiana has no to comply to train. It further suggests changes to crew operation manual not just to the 777 but all its light body aircraft.
